However, I later received an unexpected separate office visit charge in addition to the preventive exam billing. My concern was not the care itself, but the lack of transparency regarding billing. During the wellness visit, the provider briefly reviewed outside lab results I had brought with me, and I was later billed for a separate office visit associated with that discussion. I was not informed during the appointment that this could result in an additional charge. After reviewing my visit documentation, I also felt portions of the note did not fully reflect my understanding of the visit, particularly regarding the extent of evaluation performed beyond the wellness exam. After raising concerns and providing supporting documentation, the office acknowledged overlap between the preventive visit and additional billing and indicated the matter would be reviewed. However, I did not receive a final follow-up or resolution regarding the coding review or reimbursement request. I would encourage the practice to improve communication around preventive visit billing and provide more consistent follow-up during billing disputes.

I called billing, no response. I emailed and they told me they would resubmit. I never heard back. 8 months later I get notice of a new bill, now an even higher amount. I called billing, the number doesn't even work. Emailed, no response. I can't even message in the online portal. I can't seem to reach anyone, yet I keep getting notices of this bill I need to pay. I liked the dr I saw, but their billing dept is incredibly incompetent and predatory, and for this reason I recommend never going to this practice. It's incredibly frustrating for a practice to be this unresponsive.

I told them multiple times before and during the visit that i do not need an examination- i just need my immunization certificate completed by the doctor. They told me they just need to do intake to complete the form since my old doctor left but they will not charge for that. Of course, a couple weeks later i get an email that i have to pay deductible amount of $100 due to a 20 min visit. Mind you, they took their time to complete the form while having me sit in the back just to charge me for a visit i didn't ask for. Stay away!!!

I have gone to Dr. Shafai for three years before I moved away to another state. My husband and I had to drive 25 miles in the horrible northern Virginia traffic to see Dr. Shafai and it was well worth it. She has always been very caring and attentive of our needs and health issues. She not only takes the time to listen to your health issues, but also asks if there are any other concerns you may have. She is the best Internal Medicine physician I have ever had. I had major adrenal gland issues where my cortisol levels were off the charts and instead of prescribing me pharmaceuticals, she placed me on natural supplements and after my follow up blood work, my cortisol levels were back to normal. As far as Priya S. comment regarding the staff, I have never encountered any rudeness from any of them. I have had to schedule emergency appointments and they were always able to get me in that day. I have never been charged for anything other than my visit or medication I obtain from them. I would like Priya to prove that their office staff is made to write their own reviews on Yelp. Bottom line, Dr. Shafai is very thorough and compassionate and highly qualified to manage your health and well being. Also, I do not work for Advanced Health Center in Arlington, VA, I was a very satisfied patient who wishes she practiced in Washington state where I now reside so I could continue to get excellent health care from her.

    Since June 20th, 2026, when I was scheduled to be seen by Nurse Adhikari, I've had to call the…read moredoctor's office (your office) several times (at least 6 times, if not more), in addition to sending an email. This was my semiannual visit, and the whole purpose was to have a refill on my medication. The day after my visit, I went to the pharmacy, and they had not yet received the doctor's order. I called the office, and they had to ask the nurse for the prescription; they didn't have it. When I went to the pharmacy a few days later, only one of my prescriptions had been sent, and I had to call back for it. When I was with the nurse, I specifically asked for my lab results. Yet she forgot that there is a need for medication. The medicine needed a doctor's approval. I had to call my insurance company three times to ask whether they had received approval for the medicine, and each time the answer was no. I called the doctor's office, and they didn't know how to work the system to send the approval. Finally, they lied to me and said the insurance didn't approve the medication and that it has to go through a specialty pharmacy. I called my insurance today, and they did not know anything about this claim. There was no doctor's approval for the medicine in question on my file. My insurance agent called the doctor's office while I was on the phone, and I was listening for them to leave a message. There has been no response from your office yet. This is the worst I have seen any medical organization perform in my many years of experience. I did not think this level of ignorance from a doctor's office was possible.

I've been extremely impressed with the care I've received from both the doctors and their staff. In their Herndon location, they even have onsite ability to conduct standard serum testing. This streamlining for me is coupled with the extremely generous amount of time that doctors spend during appointments. I feel that the concierge practice model has provided care far exceeding what my old local medical practice had become after losing a few key physicians through retirement. I'm not sure if they are accepting new patients, but if Lifetime Internal Medicine is, I highly advise trying this 'small' team concierge solution as a return to times when medicine was practiced by Doctors instead of Doctors applying a one-size-fits-all template strategy for the codified roll-out of big-med:-)
