This is a little late...My wife spent 2 months here, Jan-Feb of 2026. Recovering from a broken leg and arm. I can't say enough good things about South Bay. She had a private room and received excellent care. The first month she was bedridden and needed help with everything! Second month the PT started and progress was evident from the start. All staff were exceptionally helpful,friendly and respectful. Meals were more than plentiful, they may have gotten boring after a month but when preparing meals for 80-90 patients you can't please everybody. These awards that this place has received mean nothing. My fiancés father was admitted here last week. Found out a few days ago that he fell on the floor and was not found till a few hours later. At that was because he was banging on the wall and called his wife. His call light was on. Like wtf? Are they not checking the light and alarm?? When we came to visit him, found out he was in a diaper while the other day he was walking to the toilet. When was this change? The nurse saying that she didn't know and she just started being under his care. Ummm doesn't the other nurses give reports??? We aren't stupid. If you care about your loved ones please do not send them here! The staff are rude and will not assist. read more

The care quality in my experience at these center was excellent Thanks to Jessica. I had a beautiful private room overlooking the garden with its own private bathroom. My dad was at South Bay Post Acute Care from aug. 31st 2024 to Oct 18th 2024. He had an aneurysm and it the left side of his body paralyzed and I chose this place for the rehab portion as the first steps to his recovery. He went to them as a fully dependent man meaning he needed help from them to do everything and the way my father was treated was appalling and sad. The amount of times I came in to see him and he was sitting in his own feces dripping down his leg, it was disgusting. He was never checked on a regular basis and left alone for hours at a time. Was never encouraged to eat, get out of bed or to even do rehab. This place is severely understaffed and on top of that the a good majority of the CNA's did not care to do their job properly, out of all the CNA's there I ran into 4 of them that actually cared and went the distance to make sure my father got what he needed and there was only 1 wound nurse that actually cared and she only works part time the other wound nurse that I ran into didn't seem to care and continually gave excuses to why his bedsore, that this place caused, wasn't getting better. While he was at this facility he got pneumonia, UTI, blood clots and a bed sore that wouldn't heal due to the bandage not being changed regularly. He had up to 100 days at this place and ended up taking him out early due to the horrendous care he got. When he got discharged he went to a long term care facility where many more problems were brought to our attention, the pneumonia never was fully recovered from, they stopped giving him the meds and he had a massive stool blockage that was never addressed, we found out he was giving stool softener and laxatives but it was never brought to my attention why. They put him on high cholesterol pills and he never had high cholesterol, they made decisions and never got any permission from me to do anything. They stole my personal items and when I brought that to their attention it took them over a month to reconcile that and a year a later they are sending me a bill for over $5,000 and on the description for it, it just say's charges. If you have a loved one that is fully dependent from an accident DO NOT SEND THEM HERE! They just want the money from you and they are laughing the way to the bank. Maybe if you aren't a dependent person you can go here but I wouldn't recommend it. The PT is also a joke, they fool you with being nice and understanding people but they have no idea what they are doing and don't even care to encourage anyone. Scripps Mercy Hospital, Chula Vista, released me to South Bay Post Acute Care for convalescence and rehabilitation of my broken hip on May 4, 2024. I remained a resident there until May 26, when I was well enough to live at home. I didn't expect three weeks at the Intercontinental: What can I say, Medicare was paying the bill. Although the facility had seen better days, i.e. my room had a missing section of floor molding and scuff marks on the paint, it was cleaned daily & the staff was competent and friendly. Certified Nursing Assistants (CNA's) provide most patient care. Nurses dispense medications and provide wound treatment. Although I never had an unsatisfactory encounter with staff, two CNA's, Emerald and Michaela stand out in my memory: Emerald for her empathy, and Michaela for her "cheerleader-like" enthusiasm. Never saw a doctor, or even a Physician's Assistant. However, a Podiatrist did visit me. When I asked who had requested his services, I didn't, he told me he was routinely called in to care for patients feet. (This caused a billing problem for me, because the facility assumed I had Medicare part B to pay for it. I don't, so had to pay the bill. Decent $45 pedicure, though. ;-) The facility made another incorrect assumption about my insurance status, and it cost me $550. I was given, unasked, a single room. When making arraignments for my discharge, I was presented with a bill for $754. A copay of $204 was what I expected, as my stay was one day over what MEDICARE would pay for without a copay. However, the facility assumed I would qualify for MEDICAL, and that they would pick up the extra $550 for the private room. I didn't qualify (if they had asked, I could have told them I wouldn't) so the extra $550 was my problem. Patient Meals were excellent. A copy of the month's planned menu was routinely provided whenever asked for. The dishes offered were varied, well prepared, and, for the most part, served at eating temperature. (A cold pancake here or there, but who's complaining?) Physical Therapy was another high-point of my stay. Either Alex or Andrew would visit most mornings, to help me with getting dressed & morning hygiene. They taught me new methods necessary for accomplishing daily activities, i.e. getting dressed. (I still put my pants on one leg at a time, but which leg goes in first now makes a difference!) My overall rating for the stay was between three & four stars. Bumped it up to four because of the staff interactions I had. read more

The staff is amazing and excellent customer service. From front desk to the girls from activities Naria, Carolina, Jessica, Giselle, Veronica, Maria and Natalie Always so attentive with my family and mother. I leave at ease knowing that the girls care for my mother as if she was part of their family. I haven't met any of the LVN's nor the RN's. I seem to get the run around when I ask questions or no one comes out and talks to me. The receptionist that works on the weekends I believe her name is Jasmin is not very approachable. She needs more customer service she can be rude. My rating should be less but honestly I am pleased with the people that we've talked to which is the front desk and the girls from activities. read more

One of the best facilities in San Diego, I had my after surgery rehab here few years ago and I have no complaint about the place. I recommended the place to my sister as she had the same surgery (right knee replacement) and she said the same thing (she's still a patient and on her 3rd week of rehab) The staff here are great, its well managed and clean. The dedication and care received exceeded all of our expectation. When my mother first arrived here I was somewhat hopeful even though the place is run down with no hot water in the bathroom, because at first the staff seemed very attentive. However after my mother's 3 week stay I found that not to be so true. I will first let you know there are no doctors on site. And ONE Registered nurse for the entire facility. Which caused a huge issue for my mother's care. But first let me speak on the visitation "coordinators." My first 3 encounters were with a young lady with a heart tattoo by her eye. She was accommodating, courteous, polite and helpful. However another woman coordinator is rude, condescending and treats people like children. She lead me to my mother's room for a visit, then left. My mom said she had been cooped up and wanted to sit outside. So I turned to ask the woman if we could go outside as she was walking away. I called on her several times as she walked away without hearing me. So I went to the front desk to ask the guy there and he checked outside propped open the door and said yes we could sit outside. Midway through our visit the visitation coordinator comes outside with an attitude scolding us for sitting outside stating there was a reason that she left us in the room because the outside was reserved for a visit with unvaccinated kids. I was taken aback by her demeanor. We didn't just walk outside and sit down. We asked first. I let the other coordinator know before we left that I didn't appreciate her behavior and that we were told it was okay. And the reason we wanted to be outside is because even though every room has a sliding door to the back, we couldn't open it because they had mickey moused the ventilation for portable ac's. That by the way leaves every freaken door unlocked and unsecure. A safety hazard as you can see in the pictures. But here is the kicker going back to the RN. My mother's only reason for being there was that she needed antibiotics intravenously. Two days before my mother's release no one showed up to give her the IV. 8 hours later I am calling the facility asking wth is going on. Turns out the RN wasn't getting there until after 3pm. My mother missed her first two IV treatments in the morning and afternoon that day. It almost delayed her release. Then when my mother gets home and an RN home care nurse comes to visit here we find out that my mother's wound which was 6 inches deep was never properly cleaned. All they did was change the freaken bandage. Be very careful leaving any loved ones here. My mother was fully aware and able to talk to me and I called her everyday several times a day. Do not trust this facility on their own with your loved one. read more

My father was admitted a little over 2 weeks ago. He is now home, having dropped 5lbs in the 10 days he was there. We couldn't stand one more day there, so I took him home against medical advice. For starters, he was never given a shower for the 10 days he was there. We asked for extra portions at mealtime and never recd those. My father is a very thin man w/some severe medical issues. He does have a healthy appetite, however, but cannot afford to lose any weight! There was also an issue w/his call button. They never came to his assistance in a timely manner. The result? A BIG mess on the floor and all over him. How humiliating! He should have had extensive physical therapy from the beginning, yet they forbade him to get up on his own. The physical therapy consisted of exercises in his bed and if he was lucky, a few steps away from his bed and back. Then they would assist him back into the bed, once again. This man is used to being up and about, walking on his own with the assistance of his walker. He was lucky if they filled a very small glass of water to last him all day. What happened to the pitchers of ice water that were filled several times a day at most other places? They never changed his gown once a day. More like twice a week. Animals are treated better than the patients there. Those poor people who are destined to stay and live there on a permanent basis! The day he left, the charge nurse along w/several others in administration, harassed him for leaving to the point where he almost called the police! They finally brought him out to leave and didn't even give him a mask as they wheeled him through the hall and out the door, throwing his personal belongings every which way in his bags. They became very nasty w/vitriol spewing out of their mouths. ENOUGH! Close this hellhole down!!! read more

South Bay has nice employees but the level of care isn't good. They kept sending my brother to the ER. I think he went to the ER 3 times during the few weeks he was there. They wouldn't help him to the bathroom so he had to poo in his pants. I never heard from a doctor during his stay or had any communication unless I called and asked. My phone calls asking for information weren't always returned. I think the director of the facility could do a much better job of running the place. The employees were always nice and helpful but they lack guidance. At his last ER visit they found my brother had medicine is his system that no one told my they were giving him. South Bay had drugged him and then thought something was wrong with him because he was so sleepy. I was glad that he didn't have to go back there after that. read more

Heroes works in this place! Not only that they have compassionate and knowledgeable staff members but they also have a very good culture. No Nursing Home is perfect and I know this place is doing the best they can to provide the quality of care to their residents and good working environment for its staff. We have my great grandma there, and I can't ask for the better care and treatment for her. The times Ive show up unexpectedly, I see the staff tending to her and other clients needs. It gives me such great piece of mind to know that she is in good hands and most and foremost that she is being taken care of. The staff are always very reassuring and helpful. My uncle was staying at Southbay Post Acute nursing home in Chula Vista. Every day I would go take him food. Little did I know my uncle had been ill for a while and wasn't eating it. They never mentioned to me how ill he was. I had my aunt call to speak with him after my visit to the nursing home to find out what was going on and he sounded very sick and he told us he hadn't been eating for a few days and didn't think he was going to make it. He was later diagnosed with COVID after we insisted something was wrong with him. I don't understand how being professionals they weren't able to notice that he wasn't doing well or failed to let us know that he wasn't eating for a few days. We trusted in them. read more

My brother was here for after surgery care. I don't know how well the care is because of Covid. He had to be transferred to Scripps Mercy because his wounds got infected. They don't always answer the phone even in the office. Someone stole my brothers shoes and pants and he's missing other personal items. This is not a place I would recommend you bring your loved one to, especially during Covid when you can't check what's going on. read more

My tia was here for rehab and this place was the worst place you can put your family member. Started with the person in charge for intake or admission.She lied to us about the building and the accommodation then when we tried bringing up our concerns- the social worker was rude and unprofessional. Some nursing staff seem to lack some training too and it seemed that they don't know what they are doing, though some are trying their best and are accommodating. The rehab was ok but was a bit rough with my tia and food was hit and miss. Overall, it was not a pleasant stay and I wish we could've check other more places who's more professional and competent in giving quality of life to our elderly. read more

I am giving this place 1 star because I can't put zero stars. My grandfather was admitted here and after 10 minutes we took him back to the hospital. This place smells horrible, the staff is incompetent. My grandfather started to complain of chest pains and the staff just stood around looking at each other in a panic. He was supposed to have oxygen ready for him on arrival and they couldn't even find the tank. Or a thermometer to check his temperature If you are reading this review you are probably in a tough situation, please do not put your loved ones here. Please do not put your loved ones in this facility, in all honesty this place does not seem like a place you go to rehabilitate, but a place you go to die. I wanted to write this review about this place because things can be misleading and this place looks like they need a fair review. When I first saw this place I was concerned on how this place operated. The place looked a little run down but nothing alarming where it would not work. There are people that really know what they are doing and like all places there are people that do not know. We did have an issue where one of the night time nurses wanted to charge my family to transport my father to the hospital. I was upset about it but emotions aside I knew to escalate the issue if I needed more information and to get the facts. My father needed medically supervised transportation because he could not walk and he had staples in his stomach that could tear with movement. Long story short I contacted the case worked and she resolved the issue and got the transportation he needed. My father says he is very comfortable where he is at and is enjoying his stay. He stated that they are treating him very well. Which is what i want to hear from him. 3.5 stars for the structure and facility 3.5 stars for the people 4.5 stars for my dad's satisfaction 3.5 Overall, Yelp does not allow 3.5 This place is doing what needs to be done for my dad with very little drama. I felt the need to write a positive review for South Bay Post Acute Care Center. My sister was discharged yesterday, and she didn't want to leave! She was very comfortable there and they made her stay pleasant. Prior to going to South Bay, she was at a different facility and complained non stop about her treatment in general. When I had heard that she was being transferred to South Bay in Chula Vista, I went on line to read the reviews and was terrified! However, it was out of my control due to her Insurance. To sum up, the facility provided my sister will excellent care. I would like to thank everyone at South Bay for being so attentive to my sister's needs! read more

I can relate to Ginny M.'s review about the quality of service being offered to disable persons such as her brother and mine. My brother Miguel S. had a kidney transplant on 12/2019 our family was blessed to have had a donor for him to save his life. Unfortunately, my brother due to his condition and recovery was very sick and spend 21 days at sharp hospital where he was treated with the out most respect. He was released and transfer to this what they call themselves rehab center on 2/10/21. Because of his medical condition he requires a special diet which they cannot provide everyone there eats the same kinds of food regardless of their diagnosis. My brother has been left with his dirty diaper while having diarrhea for over three hours. His emergency or nurse switch not working and at night nobody checks up on you. He did not have oxygen over night for 2 hours very dangerous to someone who needs the oxygen to breath. After many attempts to have the necessary changes implemented to his needs no one from social workers, facility manager, and other staff he has given up and wants to be release from this nightmare of facility before he ends up dead. Last thing, he was supposed to be discharge since this past Saturday 2/27/21 but there was no oxygen tank order placed knowing his release date to be delivered at his home for his continued care. Today is Monday 3/1/2021 he was told they have him as a discharged patient therefor can't help him. He has been given the round around all morning needs to be told if order was approved and when can he leave. I've left a voice mail to a social worker no name provided by the receptionist to address our concerns. I will be seeking legal advice for justice. read more

If I could give this place zero stars I would but unfortunately I can't. These nurses have absolutely no idea of what they are doing my dad had just recently been transfered from the hospital to this place supposedly to get some therapy for his foot since one of his toes had just recently been amputated. The nurses had absolutely no idea of what they were doing causing my dads circulation in his toe to not flow properly if these nurses knew what they were doing my dad wouldn't have to be transfered to the hospital once again. My dad has already been submitted to a total of 3 operations in a short period of time now thanks to the fact that these so called "nurses" were not doing their job and not taking proper care of my dad his surgeon said that due to the fact that the circulation in his foot was not properly flowing he might be submitted to another surgery. Please hire people whom actually have an idea of what they are doing. I would never recommend this place to anyone. read more

This place has been a big disappointment for our family. My grandmother was in no condition to be discharged so we filed an appeal. The day they denied our appeal they discharged her immediately. Without time to find her an alternate home. Two nights before that she had trouble breathing. How can you discharge someone who had no place to go and had difficulty breathing.? We had been trying to talk to the case manager Joy who told us she would help us and keep her there. That was a lie. She was always hard to get ahold of always in a meeting or at another facility. They convinced my Auntie to pick up my grandma, but come to find out that we didn't have to leave if she didn't have a place. My grandmother ended up in a hotel without any medical supervision. Family was there but they had no medical training. Shes now in a home but it took a while to find one. I am so upset with this facility! Do not bring your loved ones here. Once the insurance is cut off they will kick you out without any effort to work things out with you. It would be negative stars if I could. read more
