STOLE MONEY FROM GRANDMA. Great care overall (decent food, regular showers, attentive behavior, clean facility) but some a*****e stole money from my grandmother's drawer. My mother ultimately decided not to press charges but I am livid about it. My grandmother is no longer there thankfully, but what about the other residents? They suck my Great grandpa's neighbor dedicated himself and was their for 2 hours no one came and ringed the call for my great grandfather for the bathroom didn't come for 20 minutes and the nurse station had a person their and ignored us the whole time and I told her he needed help and 10 minutes still didn't come and we had to help him and finally came 10 minutes too late and was rude with us and didn't take him to his exercises for 3 days. read more

I would not recommend this facility at all dirty very rude I can't imagine a senior citizen or somebody getting rehab that doesn't have their ability to really speak up for be happy here. I would not put my loved one in this facility or myself. read more

This place is a nightmare! Most of the staff is rude especially the older women at front desk ,most of the nurses were put out when asked for help. Save for a family of caregivers ( a nurse, her husband, also a nurse, and their son, a CNA) this place is an abject failure! The head administrator hides from the patients and their families. Our first meeting with her, she entered the room without announcing her name, title and walked in chewing gum. We expressed our concern about a nurse having left the connecting end to my father's feeding tube lying on the floor twice! The feeding tube connects to a port in his intestine! In addition, they're in violation of state protocols and mandates. Patients are supposed to be handed a bill of patients rights and given a form discussing the care objectives. A family member is a compliance officer for a major health insurance company and expressed to the administrator that they are in violation of state mandates. read more

Hard bed, water, ice and snacks were rarely put out for consumption and food-all 3 meals of the day were terrible!!!

ZERO STARS!!! Neglect! Their staff to patient ratio is horrible. An RN told me she had just started that morning and was given a 45 patient caseload. She said it was her first and LAST day. The RNs are not friendly. The business office is pushy and actually discharged one of my mom's roommate's to a hotel becuase her medicaid hadn't cleared and they were afraid they weren't going to be paid. One patient rolls around in her wheelchair at least 12 hours per day. They change her diaper about every 12 hours. She screams from the pain of the bedsores. Soooo many possessions disappeared from there. Clothing, food items, cigarettes, a homemade quilt. DON'T SEND A LOVED ONE HERE!!!!!! It's pretty on the outside. The inside reeks of bodily fluids and disenfectant. The residents are neglected. And NO ONE CARES. Horrible level of care. Only basic nursing care was provided. And always late, by staff who are always overworked and stressed. read more

I was there for 2 weeks last fall, and found my stay very beneficial with physical, mental, and hospitality. Room was spacious and they kept the bathroom and my room extremely clean, the bathroom was like a high end hotel. Would recommend this facility to anyone I know. Physical therapy helped me to begin to walk again and be independent enough to go home and care for myself and pets. read more

I was in the facility from April 13th 2026 to April 23rd 2026. The staff was very rude and unhelpful coming to my room without a mask being immune suppressed not cooperative for medications no care for the patient or their needs only the facilities needs and what they wanted recommend that nobody ever go to this facility. I was sent home early due to the lack and the responsibility of the Care staff Rachel the lady in charge will not get back to you they sent me home with handwritten prescriptions that needed to be electronically filled they did not feel them. Had to file a grievance with my medicare against the facility still 5 days later have not gotten one response from a medical nurse or doctor from that facility left me with broken ribs and contusions on my lower leg send me home with no pain medication. It's not the place for recovery or care is not the place for anybody to be rehabbed or taken care of the physical therapy group all they do is give you a hard time. read more

The food in this place is pure slop. It is so so clean. It jas a high turn over rate. There is zero team work. The activities director Lynda is not a very nice person. Most of the people that work here are nice. I needed help getting up from my bed & the CNA told me I could get up ny myself . The food also comes cold. When they post the food menu it's mostly under cooked rice & rubber chicken. I don't get my night time meds until 10 pm at night. They are so under staffed. I have witnessed so many acts of kindness on a daily basis. A truly beautiful and well maintained building is in sync with the extraordinary services They provide. So my mother is back again. In the beginning it was rough seemed the workers were not attentive and took their time but after I talked with them everything changed. My mother made friends and the workers are so nice to her. I had the chance to eat with my mom and must say the food is darn good. Had tomato basil and it was as good as what I get at restaurants. They do have issues with ants but is being addressed. I want to thank the staff for how kind they are to the patients here. I don't understand the people that say they has horrible experience as we did not have that. So maybe it is do to bring on short term side and not on long term read more

My father was sent to Spanish trails "rehabilitation" Suites after a 14-day hospital stay. He arrived late on Friday, January 3, 2025. The facility itself is beautiful. It looks like a resort, but the care that was provided to my father was less than subpar. Upon his intake, I was greeted by an aloof nurse. I don't recall her name, but she spoke with a heavy accent. Right in the middle of intake, she stops talking, looks out my dad's door at someone in the hall, and says, "Oh, Jesus." I asked if everything was OK, and she responded with,"A patient just died." First of all, I am most certain that was a HIPPA violation(which occurred throughout my dad's stay), and second, that was the last I thing needed to hear while leaving my father in their care. Things the charge nurse, director of nursing, nurses, and case manager allowed to happen on their watch. 1. My dad's antibiotic IV was not removed after the IV bag was empty and was let in overnight. A nurse from Ukraine told me and my dad that it was unacceptable and that she would discuss it with the night nurse. Mind you, when she came in, it was already 10:30 am. That means staff came in and out of his room and ignored his requests to have it removed. 2. Do not answer bed calls in a timely manner. We timed one call at almost 45 minutes. My aunt actually walked down the hall and made the nurse go to my dad's room with her. 3. Did not notify myself or any of the emergency contacts when my dad fell off the bed on January 9th. They claimed to have called me but did not have a message for me to return the call, nor a missed call on my phone. Luckily, our family was actively involved in his care, and it was mentioned to my husband. I was told the doctor performed a physical and neurological assessment and deemed him to be "fine." My father told me prior to this fall that he had fallen before. When i spoke to the Charge Nurse, Heather, she told me protocol is to call family and ensure he is not injured. When I found out he did fall, I expressed my concern since he had two broken ribs, a break on his pubic boneion intake, AND that he's on blood thinners. Her response was, "What do you want me to do about it?" I told her perhaps x-rays or an MRI. She said the doctor said he was fine. I really find it hard to believe that he was checked out by a doctor because on January 15, when I asked to speak to the doctor, it was approximately 2:30pm, the nurse Andrew (who was quite condescending and rude), said the doctor had already left for the day and wouldn't be back. I highly doubt that the doctor came in at 4:30 am, when my father was found on the floor. 3. My dad developed a large bruise on his forehead. Of course, Andrew couldn't tell us what happened to my dad. This is when I asked for him to call the doctor because I was concerned about my dad's affect; lethargic, slurring his words, and confused, in addition to rib pain; something he hadn't complained about until he fell at this hell hole. Andrew said the doctor ordered an x-ray, and it may or may not be done that day. At this point, I made it my goal to find a real rehab facility. 4 . After I left, my sister met my aunt there. I told her I spoke to another rehab facility and Adult ProtectiveServices, and I was advised that he should have been taken to the ER when he fell on January 9th . I told my sister to tell the director of nursing, Becca/ Rebecca or Josh, the case manager (apethetic), to call 911. Becca told my sister they could not call 911 because they would need the doctor's approval. **Keep in mind, the doctor was gone by 2:30 that afternoon and "wouldn't be back", per Andrew.** My sister said she would make the call, and this time, Becca said she couldn't do it either because she wasn't the POA. I've never heard anyone needing permission to call 911, so I made the call myself. Fire & Rescue collected my dad's records and transported him to Presbyterian Hospital. His BP was low due to them giving him a muscle relaxer, hydroxyzine, a sleep aid, AND melatonin;4 things he never takes! Keep your loved ones away from here! read more

I recently had a loved one in this facility for the better half of a year. They only had positive things to say about the staff and the care they received, and I'm beyond grateful they provided such consistent care for a beloved member of my family. Thank you guys! This was a new and scary process at least for me, and if you're reading this I imagine you're in a similar position, I truly stand by Spanish Trails and the quality of the work they do here! Do not put your loved ones in this facility. The nursing staff is not well trained and they do the absolute bare minimum. Expect to wait an hour after you press your call bell, expect the nurses to try to run out of the room like it is a race, expect that you will have to throw an absolute bitch fit to get the nurses to pay attention to serious medical problems, expect to get bed sores, expect cold unappetizing food, expect to be scolded if you ring the bell too often, expect the nurse to just give you a blank stare like she has no idea what you are saying. In short, don't expect any kind of nursing treatment. The therapy department is actually very good. They are kind and caring and work with the patient. But that can not overcome how bad the nursing department is. I'm honestly surprised that this place is allowed to stay open. My mother was in the facility for a week. Sad to say, even with the nice surroundings, my family and I took care of my mother, almost exclusively. Is this place an insurance scam? I get they were dealing with COVID staff issues but really?!? I would not recommend them for your loved one. read more

Therapy- under no circumstances take your loved ones to this facility in Albuquerque New Mexico. Your loved one will marinate in their own feces in urine and the therapy staff is worthless. All they do is sit around and watch TV or spend time on their cell phones while you're loved one does hardly anything. The nursing staff is a joke and the CNAs are hardly ever around... The hospitality is terrible. You hit the call button for anything and you have to wait for two hours. So as you're sitting there in agony and pain waiting for some pain medication you have to sit there and wait for two hours. The food is like someone popped it in a microwaved and drained of its juices and it's filled with nothing but salt. You're waiting to have a heart attack. I noticed that people come to visit they bring food for the people that are there. They even know that the food is horrific and they are bringing nutrition for their family members. I wouldn't recommend this place to my worst enemy. If I knew who to call I would have this place investigated for malpractice. read more

As a nurse I worked several days at the facility back in June . If you don't want to get paid then work here for free. They won't return my calls and haven't paid me for the time I worked here. If they won't pay a hard working nurse then you don't want to work here and definitely don't want your loved ones to be taken care of here. :( read more

This place is hot or cold. My mother is staying there, and the day crew, M-W is amazing. They take great care of my high-needs mother. However, some of the nights - and especially weekends, my mother is very neglected. One night she was not changed during the night from 6:30 pm until 8:30 the next morning - my mom, and the bedding were soaked. Her meals arrive with a meal ticket - when the meals are delivered to her room, there is always food missing from the tray that was listed on the ticket. This is a new facility, with beautiful areas to help patients heal, however beware of the staffing. Physical Therapy and Occupational Therapy were fine - no complaints there. The Director of Nursing spoke with me about the issues - he was concerned and helpful. There are definitely problems that need to be worked out. read more

First day mom arrived with a transfer van he was in the room waiting to get his wheel chair because he needed to pick up others. While the nurse was showing my mom how to use the tv remote. She was more interested in giving my mom a tour then getting her medical information while this worker waited for her to finish. Then the worker proceeded to ask my mom about her medical information while this guy was in the room. I had to explain to her that she was violating all sorts of hippa regulations and needed to move my mother to a chair and give this gentalmen back his wheel chair so he could leave and then she could resume the medical questions. Then over night the staff did not get her pain meds and had to yell for help because no one came when she beeped them. Very unprofessional and will move my mom out of this please asap. Do not place your loved ones here. My mom was only there thankfully for a couple of weeks. Upon asking for assistance in using the bathroom or even a bed pan, on several occasions was forgotten or waited long enough that she had soiled herself. (Piss and shit) The "tech" who half NOT wearing masks during COVID-19 wipes her from the back to the front. Even after asking to not do so. If I have to explain why this is wrong maybe this is the place for your loved one.. Another 5 star on neglecting care of your loved one here is the driver Jerome ... who took my mother to an appointment. After her getting out from the appointment she waited for 4/5 hours to be picked back up from this person who Spanish Trails still feels is a responsible driver. I can only imagine how many others had to go through this.. We were told he only had one l.. other person on his roster that day. Upon being picked up he made up his time by speeding, running over bumps and blasting his music. I'm sure that feels great from a double hip surgery, recovering from infection. Should you choose to put your loved one here and they die, because they have a great chance of it from this place, I hope you sue and the Spanish Trails gets shutdown to spare the lives of others. I'd like to end this on a good note by thanking the people who do actually care and have empathy of others but the only thing I'm thankful for is my mother is out of this shithole. read more

DO NOT LEAVE YOUR FAMILY MEMBERS HERE!!!!!They do not care about their clients/patients at all. Unfortunately this is the only place that had an available bed for a family member. They do not check on you when called even if it's to use the restroom, they apparently only let their clients shower TWICE a week which is completely unacceptable considering the fact that they let them sit in their OWN URINE AND FECES FOR ALMOST AN HOUR. We could hear patients yelling for the nurses and CNAs from other rooms on FaceTime. We dropped off lunch for them and the nurse who took it from us grabbed it and went to go sit back down instead of taking them their food right away. We will be removing them ASAP because they can't even help with basic human necessities that are supposed to be apart of the rehabilitation for them. As someone who is familiar with the medical field this place is a disgrace. It's sad to know that they only care about the money than they do patients because it seems that they try to prolong your stay by keeping you in bed with minimal therapy. They are supposed to get a full hour with the physical therapist but we're lucky if we even get 30 minutes. We tried calling Heather the therapist director who won't return any calls regarding the lack of therapy that's needed!!!! read more

It looks nice from the outside, and even when you walk inside. However, once the business office is involved, it all goes down the toilet. Our father passed away early April 2018. It is now the end of September 2018, we are due a refund, and Spanish Trails refuses to reimburse us for the over payment. I'm getting the Attorney General involved for fraud. read more

My father was released to Rehab Care and we chose this facility for its newness and cleanliness. We were dealing with the best option under insurance coverage. I recommend every family practice due diligence when selecting a care facility. This healthcare business first and foremost is a corporate business. There are wonderful nurses and then there are traveling/contract nurses who float and fill-in as needed. I commend the physical therapy staff who provided my father with caring and focused attention. There is one M.D. for all the Rehab patients and that physician is there only on certain days and is difficult to contact when needed so most of the care is placed in the hands of nurses and nurse techs. My father was blind, nearly deaf and recovering from a hip break. We had to put up signs noting his impairments just so that the attending staff understood his condition. During his stay a contagion became a concern and the ward went on a lock down. My father experienced diarrhea and was isolated. We watched as patients were moved along or out. We practiced clean room behavior but we were never informed about the cause of this outbreak. We did have two meetings prior to this incident with administration and lead nurses during my father's four week stay. I appreciated the openness and attempt to keep us informed however my family never felt a sense that our father's care was important. My father was rushed to the ER at Presbyterian one Sunday morning when the incoming RN found his vitals in poor condition. This lead nurse felt that my father had not been attended to during the night as simple attention to these vitals would have revealed his distress. My father was released from the ER to Hospice and died ten days later. My father was 91 years old so is it fair to say that the care at Spanish Trails hastened his death? I will conclude by saying that there was little sense of true medical care. This facility employs many nurse techs that are simply functionary not medical care providers. The doctors are part-time and covering too many charts, so lack of attention is just a fact. Do your due diligence and watch over your loved one and hope for the best. read more

My grandmother is a resident and the care is nothing special. I was upset with how the business office tried to get more money out of my grandfather even though my grandmother already gives them all of her money. They already charge almost 2,000 in addition. and then without even doing their homework, said she was making more money and started billing her even more!! Her income had gone up less than $15 and somehow it validated them taking hundreds more. They can barely pay as it is ...Its just been a fight. Not something that my elderly grandparents need to deal with. read more
