During this time, HFM has managed most of my FMLA and disability paperwork. My condition worsened and it got harder to walk. I ended up injuring myself and went to my most recent appointment on crutches. For reasons I will never understand, HFM decided to drop the case at this point and stopped returning my phone calls. Without notifying me, they faxed the paperwork my employer completely blank, with a note saying that they couldn't find a medical reason for my disability, and that my employer would have to make do with whatever they had on file already. I found out about this 3 days after the deadline has passed. Fortunately, my caseworker is being patient with me, and she automatically extended the deadline for another seven days. But until I find a real doctor, I'll have no income, no treatment, and no information about my long-term outlook. It's getting harder and harder to do simple tasks, and I don't really know what comes next.

The nurse called me back about10 minutes later and performed the normal procedures, weighing, height, blood oxygen levels, etc... Dr. Wang came in about 5 minutes later. He is a friendly man and wants to hear about your health, history and lifestyle. I never felt rushed and he was easy to talk to. He explained his methods of care, his rating on what you are doing now, and any changes of action that is easy to understand. He said he wanted lab work and it could be performed on site! Wow! No fasting, no going to some lab, no waiting! The whole appointment was just over an hour and I'm done! The next day I received an email with my lab results and what he want to tweak! That was so quick! I am very impressed with the entire practice from the ample parking, and friendly staff, to the attentive nurses and doctors to the painless quick lab facilities. The dispute was not with Vicki Cooke my former NP, she had never talked to me about it, she was out of town during this time.... I guess it's common practice now to have no say so about your care. Regardless, you discharged me as a patient, fine, I will go someplace else... My complaint, you sent out a certified letter in January, or you say you did, you wouldn't show me the receipts. I never received any letter, why would I have shown up for an appnt if I had received it. I had an appnt scheduled on 6/14 since my last appnt in December of 2017. I received a call to confirm my appointment, I asked to change the appnt to today's date. Yesterday I received a call to confirm my appointment. I came in for my appnt, I was asked to update paper work and then taken back to a room.... It was then I was finally approached about the letter that was "sent" out in January. Vicki Cooke would not even come and talk to me. I was humiliated and all of this was over a dispute with a very rude staff member about antibiotics for a sinus infection. Today I came in with a black eye and a hurt shoulder from a fall yesterday, I thought I was going to have it looked at. . I guess I need to find a new doctor to take a look at it.... My advice to the staff there, if you discharge someone as a patient make sure your staff and the patient knows about it. And to Vicki Cooke, I suggest you take the time to talk to your patients before you make rash, uninformed decisions. I'm truly baffled at this, all over antibiotics for a sinus infection.

Billing and scheduling are terrible. They won't tell me if I have a bill due until it is past due…read more I even call them up to ask about the bill and they act annoyed and tell me it is process through insurance. Then I get texts and emails a month later saying it is past due. Scheduling people are rude and they do a terrible job. If they don't like you they will pretend the only available date is a month and a half out and that there is nothing else available because they aren't required to do their due diligence.
