I came here with a lot of medical needs including a feeding tube and pump, 24x7 oxygen concentrator and a lung suction device. Not a small feat to support me all day and night but The Pearl was up to the task. I've made friends with many of the staff and I look forward to returning to do some volunteer work if possible. I totally recommend The Pearl if you are in need of rehabilitation services like I was.

The Pearl provided excellent care for my husband after an extreme spinal surgery. He ended up with…read morean infection at the hospital requiring emergency abdominal surgeries. After all of the surgeries my husband spent a month in the hospital and he was kind of at square one when discharged. When he started at the Pearl he was unable to stand. After his month long stay, he was in good enough shape to slide transfer from a wheelchair to his bed or shower/commode. He was also able to stand for brief periods of time. He progressed enough to join the intensive Rio Rehabilitation Program at Legacy Health. During his stay he received top notch care from Rio staff. They were very caring and met him where he was physically. The therapists pushed him to get where he needed for additional rehabilitation. Thank you Colin and Debra! The nursing staff were caring and as timely as they could be and several nurses were standouts (like Carol and Sherry). The weekends seemed to have less staff so things could take longer. That said, they were upfront about other work they needed to do before being able to help. And when they were helping, they took all the time and care that was needed. The food was very good with several alternatives if the main menu didn't sound good. Bambi was great in helping set up wheelchair transport for appointments, Sondra helped coordinate lots of appointments and helped make Rio happen. Tawnya was amazing at coordinating transition to Rio and backup plans. Penny was the nicest therapy pooch. She lifts everyone's spirits. I highly recommend the Pearl as a skilled nursing facility. I was in Porthaven short term to recover from an injury. The physical therapists and other…read moreprofessionals in the gym are fabulous. Very competent, and I felt like I was "on a team with them" for my on care, which is what the medical profession expects between patients and providers. These therapists' competence and care are the only reason I gave this facility three stars, instead of only one star. Porthaven is described as a skilled nursing facility. But it not that, as the nursing staff is short-staffed. Skilled nursing facilities are described as being for short term care, where patients are given therapy and expected to go home soon. But it seemed to me like there are people who actually live there.....the rooms of these people had some personal furniture, and personal pictures hung non their walls. One woman told me she had been there for four years. And as a whole I did not feel like there was teamwork between myself and many others there -- as a whole I felt like a lost my identity as a competent adult, and decisions were made FOR me and ABOUT me, and not in a timely manner, without my knowledge or consent. Fortunately, I have a health advocate, and she worked to make sure I was getting what I needed. CNA staff to patient staffing ratios, I was told by one of the CNAs at Porthaven, are determined Oregon State Board of Nursing. She agreed with me that Porthaven is understaffed. I subsequently researched this ratio and discovered that the state regulations for the bare minimum staffing for safety, and that facilities are able to hire more staff. The only reason I agreed to stay at Porthaven initially is because they allowed me to have my own room, without a roommate. Initially, I had a roommate, who left her TV on day and night. This is totally unacceptable for what should be healing environment, and I told Porthaven this, that I refused to admit myself, but then they assigned me to a private room at my request.

My mother was admitted and on her first night, she called for assistance to use the restroom--no one came. She fell, fracturing her sternum and three ribs, and the facility waited eight hours before calling an ambulance. She is now crammed into an 8x6 ft partitioned space with two other patients, separated only by curtains. In the weeks she's been there, I only received a single call from a doctor about her care, this was only after I threatened to report them. I had left multiple messages and even emailed the administrator and head of nursing--no one responded. This facility is dangerously understaffed, unresponsive, and negligent. I wouldn't wish this level of "care" on anyone. Avoid at all costs.

My husband just spent 29 days at OHSU hospital after a bad fall with a brain bleed, about half the…read moretime in ICUs. He was transferred to Hope Village Post Acute Care in Canby, OR today. By the time I got there he was laying unattended in a bed he barely fit in without rails. He is legally bling without contacts and is 85. Just beginning to get his strength back. I was alarmed and went to talk to his nurse who was too busy until about 6pm to do a routine assessment. I talked to the Director of nursing who said side rails are considered restraints so Marquis doesn't use them unless you asked. I asked and they sent someone in with some add on bars which didn't fit on his bed. I had to barricade him in his bed so he couldn't get out with a bed table and some chairs. I had to stay over night sitting up in a chair so he wouldn't try to get up and fall because he can't see, sometimes has dizziness when first getting up and has peripheral neuropathy so he can't feel where his feet are. This was his first night in an unfamiliar place and not even a safe bed. They de tell me they could some pads on the floor in case he fell; not a comforting thought asI thought fall prevention was the goal. I don't drive after dark and was told the nurses check every 2 hours during the night. Not safe for an older person weak from a month in the hospital recovering from a severe brain bleed. I was shocked that they accepted him knowing he was an extreme fall risk and knowing they didn't have a safe bed to put him in. I believed the 5 star rating. I would give it a one because the nurses aid and the night nurse were genuinely concerned and nice. This is a shockingly dangerous position to put someone in. I was an RN for 30 years and just couldn't believe they were so reckless and callous to accept him knowing his condition. Thank God I can sit in this chair all night until I get him out of here!
