The RNs are in touch with the patient's needs and contact the proper medical providers when needed. Unfortunately there is significant delay in initiating proper treatment, such as antibiotics, etc. In addition, treatment managers to not contact family members routinely nor do they give details of care, planning, testing, etc. prior to the group sit down. In some cases this can be a real challenge for the patient, who may need better communication due to hearing, visual limits etc. After release, we inquired about some dates of care via telephone x 3 to the care specialist, with No response. While at Village Rehab, my husband complained about the food, did not eat, and lost weight. So I tasted the "soup"....he was right. After returning home he has regained some weight and pursues regular meals, so it was not his illness.

Reviews here are consistent with our experience!! Unprofessional, poor communication never listen…read morenor listen to understand or mindful. Transactional, stories/excuses not accountable nor integrity based with no regard for well being or psychological aspects of the patient. A couple caring, but overall rudeness and bullies that neglect via ignoring, minimum 30 minute to 45 response times would call me crying. No dignity or reverence provided diminishing and falsely profiling patients/family. Despite concerns for overmedication potentially harming cognitive and kidneys, causing weakness/dizziness,, added more including narcan and 3000 mg daily, ordered equipment delivered to room at 9pm after medicated asking 95 yr old with cognitive to sign. Use upholstered chair for storage and closets wadded up clothing saturated in urine 2 weeks because only PT can take them to potty necessitating urinating in bed with long wait times causing bed sores to never heal. Got backache and developed congestive cough while PT on week vacation without substitute felt neglected. Don't address inflammation through basic icing even after provided cryo and voltarin ourselves had to remind, not following surgeons orders, bypass primary care/cariologist specialists, not getting patients out of bed through strength building early on prolonging rehabilitation then blaming patient. Their ratings are from Medicare whom give frequent cutoff notices that add to stress loads of families. Only one shower/shampoo in 2 months and no modesty draping with two men changing mother. HVAC inadequate and rooms uncomfortably hot even for visits, miss important doctors appts, food not heart healthy nutritious, and poor quality causing 25lb wt loss to 124lbs, Belongings, Flowers Food out of view/reach,. one actual "care" meeting in over 2 months stay (used for attempt signing off rights hearing aids later lost one between facility/hospital refused 1/2 cost i/e 150 for 7g aids) or 2nd medicare notice only limited 20min vs. care, poor record keeping and never provided when requested. I could go on, but you get the picture. sigh. Care in this country is a broken bureaucracy.

I was a resident/patient at Hope Village for about a year and a half and just moved out in June…read more2026. There are many worse facilities. This establishment is clean and the CNA staff is good-but there are not enough of them-ever. A few of the nurses are excellent-many are half baked and not on the ball. There is a TV connected to each bed, however each TV is tuned to a different station-too loud. There is a physical and occupational therapy gym but the PT staff is not the best team. If you need physical therapy, I suggest you make an appointment with ProActice Physical Therapy found about 12 blocks down Ivy Street. ProActive is the A team. Food at Hope Village is basic institutional fare-think what you got in the high school cafeteria. They do make a great carrot soup. Ask for it. Rooms are way crowded with often three beds per small room. So conflicts with room mates are frequent and not resolved well. The facility is so crowded that if there was a fire, earthquake or flood- the residents could not be evacuated fast enough. If I were the Canby fire chief, I would demand fewer patients per room. Hope Village says they conduct fire drills- they are Micky Mouse fire drills. Ask the management when they had a real fire drill, where everyone was removed to a safe place. Sign up for CAT (Canby Area Transit) bus services-excellent little wheelchair transport provider that costs a buck a ride and well worth it. There is a limit to how far CAT will take you outside of Canby. Canby has a good little library and small independent bookstore, The Book Nook- both worth visiting. The CNA staff are good but frequently there are temporary staff. The MD in charge of Hope Village has two other large facilities he oversees. You can depend that he treats the management of Marquis, not you. Keep you own doctor and Hope Village will often follow your MD's directions. Sometimes not. I had to use Providence/Urgent care twice-once for a bladder infection. There are many adult care facilities far worse but that's not saying much. The upper management of Marquis is unaccountable and does not make patient care a priority. They are out for your money. Family and friends should monitor and supervise and your loved one will get better care. Best wishes.

While a few individuals on the care team truly made a difference--Damalesh, Shantay, Louie, and a handful of others were kind, attentive, and absolutely wonderful--the overall experience left me shocked and disappointed. The social services director, Sam, is the main reason this facility gets a 1-star review. He was dismissive, condescending, and ultimately responsible for the stressful, manipulative mess that surrounded my attempt to leave--saying that I was "denying care" when in reality I was taking my "care" elsewhere... Had to fight to get basic discharge paperwork (that I asked a half dozen times to get for hours on end), was pressured to sign documents without being given full information, and felt completely stonewalled when I asked for reasonable medical equipment and referrals for in-home rehab. Sam made what should have been a smooth and supportive process incredibly frustrating. As for the food? I'm vegan, and while they made an attempt to accommodate that, I can't live off bread and tofu forever. Toward the end, I could barely eat anything at all--it got that bad I understand care facilities aren't luxury resorts, but respect, communication, and basic follow-through shouldn't be optional. If it weren't for the handful of standout staff members I mentioned earlier, I would've lost my mind here. If you have a choice--keep looking. You deserve better.

I was told that if I didn't take a shower at that moment, I would not be able to take one the next day. There were enough female staff on duty that one of them could have come to the shower instead. But yet, being humiliated. I still have trauma from that night. Only one staff member approached me about what happened. I was not apologized to by Hussain nor by anyone else. I felt absolutely outraged and degraded. Yet, no one seemed to care. The only reason I was here was to rehab from cancer treatment It was much more banal at best. A complete lack of communication between staff members. Often told "oh, I forgot, or no one told me," which is absurd when dealing with people you are there to care for. On more than one occasion, I was brought the wrong tray of food while the staff jokingly said, "oh, it happens all the time", like that's ok. Another issue.. having to put a sign on my door asking people to knock before just walking into my room. It is a common courtesy to knock on someone's door before walking in... it does not matter where you are. Yet, even with the sign, some just walked in without knocking. As far as rehab.. I did better PT in my room. A couple of old bikes and a few weights. It was a waste of my time and dignity. I stayed there two weeks too long. However, that being said.. some of the nicest people you ever wanna meet. All seem pretty stressed most of the time, definitely overworked, probably underpaid. From my personal observation along with asking the R.N's CNA's, LPN's, the rehab staff and others. Each caring for patients with different needs and specifics. The length of time spent that particular resident varies, leaving others under their care basically screwed until they can get to you personally. That's why the one star, because they do try. But ,things definitely need changing as far as respect, patient care, effective communication, the fundamentals and most valuable not treating your patient as you did your last. Each patient are different in their own right, they should be respected as so. As an individual, which we are all and not just another bed filled.

My Mother, a 91-year-old retired nurse from the VA Hospital, was in need at this stage of her life for adult home care and we were fortunate to find ACMT. I really didn't know what to expect and they were the first place we visited. We had another appointment at another facility, but after our visit, we didn't have the desire or need to go anywhere else. Our gut reaction when we walked in said it all. The owner, D'Andrea, also a nurse, has a beautiful, immaculate and spacious home and the ambiance is very comforting, as was D'Andrea and her staff. Two weeks later, zero regrets and only positive things to say about my Mother's time here. They are very professional, patient and most importantly, VERY, kind. My Mother and I feel the love and good energy and are beyond satisfied in knowing that we made the right decision. D'Andrea is a fabulous cook and the meals are generous, wholesome and homemade. They take my Mother, who is in a wheelchair, for walks, play cards with her and make her feel like the special person she is. I never thought that this stage of life would come where I could no longer take care of my sweet Mom and was quite worried about the whole process. My Mother loves D'Andrea and her CNA's and they feel like family to both of us. Age Right Services visited today for an evaluation of my Mom's health and commented on the excellent care, great staff and welcoming home, while sadly stating that it is not always the case. They said we did great in finding this place and we know it! We feel very blessed with the knowledge that Mom is where she belongs.

My husband just spent 29 days at OHSU hospital after a bad fall with a brain bleed, about half the…read moretime in ICUs. He was transferred to Hope Village Post Acute Care in Canby, OR today. By the time I got there he was laying unattended in a bed he barely fit in without rails. He is legally bling without contacts and is 85. Just beginning to get his strength back. I was alarmed and went to talk to his nurse who was too busy until about 6pm to do a routine assessment. I talked to the Director of nursing who said side rails are considered restraints so Marquis doesn't use them unless you asked. I asked and they sent someone in with some add on bars which didn't fit on his bed. I had to barricade him in his bed so he couldn't get out with a bed table and some chairs. I had to stay over night sitting up in a chair so he wouldn't try to get up and fall because he can't see, sometimes has dizziness when first getting up and has peripheral neuropathy so he can't feel where his feet are. This was his first night in an unfamiliar place and not even a safe bed. They de tell me they could some pads on the floor in case he fell; not a comforting thought asI thought fall prevention was the goal. I don't drive after dark and was told the nurses check every 2 hours during the night. Not safe for an older person weak from a month in the hospital recovering from a severe brain bleed. I was shocked that they accepted him knowing he was an extreme fall risk and knowing they didn't have a safe bed to put him in. I believed the 5 star rating. I would give it a one because the nurses aid and the night nurse were genuinely concerned and nice. This is a shockingly dangerous position to put someone in. I was an RN for 30 years and just couldn't believe they were so reckless and callous to accept him knowing his condition. Thank God I can sit in this chair all night until I get him out of here!

The night med tech refused to give him his morphine despite her admitting the dose was authorized in his chart, and despite hospice nurses coming in the middle of the night and again in the morning to clarify to her and calling the med tech's supervisor to get phone authorization. He never asked for things. He never wanted to be a bother, and he was begging me all night for pain relief. 2. They knew he had died but did not warn me. I arrived in the morning to visit him. I passed by the reception area where they greeted me but let me go into the room unprepared to find him dead. I was in there alone, checking his pulse and putting a mirror to his lips to check for breath. Eventually someone from the office came into his room to say sorry he had passed.

She definitely took control of the situation and made sure I was very comfortable after that. The nurses in charge also stepped up and took care of me and my needs. I would recommend Marquis in Wilsonville yes.
