I remove my brother for absolutely the worst of the worst rehab center .I ever seeing it's call Park Manors of the woodland .they were absolutely the worst.i even will give them mines 0.you can read my comments on there website. I brought My brother to Broadmoor at creekside he was there for almost 2 months. It was the The Best service The Best staff The Best nurses and extremely clean. And The doctor was great. He come every morning and come at night. The room was fabulous.even I have chairs to lay down on . Where the other one we had to borrow a chair to sit on. The cleaning person They come and clean . The Rehab when he didn't feel good, they help him exercise on the bed. Just walk bit in the Room He was comfortable .that is No Words can explain to y a'll how great it was.the staff they work with me . they make sure he was fed and for the visitor we had a great place where you can have a cup of coffee .you cannot even compare between this and Park manor you cannot.. I wannaThanks specially the nurses aid for dressing my brother and make him look so good and clean. All together it's the best facility five star is not enough I will give them 10+++++ read more

While our mother was under the care of this facility, we identified a serious environmental safety concern involving the presence of mothballs that were being brought in, crushed, and dispersed in resident rooms--reportedly by a staff member. This practice is dangerous for use in any type of facility and presents a significant health risk. During this period, our mother developed acute symptoms including leg weakness, leg quivering, and a pronounced unilateral lean, which were observed by staff without appropriate intervention or escalation. She later confirmed she had personally witnessed a staff member crushing the substance in her room. Given the severity of her condition and the lack of timely response, she required emergency medical evaluation. This raises serious concerns regarding supervision, safety protocols, and duty of care. Families are strongly encouraged to ask direct questions about environmental controls and oversight. If you want someone to feel neglected, mistreated or uncared for, THIS IS THE PLACE FOR YOU! If you actually love someone, this is not the facility for them. My mom went to the hospital with back pain, and without warning, her condition required hospice care. Due to being in the hospital for 5 days, bed ridden, she opted to come to this facility for "rehab" to gain strength. There were a million problems upon first entering, 1) she can't move and there was no remote control to her tv, 2) her bedding was tangled under the fitted sheets, which no one came to check on in the 2-3 hours we were there. 3) the service is Terrible!!!!!!!!!! 4) there were people that looked totally miserable in wheelchairs by nursing stations that were ignored, no engagement at all for at least 3 hours. They stayed in the same place. No "let me take you on a stroll" They were totally ignored sitting in the hallway with miserable, hopeless stares on their faces. 5)My sister and I were looking for water to fill my mom's cup provided, expecting some kind of filtered water somewhere. Upon asking, the staff looked at us like we were crazy and told us to use the sink. Unbeknownst to us, we think there's a sink in the unit of filtered water, and then they rudely say "her bathroom sink." WHY DONT WE JUST FILL HER CUP UP WITH TOILET WATER?!?! This is the woodlands. We have standards. This facility should meet those standards being that they are located in an affluent area. It gets worse from there. As my sister and I are leaving, we want to make my mom comfortable. Because she is so weak she can't move. Can't lift herself out of bed. Can't walk. We ask if the lighting is okay. There are lamps in the room, none of them work. We were worried about how she was going to turn off the lights and the tv that she couldn't turn off, or change the channel without a remote, and we found tangled underneath her bed a device with a red button. We asked my mom to press the button. I saw the light on the wall light up, indicating "call placed", and we timed it to ensure someone would respond. 20 minutes passed, no one came. We went into the hallway, and ran into Elsa, the person on duty for my moms hall that night. I asked her how the red button worked (no one even explained this to us upon my mom's arrival, among other things), Elsa tells me it lights up outside the room. She points to the door lights that were lit on her hall. There were at least 7, one of them being my mom's, and she hadn't attended to any of them in at least 20 minutes. If that's how they treat your family member, your mother, when you are there, I can't even fathom how horrible and awful and despicable it is when you aren't there. It was hard to leave my mom there. She had finally had enough when no one came to answer her calls And she heard the nurses laughing and carrying on while she was soaked in her own poop. She finally called my sister, myself, and my dad to get her out in less than 6 hours. If you want someone to spend their last days in total hell, this is the right place for you. If you don't, I beg you to choose any other option available. read more

I wrote an extensive review on Google about the extremely poor service by nursing staff and nursing aides at Broadmoor . The only continual positive experiences I had there was from the Physical And Occupational Therapy group and housekeeping staff Both PT and Occupational Therapists worked together as a team and continually offered to assist the other therapists. In addition both therapy groups encouraged and applauded all of us on our accomplishments. The housekeeping staff was so kind and helpful and they offered to assist each other. The PT / Occupational Therapists and Housekeeping staffs are both cohesive groups and work well as teams. I only wish that the entire nursing staff and their aides were a cohesive team, for their guests / patients. Four different aides came into provide patient care for me but they were on their personal cell phones the whole time. That could have been a violation of HIPPA or very close to it. One aide came in around 11 pm at night and was upset because she had to come in and clean me up. That aide drug a wheelchair over to my bed and said "Mr Woods here is your wheelchair that way you can get up and go to the bathroom yourself". I was shocked and upset and said "I can't get out of the bed or walk without help." Her reply was "I only work part time and PT didn't pass that onto me." Obviously there needed to be procedures in place where aides and nurses are at least basically aware of the status or condition of a guest / patient. Or how about just instituting a communication or pass on between shifts. One last issue a counselor/ therapist came in and asked if she could ask me a few questions and I said yes. About 10 minutes into the interview and she said "Oh you are not a woman by the same last name are you?" I said "no I have not had a gender reassignment surgery ." She left my room and found the correct guest / patient. The counselor came back in and said that "I have been making mistakes like that a lot lately". She left and then came back a few days later and said "can we finish the rest of the interview" I said "no and please leave you already discharged me." Now I could go on about other issues but I won't. Now Amanda the Director hired 2 new employees "T" and Gemayel they are in charge of the nursing staff including the aides. They made some positive changes but they have a long road to go yet. I would highly recommend before you admit a relative or friend to any "skilled" nursing facility conduct your own research on any and all facilities that you might consider. My mom is currently staying there for physical therapy due to a dialysis issue. And the staff talks loudly about who has to help her go for a walk or to the restroom like oh I don't want to, she's gross and all this other crap. Don't EVER come here No stars if I could, forget this place read more

Dirtiest place ever. Only good thing is the physical therapy. Looked great on tour but absolutely horrible where they put you. Nobody can eat the food. same thing served for breakfast, lunch and dinner. Never cleaned the room and took 2 days before they filled hand soap when we got there. At least 20 minutes before they responded to nurse button and they were super slow to help then too. No liners in trash cans and there's poop in there. Never touch anything around there because it's never been cleaned! We left as soon as we could and that took 5 days. Nothing is done on weekends so I just took my dad home. Social worker said she called insurance to set up home health but insurance verified that she never called. I lost 3 days waiting on her. Absolutely horrific experience! read more

The most horrible place I've ever seen!! At first glance this place looks like a nice hotel. The first night after my 75 yr old mother's hip surgery they said they found her on the floor at 2am! No bed alarms? Well, after the ok from the emergency room my mom went back. My mother has been sitting in her feces and it looks like it's been days! Called the nurse when I got here and it's been 30 mins!! My mom has a huge sore on her hand and her hand looks like it's turning black! Bloody face and ear, sores, ect. She's saying no one here likes her which makes sense because everyone has a sour look on their face like they don't want to be at work! She looks like she's dying. The smell, the absent faculty! What if I wasn't here, would anyone ever come? Well, someone came and guess what? They are mad, attitude! This place is HORRIBLE!! Do not bring your precious parents to this AWEFUL HORRIBLE PLACE!!! My wife is a long term care patient at this facility and recently I sent her two boxes through the usps which I have confimations that they were delivered on 8 may2023 but she has never received them. I sent a message to the business office with the confimations that they were delivered to staff at this facility, but she has not received them. I called to speak with the mailroom supervisor and was place on hold for over thirty minutes and finally was diconnected. It seems that someone in this facility has nothing better than to steal patients packages. The business office has yet to let me know what has happened to those post office packages I sent so now I have to spend time with the usps postal inspector and file a criminal complaint. Katia, I am Kathy Winn, Charlie's Winn wife. He was transferred to The Broadmoor on Thursday night. You had told me he needed to go to a highly skilled nursing facility and I can promise you The Broadmoor does not meet those standards with the nursing department! The PT is great and they do everything above and beyond! The nursing staff is almost non-essential! They may take over an hour to get to the room, if they even come to the room! His diaper on Friday was dried up poop! Which was disgusting and there were no wet wipes so I could clean him up like I did at the hospital after he set on the potty chair! I called the nurse and she finally showed up and cleaned him but that even took a while for her to come back! Charlie's daughter was there yesterday and tried to get a nurse to come to his room after using the buzzer and she went to the nurses stations 2 times in an hour to ask them to come for help! Today I was there and Charlie's catheter bag had not been emptied since Friday and if I had not been there today, they would not have checked it! Last night the nurse got mad at Charlie and left him cross ways in the bed and he was miserable but no one would answer the buzzer to help him get comfortable in the bed! Yes he is frustrated and yells but I have been there 2 days and a lot of patients are yelling because they are not getting proper care! No one checked on his diaper today and as I said his catheter bag was full! But the nurse was quite concerned when the door was open and I was not wearing a mask and closed the door but NEVER came back to check on Charlie! Charlie told me he had better treatment at The Medical Resort than he is receiving at The Broadmoor which some how I believe! We were just frustrated that no one would talk to us! I assured him that at least we can see him and he smiled. Katia I really wish the transfer for Charlie would have been to Encompass at the Vintage at least they take care of their patients! My friend's husband that had a stroke in November was discharged in December and had excellent care from both the nurses and PT! Katia you told me last week you had the same problem with The Medical Resort that they would not answer their calls. I know you cannot do anything for Charlie right now because he has been discharged from the hospital but truly I wish your team could go visit these facilities that you are sending patients to instead of relying on the internet or whatever source you do, please visit these places you are sending especially older individuals to the more respected HIGHLY "Skilled Nursing Facilities"! You will learn a lot just walking into the facility! read more

I am presently at Broadmoor getting physical therapy for a broken pelvis. I want to give honor where it is due to Terri Murphy an outstanding nurse who usually works night shift. Her constant excellent care and cheerful attitude no matter what are highly commendable. This goes a long way to overcome any other shortcomings one could complain about. Also the PT dept. is stellar. Experienced, attentive, funny, giving you constant encouragement and encouraging you daily to move forward. Brian, Dylan, Evan, Frances, and Jeff. Great team. read more

We almost didn't send my Dad here based on the reviews. Fortunately, we toured and shared our concerns with the the staff and moved forward. Dad was not happy about going to a facility period. Now, he doesn't want to go home. Jeff, Brian and Dylan with rehab are amazing. They had him looking forward to his twice daily rehab. He is up and walking with no help. The support staff was great as well. Christina was with him most days and wonderful. There are many more behind the scenes that made this a positive and successful stay. Thank you! read more

The facility itself is probably the nicest in The Woodlands area and they say all the right things when you tour regarding the facility and services provided and they addressed all the negative past reviews by saying new staffing have been hired for key medical positions but those jobs are still posted on their website and staff have told us they did not have new people in key positions. Unless you can be there everyday to supplement the nursing care due to staffing don't place your loved one here. The communication with medical staff is awful including questions regarding medications not being given and what exactly IS being given. You rarely get a direct answer. We did like the Doctor but he visits only 3x week and says he will follow up with nursing staff. The physical therapy department has great therapists which is the most important thing for my 83 year old dad recovering from broken hip. The only phone number published anywhere is the receptionist desk which is supposed to be staffed until 10 p.m. They have to transfer you to nurses station and the phone is NEVER answered ( like the call buttons from patients! ) I requested direct number to nurses station and was told it had to go thru receptionist though they advertise you can reach someone 24 hrs a day. The 24 hr access to facility you are supposed to have from front door is not true. Family member rang doorbell where camera was at front door for 40 min before going around the building to all entrances in pouring rain until he got someone's attention when my father was brought by ambulance at 9:00 pm. There are some very good staff but they can't do it all. We were promised "Circle of Excellence" meetings with care team but had one meeting only which primarily covered financial obligations. read more

If I could give this facility zero stars I would. It appeared to be a decent place from my walk thru of the facility. Nicely appointed, too bad the care sucks. My father was sent there initially to recuperate from a (second) brief stay at Mem. Herm. Woodlands for very swollen legs, a UTI and severe lower back pain being surgery was not required & because they are an "IV med friendly facility." Upon check in, the head nurse who called himself "the General", stated that he would see my father every morning/evening to assess daily care/improvement. That did not happen. I also had to bring some of Dads meds from home because they did not have them (Oxycodone, metalazone and a few others.) Dad was on a strict pain management regimen and required his pain meds every four hours - Doctors orders. We got him settled and dosed by the time they served dinner. His next dose was due at 8pm. By 10pm he called me crying in agony. No one gave him his meds yet and the nurse kept saying she was waiting for them. I called the facility and no one picked up at the desk. I called the General on his cell phone at home. He said he would take care of it. An hour later, my Dad calls me back and he is frightened. I can hear someone ranting and raving loudly in the background. When I asked what that was, he said it was the nurse and she was highly agitated, yelling/pacing up and down in front of his room, threateningly. I could hear her yelling "I don' have to take this sh*t from these as*h*l*s" and a quite a few other choice words I won't won't repeat. I told Dad not to worry and called the General back again. I told him I am minutes away and he better pull that nurse from the floor because I will not tolerate my 83 yr old father being threatened by someone who was spoken to for not doing their job. He said he would go back and take care of it. My father FINALLY got his meds near midnight. 4 hrs beyond the RX. Of course they did almost nothing for the pain at this point and he never saw the General that night. The next 24 hours were a nightmare for him due to extreme pain and lapses in dosing times. The following day I called the staff physician to complain. He said he would make it clear to the staff that he is a pain mgmt patient and meds MUST be given on time. Dad spent 11 days there and the meds were never given on time nor did his condition improve. It was a constant battle. Most of the staff could care less. In that time, his extremely swollen legs never went down and they were turning dark red from below the knee all the way to the toes. They looked horrible!! His pain had worsened in his spine. Finally, the Dr returned him to Mem Herm by ambulance. He still had the UTI, diagnosed his legs with cellulitis (turned out to be wrong later) and now he had a bed sore to deal with. Still, (for a 2nd time, no one could figure out why he was in so much lower spine pain. TWO MRI's (one each time)They just kept dosing him with heavy pain killers.) The hospital kept him TWO days and against the pleading from myself AND the nurses caring for him, they sent him back to Broodmoor at midnight on May 3rd . (The nurse was crying along with me as we both felt he needed more hospital care. She even called the hospital Dr at home to try and give him another day or two. He refused.) Sadly, the same treatment occurred as before at Broodmoor. After 48 hours of excruciating pain, my Dad tried to end his life using the blinds around 2AM on May 5th. In his goodbye note he wrote: "I hope you will forgive me. I cannot bear the pain anymore. No one seems to have any solutions for me. I hope the Lord will forgive my soul." Broodmoor waited several HOURS to notify me. I was called in the morning. I was livid. Upon mine and my husbands arrival, the staff was surrounding him in his room. I made them clear the room and spoke with Dad. Again the meds were not being given as prescribed. We spoke with the facility mgr, the "general" and a nurse. They all made excuses about the care he received, when he got his meds & advised us he had to go to a mental facility. I argued about it as he was still very, very sick; that his mental state was CAUSED by the lack of care, to no avail. NO APOLOGY for the care. A staff member pulled us aside on our way back, & advised us that even though they sign the meds out on time- it can take up to 3 hrs before a patient may get them. (Depending on what is going on, on the floor.) He was transferred that night. By the next morning, the new facility sent him to St Lukes because his legs were seeping. That turned out to be Congestive heart failure. He spent a week there. MRI of spine showed nothing. (3rd one) The pain? Turned out to be 2 spinal fractures that both hospitals missed!!! Was caught on an x-ray at a corner ER! Thankfully he has since recovered. The staff was cold, the care was negligent. Spare your loved ones. Steer clear. read more

Completely horrible experience from beginning to end for my mom in rehab! Thankfully she endured but do not send your loved ones here. They do not care about their patients and they will not service. It's a fraudulent that they take money from people for what , they provide nothing except for the PT service. The nurses are unresponsive and they don't clean after the patients. It's disgusting what they put my mom through not even welcoming her and not provided medicine requested or providing towels or showing up after 5 hours of her calling. Definitely should be investigated for the opposite of a care home. read more

Most of the nursing staff have horrible personalities and treat patients without compassion. My mom could ring for the nurse and it could take over 30 minutes to have someone come in. Poorly trained, they nearly dropped my mother twice (that I'm aware of.) Had to readmit her to the hospital for a severe uti (because they didn't change her regularly and she'd sit for hours with a soaked briefbrief through her clothes and sometimes her bedsheets) and discovered she had bedsores. Last straw was when they thought she may have had another stroke and called medical transport and not 911. Of course they backtracked their story. I would never consider this place for any family member again. read more

If giving zero stars were an option, I would give that, without hesitation! Please, do not leave a loved one at this place. I should've listened to the reviews before sending my dad to this awful place. Also, please do not trust the positive reviews, they are just made up! I was there every day, sometimes twice a day since it was nearby my work, and every time I was visiting my dad, there was an issue, which I would address with staff, however, nothing was ever done to address my concerns. My dad was not ready to be released but he couldn't bear to stay there any longer, here is a list of a few things that made his stay so miserable: -When he arrived, he was dumped in his room. Nobody was there to guide him, there was no nurse, doctor, or any staff. He was not seen by any one until the next day. - He was not able to walk and needed help to go the bathroom, a couple of times he called the nurse for assistance, but they never came and he ended up soiling himself! - One day they left him in the wheelchair and he wanted to go back to bed to sleep at night. He called the nurse and after a couple of hours of waiting he decided to go and look for help and he got scolded for being in the middle of the night roaming around the hallways! - They would bring medications that he was not prescribed. Including laxatives, which he didn't need, at all. He needed pain medications and he had to wait for hours before they would finally bring them. He called me several times in pain. - His main medication need was his sleeping pills and there were times that they wouldn't bring them, and if they did, they would give them to him at 2:00 AM! Other times they would say that they run out of them, even though I brought a bottle of pills with his prescribed medication. - He was there for two weeks and got only 3 showers, and only because I had to chase the nurse and CNA and ask them not to forget to help him shower. On several occasions, I reminded them that it was shower day and wanted to make sure he would get one, they never showed up. - The temperature in the rooms cannot be controlled and either he was super hot or super cold and once again getting someone to help him was impossible. - They forgot a couple of times to bring his food, so he called me and I had to go and bring food. - Every time that I was there I had to empty the urinal because no one else would do it for him. - They never cleaned the bathroom and it was just gross. - The water pressure in the bathroom was a joke, I complained several times about it and they "fixed" it with the same results. - Most of the nurses are temps and they do not know anything about the patients, procedures, rules, etc. One of them tried to help but couldn't do much, the other ones showed little to no concern for my father, who was in their care. - The nurses and CNAs rarely come when they are called but you can find them in the "lounge" room chit-chatting, and giving attitude when you need them. - My dad had several arguments with the nurses because they did whatever they wanted and wouldn't listen to my dad. He was there because he fell and needed to get back on his feet but mentally he is a sharp person. - I asked to talk to a manager and they didn't even give me her/his name so I was not able to complain to anyone. - The place reeks! Thank God we need to wear masks so visitors do not need to smell the strong urine smell that permeates the building. - The food was not acceptable, there were several items that were borderline inedible. My dad didn't get coffee or milk with his breakfast even though it say on the ticket that it was part of his breakfast. - In general, the staff is not compassionate, caring, or trustworthy. - There is no order, no one in charge, and no one who you can really trust. - I talked to a couple of patients and they complained about what a terrible place it is and how the staff does not care about them, so I am sure this is not an isolated incident (again, I should have listened to the reviews). My father was hesitant to even let me voice concerns for fear of retaliation from staff. This should never be the case when leaving a loved one in the care of alleged healthcare professionals. My dad is a positive person, always joking but he was very depressed while being at The Broadmoor, and I was sad and stressed out, no one has to go through this! read more

BEWARE!!! MAKE EVERY EFFORT TO AVOID THIS FACILITY. It needs to be shut down. The nice property is very misleading. My loved one was here for 5 days for high dosage antibiotics. Luckily, she was very coherent, mobile and able to take care of and speak for herself, so she was not in any danger. However the neglect and lack of organization was off the charts. They failed to administer the meds according to schedule, in fact did not even have the first dosage, but told us they did. They left IV's in all night when they should have been removed after the infusion, recreating a fall hazard. They did not respond to night time calls, luckily it was never an emergency. The food was never brought on time and even after confirming they had certain food items upon her request, they were never brought. There were stains and drips of some type of red fluid on her walls and the cleaning crew sprayed the floors with cleaners but never wiped it up, making the floors very dangerous. If you have to have a loved one be here, plan on visiting everyday to ensure their safety. The majority of staff that we encountered (save one or two) had no interest in doing their job or helping. More than once, when we could not get a response from the call button, I would find 2-3 staff remembers sitting at the middle desk, just eating and visiting or hanging out up front. If you try to call, you will be told the leads are in a meeting and there is no way to leave a message. Each day I communicated issues with Landon the Director, Ruta the head nurse and Rebecca, who handled all the registration paperwork and they all assured me everything would be corrected. Each day we had a new set of horrors with no resolution to the problems. We cleaned the floors ourselves, we sought out items that were requested and we had to advise the nurses when it was time for an infusion. Now we are trying to get statement for insurance claims. After 3 emails and 7 calls, I still cannot get ahold of the records person, nor can I get anyone to call me back. This is NOT a place you want for your loved one, especially if they cannot speak up or take care of themselves. read more

This facility is corrupt and should be shut down, they do not provide all medical needs for patients as they claim to do. They have hindered my mother's healing process by not providing her prescribed Bone Stimulator she needs for her broken femur. The admin staff are monsters and only care to collect government funds but fail to provide care needed for their patients. My mom is currently in here and will need to transfer her as her medical needs are not being met. Please call 800-medicare and The State Survey Agency 800-458-9858 to file complaints. read more

Good: beautiful environment, have some rooms with view and door to outside, Some employees friendly (not all), room seemed clean NOT SO GOOD/HORRIBLE/NOT ACCEPTABLE husband arrived around noon, discharged from hospital, and was not seen by a nurse there until 8-9pm that night. He had a wet cough (congestive heart failure and kidney issues) and she indicates they will order an 'xray' tomorrow. They knew he was coming the prior day and did not have ANY MEDS LINED UP FOR HIM - NONE. So he had no medicines allllllll day long. I fault the releasing hospital MEMORIAL HERMAN and THE BROADMOOR for my husbands death less than 17 hours after arrival there as I do not believe he was medically stable and just total negligence on the part of BROADMOOR. I sounded many alarms and concerns to hospital and Broadmoor night nurse/attendant(?) before I left that night. SHAME ON THEM. read more

A friend had her mother in the Broadmoor At Creekside Park a few years ago and had a positive experience. But things change or time, including the ownership. Maybe the COVID lockdown impacted their operations but based on our short experience with them recently there is NO way would i recommend this facility until they make monumental improvements. The staff in the wards is still friendly but there appears to be a staff shortage and management issue. The promised PT did not happen, not because of the PT staff but because they were too busy changing bed sheets, helping give showers, and perform what minimal nursing occurred. read more

If you or your loved one is able to walk, this facility may be ok for you. It is a nice building. Bring your own pain meds, you will need them upon check in. You may get pain med in 8 to 10 hour after check in. Pain management is spotty you will not get meds on time, usually 30 min to 3 hrs late. If you cannot go to the restroom unassisted be prepared to stay in your own bowel movements for a min of 30 min up to 4 hours. If you have bed sores or other open wounds this can be more problematic and feel very degrading. There is no wound care or bowel movement clean ups when the 3 meals each day are being served, (The same people that are serving the food are responsible for the bowel movement clean ups and they are afraid of cross contamination issues, I really hope they have access to soap). The facility is critically understaffed. This causes many of the problems noted above. Shift changing of personnel is not staggered, this causes an approximate 1 hour shutdown of services. The facility seems to run out of sheets on a daily basis. Soap dispensers are empty but staff will bring you a plastic cup filled with liquid soap. Carpeted floors in the rooms are stained with body fluids, and other vicera. Other less medically important stuff: they put nice looking paper towel dispensers in the bathroom that are locked to safeguard the towels. When you run out of towels and ask for more the set them in top of the pretty dispenser, no one has a key. The cafe on the south side has "premium beverages", (as advertised), consisting of hot or cold water, tea bags and coffee sometimes. The coffee machine is down daily at times of its own choosing. The food is consistently fairly edible and comes in quantities of one teaspoon to half a cup. Very little care is taken with how the food is plated, crushed, mixed-up, and missing foodstuffs are the norm. The staff seems to eat "out" mostly. Staff consistently wakes you by knocks on your room door between 7 and 7:30 each morning, while flipping on your overhead lights and greeting you with questions as your recover from the startlingly blinding light and try to immediately wipe the sleep for from your brain as quickly as possible so you can figure out if there is some type of emergency. Usually by the time the sleep for has left, so has whoever came into your room for whatever reason. If they were nice they may have even turned the blinding lights back off and not moved your bed tray out of reach again. There is no designated place in you room for the staff to put their medical supplies. Thus, they go through your clothing drawers, food pantry, shaving cabinet and desk drawers multiple times a day looking for their trash bags, baby-wipes, if flushing syringes and etc. All the staff seem to wear the same type clothing. This practice makes it difficult to know if you are talking to a nurse, a medication aide, a nurse assistant, or a therapist. If they all come to work that day, you may speak to several of each in a 24 hour period. All in all, I suggest you not come for the "skilled nursing" but the PT or OT. The staff is overworked and fustrated. The employees forget that the patients live here and that they (employees) only work here. This is my 2nd stay here, my husband was also here twice before. The facility is beautiful and well kept. They said they got a lot of new nurses and staff here, but I was happy to see some familiar faces when I got here. Even the food is better now. Physical therapy staff is wonderful and I am so grateful for their help so that I can go back home. Would definitely come back again. read more

I came to the facility for therapy services. My stay was pleasant, nursing staff was fine, facility was nice. Therapy services were very, very good. Good needs a little work, but I'm going home stronger. Overall, I have enjoyed my stay. read more

Neglect and mistreatment of and 88 years old women who suffers from late stage of dementia. at 4;45 she wasn't washed, fed, or cleaned the pictures and recording prove the disgraceful treatment. When approached the head nurse Princess we were advise we broke the law by taking photos of the neglect and mis treatment. She was rude,loud and defensive to both her grand children, her daughter and son in law who is a doctor himself. After 3 days I feel her safety is compromised if she is not moved immediately to another qualified and competent facility. I will fill at a complaint with the state health Medicare and other medical boards of the state of Texas enclosing pictures. I am so disappointed. The elderly depend on us and this treatment and lack of concern is so sad in America. read more

We trusted this health care facility with my wife's care, rehabilitation and recovery and they violated that trust and failed miserably. My wife's health and recovery deteriorated and went completely backwards. We had to EMS her back to a hospital as this place set her back severely. They displayed unbelievable neglect, evasion, deception, unresponsiveness, unprofessional conduct and consistent failure to follow my wife's physicians orders. This place has got to be the worst. Almost nothing is good there. A complete nightmare. Bad to the core. Critically understaffed most of the time to the point that nurses and aides are overwhelmed and reasonable care seems effectually impossible for them. Every bad thing you might be speechless about was happening. The administration staff are monsters in sheep's clothing who showed no real concern or follow-up for my wife being well or getting well. They lied so much that a major part of their lies were to cover up previous lies. There are lots of bad reviews on this facility and it's owner and we have every reason to agree with them and to believe that they are all true. This is the most horrible and unprofessional experience, by far, that we have ever had with any health care facility and medical staff and we are in our eighties so we have considerable experience.
