I cannot overlook the serious lack of transparency and communication this facility demonstrated with my family. My father was checked in on a Friday night without any family present, despite explicit assurances that the move would not happen until the following day. When we visited him Saturday, he was highly agitated and rather than addressing it proactively, the staff dismissed it as manageable dementia-related behavior. He was sent back to the hospital, where a psychiatrist evaluated him and prescribed medication that significantly stabilized his mood. He was readmitted to Harborage (this time with me present) and had a calm night and a good day, only to be sent back to the hospital again. This time at the request of his PCP due to a positive blood culture. A medical issue unrelated to behavior. Despite receiving multiple calls from Harborage staff indicating he would be welcomed back, we were then informed that corporate had denied his return. With no explanation and no appeal process offered. My father had not even spent a cumulative 24 hours in this facility, and he is already being turned away. I want to be fair: the 4th floor staff was genuinely excellent. But the lack of honest communication, the disregard for family involvement in care decisions, and the abrupt, unexplained corporate denial have made this an incredibly distressing experience. Families deserve transparency and an avenue for recourse, not dead ends. Shame on you people and thanks for gaslighting us. read more

Mom ,dementia patient, went in for sacral wound care. We thought having her in a facility would provide the special attention she required, turning her every 2 hours, and making sure the wound would be dry. The view was spectacular from my mom's room, but the attention was far from it. We signed her out after the second visit in almost two weeks we found her lying in her urine. Hope they improve their practice, so no one else experiences such dangerous situation. read more

This is the worse place to leave a loved one. It's horrible!!! My mom went from being bullied, to been left with dirty diapers, given wrong medications for things she didn't have, and EVEN wanted to give my mom diálisis without my mom NOT needing any of this!!!!!! Communication with staff was horrible!!! Never answer the phone unless you call like 10 times, make fun of patients, and just don't care about them, they want the family to do all their work. They're all unprofessional, don't even know English many of them. My mom came home, and now she's herself again miraculously!! This place left my mom traumatized and crying. Please don't take your mom or dad to this place if you love them. Please!!! read more

I wish I could give this place "zero stars". They are so understaffed it's ridiculous and it's the first thing that comes out of everybody's mouth every time you ask a staff member for anything at any time. That is all they say "we are understaffed". They really need to give them the T-shirt. This place is a three ring circus. It's really scary that we have our elderly people either living or in rehabilitation there trying to get better and having to put up with an incompetence staff. I was a patient for 24 days,it was the worst experience of my life, I was in a lot of pain and couldn't walk.when I pushed the button for help someone would come in turn the light off and say someone was coming,and leave .One night I spilled a drink on myself, couldn't move,I called my daughter at one in the morning crying,she was calling the facility,no answer she stayed on the phone with me for 2 and a half hours before someone finally came. They are unable to handle all the patients they have,a lot of the staff need training in compassion.They just provide a bed the rest is up to the patient. I was afraid at night I didn't feel safe...I could go on but I'm pretty sure you get the picture."Don't Go There " read more

Many of the nurses and assistants are very good. Administrators are the worst. Cleanning was good at first; However, our family is the one cleaning and changing the bed now; they are very understaffed. If you have any type of situation, good luck!!! Getting admistration to help. All they do is give you the run around. Extremely under staff. My mother's roommate is left with a wet pamper; all her sheets wet. Many times, I have to beg she gets clean. Do your homework before coming. Make sure you check on your love ones frequently. read more

The rehab part of the Harborage was ok but the nursing home section was awful. We paid out of pocket for our mother to be there and also a private aide all day just to be sure she got attention. The place is totally understaffed but the big issue is hospice there. Rather than helping to make a person's remaining time as pleasant as possible, the main idea is to keep them quiet and drugged. There was no effort to help her use the lavatory. Instead, they put everyone in diapers and then don't change them enough. Immediately the day after our mother was admitted, we notices peculiar behavior with her. The next day she was basically comatose and we were told she was dying. She actually pulled out of it that time. The truth was that she was given three doses of Morphine that her elderly body could not handle. When we found out and she immediately said she didn't want morphine, and I met with the social worker and hospice coordinator to stop the morphine, but was ignored. Our mother was given the next dose two hours after our meeting and continued to be drugged until she died. She caught COVID twice and quarantine was horrific. One particular nurse ignored her and kept her in a wet diaper when she had a bed sore, for hours. Our mother died alone in her room next to another patient who was doped up. I suspected she was still being given morphine and requested her medical records which was ignored. My lawyer had to make phone calls to get those records. I was right. the morphine continued after we said to stop it. That explained her confused state and their insistence that she had dementia. Horrible. I am haunted by the memory of not being able to get our mother out of that place before she died. read more

Staff is great. They took great care of may father and attentive to his needs, always kept him clean and truly cared for his well being. I highly recommend this facility. read more

My friend was there for 3 1/2 years. The last month she was there she had no phone. This woman had no family. She was older so most of her friends were gone. I tried for a month to have them repair her phone. Nobody paid attention to me. She died without been able to call or receive any calls. I kept telling them this was the only connection my friend had to the outside world. Nobody paid attention to me. Rip Diana I really tried getting that phone fixed. read more

Just ok, they are an older place and outdated equipment but they are a little better than other places. They take care of the patients the best of their abilities with a small staff. They do have limited English speaking CNAs and even some of the nurses. The biggest issue, is the food. Horrible junk food. Hot dogs, bologna's sandwiches and pizza and sometimes hamburgers. read more

The facility is very dated. Food is decent for a facility. The head nurse Nancy cares tremendously about her patients. The staff is spread thin. There are better places to go. If anything goes wrong, Palisades Hospital is not a top shelf ER. read more

I haven't gone back to finish my review of this place that I started 9 months ago. I stated that they left my mother in a wet diaper which caused the UTI. She went back in the hospital and she passed away because of the UTI. So f*** the harborage, f*** the staff, f*** the doctors. As far as I'm concerned they killed my mother. Don't send your loved ones there whatever you do. read more

My mother was in rehab at Harborage for 20 days. Mom is 89 years old and broke her hip. She received the best care ever. The therapist were excellent. They were dedicated to her and helped her learn how to walk again, go up and down steps, shower, eat and listen to her when she felt she would be in a bed the rest of her life. All the staff was pleasant and understanding. I could not ask for better care. I would recommend this facility. It was clean, food was descent and their was various activity to help through the long days. Thank you and keep up the good work. Jane read more

Ratio of aides to patient is 12-14 per patient. Not a good ratio for patients. Great nursing staff, physical therapist and occupational therapist. Depending on your needs it's decent. BTW food aweful and served warm to cold. read more

My father has been in the Harborage for over 5 years now. I had no complaints until now. The place is deteriorating rapidly. They only have 2 nurses sometimes working 2 shifts, not enough CNAs, not as clean as it used to be and temperature control is a mess. I must say that the nurses and CNAs are the best they really care for the residents and they treat them with so much love. Also many of the nurses and CNAs are leaving because of working conditions, this is not good for the residents that have become attached to them. I hope things go back to how they used to be it's a shame to see whats happening after being #1 for so many years. read more
