I had visited a friend there several years ago and was impressed with the facility at that time. My mom fell in March, and after a week in the hospital, we had to go to a different rehab facility at first because Santé wasn't on her insurance. However, that place didn't take good care of her, and she ended up with a Stage 4 pressure sore and sepsis, resulting in another, much longer, hospital stay that included three weeks in intensive care. When she was ready for discharge to rehab that second time, Santé was covered by her insurance, so I insisted on going there. We were at Santé for about six weeks, beginning in June. Some have said in other reviews that the décor is "dated," but I disagree. The style is Southwestern, which is quite appropriate for Arizona, and it never goes out of style. The photographs on the walls are of beautiful Arizona scenery, and the rooms are set up like hotel rooms. Santé also has a Starbuck's café and some beautiful outdoor patios where guests can get some fresh air. The food here is much better than most hospitals and rehab facilities provide, and guests can order just about anything they can think of, even if it's not on the regular menu. Each room has its own refrigerator and microwave. Most of the rooms are private, so most guests don't have roommates, but the shared rooms are really semi-private, with each guest in his or her own separate space, and the kitchenette being the only shared area. The physical therapy gym is wonderful, with many different types of equipment for different needs, and the therapists encourage family to get involved in the therapy, which takes place six days per week -- far more and far better than the first place my mom stayed. The wound care nurses took excellent care of my mom's pressure sore. By the time we left, it had filled in far more than I had been told to expect, all thanks to their superior wound care. Everyone was happy to answer questions and help however they could. My mom's case manager, Angie, was very kind and patient, and she gave me lots of information about post-rehab care possibilities. With her help, we were able to get post-rehab assistance that worked best for our situation. I would also like to give special thanks to Nurse May, Physical and Occupational Therapists Cisco and the two Lisas, and especially Speech Therapist Aaron, who went above and beyond for us. I was happy to take my mom home when she was discharged, but I was also sad to leave because the staff all seemed like friends. Most of my mom's allotted rehab time had passed before she was really able to start getting back on her feet, but I'm happy to say that, in continuing at home what I learned while participating in therapy with her at Santé, she's now improved by leaps and bounds and is well on her way to resuming the activities she enjoyed prior to this misadventure. I look forward to bringing her back to see everyone after her recovery is complete. I can't say that there weren't ANY problems, however. One female CNA was changing my mom's brief one morning, and some fecal matter had gotten into her wound. I asked the CNA to make sure she cleaned it out thoroughly, and she tried to tell me that the fecal matter had originated IN the wound, which was impossible in this case. When she finally left the room, I finished cleaning out the wound and asked the wound nurse to double-check that it was clean. This same CNA also used my mom's phone charger to charge her phone without asking, which I found rather presumptuous. The only other problem I experienced was that I always took my mom's laundry home to wash, but I was gone for one full day and night, and during that time, they managed to lose one of her nightgowns. I provided the price of a comparable nightgown for reimbursement, but to date, we have received nothing. Those minor problems aside, the gist of the matter is this: if you or a loved one needs to stay at a rehab facility for physical therapy, I cannot recommend any facility higher than Santé of Mesa.

I felt safe and cared for. Daily physical therapy was instrumental in my 'at home' recovery. I had several nurse visits and physical therapy sessions in my home, which was another layer of recovery care! I'm grateful to be able to do the activities I enjoyed before Osteoarthritis affected my knee and have lost twenty pounds by resuming my activities! Thank you Sante' Mesa!

When I was discharged from the hospital, I did not have the "PROMISED" option to select my rehab facility. I have to say, that the best thing about Citadel Post Acute, was the morning oatmeal!! As a newly diagnosed diabetic, I received NO guidance on diet, nor were there any menu choices suitable for a patient with diabetes!! Pasta & bread at every meal?? One whole week with NO salad options?? PT, OT workers were caring & encouraging & they did get me up, standing & walking again. There were two night nurses who actually seemed to give a darn, but the rest of the "caregiving" staff did not seem to give much of a darn about patient care. And don't even get me started on the "medical care" - I contracted an eye infection that the doctor (?) did not look at until after they had been treating it with saline drops (no improvement) for over a week!! And in the 6 weeks that I was there, they stuck me with 4 different room-mates, & apparently because I was not a "complainer", each one was worse than the one before! One even tried to smoke in the bathroom while I was 6 feet away on OXYGEN!! Stay away!!!

I researched the facilities that were within my medicare network. All of them received mostly negative reviews. My daughter knew a former of Mi Casa employee and she had mostly good things to say about the place. Thus, we selected Mi Casa for my wife's care. The facility is old and somewhat dated; typical Arizona decor, beige and brown. However, the facility is clean and the cleaning lady did her room daily. The nurses that cared for my wife were very good and caring. The majority of the CNAs were also good. Weekends usually had fill-in staff who were mostly OK. The physical, occupational and speech therapists were exceptional and caring. My wife made a lot of progress while there. Criticism of the food is warranted. Most of the entrees were excessively salty, as was the daily serving of instant mashed potatoes. The vegetables were what my wife ate mostly. My wife is lactose intolerant, but we sometimes received a carton of milk with the meal. The kitchen staff does not follow instructions very well. After two months of care, some desk jockey at Humana Medicare Advantage decided she did not need any more SNF care. Too bad, as she will require considerable physical therapy for the next several months.

One day I arrived to find stomache contents all down his chin and front of chest, it was all dried on, this was at 3pm, when I asked the nurse how long this had been on him and he said since morning, I said ok let me clean it off, so I did. Everything my father has in his mouth goes into his lungs, so the fact he was aspirating was dangerous. Everytime, I arrived dad was being fed, that being the case he was being overfed. I checked dad when I arrived at 3, told the nurses he was soiled with both and no one came in to change him, I would have at this point but there was no changing stuff around. I left at 5:30, still not changed, I told the nurse I'm taking my daughter to dinner I hope dad will be change b4 I get back. I arrive at 7:30 and they were just changing him. They also were putting him on a c-pap machine with his oxygen because the test came back he had pneumonia. Yes, he had pneumonia because he aspirated into his lungs and was struggling to breathe, I wish I had said take him to the hospital right away, but I hadn't put 2 and 2 together yet. I went home and got a call later that night he was dead and they were doing cpr. He aspirated because they were feeding him too much, it was running every hour I went there and it shouldn't have been. I'm convinced he was throwing up because he was being over fed. Everyone knew he couldn't have anything orally and knowing this, they should have clued into that and sent him back to hospitalthat day. My entire family feels they killed our father. I do have to chime in on the smell of the place, it was horrible, so busy, patients blocking hallways in wheel chairs and the first room dad was in down stairs was super dark, just shoved in there and I told them he was a stroke patient and needs light and stimulation, they get kuddos for moving him upstairs right way in a more lighted area. This is not a place for people that need to be looked in on. All the times I was there except for first time, they came within 2 hours of my arrival, but all other times no nurse ever came in unless I peeked my head out of room looking for something. There is one visitor chair for 2 patients. Everytime I went I had to search for a chair. Dad didn't have a side table, everytime a nurse came in they'd ask where it was. The IV stand had a missing wheel so it was lopsided. I had to stick it on the bed leg for it to stand up. Please, if you have any other choice do not take your family here! I get they are over worked and all but they also don't do their job correctly. My father should still be alive. Oh forgot to mention, they hadnt given dad his dailysis in 5 days, his blood pressure was too low, well they were giving him blood pressure medicine. He should have had dialysis. There was mass neglect. I even asked roommate how often they came in and he said they don't, unless I call them in. The alarms going off forever and ever, and no staff in sight! It was horrible and the smell was so terrible, I feel for those working there. I would Rate this place a zero but they get a 1 star because they moved my dad out of the smelly dungeon.

When my mother…read moresuddenly took ill and needed 24/7 care temporarily ,I called them a bit frazzled and they put together a team very quickly. This was done on Thanksgiving week! Everyone they sent was so kind, friendly and attentive. There were household chores and doctors appointments (some that were not close) that they had to help with and did it all with smiles. Apart from a few scheduling issues, everything ran smoothly. Again, many thanks to them.

    We have been using TheKey (San Jose) for 24 hour care since October 2022. Aside from a couple of…read morereliable caregivers, we have had poor quality of care exacerbating an already stressful situation. There does not appear to be a matching process or vetting. There has not been alignment between caregivers, staffing manager, and care manager causing confusion, multiple miscommunications, poor continuity of care and additional stress. Calendars (if we get an updated one) are never accurate and there does not appear to be accountability for who is responsible for what. My family has had almost no peace of mind during this difficult time and I have spent countless hours managing problems with both caregivers and Keys leadership multiple times a week. Two night caregivers have slept through multiple call button rings, two caregivers have no showed, one caregiver walked around the house all night keeping everyone awake. After asking the care manager all this week to verify coverage for this weekend, another no show tonight - Christmas weekend. These are only a few examples of problems that have occurred in a short period of time. Why not fire The Keys? Easier said than done. While my parents have me advocating for them, it's sad to think about other vulnerable elderly folks that need support and are being taken advantage of by what appears to be incompetent leadership and a poor for profit business model. For the amount of money Keys charges, service should be seamless. Do better. You are certainly not helping older adults live well in their own homes or improving their quality of lives. We know that stress and lack of sleep exacerbate medical problems. Sadly, The Keys has succeeded in accomplishing this.
