It was…read moreonly supposed to be for a few days, but we still did our due diligence to identify a reputable facility. From that perspective, this location seemed reasonable. But, it was not. Upon arrival, their intake process did not cover anything specific about why she was there or any particular concerns. This was a problem because there were important diet restrictions. Further, the nurse doing the intake seemed incredulous -- responding "you don't believe in anything" -- when my wife answered "no specific religious beliefs." The nurse skipped the "do you feel safe" questions commenting that she would skip those because I (her husband) was in the room. That's exactly why you ask those questions AND YOU DO NOT SKIP THEM. After that, they brought in her dinner, before checking her glucose, which was precisely what led us to the hospital to begin with. When I expressed my concerns with the charge nurse, they came to the room to discuss and indicated that we could always reach them by phone -- a phone they always have with them (though he had inadvertently forgotten it at his desk, so he did not -- in fact -- have it with him). The PT/OT was okay, it was slated for 3 hours a day. But outside of that, my wife could not do any of her own recovery or work -- including walking. So she'd be sitting or lying for the remaining 21 hours in the day. That didn't seem conducive to recovery. We ended up leaving the facility after one night. The facility itself was fairly nice, and a handful of the staff were helpful, but overall it was not a place that I would recommend. I have zero confidence in their ability to provide the level of care expected or desired.

The evening she arrived, she was still able to walk. Unfortunately, the therapy she was supposed to receive--at minimum 3-4 times per week--rarely happened. Sessions were short, inconsistent, and often skipped entirely. Instead of helping her improve, staff frequently left her lying in bed or sitting in a wheelchair for most of the day. Over time, her muscles deteriorated significantly, and walking became nearly impossible. Eventually, she could no longer take steps without the assistance of a walking device. Basic care was severely neglected. She was not properly bathed or given adequate peri-care. Staff tried forcing her into Depends, despite the fact that she had never needed them in her life. As time went on, it became obvious this was simply because staff did not want to assist residents to the restroom. There were many times she would call for help and wait 30-45 minutes before anyone responded, often resulting in humiliating accidents. Her medications were mixed up on several occasions. We also witnessed feces- and urine-soaked bedding and clothing being left in her room and even thrown onto the floor. Our family met with management, including Heath, multiple times, but despite plenty of promises, nothing ever changed. We ultimately removed her from the facility, and within 12 hours she had to be hospitalized with a severe blood infection caused by unsanitary conditions and neglect along with bed sores on her back side. The night staff were by far the worst. My mother began recording the amount of time it took for someone to answer her call button because the delays were so excessive. She also suffered falls there--despite never having fallen before in her life. On multiple occasions, night staff left her lying on the floor with only her own pillow placed under her head while waiting for another employee to help lift her, sometimes for 20 minutes or longer. There were also several times our family arrived and found all the doors locked, forcing us to wait extended periods before anyone answered. Each visit, our mother appeared more neglected and distressed because of the poor care she was receiving. Late at night, children--presumably employees' kids--would run through the halls loudly, disturbing residents. Her water containers were frequently left empty and dried out. This place was an absolute nightmare. We personally witnessed verbal abuse from staff toward other patients in common areas. Based on our experience, I strongly urge others to stay far away from this facility. Their negligence nearly cost our mother her life.

The nurse I worked with was so…read moreawful & unprofessional I returned to Wellsprings the following morning waiting for them to open to report this nurse to HR. The nurse I worked with was VERY unprofessional in his remarks, attitude, & tone toward me. This nurse did not help me with fulfilling patient needs when I specifically ask him. This nurse made a point to tell me 2 times that "ya know, I am a good nurse". Never asked him if he was, this came as responses to asking him for help by grabbing more bleach wipes when I was in full PPE in a room assisting a resident. He never helped me once. This nurse created issues between a patient & myself that did not exist. He would not let me speak. He believed his conclusion. The resident corrected the nurse in his wrongful accusations toward me. Another incident while making rounds, I knocked on a residents door to check on her again. This nurse was angry with me because he was in the room I "disrupted him while talking to the resident". This nurse was very egotistical. I didn't understand why he was so upset with me. I had not done anything wrong. This nurse scolded me again & told me to stay out of this particular patients room that the issue was with. That I'm never to go back in there. I didn't understand why or what happened. A couple minutes later this nurse completely flips out & starts talking VERY loud in the common area where all the residents & their families could hear & see him and loudly said that "he doesn't trust me with any of these residents!" Then this nurse, not a manager, not HR, but this LPN nurse tells me to just leave now & go home that he doesn't trust me, to stay away from all the patients!! Of course it's nearing the end of the 12 hour shift &another CNA had showed up. But everything was ok until he knew he had to cover his lies & get rid of me so the truth wldnt be exposed. He wldnt let me speak or listen to me. He would talk over me or cut me off. I was absolutely shell shocked. This was my1st time working at this facility & I did a stellar job, especially having the limitations of it being a busy holiday shift & many families were visiting. I did my job & I ALWAYS assisted & cared for the residents as if they were my own family. The issue was a nurse that needed to cover himself & the mess he alone created & wasn't going to hear about anything else. He could have simply said he was wrong about what he thought had happened & I would have said it's ok, no big deal, water under the bridge. Now, I am happy to write this truth on the staff & the lies they will create to shift the blame &create scenarios that never happened. The Nursa app has evolved & now we can see the reviews left on our performances. Wellsprings of Gilbert has even left me a negative review about my quality of work. I was very angry at how I was talked to & treated by this nurse during my shift & I expressed that to Wellspring HR. I told them to watch their cameras! The resident did correct the nurse of what he thought happened or said but he didn't want me to speak to that resident again. That's when he flipped out. I am absolutely shocked that their management believe lies over truth. There were so many other incidents that happened with this nurse during my shift I would just shake my head in disbelief & continue doing my job. One that I will note is one resident was new to Wellsprings from the hospital. This resident had a couple incidents that pointed toward having C-Diff. I made the nurse aware & suggested to put a contact precaution on the patient's door so if in-fact she was positive for C-Diff, anyone who entered would know to take the proper precaution & prevent spread as it is highly contagious. I ask the nurse 2 times about doing this. He was angry at my suggestion &said he didn't like me telling him what to do. I told HR about this also. I explained to the nurse and HR I'm only reporting from previous experiences I have had with patients & hoping for protection by taking an extra precaution. This nurse never took a test to send off to see if the resident was positive nor did he put up a contact precaution sign at least until a test could be done to be determined. The resident did have several episodes & symptoms that pointed to C-Diff. I just report what I see. I was met with being scolded with egotistical remarks. Shame on you Wellsprings for turning on me. I brought this to your attention! I am a good, honest, hard working CNA. Yes, I was very angry & said colorful words as adjectives of behaviors I experienced, never direct name calling. I shared my experience with other healthcare management & they all agreed that I should have reported Wellsprings & that nurse to the AZ Board Of Nursing. I regret not doing that. I'm not perfect but I know God saw ALL of it and I can stand before Him blameless in your accusations.

The business, Like Family Home Care, I was approached by the owner, Adam, who conducted an assessment of my home. He demonstrated exceptional attentiveness to my needs, which immediately impressed me. He was very experienced. I was subsequently assigned to five caregivers, with Kelsey being the most present. Kelsey is exceptionally caring and passionate about her work, prioritizing the well-being of her clients. She effectively assisted me with showering, dressing, mobility, and transfers. Kelsey gained valuable experience in cleaning and dressing my wound, as well as helping with my daily chores, including cooking, laundry, and cleaning my home. I highly recommend Kelsey. All the caregivers provided exceptional support throughout my recovery. Adam and Amber were particularly helpful in every situation. I wholeheartedly recommend their service to anyone in need of personal care assistance. I genuinely felt like part of a family during my time with them, and I was saddened to see them depart.

When I was discharged from the hospital, I did not have the "PROMISED" option to select my rehab facility. I have to say, that the best thing about Citadel Post Acute, was the morning oatmeal!! As a newly diagnosed diabetic, I received NO guidance on diet, nor were there any menu choices suitable for a patient with diabetes!! Pasta & bread at every meal?? One whole week with NO salad options?? PT, OT workers were caring & encouraging & they did get me up, standing & walking again. There were two night nurses who actually seemed to give a darn, but the rest of the "caregiving" staff did not seem to give much of a darn about patient care. And don't even get me started on the "medical care" - I contracted an eye infection that the doctor (?) did not look at until after they had been treating it with saline drops (no improvement) for over a week!! And in the 6 weeks that I was there, they stuck me with 4 different room-mates, & apparently because I was not a "complainer", each one was worse than the one before! One even tried to smoke in the bathroom while I was 6 feet away on OXYGEN!! Stay away!!!

Walking in I gasped with excitement. I see the wonderful Halloween things right away upon walking in and I was like a kid in a candy store. For me Halloween is my favorite holiday so my eyes light up like I am a kid again. Going down the aisles I could see that they were still stocking things and there would be more to come, but there was enough to keep me interested and scouring the aisles for what I wanted this shopping trip. The aisles were clean and clear, and the employees were nice whether they were in the store on the floor or at the cash register, which made the experience very pleasant. It's always so tempting with all of the ghouls and goblins and skeletons and pumpkins galore. Today I left with some cobwebs for my fireplace mantle for only $2.99 wow! What a deal!! It will look absolutely perfect.

We ended up leaving with a mantle decoration, but they already had everything from the giant skeletons and yard decorations to a few inflatables and plenty of indoor décor. The store was clean, easy to walk through, and the cashier was nice and friendly, which made for a pleasant checkout. Their prices are kind of a mixed bag. About half the time you find something and think, "That's a really good price," and the other half you're thinking it's a little more expensive than you expected. The only reason I'm giving four stars instead of five is because it seems like a lot of their larger decorations end up with some kind of damage. If you're buying one of the bigger items, it's worth taking a few minutes to dig through the stock and find the one in the best condition. Other than that, it's a fun place to browse, especially if you're ready to start decorating for Halloween way too early like we are.
