My 80 year old wife spent an excruciating six weeks at The Pearl at Watkins Centre (fancy name) Memory Center in Midlothian, Va between late Nov and Dec,2025. She and I both could not wait to get her out of there. Total neglect from the staff, (except George who is the only employee that I saw who actually cared), from top to bottom. Ashley, chief person in charge, Jemel, next person in line, ignored my telephone calls. My wife was sick between Christmas and New Years. She had diarrhea and vomiting. She couldn't make it to the toilet. She pulled the emergency cord in the bathroom. No response until the next morning when a staff person finally showed up. When she asked for someone to clean up the mess that person told her, "that's not my responsibility" and left the room. Another major area of concern was her security. From the get go her room was unsecured. Her key fob did not work from day one. Consequently, other residents would come into her room during the day and in the middle of the night. She was very frightened in the middle of the night. We found a so much better place to take her. Literally night and day. Spring Arbor in Midlothian. I have since found out that 5 other people from Pearl had escaped and were living there. I think this facility should be either greatly improved or shut down. I am sending this message to the appropriate state regulatory agencies in Va. Bottom line. If you care about your loved one, DO NOT take them to Pearl. Over the last year of my life, I have sat at my laptop more times than I can count to write this review. Sharing what I saw is excruciating and I want to be very clear. Please, I beg of you, you must not consider this facility, The Pearl at Watkins, for your family member. I did not have a family member at The Pearl at Watkins, but I learned to love many residents struggling to survive the unspeakable inhumanness of the "care" given by The Pearl at Watkins staff while I worked there. I was hired by an outside source. A family hired me to visit their father once a week for therapy. There were many weeks, I visited more then once, on my own time, to assure his safety. At times I found their father hunched and falling out of his wheelchair. Their father needed help eating his meals from Parkinsons disease and I would find him with an untouched plate of food because no one would help him eat a meal. "IF" I found him in a clean diaper, he was not wiped clean after bowel movements and his bottom was covered in feces with a clean diaper over the soiled and infected skin. I witnessed the maintenance man administering medications to residents on multiple occasions because The Pearl at Watkins was "short on staff". I found my patient drugged and The Pearl at Watkins and given medication without communication with the family. I was given permission to enter and do my work when families were not permitted visitation during Covid. I had to bring my own hand soap and I found many residents unkempt. The neglect and deterioration of residents during Covid was unbearable. I have years of stories and this is just a review. I can promise you that the day you tour The Pearl at Watkins will not be the environment you leave your loved one in. The website shows a fireplace and I have never seen it turned on once during my years of visiting. The Pearl at Watkins is a new facility so the reviews of it being clean means nothing. Your loved one is in danger at the Pearl at Watkins and I need to stress that while the revolving door of individuals hired to care for your loved ones are beyond oppressive, the root of neglect and criminal behaviors lies within the managing staff and Pheonix Senior Living company. read more

This facility is a rare find! When I talked to the Aministrator about moving my husband to the Pearl, she promised that they would love him as if he were their own loved one and that they would consider our family as theirs. These were not idle words, as I had heard before. I come everyday to the facility and constantly watch the entire staff, even down to the maintenance crew as they pass through the hallways, helping and loving the residents. After being there for only a month, my husband was diagnosed with terminal cancer. When I brought my husband back to the facility from the hospital, I was met with compassionate and loving arms and reassured that they would see us through. I feel so loved, and know they truly care! They offer support for the spouses of their residents, which is a tremendous help. Because of the programs they have in place, my husband has spoken more in 3 months there (because of the buddy system), than in the last 3 years combined. The nursing care is top notch! They have been keeping his pain under control and they make sure that the staff handles him with care during transports from wheelchair to bed, using two CNAs to keep from causing him unnecessary pain. They put up with a lot of abuse from the residents due to their Alheizmer's/dementia conditions, and I am amazed at how kind and tolerant they are, and how professional they handle these situations. There are not enough stars to rate how wonderful this facility is. It is comforting to me to know that my husband's last days will be filled wih happiness from his buddies, and the love and expert care of each staff member here at the Pearl, which we consider home! read more

Are you looking for a memory care facility that will forget to feed Mom or Dad? How about one that does not bathe them? Or maybe one that will tranquilize them without consent? Perhaps you have been looking for a "caring community" that allows them to sit in their own feces without being checked. If so, I have the perfect place for your loved one, and it's called The Pearl at Watkins Centre in Midlothian, VA. In short, words are not enough to describe what an absolute disgrace this facility is to the healthcare profession. When I first moved Dad here, I thought, "Great, this place appears clean, it's recently built, and it appears properly staffed." Within a very short time, however, it became clear that it was not what it seemed. Upon visiting, I noticed that Dad would not have been bathed, teeth brushed, nor hair combed; this happened repeatedly. Countless times, I would find him unchecked, sitting in his own waste or in his urine-soaked Depends that had not been changed for hours. Or better yet, there were times he had been tranquilized without consent. And there was ALWAYS an excuse every single time I visited-zero accountability and zero competence. And how could I forget my father's "care plan." HA! What a joke that was. At no time was my father's "care plan" ever once followed properly, and when asked if they even knew what his "care plan" required, I was told (verbatim), "Well, we know where it is but have not really read it." Perhaps this was because this facility has been severely understaffed and in constant violation of VA health regulations; there were instances I arrived to see 20 or more residents being supervised by one staff member. Where were the other staff? There were no other staff. And then there was the staff themselves. There were a few who were genuinely kind, conscientious, and very caring...the others did the absolute least work possible and mostly stood around looking at their phones and talking rather than caring for the residents. And how could I forget administrative staff. What administrative staff? This place has constantly been without proper on-site supervision, and the "administrators" there have been ineffectual, utterly incompetent, and CONSTANTLY full of excuses. Emails? Forget it...they went unreplied to. And then there was the food, or what can also be called Grade C garbage dumped from a can or microwaved. It was absolutely shameful and an insult to the residents. Last but not least, you might be wondering how much you will pay for this level of neglect: around $7K a month. That's right, while your family hemorrhages their life's savings, you can at least know that your loved one is in caring hands.
