    My 92 year old mother was a patient at HDH/Forest for 10 days for CHF and AFib. Firstly, she was…read moreadmitted to an Oncology/Med Surg Unit for heart issues and did not receive a telemetry box for at least 18 hours. The nursing / hospitalist care was horrific. She received too much IV Lasix by day-3 and was weak, shaky, and severely dehydrated. Nurses were instructed by the cardiologist to give all her meds before her Cardioversion and she didn't receive any (including her blood thinner). Also, when one of her blood thinner pills fell to the floor, the male nurse told her she could skip it for the day. She was left alone in her room at the end of the hall with the door closed tightly when she became disoriented, agitated, and confused. One morning, her 9am pills were still sitting on her table at 12:30pm bc the nurse didn't make sure she took them (when she was confused). When the "nurse" was confronted about her untaken meds and the door being closed, she put her hand in my face and walked away stating she was "going to give me a minute". She had to use the call button repeatedly to ask for help reaching her food tray, only to be ignored, and finally received help when the hospitalist came in her room. She did not receive any hygiene or oral care unless I asked for clean sheets, washcloths, and towels, and then I did the care for her - not the employees. Apparently, hygiene and oral care is no longer a routine part of nursing care and patients can go days without it, unless they have an advocate to facilitate it. Every day she was a patient at HDH she became weaker and had more issues. Finally, there was a huge cockroach in her bathroom that I smashed with my shoe. The nurse manager argued that it was a water bug...it was NOT. This is actually the second time I've seen a cockroach in this hospital. I have vowed to never return to this God-awful facility and have asked my family (and you) to go somewhere else if they want to live. For a previous ER Visit for my 92 year old mother with CHF and shortness of breath (on 12/16/25), the ER physician told us that "her case really isn't an emergency because the ER is for traumas, gun shot wounds, and heart attacks", but he proceeded to give her IV Lasix and admit her for CHF. Ugh...really? His personality was not suited for emergency medicine.

While there are some employees here who actually work and care for the patients, namely Jules and Basil, there are many who don't. They're lazy and too busy on the phone. the patient to caregiver ratio is grossly out of balance and the quiet ones, ie stroke patients, are neglected. And if you show up for a visit and ask any questions, they are insulted and defensive. I actually had to call 911 during a visit because my LO was in so much pain and no one would help AND THEY REFUSED TO LET HIM LEAVE. This place is beyond scary IMO especially when the Medicare website even gives it a one star rating.

My brother required full time nursing care after a fall in August 2023. West port is like many nursing homes, all have good and bad qualities. One of his nurses, Greg, was excellent. Also Donna -she was in charge of the floor/wing ,was very good. Both kept us informed as best as they were able. There is supposedly a physician at the facility , whom was never seen ,,,if he even existed. The physical therapy department was very good. Unfortunately my brother did not put his best efforts into his rehab. Food is institutional and left a lot to be desired. We visited my brother every day and had to bring him food so he would eat. The weakest link here is the business office. After my brother ran out of his Medicare time allowance, he was a self pay resident. $400/ day ~ $12,000 month. As I was handling his business affairs, I was paying his bill every month with his funds. Payment was expected by the 10th of each month and his bill was always paid within the 1st 2 days of each month. As he passed away mid month, there was a debit balance of $2945.13 left and owed back to his estate. Now as the administrator of his estate, I have been trying to get a check issued for this amount since June and all I get is the runaround. Supposedly corporate has to issue the check , but on one call tell me who to contact .

I am appalled that this establishment continues to operate without accountability for the neglect and abuse I personally witnessed during the 48 hours my mother was there, which ultimately contributed to her passing. During our brief stay--where we were with her approximately 90% of the time--I observed numerous issues that no elderly individual should endure. These included cockroaches, medication, and feces on the floors, which remained uncleaned throughout our time there. There were dirty diapers under beds, in sinks, and in hallways, and the overwhelming smell of dehydrated urine was pervasive. Patients requiring assistance with feeding or hydration were not provided with food or water throughout the day. Staff failed to check vital signs, wash, or change patients' gowns. Many patients were found completely unclothed, with no efforts made to dress them. The staff appeared disengaged, often sitting in corners eating and using their phones, neglecting their responsibilities to the patients. Additionally, beer cans and dirty gloves were observed in the parking lot. When my mother was scheduled to be placed in the dementia ward, we arrived early to meet her there. The conditions were shocking: urine on the floor, patients wandering without supervision, and a man walking around without pants. I immediately informed staff that my mother could not stay there, and they agreed to move her to the first floor. Upon her arrival, she was placed on a broken, dirty bed in a deplorable room. She was begging for water, but staff refused to assist. Due to her pneumonia, she needed to be positioned upright to drink, but because the bed was broken, we had to manually sit her up and provide water ourselves. My mother is unable to use her arms and legs so she requires full assistance with all of her activities. Once moved to the first floor, she was left unattended--without food, water, vital checks, or any form of assistance. We had to hunt down the nurses and beg them to take her temperature and give her cough medicine. She was crying in pain and they said there was nothing they could do. We were trying to move her out of there as soon as possible however this was a weekend and making new arrangements was not possible until Monday. We cared for her during this time, but when we left for a few hours during the night to rest, we returned the next morning to find her unresponsive. We had to insist that staff measure her temperature, which was 103°F, and call emergency services. They failed to check her oxygen levels, which were dangerously low. She was transferred to the hospital, where she was placed on a ventilator and later passed away. No individual should be subjected to the conditions described here. This place is what nightmares are made of. This facility must be shut down or held criminally liable for neglect and abuse. The staff involved should be terminated immediately. I have documented evidence, including photographs, which have been provided to Adult Protective Services. If you are seeking a nursing home for a loved one, I recommend consulting resources such as Medicare Care Compare. As of now, Canterbury has a two-star rating out of five based on health inspections and other criteria. The hospital informed us that the patient's they see that come from Canterbury are always in terrible condition and that there is new administration there because of all the problems they had. I spoke to the new administrator and she did not seem surprised by what I was telling her. My understanding is she has been there since May but she needs to hurry up and do her job and protect the people that are currently in their care.

The presentation of services to be rendered was completely misleading... overstated and vastly underdelivered. I was told the nurses were skilled and would understand the needs of my mother and right out of the gate the woman they sent was clueless. My 83 y/o mother had just received a hip replacement and we were looking for someone to help monitor her during a difficult phase of her recovery... basic stand and pivot to bedside commode... Turning in bed... monitoring blood pressures as she was, at the time, constantly fluxing with orthostatic hypotension. We were told the nursing staff was competent and would be able to do this and more, even light cleaning, meal preparation, full caring and respectful bed bath and some laundry service too! It sounded like a dream and too good to be true. It was exactly what I was looking for in terms of service and care for my mother. It was all a lie! First off, they were unable to find anyone for several days to come out and help. We even had someone sheduled and they didn't show up. When finally someone came, AN HOUR LATE, the woman literally huffed and rolled her eyes at me when I opened the door to let her in. She did not introduce herself in any way nor did she say two words as I led her downstairs to my mother's care area. She was very defiant, contentious and acted like she definitely did not want to be here! I was so uneasy and uncomfortable at the encounter I wanted her to leave then and there. I asked her if she knew of my mother's posterior hip precautions... no response! I asked if she was aware of how to do a stand and pivot and help mother out of the bed so as to avoid another hip dislocation, she looked at me like a deer in headlights. I then asked her if they had updated her on my mother's condition and the care needed... her only response was "yeah, they said she was real hands on." I proceeded to explain and instruct her as best I could. She then proceeded to sit in the chair across the room and watch the football game that my mother was watching at the time. I thought to myself "you've got to be kidding me! This is what you call a qualified caregiver and supportive palliative care!" She did not engage with my mother, speak to her nor even introduce her and give her name or offer to do anything other than, "you need me to fix her some food?", to which I responded, obviously no, I had already fixed her lunch in the hour I was waiting, which she was eating at the time of this woman's arrival. The whole experience was incredibly uncomfortable, but I had to go out and replenish supplies and groceries and other errands. I was so hesitant to leave but mother was having a really good day and was in bed watching the game, so I thought I could run out real quick and then just get back ASAP. I got as far as the parking lot of the store a few minutes away and immediately turned around and came back to dismiss the woman and figure out another solution. I called the oncall staff at Brightstar and explained the situation. They told me the woman had wanted to call out and not work that day so that was the excuse for the unprofessional behavior... I was thinking... why would you send that in the first place if that is the best you have... They tried to assure me that they did have really good qualified individuals and for me to give them another try the next day. They said this woman was no longer work with the company... Which I thought was strange to inform me of their employment decisions. They tried to convince me again to have them send another woman. I told them no... I would make other arrangements as they were obviously unable to deliver adequate staff to begin with. The woman was there all of 30 minutes. I dismissed her immediately upon my return home. Some time later I noticed I noticed an automatic debit from them to my bank account for $98. I disputed it. They were incredibly rude on the phone and very short with me stating it would be under review. I guess it's still under review because the charges were not reversed. Very unprofessional, very shady and very inappropriate business practices I advise anyone and everyone to look elsewhere!

I expect them to close their doors and file for bankruptcy within a few years. They act like they have been given a box of snakes and a full time job. However, other companies have no problem with medical + non-medical staffing.

    My mother was there for rehab. The PT folks were professional and amazing. I can't say much for OLH…read moreleadership. Evening shift for nurses and CNAs provided less than adequate care. While there she was dropped, left in a chair for unacceptable amounts of time (after telling me she was sitting for sometime an hour in a half I came and sat with her) I got up after 45 minutes and found everyone around the nurse's station on their phones and talking. Director of Nursing would never speak to us though requested on multiple occasions. Social worker was less than helpful. And head of facility avoided us. Not to mention the receptionist was down right rude. The most stressed I have ever experienced was while she was there. It was awful. I would never put anyone I love there again.

I am told to call the main line. Funny thing is the only direct way to talk to anyone of importance is only is your admitting someone!
