Can someone please tell me how a patient has not been given a shower in 10 days? How physical therapy consist of 10 min walks in hall with his walker, or sitting up and down in a chair. How a occupational therapist tells patient on Wednesday " I will be right back for your therapy" we are now at the following Monday and she has not been back as of yet...are you serious? Also why is the front door being manned by a 90 year old woman in a wheelchair and no one at a front desk? I would imagine that is a HUGE security breach...My son called to state his complaints and got nowhere with the social worker. No workers seen this past Sunday...he walked halls and saw no one....Also why when a patient has to be transferred to a hospital 10 min away, it took almost 2 hours....Can someone please clean up the urine that is all over the bathroom floor...this is NEGLECT to the max, and I will be working on getting the patient out of there ASAP!!!!!!! This facility is extremely understaffed. Communication is beyond bad. Night staff couldn't even explain visiting hours or facility rules to me. Good luck getting anyone to answer the phone in the evening. My mother was transported there for PT following a severe infection that left her unable to walk. Within 48 hours she was transported to an area hospital covered in bruises from her fall out of bed -- she was classified as a fall risk. The worst part was I heard the news from the ER Nurse at the hospital --- no phone call at all from the facility. The night and weekend staff is lacking on so many levels, there is not enough room to even go into the details. When I finally got to speak to a supervisor - there are none onsite at night or on weekends, she said they had no contact info. Really? They called me from the facility she arrived, and wrote down mine and my brothers info when I arrived, not to mention all of the records they would have received from the hospital. Understaffed and incompetent are two words that come immediately to mind. Thankfully she didn't suffer any brain damage or broken bones from this fall. The extent of these injuries could have only occurred if that bed was left up as high as it could go, and side rails not put up. I don't want to even think about how long a confused 87 year old woman laid on the floor before being found. She was so scared when I go to the hospital. Don't miss the photos. We went for the first time last night. Nice little place. Was seated quickly and ordered our drinks. Food was ok and we all agreed we probably wouldn't come back. We were finishing our drinks and was asked by the owner if we could take them to the bar, ( bar was packed ), because people were waiting for a table. The facility isn't clean, trash every, and smells terrible. The staff is very rude, especially Julia in payroll.

Physical Therapy is excellent and major saving grace . Main priority of staff is to insure trays go out on time and picked up timely. So any calls for aide during those hours are put off for hours . Pain meds take an average of 2 hours to get if at all. Meals are excellent if you love gravy on EVERYTHING and no special diet choices. ( like cardiac diet)one day we had raw biscuits. Management is continuously forcing patients to move to another room. They promise You one room and after you are there for a bit you are "requested" to move to another. Intensely understaffed and impossible to get Drs to address worsening medical issues. read more

My husband was there just recently from a Wednesday evening to a Saturday morning. It was a terrible experience. Very little communication between the nursing staff and the family. When the nurse called me to tell me that he had fallen but had no injuries, she put me on "hold" so she could pick up her cell phone. I could hear her entire conversation with someone about transportation for her children. No visiting because he was new and their current policy is to wait two weeks. I would not recommend putting your loved ones in this facility. They are poorly staffed and I can guarantee that your loved one will be neglected. We have had a family member staying there for a month while waiting for her permanent residence to open up. In the four weeks that she has been there, she has not had a bath or a shower once. She is not able to move around in her wheelchair on her own, so she has to call if she needs something, and I have waited over 2 1/2 hours before for a single request such as a glass of juice for her. They don't take you to the bathroom. They just put diapers on you and change your diapers. They never put the residents in the wheelchairs and take them outdoors or to any activities if they are unable to move themselves. Usually there's no one at the front door. Every time I walk to the nurses station, there is no one there and you have to wait at least 15 minutes to find a nurse. The entire place smells like diarrhea, and it is not clean. There is trash all over the floor. If you were in a desperate situation for respite care, I guess this would be fine, but I certainly would not put someone here long-term. read more

For the life of me, I have no idea why this nursing home has so many stars. I would honestly give it no stars. Unfortunately, my mother is currently in there (Of which we're trying to change that asap). Where do I even begin? The staff doesn't seem to take her health needs seriously at all. My mom has a history of degenerative disk disorder in her back with several broken vertebrates and a couple of back surgeries, and the resident doctor whom she was seeing not only didn't bother to look at her past records, but told her that she didn't have any back problems at all! Two times, my mother's health worsened, and I had to insist that she be taken to the hospital, due to THEIR negligence. As it turned out, had she not gone to the hospital, both situations could have actually been lethal. Communication between the nurses and the CNAs are extremely poor. The food is absolutely terrible. Many times, there is only one nurse or assistant in the hall to actually help. Often, they will tell the resident that they will be right back to assist them, but then never return. I could go on and on about the horrible experiences we've had with this place. I would not recommend sending a loved one here at all. read more

Unfortunately, my dad is already in this place but I would not put a loved one here unless you were able to constantly advocate for their needs. The bottom line is $ for the corporation. The food is of poor quality. My dad tells me patients yell for assistance and no one comes or does but they say are the only one available. Communication is extremely difficult once inside. Luckily my dad has a cell phone but his phone didn't work for a week. Nursing staff is of questionable quality and definitely spread thin. read more

95% of the staff here are not working for a paycheck. They are working because they care. I think the place is kept clean, housekeeping is fantastic, everything is cleaned on a daily basis. Laundry is returned and hung up within 2 days. The medication is passed within 5-10 minutes. Social work sets up dental, optometry, and assists with applying for call a ride, and assists with anything else. Physical therapy does an excellent job. I went from not being able to lift my leg at all and now can lift 6 pounds on one leg. Therapy allows me to have workouts on my own, which I feel is a BIG plus. read more

Horrible, just horrible. My dad has not even been there a week & we are moving him to a new facility today! He went in for wound care on a Thursday evening dressings not touched until Saturday. He was left on a bedside commode for over an hour without his call light in reach. Not acceptable for a bilateral below the knee amputee. One great nurse but she can't work 24/7. Director of Nursing no help. read more

I was recently injured and stayed for 2 weeks at this facility. The staff was attentive within reason, the food wasn't gourmet but perfectly good for a facility like this one, and the therapy team is second to none! I didn't want to leave!! read more

I cannot give this place even half of a star. I urge anyone please do not take your loved ones to Westchester if you do not have to. The whole entire place is pitiful. Not one person cares about any of the patients, they neglect and abuse them. My grandmother was there 3 weeks and could walk and talk before, she left In ambulance and had half the abilities she had before. She was taken out of there by ambulance. During her time there she was not bathed, not fed, contracted e coli, missed medication. She left with sepsis and we were told she would have been dead by morning if a family member wasn't there to see her condition. Each disgusting instance and mistake was brought to the directors attention and was met with excuses and no level of compassion or acknowledgement. There will be an investigation going on and hopefully the other residents can be saved from this torture chamber they call "state of the art". read more
