I can speak on this directly because I accompany her to every appointment and procedure. My wife was a patient of Dr. Trevor Jorgensen at Northwest Pain Institute in Cornelius for nearly 3 years. She has a very long documented history of legitimate chronic pain due to spinal spondylosis, spine foramenal narrowing, occipital nerve damage, and autoimmune inflammation. She was previously stable on agonist pain medications for over 14 years prior to her provider retiring. After which the nightmare began. My wife's previous provider (Dr. Vladimir Fiks & Bridgett at Advanced Pain Management) abruptly cut her medication multiple times without warning which threw her into unnecessary torturous withdrawals. She was forced the 1st time without any warning or options, and the second time was accidental due to an admitted calculation error on their part. However what was unconscionable was that the error was not corrected and Dr Fiks said "oh well.. I wouldn't have done it that way, but what is done is done." She tried another Pain Provider. He reviewed her chart and stated "Her history is just too complicated for him to take on." Now desperate to get away from Advanced Pain Management she was given a referral to Northwest Pain Institute. Dr. Jorgensen immediately pressured her to switch to buprenorphine, ignoring her concerns about pre-existing mouth issues, weak enamel, and chronic mouth sores. He insisted it was better for pain and downplayed all side effects. The transition was an extremely traumatic experience. She was given vague instructions, no after-hours contact, and no safety net should something go wrong. What followed was an intense withdrawal episode that I can only describe as inhumane. The distress and suffering I witnessed from her experience was heartbreaking, and the office offered no acknowledgment, compassion, or accountability. Months later, she developed severe increased acute rib pain. Her previous provider (Advanced Pain Management) believed it to be a trapped intercostal nerve but offered no diagnostic steps toward a resolution other than a temporary steroid injection, and pain meds. Neither treated the pain or made daily movement and even breathing tolerable. Similarly, at Dr. Jorgensen's office, despite the stabbing rib pain becoming her primary pain concern, she continued to be prescribed buprenorphine and was never properly examined for her principle pain issue. As her husband, I also raised concerns that buprenorphine is known to block opioid receptors--meaning other pain medications wouldn't be effective during procedures like nerve ablations; this was downplayed. When I asked directly, Dr. Jorgensen falsely claimed that "buprenorphine doesn't block other pain meds," a statement easily disproven by numerous medical studies. Knowing she had spinal spondylosis and previous ablations with a provider 10 years ago, she repeatedly asked if diagnostic testing, scans, or other procedures could be done to address the **root cause** of her pain. She was given nothing other than Buprenorphine or a nerve ablation. Not even a referral to a neurologist for an MRI. During her nerve ablations, she was given only a minimal dose of fentanyl, which wore off before the procedure ended. One time, when she was unable to skip her buprenorphine dose the day before, she was left feeling every burn and needle insertion. Even in obvious agony, he refused to offer additional relief, stating she'd received "enough." It felt medieval and cruel. Over two years on Buprenorphine, she developed severe dental decay, repeated infections, and lost over 70 pounds from being unable to eat due to oral pain. I can say with certainty that the only new factor was buprenorphine. Her dentist confirmed that her mouth would never stabilize while she remained on the medication. At our final appointment, my wife pleaded for an alternative--explaining that she was in constant mouth pain, unable to eat, and going broke from dental costs. Dr. Jorgensen refused to see her personally and sent his PA to deliver his response: "No." That was it. No conversation, no empathy, no effort to help someone suffering and pleading for options. Just "No." In my opinion, this is not how a pain management provider should treat any patient. Is it not your job as a Pain Management Physiatrist Doctor to help: 1.) When a patient is in tears begging for help, asking for some treatment of the root cause and not just being fed damaging drugs like Buprenorphine. 2.) When that patient is clearly suffering from the side effects of the very medication they were pressured to take, you dismiss them without care or alternative options, refuse to talk to the patient and consistently send the PA is unacceptable. Please consider all of this carefully before trusting your care--or the care of someone you love.

Dr. Jorgensen is, in my experience, arrogant, dismissive, and lacks compassion. He has a fragile ego and treats patients who question him with condescension. I was a patient at this clinic for 2+ years, referred by my rheumatologist. I have a lifetime of documented pain, injuries, and autoimmune complications and have been on pain medication for 17 years. I qualified for disability on the first attempt due to extensive documentation of chronic pain. Despite that, Dr. Jorgensen immediately pushed me to switch to buprenorphine, dismissing my concerns about mouth sores and weak enamel--even though I told him I already suffered from chronic mouth issues. He insisted it would be "better for pain" and claimed side effects were minimal. The transition process was a nightmare. I was given no clear instructions, no after-hours contact, and no warning about what to do if something went wrong. I followed the instructions on the bottle (3x/day) and took a second dose during intense withdrawal. My husband said it was like watching a horror movie. I later found out I "wasn't supposed to" take more than one dose-(over a 20+ hour period) information no one provided. I was reprimanded by the office manager for not knowing what I was never told. Months later, I developed debilitating rib pain believed to be nerve entrapment. I had been working with the PA (Makayla) for 2+ years, only seeing Dr. Jorgensen for procedures. Makayla suggested seeing a gastroenterologist (for rib nerve pain) and never performed a physical exam. Dr. Jorgensen also told me that being on buprenorphine "doesn't block other pain meds" - which is factually untrue. Research clearly shows that high-dose buprenorphine blocks pain receptors, rendering opioids 90-100% ineffective. He performed nerve ablations with only a small dose of fentanyl, which wore off before the procedure ended. When I couldn't skip my buprenorphine dose before the next ablation, I was in excruciating pain, and when I attempted to discuss, I was told "it happens" His opinion overruled my reality--and medical literature. Over two years on buprenorphine, I developed severe dental decay, constant mouth infections, and lost 70 lbs because I couldn't eat. My dentist told me, "Your mouth won't stabilize until you stop this medication." I was in distress for months. Makayla said she'd speak to Dr. Jorgensen. At the next visit, I was told he said, "She only has fibromyalgia--she's not in enough pain to warrant a different medication." He refused to speak with me. When I said this felt like a violation of medical ethics, Makayla said, "I can't tell you if he cares or doesn't care." I clearly stated I could not eat, I was in constant pain, and my dental issues were financially devastating. His response was still "No." I have audio recordings of this interaction. In my firm opinion, this office treats chronic pain patients with dismissiveness, arrogance, and a shocking lack of compassion. I urge you to find another provider. What happened to me should not happen to anyone else.

When I call to reschedule an appointment because I was just in the ER and was unable to drive due to pain and pain meds, I shouldn't be asked "do you even want this procedure?" when I've had years of showing up and attending my appointments and procedures. When I show up to an appointment and find out, they scheduled it weird, so the front desk couldn't find it clearly and the lady in font repeatedly tells me "we don't do that here, and you don't have an appointment" without verifying in the information with anyone in the back. You tend to get a sour taste for the company and rethink options.

When I finally got in I advised the spasms would sometimes go dormant for 2-3 weeks before returning. Rather than waste the appointment, I inquired about possibly having a separate issue with my back treated. From there it went downhill: **MRI order was sent to the wrong facility. It took weeks of requests to have this corrected. **The earliest follow-up was another 2 months out. This appointment could have been a 5 minute phone call, something that should be considered for patients dealing with chronic pain. **After waiting for 2 months I'm told at the follow up that they never received the medical records from my previous doctor, which they need to submit a claim for the procedure. I pointed out they had 2 full months to get the records and asked why they waited until I was in the office to request them, the doctor said he was unaware they didn't have my records. **Weeks before the appointment, I sent a note through their patient portal to let them know the nerve pain I was originally referred to them for had returned and requested the info added to my chart. No response or confirmation from the office. During the follow up, I asked about next steps with regards to addressing the nerve problem. Rather than write up an order for imaging to investigate, the issue was completely ignored. **Staff said they would send a request for bilateral facet ablations. I made it clear I've only had ablations on my left side and do not need them on the right. Staff doubled down and said they'd submit a claim for bilateral, but the doctor would do the procedure on just the left side if that's what I want. I advised insurance won't approve a claim for bilateral ablations since I've never had the procedure on the right side (diagnostic injections are required prior to ablations). They called me a week or so later to let me know they'd request the facet ablation for left side only. Two months have passed since the follow up visit and I have received absolutely no contact from the office to schedule the procedure. My insurance provider hasn't received a claim from them either. I imagine the excuse is that they're still waiting to get my medical records. That would be ironic, if true, because my spine care team at UCLA (which is very responsive) hasn't gotten any records requests. It is unreal to me how a practice this inept is staying in business. If you are referred here or are thinking of going to SP&SS, please do yourself a favor and go somewhere else. I had hoped to find a local practice to treat my pain, but I don't hate myself enough to wait for this office to get their act together. I'd also like to include an honorable mention of how after having said that I prefer not to take pain medication, the doctor casually asked if I wanted muscle relaxers as we're exiting the exam room. I declined the offer and stated my pain isn't muscular.

Dr. Khemlani met me for my first appointment, accommodating a scheduling conflict to make sure I was seen in a timely fashion. Dr. Kehmlani went to bat for me with my insurance to ensure I got an approved prior authorization. He truly had to go the extra mile. Further, the next day I was seen for the actual procedure. I was honestly terrified. He took the time to stop and speak calmly with me, reassuring me all would be fine. He took time. He showed compassion. He was above-and-beyond professional... but more importantly, he was kind. Providers like Dr. Khemlani are FEW are FAR BETWEEN. Providers that truly express empathy and grace are a rare find any more and I'm quite lucky to have been referred to him. Even better, he has a ridiculously good sense of humor and made the rather unpleasant procedure 100% tolerable... painless one would say. I can't speak highly enough of Columbia Pain Management and Dr. Khemlani. He and his staff were all spectacular and I appreciate everyone for their great care. I had to wait several months for an appointment for this place I mean like it was like six or seven months. 2. When I went in for my first appointment, my ID was a little messed up. I have a puppy and we chewed it. So part of my picture of my ID is not there. But the small one in the other corner is there I had probably 5 to 7 other things in my purse that had my name on it and the front desk lady said oh I can't take this. It won't scan and I said can you use your eyes and look at it you can tell it's me. I have other things in my purse with my names on it other insurance cards with my names on it etc. etc. she said no we can't take it. I said you need to go talk to your office manager and ask them because you could obviously see it just because it doesn't scan it doesn't mean it's not real. And I really wanted to tell her just to use your brain cause she obviously does not have one and she has no care for patience either. Should be back and talk to somebody and said yeah we can take it. But she was still not friendly whatsoever. 3. So I wouldn't end and saw the doctor. I have fibromyalgia amongst degenerative disease, bilateral osteoarthritis of my knee, I've had a neck surgery and spinal fusion (of 4, 5, and six), not to mention other injuries from shoulders, tendinosis, etc. He looked at me and said we don't do pain medicine so the only thing that he could do for me was the joint injections and take over my "" pain medicine which he's talking about tramadol, which does nothing for anybody that has chronic pain.. He was not friendly. And I think he was treating me as a pain pill seeking person. Which I am not I've had this issue for 56 years now. And I rarely ever take pain medicine. Because I detest it and I don't want to ever take the chance of being addicted to them. But my pain has gotten worse and it's unbearable. There are days that I just feel like I'm surviving and not living. It's so bad that sometimes I lay in bed feeling like I'm gonna go crazy from the pain. So I wasted all that time just to go up there to take a pee test, which I could've done anywhere before the appointment and actually done something at this appointment. And they scheduled me for next appointment to "" take over the medication. I complete waste of my time! 4. I got a call from the billing department saying that the insurance would not cover my joint injection so I would have to pay for it. You would think that they would put two and two together that most people that are on Medicaid and Medicare obviously do not have the money for these types of things. 5. So I asked to talk the office manager, and I got transferred to the office specialist or billing specialist. Whatever she calls herself. I asked since I didn't want to waste my time going up there for nothing if they were actually going to do anything for me if I followed through with the appointment. And she said yes, they're planning on doing a joint injection. I explained to her that I could not afford that that's why I had called in the first place because the billing department says that my insurance will not cover it. So I wanna know what other options they have to help with my pain. She said that well, you'll have to go to the appointment and discuss that with them. I said so this disappointment is just to take over my medication and for me to sign a pain contract. She said yes she'll have to discuss it with the doctors. I'm not wasting any more time if they don't have other options. They should not call themselves pain care specialist if they're not willing to actually care for the patients. You would think that a pain specialist were actually taken into consideration what can be covered by interest in what can't and actually do something that is viable for the patient. I hope they take this into consideration, but from the other readings, they obvious they will not. I wish I would've read the other readings before going to this place or I would not have wasted my time. I would also implore to doctors that are referring your patients to this place to think twice. Do your own investigation read the other ratings and maybe speak to your patient that you have referred to this place and see what they're facing. And what they've gone through. I would give them zero waiting if I could or even a negative one if I could!
