H2O location is awesome, great parking, it's for PT and rehab...it's a nice new facility with good equipment and amenities. REVIEW is part 2 of "Aqua Zumba " soo, I originally paid for 5 classes and I've now completed 3. (hindsight fail) Now, all of you know that Zumba is fast dance moves, right? Starts out high impact then eventually slows its pace, correct? Ok...so, tell me, how does a teacher justify teaching water Zumba without: 1) getting into the pool 2) without modifying the dance moves I mean, she does realize we are floating, right? We are buoyant, right? We can't ever move fast, right? Geez! But, hey...these old ladies appear to enjoy themselves with this pseudo workout (and maybe that's why, LOL?!) I too, am 60 so I can say "old", ok, but, I do not enjoy or get anything out of this class. I modify the exercises as best as I can, I'm no expert but, I want to get a workout; and gawd knows the teacher definitely gets her work out on...I mean, she's on land doing regular Zumba exercises. I will complete the last 2 classes ONLY because I prepaid... but, it's a hard NO on any recommendation to Aqua Zumba. My suggestion: Modify the exercises to accommodate water buoyancy...teach the class in the water.

My therapist, Tisha Enriquez, was awesome. She was patient and gentle, but still encouraging enough to force me to do things that didn't feel so great when I first started. And by the end of my six weeks, I was doing them almost perfect - well, ok, not perfect, but definitely made a big improvement! I was quite proud of myself as a 70+ year old! The pool was clean and a perfect temperature for spring. The equipment was easy to use and well kept up. The staff was friendly and knowledgeable. It was a pleasure to get in the pool, move around with less pain, and improve my health at the same time. After six weeks there, I not only gained more strength to help me deal with my disc problem, but also the knowledge to continue the exercises at home now that I am done. Highly recommended and I'll go back as soon as my medical insurance will let me!

I was so pleased with the compassion and care he received. The facility itself is clean, new and has a rehab gym that has mental exercise equipment, occupational therapy equipment and physical rehab. They schedule 3 hours a day and accomplish so much more than a skilled nursing facility. We've been to Encompass in comparison and prefer Exalt.

The Rio is only a year old, so the facility is still in good repair. They have both private and…read moresemi-private rooms. Each room has its own climate control system. Only the private rooms in the Skilled Nursing Rehab side have their own full bathroom. The long term nursing facility side only has a toilet and sink. Things to note: The ice/water dispenser in one of the communal dining areas has black slime growing on the water nozzel and running down the rear splash guard. The tray below the dispenser is filthy and has slime in it. The coffee dispenser has had an "out of service" note on it for a while. I also observed a staff member telling a resident to "shut the f*** up". He had been sitting out on the hallway calling for someone to help him go back to his room, and a staff member came storming out of another room and told him to "shut the f*** up". The only saving grace is this place is new...but aside from that I haven't been impressed.

Number of patients roam hallways or just sit around in wheelchairs. Mom moved which is great, looks can be deceiving. Staff hit/or miss some are warm and welcoming others are rude and condescending. I pray they don't have family members in this zoo. No communication among staff, and patients suffer the consequences. Staff very unprofessional stating my mom didn't have insurance. The insurance changed first of year because plan changed. They received Dec/Jan payment until she was placed back in hospital for pneumonia. But things happen for a reason and she is at a lovely well ran no demirts or fines faculty with people who care.

Turns out why they would want to discharge an elderly because Medicare had stipulations on weight statistics on patients. They LIED and also stated the patient gained 2 pounds which is absolutely should be criminal. This place definitely needs to be investigated!!!! The facility is nice looking. BUT.... If your elderly family member goes in as a patient here please keep a close eye on them. The meals they serve to them are COLD (Approximately 80% of them). The food is horrible. If the patient calls for care, they respond LATE or forget. My family member sat in their own urine for over an hour (this happened twice with me there). My family member lost over 50 lbs in just one month and LIED about it when they were discharged. They stopped patients pain meds for a short time even though patients doctor prescribed it. They denied patient VITAMINS even though patient stated they were weak feeling and requested them. They did NOT keep patient hygiene up to par. They lied and stated patient denied care. They deliberately made patient miss doctor appointments. Patient asked multiple times to speak to a doctor onsite but was DENIED. Seems like this place is run by nurses only. They manipulated Medicare and discharge too soon even though patient also had insurance and need further care. I would NOT recommend this place to anyone.

Greatest lesson learned for our family is to never ever prioritize the convenience of being physically near your loved one over quality of care. We faced multiple challenges, but here are the top cons: 1. Bedsore/Wound Care Issues & Lack of Care: Despite entering the facility with clear skin and no open wounds other than a surgery scar, my father developed bad bedsores (puss-filled, hole in flesh) which is simply caused by neglecting and delaying the repositioning of patients and not attending to their basic hygiene needs in a timely manner. The family reported the initial bedsore, not the staff, and the wound care physician didn't see him for nearly two weeks after. Dad needed to have a surgical procedure by then. 2. Lack of Medication Management: Meds have been administered multiple times without nurses performing the essential blood pressure check first, which could cause a major medical crisis. He was also given an increased number of pills at one time, which was greater than the max he should have received. After multiple inquiries, we still don't know any details. My father decided he did not want to ask anymore because "at least he did not die." 3. Severe Gap in Overall Medical Wellness Coordination: We didn't see the different disciplines working together to provide holistic health care. For example, he was being moved in a hurtful manner, etc. by nurses and aides who told us they were not aware that he even had a hip replacement! He was a heart patient and had bedsores, but there was no attention to diet, which is key to healing. There were multiple instances of one team member reporting an issue that was then not addressed by the nursing team. We waited about a week to have an issue with stool be addressed, which also caused major concern for infection with the bedsores. This is detrimental, especially for the elderly. Despite reporting his worsening condition, including pain and diminished mobility, there was a notable absence of timely medical intervention -- the attending or servicing physician did not see him for two weeks even though we were being told that he had been seen. Finally, we did not have an official, informative "plan of care" meeting until 15 days after admission. When my dad became very ill a few days ago, after waiting for some time, we decided to request that he be taken by ambulance to ER vs. chancing waiting even longer on a response from staff given their history. He was admitted to the hospital that day. 4. Consistency/Quality of Care Gaps: Having temp agency workers in and out resulted in a compromise in the consistency and quality of care provided. Patients and families should not have to tell staff what's going on and what's needed medically. It's important to note that these issues occurred even with the family paying out-of-pocket for a day-sitter and having our immediate family checking in on him + stay with him throughout the afternoons and evenings. That indicates to us that as soon as the family or friends are not around, things are dropped. Despite these issues, the therapy provided, particularly by Mr. James, was commendable, with thorough explanations and personalized attention. There are several others who I don't have names for, but PT and speech are included, and my father bragged about them. Additionally, I can say the Director of Nursing, Karla, addressed our inquiries and concerns when I brought them up to her. The administrative staff also demonstrated professionalism and helpfulness in navigating insurance and care-related matters. We met some very caring and professional care team members during 2 two-week stays in the last 45+ days (Stephanie, Johnnie Mae and 2-3 others who I can't name at the moment, but unfortunately, the majority of the others are no longer employees. I could not, in good conscience, send my father back here.

Both my wife and I have been patients of Hope Rehab. I twisted a knee that did not require surgery…read more(couple of years ago). She had a broken ankle that required surgery (ongoing). Rob and his team of therapists did an excellent job of getting both of us back on track and better. My therapy lasted a few weeks and ended about 18 months ago. My wife's therapy just started and is ongoing. The front office staff communicates well and the PT staff does an excellent job with the balance of pushing you to do well during your therapy but not so far that you feel like you've gone too far. Bottom line - An easy five-star review. I highly recommend them if you ever need physical therapy.

My…read moreexperience, although I am not proud of the reason I had to come, was one of the best decisions I have made in my life by far. I was treated with the upmost respect and kindness while still understanding that I was there to get well. From the receptionists, to janitorial staff, they all played an important part of my recovery. I have to give a shout out to Zac the V.P. who counted it not robbery to come out and fellowship with his clients, rather it was planning a 4th of July event or hanging out with us a the Movie Theatre, he was all in! Into Action Recovery Center (Houston) planned events that nourished our minds, souls and physical bodies. So from me, Dan Jackson you receive three thumbs up and five Stars!!!!!
