He slept in a recliner with metastasized pancreatic cancer for 5 days before his hospital bed arrived, no sheets were provided and they had not made any arrangements for him to be lifted from his recliner into his hospital bed. Further, I was told that they provided 3 visits per week from a nurse's aide to do things like bathing, shaving and so forth, but today marks an entire week since he came home and only after SEVERAL calls and text messages did they finally send someone on Saturday who said she was UNABLE to do all those care items because poor accessibility due to him not being in a hospital bed! She also noted that we should have already received all the supplies for her to use (bed pan, wash pan, etc), yet they have still not arrived after a week with Vitas! Today I asked when I could expect an aide to return since it's been a week and they only showed up once after I had to beg them to send someone, and they told me that nobody was on a schedule for this week!!! His "nurse" called me shortly after I complained and said that while the aide should visit 3 times a week that they'd only bathe him once and that "the family needs to step up and help some"!! He's been home for a week, sleeping in a recliner while Vitas has had a nurse come over twice to check his blood pressure and temperature -- not exactly wonderful care! If you want your loved one to die with dignity and you don't want to have to beg hospice to provide a minimal level of service, avoid Vitas at all costs.

Before admission, I specifically confirmed with facility staff that her prescribed oral chemotherapy medication could be administered at Sandgate, provided we brought the medication with us. We were told this would not be a problem multiple times and were instructed to bring the medication, which we did. Upon arrival, the facility was not prepared with the breathing equipment she required. They reluctantly looked for and found the equipment. My wife had not eaten for many hours due to a 3- hour ambulance ride, but I was told it was too late to get her food, so I had to leave and purchase food myself. Her room also had several issues, including no guard rails on the bed, and non-functioning TV and air conditioning. I was told I was not allowed to stay the night with her. At approximately 4:00 a.m., I received a frantic call because staff could not locate equipment needed to assist my wife's breathing and I heard my wife gasping in the background. The equipment was eventually found and the situation was rectified. The next morning, a physical therapist and I arrived at the same time just in time to prevent my wife from falling from the bed. When I asked whether she had received her chemotherapy medication, I was informed that Sandgate could not administer it after all and she must go to another hospital. This directly contradicted the information provided several times to me, Moffitt, and my wife's case worker before admission and was the primary reason we chose the facility. Efforts to transfer my wife to a hospital were delayed, and there were significant communication and coordination problems throughout the process. I was told by nurses to make sure I got the director's attention again before 4:30 because she leaves the office regardless of what is happening. They also did not seem surprised by the misinformation I was given. Later that day, after my wife's vitals had not been taken for 8 hours, a nurse determined that my wife required urgent hospital care, and she was transported immediately to Lawnwood Regional Medical Center. My wife passed away at Lawnwood four days later. While I cannot say whether the outcome would have been different if she had gone to a competent rehab facility, I was deeply concerned by the lack of preparation, misinformation, communication, and consistency regarding her care, as well as the condition of the rooms. Based on my experience, I would keep my loved ones far away from the facility.

The telephone number attached to this listiing is never answered and no calls are returned. There is no value for the children involved, because any child unfortunate enough to go there will be greatly disappointed. And if, as a parent, you were careless enough to send your child there, you would be doing them a great disservice.

First the good: most of the nurses there were quite capable and caring. Special shout out to the night nurses Osbaldo and Marie as well as the day nurse Evelyn who were all excellent and responsive. The CNAs there were just average with only one, Charity, being a notable exception. She was always working and very good to me. Some didn' t seem to care at all. The physical, occupational, and speech therapists were all excellent and dedicated. The dialysis department was outstanding with Sheyla being a delight. On the negative side, the administration was quite poor, rarely following up, getting things wrong, telling me something was done that wasn't, matching up very ill patients who need constant care with those who don't. Putting me in the wrong dialysis facility after discharge. Had to be changed. The facility does not make it a priority for patients to get a proper night's sleep, They come in at all hours of the night sometimes just to take out trash. Hard to get more than a few hour's sleep. The food service needs to be overhauled. Quality is poor, and more importantly the diets that were carefully detailed were not followed at all. As an example, I was on a renal diet that was restricted and was constantly fed off limits food despite repeatedly telling them in writing that the food was not appropriate. Access to doctors were limited. I only saw my primary doctor once in almost 4 weeks and that was after constantly asking. Once you could see the doctor they were quite capable and professional, This could be a higher rated facility but they need a lot of improvement in important areas.
