However, the therapy department staff is knowledgeable and friendly. Katish and Moe deserve 5 stars, and so does Kevin. The nurses are actually better than expected. I hear what they have to put up with. Some of the names I have heard night staff be called because they weren't there immediately. I get why the aren't in the best of moods, 4 stars. It is not like this happens often but I am not the one who upset you. Now there are a lot of people who come in and out. Not all of them nurses and I cannot tell who is who sometimes. Especially when I am half asleep. Now for Maxine. There aren't enough stars available by their scale. 10 stars. She has been amazing, she consistently goes above and beyond. I thank everyone staff wise. It is the facilities you lose a star on maybe it should be a 3 star because them, but the staff gives you a boost. If you have options and can pick somewhere you go. I recommend going somewhere else and for me Staff has been great but the facilities, equipment and pest issue. Until these issues are corrected. do not choose here.

    Heartfelt Review of My Sister's Care…read more My sister is currently a resident at this facility, and as both her family and a nurse, what I have witnessed has been heartbreaking, overwhelming, and unacceptable. I feel compelled to speak out so other families can make informed decisions about where they entrust their loved ones. My sister has a deep, gaping wound on her coccyx -- roughly the size of a grapefruit. Despite the severity, I have not seen the level of care, urgency, or clinical judgment that a wound of this magnitude demands. She spends nearly all day in bed, wrapped in layers of blankets -- not for comfort, but to mask odors that should never be present if proper wound care and hygiene were being provided. What hurts most is that I tried to have her transferred to a facility closer to me, where I could visit more often and personally monitor her healing. Instead of being treated as a concerned sister and a healthcare professional, I was dismissed and spoken to as if my knowledge and advocacy meant nothing. I suggested a wound vac -- a standard, evidence‑based intervention for wounds like hers -- and was told she "can't tolerate it." I can't help but question whether this decision was truly about her tolerance or about the facility being for‑profit, prioritizing cost over care. If my sister didn't tell me what was happening, I wouldn't know. I work long hours, and I trusted that she would be cared for. That trust has been broken. This review would be completely different if I felt the staff respected family involvement, honored patient‑centered care, and communicated openly. Instead, I've been made to feel invisible -- and my sister has suffered because of it. They may have changed their name, but they have not changed their care. I urge families: Do not send your loved ones here without thoroughly reviewing their history, inspections, and violations. Your loved one deserves dignity, respect, and competent care. Through all of this, I hold onto faith. God is the greatest of all time, and I pray for healing, protection, and accountability.

The moment you walk in, you're hit with the overwhelming smell of urine. When she rang the bell for assistance, she waited--and waited. Yes, we understand aides have other residents to tend to, but waiting 45 minutes is completely unacceptable. Especially when that delay led to my mother falling--something that never should have happened. then to cover it and tell us that she didn't fall, because her bottom did not hit the floor stating she slipped out of her wheelchair per the nurses on duty, no report ever was written. Let's talk about the lack of basic care: they would take her to the restroom and not put her oxygen back on. We found it multiple times just lying on the floor--completely negligent and dangerous. But the final straw? She was left in bed all day long, struggling to breathe with pneumonia water in her lungs and a horrible UTI .I had to call and insist she be sent to the emergency room. Did they send her? They waited until my brother arrived 40 minutes later and asked him again if she should go to the hospital, as of today, my mother lays in the hospital fighting for her life. Our family is heartbroken and outraged. In our opinion, this facility should be shut down. We will be making a complaint and calling onbunsmen on them . Working in the healthcare system myself you bring your loved ones to be cared for not to be left in their bed struggling to breathe because of the lack of care our loved one received. Stay away from this facility! I would definitely rate this place a negative one.

She recuperated from a mild stroke(heart event) after 3 months.Because everyone, syblings,people she knew , associates and this nursing home were seriously after her money, my mother was held and drugged against her will, deemed incompetant,and remained there for 8 years. In that period of time I visited her, and supplied her with clothing, books, food,journals, photographs,letters,cards,music etc. and as much phone contact as I was allowed by this nursing home. I lived 3800 miles away. I attempted to get her legal support, but this nursing home would not allow the lawyer to visit my mother.To get her a phone, and a doctor to remove the wax in her ears so she could hear again was a legal struggle for me. I had to hire a lawyer inorder to fight off as much corruption as I could.The administration robbed her of a decent supportive enviroment the last 8 years of her life. She contracted sepsis and I was never contacted...returning from the hopspital to this facility she fell and was moved an Okeechobee nursing home facility. Laurel Point did not forward her medical information as the Okeechobee nurses were unaware of mothers COPD needs. Laurel Point also did not keep up with her life insurance payments making her policy closing 8 months before her death in 2018.This woman, my mother, was so unindated with villans and theives it was a horror show. My own attorney dropped the case because she stated "she felt like she was in the movie "Deliverance"...she was, I was, mother was. Do not under any circumstances put your relatives in this place. My sister was the one that put mother here and left her here.Mother asked the court that I be her guardian and the Okeechobee judge denied her request. The court appointed lawyer for mother ,(which she did not need, because she could have afforded one)...was paid with her money...the lawyer for my sister was paid with her money...the nursing home was paid for $6000-$7500 per month, with her money...my lawyer was paid with My money. This was a medicaid subsitdized nursing home. My mother paid top dollar! Do you see where this is going?Do you see how much money was involved? When I pursued this nursing home for neglect, they stated it had been sold and changed names in 2018. Apparently I was lied to ,by whom- ever answered the Laurel Point Nursing Home phone, this happened for weeks after her death. Don't go here! is my warning to everyone....

The day I saw my mother for the first time (3/13/2021) was "supposedly" the day she had arrived from Lawnwood Hospital. When I saw my mother, I was appalled and in so much shock. They had her all the way in the back of the facility. No television. No phone. Nothing and barely naked. She had bruises ALL OVER HER BODY!!! She kept crying that she's been sitting in her own feces for 2 days. I kept reassuring my mother that she would be okay and that the new medication was making her hallucinate (or so I thought bc thats what the nurses told me). She kept saying they were injecting her with laxatives and then letting her lay in her bed but I didn't believe my own mother. I wish I had. Now I will admit, my mother is known to be a bit over dramatic and somewhat of a diva. The nurses there were already giving me attitude and staring me up and down. They act as if getting up is SUCH an inconvenience to them. It's pathetic. On 3/15/2021, I tried to visit my mother again. This time, I was told that when a new patient arrives, they have to quarantine for 10-14 days. So then WHO LET ME IN THE FIRST TIME??? Okay, fair enough. I understand and respect that. On the 15th day, I was so excited to see my mother. I walked up to the front with a stuffed animal, her personal items and a boutique of roses ONLY TO BE TURNED AWAY AND TOLD I WAS NOT ALLOWED INSIDE! As I did my research, I realized my mother is unaccounted for on 3/12/2021. My mother was RELEASED FROM LAWNWOOD HOSPITAL ON 3/11/2021 and documentation shows that someone from SANDGATE GARDENS signed receiving my mother as a patient on 3/11/2021. WAIT A MINUTE..... When I went there on 3/13/2021 - the nurses told me that my mother was crazy, a liar and hallucinating. I had them print me a form that showed the day she arrived and it still says 3/13/2021. TURNS OUT - MY MOTHER WAS THERE FOR TWO DAYS BEING INJECTED WITH LAXATIVES ONLY TO HAVE TO SIT IN HER OWN FECES FOR 1-2 DAYS!!!!!! Turns out, my mother was not lying nor hallucinating like they stated. They were abusing my mother. Neglecting my mother. Refusing to help and give my mother her medications. Once I mentioned about the investigations I've heard about this place, their attitude changed QUICKLY. But now, we are back to square one. Doris H. is the most unprofessional "manager" I have ever met. She was extremely neglectful in regards to my questions. She refused to discuss anything with me in regards to my mother EVEN THOUGH I HAVE POWER OF ATTORNEY OVER MY MOTHER. India is new and at the front office but definitely needs an attitude adjustment. Then there is Alexis, who has ignored MANY of my voicemails and calls. It's so sad that people who work in the medical are stealing my mothers prescriptions, injection her with laxatives, making her suffer as well as not allowing anyone to see her. They forbid anyone to see her because they are EXTREMELY NEGLECTFUL. I can't wait until she is out of here. We have an attorney as well as a few social workers that have been visiting and investigating this place for us. I WANT SANDGATE TO KNOW HOW HORRIBLE OF A FACILITY THEY ARE. THEY NEGLECT THEIR PATIENTS. THEY ABUSE THEIR PATIENTS. THEY STEAL FROM THEIR PATIENTS. If I had more money, I would get her out immediately. But unfortunately, I do not. All I want is my mother to be taken care of and it's not happening. They are keeping her away from her own family like she is a prisoner. To me? That seems like they are hiding her because they are abusing her. I will not stop until this place is shut down or I sue them enough to buy the place and demolish it.

When I 'd pop in her hair was getting done or had already been done. She was in the memory care unit and never smelled when I saw her. She was gaining her weight back and was beginning to interact again after a considerable downturn at a horrible place in Jacksonville Florida landed her in the hospital. She has since passed but I am very appreciative of the staff there.

He had been in and out of intensive care while being bedridden in a different rehab and skilled nursing facility. After many months in that facility he had recovered enough to eat and swallow soft food but he was still unable to walk. Then he came to Lynmoore, just before Christmas in 2023. My wife and I live in California, so we can't visit him in Florida that often, but we did visit for a few days in June, 2024. What a difference! We walked in one day just after his physical therapy session ended; he was walking back to his room with the therapist by his side, not in his wheelchair! That never happened before. This alone would rate five stars, all gold, but there is more. During our visit we interacted with many people: the nurses, the therapists, the community engagement director, the records office, the business office, even the dining hall staff. Everyone was friendly and professional. Residents coming and going were greeted by name. The buildings were clean and well maintained. The food was good; I ordered the same thing that my brother had when we ate together in the dining room.  The affiliated home healthcare and pharmacy services are excellent; I continue to interact with them. However, there is even more to the story. My brother was in such bad shape when he first came to Lynmoore that, during our June visit, a couple of staff members admitted to me that they didn't think he would last in assisted living; he was that bad off. I truly believe he was admitted and flourished because the Wellness Director believed in him, motivated him from the very beginning, and everyone took good care of him around the clock.

My mother was admitted with a broken ankle and I wish I never drove her there and made the extra long trip to Port Saint Lucie instead. Patients are just empty vessels for the staff to get their paychecks. No bedside manner, no extra care, no soap in the bathrooms, my mother is afraid to drink water there because no one was there to assist her in the middle of the night to use the restroom as her bed phone fell on the floor and she could not see the button to push on the bed. I requested the floor manager nurse and corporate contact information and I was told that reception would have someone call me an email me. Still waiting for someone to contact me after a week! I picked up my mother today to hear more bad news about a nurse letting her sit in urine for an hour after she notified the nurse the bed pan spilled. Her ankle started bleeding and she asked for a new bandage and was denied!!! Really!?! Hope she does not gwt a staphinfection or worse. Our concerns will be promptly filed regarding the hospital neglect, cleanliness, unsafe stafging ratios, and unhelpful staff with AHCA, Florida Department of Health, US Department of Health and Human Services, Office for Civil Rights, and the FL governors office.

I was taken…read morethere by a rescue squad and once there of course they tried to address the condition that the EMT team indicated was my condition. Then what is report accurate or inaccurate becomes part of the hospital notes. I am over 80 years of age. So the hospital ties me to the bed. Puts cathedra in me and won't let me out of bed for FOUR days. I do not recall getting any food, and on top of that when a make shift nurse is about to put more medication in me I ask what it is and to see the Dr. Signature. That did not happen. My wife witnessed a couple of the test done and indicated it could not be accurate. After 5 days I was released from the hospital on medication. I called the hospital and told them the medication was really making me sick. Of course I could not change without Dr order. I ook the medication for two weeks and finally checked myself into Cleveland Clinic as I could not eat sleep and was just getting weaker. After 2 days there the prescription was changed and was already feeling better. The medication HCA put me on was messing up my body chemistry etc. No sleep, can't eat was not good. Bottom line stay away from HCA hospitals and I might be add HCA doctors as they all believe the report coming out of the hospital even if it all BS. Here a second opinion was very important. I told the Dr at Cleveland Clinic to trash the report from Lawn wood and generate their own report. Here a second opinion was the best I could do. or I might be talking my dirt nap at this point.

    My experience at QPR (Quality Performance Rehabilitation) in Fort Pierce, FL, was unproductive,…read morevery disappointing, and regrettable. I went for approximately ten physical therapy sessions and in that  time, I had four different physical therapists. That is a lot of different 'hands on' in such a short time. Other physical therapy clinics have agreed this was "unusual" and "not ideal." If consistency of care is important to you, then consider this problematic policy of using many different PT's to treat you.  The interior set up is quite cramped and lacks privacy. During these days of Covid-19 and 'social distancing,' I would be very concerned about the physical limitations of this particular facility. Most importantly, I had an entirely new medical issue added to my condition, an impinged ulnar nerve probably caused by an overzealous, part-time physical therapist. The extent of this injury is still unknown. I spent the majority of the ten sessions trying to 'undo the harm' that was done. So, not only was I no better off than when I started treatment, I was demonstrably worse. When I approached the owner he apologized, but did not want to take responsibility.  I take no pleasure in writing this review but thought it was my obligation for others to know before they consider attending physical therapy QPR in Fort Pierce.
