I brought my entire medical record, printed from my provider's patient portal. It included documentation of every visit since I became a patient, listed every diagnosis with CPT codes, each med I'm taking, all the labs I've ever had, etc. There was no "signature" but isn't the purpose of the visit with the MMTCFL doctor to verify each patient's medical issues and then independently make a recommendation for a medical marijuana card? The receptionist told me multiple times "the state of Florida won't accept records without a signature". Why are they uploading another doctor's signed records for their certification? The bigger question is, why the deception? Why don't they tell people exactly what their requirements are? Instead they let you come to their office for your appointment and find out. I left without even seeing the doctor. What a colossal waste of time.
