I would like to add this, make your appointment far enough in advance to allow the doctor to review your documents (I believe I gave it approximately 3 days). Ensure the paperwork you provide has your medical conditions documented in writing, you are gonna have to show proof. Be sure bring a copy of the documents with you on your visit, technology doesn't always work on our side. Providing all of the information he requests prior to your appointment will help the process.
