No patient intake…read moreevaluation after over 24 hours of transfer, CNA and Director had not reviewed hospital discharge instructions or act in over 96 hours and still hadn't after complaining. Patient still in hospital gown and not washed or cared for in 36 hours after arrival, despite having personal clothing and supplies, which were still in a bag tossed on the floor. Room and bed infested with bugs resulting in bites on patient, with no care provided to patient. Their solution was to move to a different room, but was not going to inform or move room-mate. Over 30 min. wait for call light response to use the bathroom, which resulted in self-care resulting in a fall, at which time the CNA told the patient to get up on his own, and failed to report. When director was questioned about the level of care we were suggested to just have the patient moved to a different facility. Call light buttons taped together or out of reach of patient. Extremely unhappy staff and was disclosed to us directly by them. Multiple regulatory violations observed which should be criminal. For the amount of money they receive from Medicare and the level of care provided, the lack of attention and care not provided should be criminal.

After spending time in the hospital and not moving much at home, I started with very little…read moremovement in my body. When I started at Optimal, I wanted to stop going because of all the pain and muscle soreness. I thought they were being hard on me, but it was so good for me. After several weeks, I noticed that I could do a lot more without having pain. I am so much more active now. They are the best in the area. All the staff are there to help. After filling all the paper work and given them my insurance information i was completely covered and approved for 20 sessions. The staff is polite and friendly, but that is probably all i can say on the positive side. After the 20 sessions, i felt no improvement so i did not bother to contact insurance for more approved appointments. I moved out in 2019 to my own place, and was contacted by collections in 2023 (5 years later) for an unpaid $500 bill. Allegedly they had mailed multiple bills to my old address and it was never forwarded to me. I work for GM and at the time we had theramatrix as our insurance provider, but parted ways in 2021. Im not going to get in the details of the run around Optimal Rehab put me through to confirm with my old insurance that i had no out of pocket expenses or deductibles, but they basically wanted me to provide the EOB (explanation of benefits) to confirm this information. I was told by the owner if they were at fault for filing the insurance papers incorrectly, he would apologize million times over. One apology would have been sufficient, but after not hearing anything back from them in the last three weeks, i assume they have taken care of the matter on their end. Hopefully they do not try sending the $500 bill to collections again in 2028. Mind you, though my address had changed my phone number on file stayed the same the entire time but it is not their policy to call you in regards to a 5 year past due bill. My recommendation would be to go to Diversified Physical Therapy. They have way better techniques and equipment to help with your physical therapy. They also know how to properly enter in insurance information. If you do end up choosing Optimal Rehab though, triple check to make sure they are entering the correct information on each of your appointments so nothing similar happens to you. More importantly, hopefully whatever injury or soreness you are dealing with gets better wherever you end up seeking help.

My doctor recommended 12 sessions of physical therapy all fully covered by MediCare and Blue Cross. There are other options in Frankenmuth, but thank God I chose Diversified Physical Therapy, their highly qualified and pleasant staff :-) Lisa and Christine had me out of pain by 2nd visit, then taught me new habits, stretching and wellness exercises I now do regularly for sustained good health. I initially chose Diversified Physical Therapy as they were highly recommended and have a special connection with Diversified Fitness Club. After initial 12 physical therapy sessions, Sara set me up with an individualized next level fitness regimen at the gym with sauna and steam rooms that boosted my mind, body, spirit now feeling better than ever.
