My elderly family member was admitted here short-term. Within the first 18 hours alone, we experienced multiple alarming issues including missed medications, repeated unanswered call lights, transportation failures, unsanitary conditions, etc. SOME issues during the stay were: - My family member repeatedly sat in feces for extended periods waiting for assistance after pressing the call light that resulted in skin rash. (Unanswered call lights were a HUGE issue at this place). - There were flies in my family member's room, they gave my family member half cooked meat (pink inside [NOT STEAK]), & there's strong foul odors throughout the facility. - Dialysis transportation coordination was HORRIBLE to the point where family member almost missed the dialysis appointment twice because they did not coordinate the rides properly at all. I have no idea how this place is still open & running. I've informed Kaiser that they should take this place off of their list. Weeks after my family member was discharged from this place, I spoke with a different social worker and this social worker informed me that "Many families have complained about that facility." My mother was…read moreplaced there following hospitalization and required very specific medical care, including strict dietary restrictions related to a feeding tube and recent surgery. Unfortunately, there were serious communication and care concerns during her stay. Despite repeated efforts to inform staff about her medical restrictions, she was given food that she was not supposed to have. This led to complications, including her feeding tube becoming dislodged and requiring an emergency hospital visit. Even after the hospital provided updated instructions, it felt like important information was still not being clearly documented or followed. In addition to the medical concerns, it was very difficult to get timely responses from staff, and we often felt that our concerns were not taken seriously. This created a great deal of stress and worry for our family during an already challenging time. We ultimately requested a transfer to another facility closer to home so we could better advocate for her care. I strongly encourage families to stay very involved, ask detailed questions, and closely monitor care plans if their loved one is placed here. I am sharing this experience so that other families can be informed and prepared.

I'm assuming the social worker would know what the care that my dad would need there. As having a Plorox catheter to have fluid drained from his lung. A urinal catheter. I had talked to a nurse Friday cuz my Dad was supposed to have his lung drained every 2 days the staff there did not even have the plurix bottles kit they only had one bottle there after scrounging around for 5 minutes and their facility room... I actually had to go home to get the rest of the kit thank goodness I live only 10 to 15 minutes away. They were supposed to have ordered new equipment far as Friday my dad said they only ordered one bottle that he told me today which was only a 500 l bottle and that did not even have the kit so country oaks was unable to drain my dad's lung today being a Tuesday.. they had to send him over to the emergency room back over to Pomona Valley because they did not have the equipment to do it this is very unsat for a care facility of 78 people as I had talked to one of the directors today... Also meals there are horrible as I've read in the Yelp reviews in the past and I could see why they have a 3.3 rating their sandwiches only have one piece of meat on there which is probably about five thicknesses of a piece of paper.. my dad even requested to have a grilled cheese sandwich one night and the people says they can't make a grilled cheese sandwich my dad asked why they said because they were out of cheese"""really the other night he had a pizza there which was on a tortilla with tomato sauce and cheese.... I don't know how that's called a pizza but in the Mexican culture I think it's called a quesadilla.... But however their menu said Pizza... I think my argument over the phone was pointed in the right direction... One thing I do have to say is the place is air conditioned compared to another place that we looked into a couple days that had two safety violations that was there for the first 10 minutes I was there.. course the man next to my dad was coughing and the doctors now saying that my dad may have pneumonia again as my dad is a stage 4 cancer patient with a very weak immune system that was a reason why I wanted to have a covid test done on him he was negative thank goodness. However I do not recommend country oaks since they do not have proper equipment to meet patients needs if you're going to accept a patient from a hospital you need to know what their needs are if you cannot make their needs why make the patient suffer and have to go back to the emergency room because you have a lack of equipment...... My sister, Deborah, contacted different addiction specialists and she found one from Pomona. I agreed to meet with the doctor and they brought me in this place. I thought, there was no hope for me to recover because I always experience relapse, and it was hard. But thank God, I was able to recuperate. The claimed weekly 5-day therapy was not conducted, job entry with no integrity. Years ago, my family ever underwent a rehab in a nursing home where occupational and physical therapy performed progressively with encouragement and reward achievers with certificate. Medical staff are professionals whose dedication is admirable. The knowledge and skillset should be applied to motivate and reinforce a safe, correct and healthy living with risk mitigation.

A girl answered and I gave her my name and explained she knew exactly what I was talking about with the insurance and then asked for my group name. It's new and I just changed it. So I said I don't know what it is they didn't give it to you asking questions being confused, I then offered to give my medical number and as I did she completely cut me off and in a rude tone says "like I said I need the group name". And I used to be a medical assistant and to speak to a new patient like that is an absolute no.. so I called her out and was like it's very rude how your speaking to a new patient there's no reason for that I'm confused and just asking and since she said she was aware I figured she knew. Then she's like I'm not gonna sit here and argue with you and I told here that I wasn't arguing I was just sharing how rude she was being and thought she would know or explain what I need instead she was being rude. She then said I was yelling at her and I absolutely wasn't yelling I explained to her again what she said and why I told her why I was bothered about her cut off and tone and she then sighed said I don't have time for this I'm not gonna have someone start my day like this. I was like you're the one with the attitude because I called you out is why you're mad so have a better attitude ... which then ignores me aand I can here someone started talking to her. So I asked if I can speak to someone else she said nope only me. So I said then what do I need should I have my insurance call you ignored me again talking to someone else. I could hear her talking to someone in the back she was asking how does she block a number. So I ask the same question about my insurance calling to verify. Then she said yeah have your insurance call to give me the group number. I said okay hopefully there will be a better attitude if I call back. My insurance called and they told me they'll call me back to schedule an appointment. I don't have much faith they will or especially her since there's no one else but we'll see if they actually call me back so we can see our dr asap.. phone call wasn't at all what I expected hope my dr is better . I'll post an additional review about them scheduling or not and my visit if it happens!
