We had to find a place fast for my mother in law after her surgery and Trellis kindly accommodated her and our family. New facility, literally walking distance to Chino Valley Medical Hospital. Safe neighborhood, easy access location - very close to the 60 freeway. Facility is big and very clean, nurses are attentive and rehabilitation center looks excellent. Daily rehabilitation with patients and very good food service. My mother in law has her own private suite room with a window and the bathroom that is very big. DO NOT TRUST THIS FACILITY WITH YOUR LOVED ONE. If your loved one needs professional medical care after a hospital stay, please think very carefully before choosing this facility. While the building looks nice and the rooms are private, that is where the positives end. In our experience, the medical oversight was deeply concerning. The physician refused to adjust treatment plans despite clear and ongoing signs of decline. Even as our loved one showed both objective and obvious symptoms worsening over time, no meaningful changes were made. There appeared to be a lack of proper assessments, and when serious issues were identified -- primarily because family members insisted on testing -- there was little urgency or corrective action. Staff did not consistently follow orders and did not seem to recognize signs of medical decline. Concerns raised by family were often minimized. For three days we requested that our loved one be transferred to the ER due to significant changes in mental status and severe migraine symptoms. The physician refused. When EMS was finally called, emergency personnel immediately recognized the seriousness of her condition. We had to sign a waiver to transfer her to a hospital 20 minutes away because she was considered unstable -- yet staff at the facility continued to say she was "fine." Within days of transfer, her condition deteriorated to the point where she could not speak or swallow and became nearly unresponsive. We were preparing funeral arrangements. She later described experiencing severe hallucinations and believing she was in hell. It was profoundly traumatic for her and our entire family. We also discovered she was not receiving the correct dosage of a critical medication that had been ordered by her previous facility. This likely contributed significantly to her decline. Had we left her there even one more day, we firmly believe she would not have survived. Our loved one had none of these issues prior to admission to this facility, and since receiving proper hospital care and being restored to her previous treatment plan, these issues have resolved. We will be filing formal complaints with the appropriate medical and licensing boards. In our opinion, this facility is not equipped to safely manage medically complex post-acute patients. If your loved one only needs stable rehabilitation, it may be adequate. But if they require close medical monitoring, I strongly urge you to consider other options. Our family went through an avoidable nightmare. Please advocate aggressively for your loved ones. read more

Stop allowing students and employees to park in your parking lot in all of the front spots, including handicap spots & not using a handicap placard (after coming here for a while, you start to learn who's car is who's). When we come to visit my family member, I'm often unable to find any parking, including any handicap spots (I have a placard, and the cars in these spots never have one displayed). I see employees and students lingering in their cars in the parking lots. They should be in the overflow lot across the street. Today I was unable to visit my family member, because the parking lot was completely full of students and employees. To add: the receptionist is often not present, and technically anyone can make their way into this place & go anywhere they want, making it seem unsafe for our elderly family members who reside here. My husband has received care at Trellis twice. The entire staff is amazing. They are friendly and attentive to all residents. They take time to answer many questions. The facility is always clean. I would highly recommend Trellis to anyone who needs care. You won't be disappointed! My husband has been there 6 months and will be coming home soon. read more

Because this is my second time at Trellis my favorite person is Adrian Racpan he is the administrator here and Adrian is there when I call him at 3am 4am he answers and believe me I have called and those times. if you truly love your family member i suggest you dont take them here. A lot of neglect happened to the patient when I came to visit their. Their were couple of times where the staff were no where to be found when i needed them. When i asked one of the male nurse to come check on him he would cowardly refuse and said he was ok. When i called for help the management T picked up he seemed to be annoyed to help. i give them credit though while the facility in Trellis Chino is indeed clean, the lack of help my family went through was poor. Trellis is a very clean facility, the staff is friendly and the environment if beautiful. My mom said the food was very good too.

My father in law was admitted in February, not walking, very weak and it wasn't looking too good. As time went on he progressed tremendously with PT/OT/ST. He is now walking, eating regular food and back at his baseline. He loved Trellis so much he didn't want to leave lol it's a very clean facility and the staff is really attentive, friendly and nice. His DR was DR. Velasquez and he was great hands down!! Overall his Trellis stay was amazing and they really did a great job with getting him back up and running. My mother who is 87 years old stayed at this facility for about a month. Despite my mother's language barrier, impatience when she needs something (rings the bell repeatedly), and her adverse attitude towards healthy meals, she was treated very well. The staff did their best to communicate with her (some actually used translator apps), they didn't get angry at her when she kept on ringing the bell, and accommodated her fussy eating habits by providing alternate meals. All the staff that I came across are professional and polite. Amanda, responded to my communication in a timely manner and got any issue resolved immediately. Yvonne, had things in control at the facility, and Magy, the awesome physical therapist who helped my mother to get back on her feet. Magy went above and beyond her assignment to make sure that she was comfortable and received all the exercises that she could handle. If you live close to Trellis Chino and need a temporary place for your loved elderly relative, this is the place to choose; look no further. Having had experience with 2 other "skilled" nursing facilities I must say this place has been a godsend! We had Casa Colina to compare with which gave great care and had good rehab but it is more of a hospital setting so not much room to move about and not being able to have grandchildren visit was tough since age restriction was 14 and visitation hours were strict. Then Glendora Canyon was another to compare with and I took my dad out and straight to the hospital after sharing pictures of his lack of care with his Dr who was not happy and requested admission to re-evaluate where to send him. Then I took a tour of Trellis and knowing the previous place had said they could do the needs for his care but didn't live up to any promises of that care I was skeptical BUT Trellis has delivered on all they said they would do!!!! He is receiving PT daily as the orders say! The wound care is amazing! Eddie is a true gem! So much so that the City of Hope surgeon following my dad is so impressed she wants to steal him... but Trellis deserves him too and is so lucky to have him from what I have seen other places care like he provides is not happening THANK YOU EDDIE! I have not even once had to request wound care and they just are doing it routinely and are very organized to be sure it is done and done correctly. You have literally taken my dad from wanting to give up because there was no progress to happy and motivated simply by keeping him informed on the progress. Now this is a skilled nursing facility so there are daily moans and groans of patients and sometimes that is the nature of the person and the mental ability to process why they are there. I in no way see any lack of care and my dad is 100% capable mentally and building physical strength back but he is great at evaluating and has said how wonderful everyone is. The facility is so so clean! I see maintenance cleaning the floors to perfection and spraying and wiping all surfaces, great maintenance staff keeping this place up! The layout is so well thought out so there is common areas with nurse station in the middle. Tables around with couches and lounge areas which in the very center is a great outdoor space. My 3 young children come with me to visit and it's a great environment we can enjoy too! Food is food, and by that I mean some days are great others are ok but nothing to complain about because you are always fed and it's made with nutrition in mind. I love how they put out a coffee bar which allows you to move around if able and gather if wanting to it's a nice way to serve! The room is great it's private with its own cabinet, end table, tv and bathroom with shower. Has made it nice to build that transition to home life! Bed has been comfortable and amazingly everything works which seems easy enough to expect but practically nothing worked at previous places. Thank you from the bottom of our hearts for all you have done to get my dad stronger and healthier and finally healing after a long journey! Everyone from the admissions person who promised us we would get good care, the young lady signing us in, the kind students, on top of it and motivating PT team, the efficient and kind nurses, chefs, maintenance keeping things spotless, to the Drs we appreciate you all and cannot praise you enough for your wonderful care you provide. read more

Excellent Rehabilitation! My relative arrived at Trellis Chino in mid May in poor physical condition due to a medical condition and less than optimal care at another facility. The team did their full assessment and staged a programmatic approach to build core strength, build back nutrition and treat every muscle group to allow for a return to her optimal level of care.. The initial road was rocky. Speech started working to develop poor conditioned face muscles. Emily was outstanding, rapidly building exercises and changing diets to allow for increased protein consumption. Nursing assured turning along with the wound and skin team . She was seen weekly by the wound physician, she was seen daily by Eddie or Jose to assure protective measures were taken, all areas were addressed, cleaned, dressed and managed to assure no skin breakdown. Excellent approach with positivity and humorous chatter. Physical and Occupational therapists listened closely to her desired outcomes and worked in small steps to give encouragement and assure progress. Roya and Robert were the main team backed by Raymond, Lindsay, Eunice and the rest of the team. Room exercises were used until she could manage the gym. The gym was sunny, airy and tough but manageable. Medical staff followed closely, the psychologist checked in to assure the psychological impact to a long recovery was managed. Pain management was also addressed well. When there were a few stumbling blocks, (night shift issues, phone, call light answering on some occasion), the Administrative team was approachable, listened and took actions appropriately as needed. On occasion, staffing was an issue. The Director of Case Management, Director for Staff Development and the Director of Nursing worked shifts to assure safety. The regular food menu had a few issues with lack of variety and dryness or volume issues. The assoc administrator tackled the issues and it is a work in progress. Our profound thanks to Trevor, Nicole, Adrian, and their team. Gratitude to the licensed staff RN's and LVN's Jodi, Ben, Lila, Liz, Arranza, Tracey, Nicole and Gio. Nursing assistants Mayra, Daisha, Anna, Maricela, Merlin,Jessie, Jonathan, Guillermo. were absolutely outstanding in encouragement, care, and tolerance throughout this extended stay. Yuri, case manager was key in communication. Bottom line, excellent facility in physical attractiveness, clean at all times, attention to detail and responsive to issues as they came up. There are no phones in the room, just a phone shared by the building. 2 phones for the whole place, we believe. Getting through is difficult, even to get an answer at the receptionist. During the time our elderly family member was there, the tv was out (his main diversion), the phones went out, and there was a COVID lockdown. Visiting hours are limited. The food seemed inappropriate for someone recovering from heart surgery. The people seemed nice and he liked having his own room (the rooms are individual). My mom stayed here after a hospital stay and while the facility is beautiful and the patients all have single rooms, there are issues with staffing. During my mom's stay she noted that care staff dug their knuckles in her back when she wouldn't move quick enough and they were not gentle with healing broken arm and hurt her while dressing. In addition, the case manager was not on top of my mom's case and doctor's orders for regular blood work was not adhered to and follow-up appointments were not scheduled in a timely manner. The facility was kept clean by a hard working crew Therapy was good and persistent with me Food was OK - it could have been good, if quality products were used Nurses were busy - often working four days of double shifts-Less time to explain what was happening. No registry nurses were ever called. Management would work a shift if there was a missing nurse-two jobs at one time - less time for patients and management. Patients and family were asked to perform tasks that Management traditional do like getting report to dr, making transportation plans, arranging home health care. Supplies would run short on weekend nights - making patients uncomfortable Patients with dementia would be yelling through the night - not good for sleeping Don't stay here if you have to be your own advocate bring a family member. This is a beautiful facility with private rooms for patients to recover and rehabilitate in a safe, stress-free, and comfortable environment. Almost all of the Trellis staff were absolutely fantastic with their care of our Uncle! There are too many to mention, but we wanted to give a shoutout a couple of real SuperStars, RN Ben and Jodee (I think that is how her name is spelled), that were exceptionally compassionate, helpful, and very patient. The Physical Therapists were also great, working diligently to ensure Uncle was getting stronger, and had good balance and the ability to get back home safely. My friend was placed here and they were quite impressive. Very attentive to the safety of their patients, had to wear a mask and face shield due to Covid protocols. The rooms were extremely spacious, clean, private and even had a wonderful view with a window. The attentiveness of the staff was wonderful, I can't remember the name of everyone who assisted, but can say that the ladies at the front desk were an absolute pleasure and that Eddie was phenomenal. My dad went sepsis, they refused to call the doctor for evaluation. The only reason he didn't pass away at this facility is because I ended up calling 9-1-1 from inside the facility and had him taken to the hospital. In addition they left him at the dialysis center, called me at 12 in the morning and asked where my dad was!!!! Neglect! Neglect! Neglect! Do not allow your loved ones to come to this facility or any! Take care of them at your home where they are loved and cared for! read more

Do not be fooled by Trellis's beauty. I had taken my father for rehabilitation a few months ago and everything was great! The nurses were the knowledgeable, caring and professional. These particular nurses were 100% about the patient and helping them regain a sense of normalcy. I have to say they did an awesome job. Well my dad recently had a fall a couple of weeks ago and I took him to Loma Linda to deal with his eating issues. He has not really eating and became weak so after spending a couple of days Loma Linda stated that they feel he should go to skilled nursing facility. We chose Trellis because he had been there in the past and the staff was great but needless to say they lost 95% of that awesome staff they had. There were so many issues it would take too long to explain. I will give you the last my father called at 4am to wake me up to tell me he was in a great deal of pain and he is calling the nurses and they aren't answering him nor coming so I had to call them to tell them my father was in pain. I tried to check on my father the following morning starting at 9am and I called his nurse 3 times to find out how my father was doing. I was told she was passing meds 2 times and the other she was at lunch. I was told to leave my number and they will make sure she called me but she failed to call. One of the nurses suggested I should call Director of Social Services so I did and left a message,no call. Last example they took his sugar level and it was 79. When I asked the charge nurse she was clueless and wouldn't even take time to figure out what was causing the drop. When I asked the so called clueless charge nurse she brushed me off to a nurse that just clocked in and she didn't know because she just came on. If you're wondering if they gave him something to bring sugar up that would be a NO. While talking about sugar it reminds me that one evening my dad ask for a snack since he is there to address his eating issues due to cancer and he was denied. When he asked to speak with head chef they sent their assistant and she showed him the planned meals for the week disregarding the fact that he was there to help increase his appetite. This is for real the last example. My father had to sit in his room with no TV for a day. I told several nurses but when I got there the following day it was still broke and he was still sitting in the room with no TV. Many of the female nurses were lacking professionalism and medical knowledge. I couldn't tell the Director of Nursing from a visitor seeing a patient. She also was clueless as how to run a facility like Trellis. I believe she is not equipped to run such a place. The social worker was also clueless and not professional. I lied there is so much more to tell! I will give you a bit more because I will be writing all night.I decided to transfer my father from there after she emailed paperwork to the new facility she told new facility I'm going home and if you need me you can call me Monday. In addition she did not come to see us off, apologize or see if we needed anything, she just left. Let's talk about the front desk the one that greets you at the door. She ( Sophia) portrays to be professional but she is far from it. This person is arrogant and controlling. We would be mocked by this person everyday. This just shows how clueless they are by putting someone like that upfront that's miserable. Update, I want to thank few people here on Yelp. Housekeeper Christine is a sweetheart, she helps brighten my day when she cleans my room. Liz is a amazing CNA she goes above and beyond during the day and Oggie goes above and beyond at night. They both check on me and make sure I'm doing okay. Wound care Dan is a great Nurse and cares about the healing processe. Jessica in the kitchen has been making sure my food is on time and prepared correctly, she checks in with me periodically. And Joseph also, he helps to make the food great. Jen Morales is a amazing Nurse, she is helping my case with my insurance company she goes above and beyond, i never meet someone so nice and caring Jen, thank you so much. Taajee from activities is a amazing person she always makes sure i have refreshments and asks about my day, she helps brighten my day. Doctor Wynn is great she comes and sits and talks to me for a hour and helps me figure out goals for the future. I really feel like trellis has been my family in the past months, that really cares about you. Best experience from a horrible injury was Trellis Chino. We quickly realized that would not be the case and we could not be more grateful for the care he received at Trellis! The facility was extremely clean yet homey with various activities for the residents. The entire staff was absolutely amazing! This is one of the nicest facilities I have seen. Everything is clean and very comfortable to sit around while the patient is resting. People here have been very nice and considerate offering us food and bringing extra chairs to the room. This facility may be ideal for some residents with zero complications and a fat income source, but it's a nightmare for anyone who isn't a profiting commodity. Its only redeeming factor is its professional and caring nurses, at least on the surface. The physicians don't do their due diligence or even stick around long enough to know the patient's diagnosis or history. The patients' rights advocates and especially the social worker are unprofessional, illiterate in the most rudimentary medicine/science/logic, and counterproductive. The facility's administration lacks clarity, follow-through and compassion -- notwithstanding the director's (Gary) patience and ability to sympathize with grievances, even if his understanding is also based on faulty information. This facility blatantly disregarded the transferring hospital's directives for a safe discharge/plan of care with a brief transition through this SNiF, and even our mother's diagnosis. Within a day our mother, who's been diagnosed with FTD-bv and epilepsy, was discharged against the directives of her transferring TEAM of physicians and neurologist (in consult with Trellis' own resident physician) -- courtesy of the grossly ill-informed Patient's Advocates Rights group led by the social worker, who rejected and ARGUED our mother's diagnosis, refused to even look at her reports, accused her family/caregivers of manipulating her diagnosis to dump our mother in "jail" through a misuse of the healthcare system, even so far as repeatedly insinuating elder abuse and falsely suggesting our mother had signed documents to remove all her physicians/family from her care. Once the deed was done, when our mother didn't show up at the door for over an hour, the very same social worker claimed no knowledge of our mother's whereabouts. After finally locating her, upon pick up, our mother walked out barefoot from the facility with nothing but discharge papers. Immediately upon returning home, in the thick of the pandemic, our mother wandered outside the house and seized again. Due to the social worker's dangerous accusations, our mother's care took several steps back as we did absolutely nothing and she deteriorated further until an investigation concluded no paperwork existed to bar us from her care. Upon receiving my lengthy documentation, the only response I received indirectly was that I should get our mother's POA or this will keep happening. This place has traumatized us so much that we won't let our mother step into a nursing home ever. It has also caused her care to be inadequate, but it seems either our broken healthcare system has no solutions or this place is a few more reviews away from being investigated for gross negligence and abuse. The social worker needs to be fired and NEVER enter this profession again. She undid nearly a decade's worth of work to finally diagnose our mother and single-handedly destroyed our mother's plan of care. We are STILL awaiting scans that got delayed as a result of this bullshit. Words to the Unwise: Discharging doctors should consult the patients' physicians and Trellis' own resident before making a foolish call. Social workers need to consult physicians and not fight the patients' caregivers with lies and deceitful games. And the patients' rights advocates need a course on what patients' rights actually look like. You can't hand the keys of a moving car to a patient with dementia unless you're in the business of speeding up their demise. Proceed with caution and if your loved one has any dementia, keep away from this madhouse. read more

I would not recommend this facility. Unfortunately my relative was in this facility for 3 weeks. The staff nurses and administrators lacked assessment skills and should have moved my relative back to acute care. We met with and pleaded with them and they just kept procrastinating. My relative of course got worse every day until he was found unresponsive. They finally moved him to the hospital, where he was intubated and died a day later. The facility looks great, brand new and in this case looks can be very deceiving. The staff should have called the doctor on call and advocated for my relative. My relative was a very sick man just wish they would have taken better care of him. read more

Last evening I went to Trellis to visit a client and was graciously welcomed by the front desk person who inquired as to the purpose of my visit. She shared with me where I would find the person I was visiting asking if I was comfortable finding the room myself or would like her to accompany me. the place is immaculately clean, smells fresh, I received smiles from the two staff members I passed in the hallway. The person I was visiting shared that he could not be happier with his selection of this facility and the physical therapy team that is caring for him. Selena and Janet from admissions caused me emotional distress, more depression even after knowing what I had gone through and crying to them twice . I'm glad to not have returned to a place where they treat you like absolute crap starting from the office. They gave up my bed after going to the hospital for getting sick and mentioned there was no more beds available but their sister facility would take me in for a week meanwhile something opened up, a week passed and I got excuses after excuses . They took away my physically therapy for two weeks witch delayed me , my physiologist who I still have not seen and it's been two months since . I have more depression then ever because of their nonsense . I hope a hire up sees this as I've also tried reaching out and no call back. read more

First of all I want to thank the nursing staff and CNA's they were wonderful during my mothers Stay. Tonya & Austin were among my mom's favorites while she was there. Here are my concerns and I hope they will address these and make the experience better for future patients coming in. 1. Remember you are your loved ones advocate, speak up when you see something. My mother contracted sepsis while there at the facility. We had to be the one to push for her to be transported to the hospital. I think that this should have been an obvious observation, we should not have to diagnose it ourselves. In the time my mother was there the Dr. on staff, Dr. Davis did not come in personally to see her. 2. Upon her return she went 2 weeks into a depression,again we had to push to have her evaluated with a Psych consult to get her on a anti depressant and an appetite stimulant. She lost 17lbs in 2 weeks. It would have been appreciated if this had been suggested by the staff instead of us insisting. The only reason we thought to was from a suggestion of family friend whose mother went through the same symptoms. 3. Communication between the family and head of PT was non existent until you were told that she wasn't making enough progress and they wanted to discharge her that week. I had to go in and ask what she needed to do to "show progress", we lost 3-4 weeks of rehab with the Sepsis and depression. I made arrangements to be there at a certain time everyday to take part in her rehab. Her progress was slow, but she was making progress, but apparently not fast enough to meet Medicare's criteria..although never told what that criteria was. 4.Upon her discharge because Trellis said that again her progress was not enough to warrant her stay there, we asked that she be moved to a facility that 1) would still offer PT to her & 2) that she would still be able to use what was remaining of her 100 days of skilled nursing. We were assured by Danica that she would be able to use her remaining days. She offered to assist with finding a place to move her to and we provided her with a list of 3 spots. This is where it really went of the rails for us. We were told that Inland Christian said they would accept her and that she was all set for transfer to the facility that Saturday at 4pm. My sister and I left my mom that Saturday and went to Inland Christian to see if we could set up her room ahead of time. At that point we were told that she was not set for transfer, but the RN was not in until 3pm and she might have the transfer order. After speaking to the RN and the enrollment clerk we found out 2 important details. 1) they had contacted Trellis and asked for more clarification on her medication and Trellis did not return their call. 2)Trellis had me sign a NOMNOC which officially ended her Medicare coverage. When I informed Danica this she said that no that was not the case and that the new facility could get it reinstated and that we should go to our 2nd choice because they do it all the time and have never had an issue. We then spoke to our second choice and they also informed me that the NOMNOC we signed ended the coverage and we would now be private pay. This wrong information cost my mom 8k, because she was in no way ready to come home. She was still on a Catheter, and could not stand or walk without assistance. We ended up at Ontario Christian where my sister and I proceeded to continue with her PT. She is now walking on her own with a walker and is off the catheter and able to use the bathroom without assistance. She is home now. We chose this place for my mom after having her second bleeding stroke because of the look. It looks like a hotel and is very nice inside. But looks can be deceiving. My mom was there a week when we started getting phone calls telling us that they were no longer going to cover her care because she was not progressing. How in the hell can they make that decision within a week of being there I tell you??? She was laying on a matress that was soaked in urine due to their negligence with her catheter. They blamed it on my mom not drinking enough water which was complete BS. My family and I went to see my mom every day and did not see any progression with the so called therapy they were giving her. One day my dad and I went to visit with her around the time she was supposed to have dinner. They brought it in and left it with us. My mom was sleeping, and sweating on the back of her head. We woke her up and tried to help feed her. She was not chewing or anything to which I got scared and went out to get someone. One girl came in and looked at my mom and went to get a nurse or whatever she was. She came in for a few minutes and then told me that they called 911. Scared the living hell out of me. Come to find out in the hospital that my mom was severely dehydrated and her sodium levels were off the charts. A worker there at Trellis pulled me aside and told me that he knew something was off with my mom earlier that day because she wasn't responsive. He stated that he notated that on her chart. AND NO ONE DID NOTHING!!!! It took me to tell them how to do their job. After this we called Trellis several times to find out why this happened and the director of nursing NEVER returned our calls. This facility is new and beautiful...please do not be fooled by their appearance and DO NOT BRING YOUR LOVED ONES HERE!! The Care is Horrible!! My Elderly Mother was there for IV antibiotics for a severe UTI and Diarrhea. While taking medications to control her diarrhea, they gave her stool softeners every day!! It was a week before we found this out. They said it was "routine". Really??? You've got to be a total idiot to give a patient with severe diarrhea stool softeners!! Because of the diarrhea, my Mom needed changing regularly. She pushed her buzzer and got no response for up to an hour!! Inexcusable!! When her antibiotics were discontinued, we asked if her tests showed she was clear of infection and were told she didn't need any testing, that her urine didn't have any odor. They wouldn't test her!! She is now hospitalized with the same infection and Sepsis!! She probably will not make it through this. I totally blame their lack of testing, and lack of care while she sat in a poopy diaper. My Mom is 88, but she was clear headed and spunky before this...they robbed her and us of quality time together. read more

The facility itself is beautiful. Not repellent like most other facilities. My boyfriend has been here a week and so far the staff have been very attentive and caring assuring that he is getting the services he needs to prepare for his prosthetic foot. The food is the greatest challenge thus far. They advertise a restaurant experience in their virtual add but that has not been the case. I would not order anything on their limited menu. They don't keep in mind cultural preferences for food such as Mexican, Vietnamese, Thai Food or Italian Food. No opportunity to customize a food order even though it's advertised. The only plus is you can get a hamburger instead of what they call food on the menu. We were hoping to avoid take out but so far it does not look promising. It's not feasible to eat burgers every day. I know the kitchen is trying to be accommodating to my boyfriend's Vegas Restaurant cravings but perhaps more resources need to be allocated to support the chef. I would suggest a smoothie/ juice option as well. read more

Update on the review I posted, we had to seek out assistance ourselves, as the referrals for home health care never materialized. Almost a week and a half after discharge, we were still trying to straighten out the mess. Along with never receiving a wheelchair or other supplies, that we were promised, he came home with a pressure sore and MRSA. WOW, Trellis, you have such a beautiful facility, do not why you cannot get your stuff together. Your not cheap and tout as being a prime facility. So glad, when my mother needed her rehabilitation, and after having my neighbor in the past at your facility and really liking it (brand new at the time), that Methodist Hospital of Arcadia was able to accept her in the rehab department and she did not have to go to Trellis. I was so disappointed at the time and wanted her in Trellis, as that was closer to me. Follow through was great at Methodist and she did not come home with MRSA or a sore, all during Covid 19 time. Since we could not visit my neighbor at all in Trellis due to covid, I actually wander what the care was. I also did not get to visit my mother, because of Covid protocol, so am so glad she didn't make it to Trellis, as I had hoped initially. My mother is 100% alert and oriented although in her late 80's, so she might of had better care there at Trellis as she could advocate for herself, rather than my 90+ not always oriented neighbor, who care was really questionable. All I can think about is the elderly clients you have that are sent home with supportable follow up support of home health Nursing, PT, OT and CD, and do not have the resources or the people that have the time and care about them to help. My neighbor would have come home with nothing, unable to get around, no wheelchair, no follow up for his sores and then within two weeks dx with MRSA. Shame on you! read more

I would not recommend this place to anyone personally. My grandmother's health declined tremendously just being there one month. I honestly would like to know how these nurses and respiratory therapists still have jobs there. The food does not look appetizing at all. They say that the patients can put in special requests for something else to eat but nothing gets done. The lack of communication between the nurses is mind boggling. We were told my grandmother would be put on a list to get physical therapy, and it never happened. My grandmother needed to see the podiatrist, we were told that she would be put on the list to see one, and again it never happened. The respiratory therapist would put my grandmother's oxygen cannula on wrong, and multiple times it wasn't even being administered. It is a nice and clean facility but it is not staffed with smart people. read more
