I visit her once a month (or so) and we have gone to the resident dining maybe twenty times. From the first time I was shocked by the quality of the food. My first time I was expecting an old-school cafeteria-style food service with metal trays and food slopped onto the plate. This was completely different. The food was shockingly good, and the meal of the day (lunch) always has a soup, a main meal and a dessert. It's not overly fancy but the food in actually really tasty and they give you good size portions. The staff are very attentive and polite and the experience is a very pleasant dining experience. Today we had garden vegetable soup along with pot roast, scalloped potatoes and peas, with apple pie for dessert. The pot roast was excellent and the potatoes were exceptional. You know the food is good when the head chef/manager of food service is there talking with residents to make sure she gets feedback concerning the food that she prepared. Currently the dining area in being remodeled and should be done by the end of this year. So, the dining area currently is less then amazing, and what they can offer is limited. I'm looking forward to the new dining experience once they have their grand opening later this year.

My mother was sent here to strengthen the bacteria balance in her gut. She was forced to wear a diaper and was not permitted to use the toilet, and she is not incontinent. She would tell them I have to go to the bathroom and they'd say well then go in the diaper and I'll come right back and change you, and they wouldn't be back for hours leaving her to sit in her own waste. The food she was served was so bad sometimes she did not know what she was eating, but she had to eat because she was so hungry. I sent her food from Instacart so that she would have some healthy food to eat in her room. They would limit her water so that she would not urinate which resulted in delirium. She called me in the middle night and her mouth was so dry. I could hear her lips sticking together because they were refusing to give her water. They ignore the call lights. They would leave her in her poopy diaper for hours at a time, causing her to get UTIs. The delirium that she acquired at this place from their inadequate care was so bad that my California family thought that she was in her end times and was going to leave her there. When I called the office and kindly asked them to change her diaper for three hours to change one diaper, they hung up on me. I heard the nurses with unkind angry yelling tones yelling at my mother while I was on the phone and they did not know the phone was open. I texted the management of the facility trying to get my mother help and 90% of the time they ignored me. When company is there or they know the phone is open they are not hostile but as soon as there's no one around, they are. I've already called Ombudsman's to check out this facility. It should not exist; It is that bad. I drove 1800 miles to rescue my mom from this facility two weeks ago. The emotional abuse is one of the hardest hurdles to help her over. Do not take any loved ones to this facility. I will be reporting them to anyone who will listen. My son sent this up accurately when he said "that facility is nothing more than a pound for old people."

Safe neighborhood, easy access location - very close to the 60 freeway. Facility is big and very clean, nurses are attentive and rehabilitation center looks excellent. Daily rehabilitation with patients and very good food service. My mother in law has her own private suite room with a window and the bathroom that is very big. DO NOT TRUST THIS FACILITY WITH YOUR LOVED ONE. If your…read moreloved one needs professional medical care after a hospital stay, please think very carefully before choosing this facility. While the building looks nice and the rooms are private, that is where the positives end. In our experience, the medical oversight was deeply concerning. The physician refused to adjust treatment plans despite clear and ongoing signs of decline. Even as our loved one showed both objective and obvious symptoms worsening over time, no meaningful changes were made. There appeared to be a lack of proper assessments, and when serious issues were identified -- primarily because family members insisted on testing -- there was little urgency or corrective action. Staff did not consistently follow orders and did not seem to recognize signs of medical decline. Concerns raised by family were often minimized. For three days we requested that our loved one be transferred to the ER due to significant changes in mental status and severe migraine symptoms. The physician refused. When EMS was finally called, emergency personnel immediately recognized the seriousness of her condition. We had to sign a waiver to transfer her to a hospital 20 minutes away because she was considered unstable -- yet staff at the facility continued to say she was "fine." Within days of transfer, her condition deteriorated to the point where she could not speak or swallow and became nearly unresponsive. We were preparing funeral arrangements. She later described experiencing severe hallucinations and believing she was in hell. It was profoundly traumatic for her and our entire family. We also discovered she was not receiving the correct dosage of a critical medication that had been ordered by her previous facility. This likely contributed significantly to her decline. Had we left her there even one more day, we firmly believe she would not have survived. Our loved one had none of these issues prior to admission to this facility, and since receiving proper hospital care and being restored to her previous treatment plan, these issues have resolved. We will be filing formal complaints with the appropriate medical and licensing boards. In our opinion, this facility is not equipped to safely manage medically complex post-acute patients. If your loved one only needs stable rehabilitation, it may be adequate. But if they require close medical monitoring, I strongly urge you to consider other options. Our family went through an avoidable nightmare. Please advocate aggressively for your loved ones.

The PT team works excellently with my father's rehab, and honestly, without their dedication to my father's mobility, he would not have the quality of life he has right now, which is eons more than what it was at the start. What happened over the next several days was completely unacceptable. On his first day, he was not given a shower or an examination. His pain medication was not given in a timely manner and was sometimes hours late. Even worse, throughout his entire stay, no one came to assist him with using the restroom. For someone recovering from surgery and sent to a rehabilitation facility specifically because they need additional care and assistance, I find that absolutely inexcusable. I spoke with a supervisor, Jojo, about the problems with his pain medication and the lack of assistance. When I called my grandfather on August 12, the same problems were still happening, and he STILL had not started physical therapy. At this point, my grandfather was already fed up with the facility. I convinced him to give me an opportunity to go there myself and speak with staff in person before we made the decision to remove him. On August 13, I went to the facility and again spoke with Jojo because, quite frankly, she seemed like one of the only people there actually doing work. I specifically explained that my grandfather needed to start physical therapy, needed a shower, and needed his pain medication on time. She appeared surprised when I told her that he had received neither physical therapy nor a shower since arriving. I was assured these things would be addressed. While I was there, I was also extremely unimpressed with the condition of the grounds. The property looked dirty and poorly maintained, with dying plants and an overall neglected appearance. I learned that the property apparently used to be an orphanage, and unfortunately the entire place gave me the feeling of a place where forgotten people are sent and left behind. After watching how my grandfather was treated, that feeling only became stronger. On August 14, he FINALLY received a physical therapy session, three days after arriving. However, he still had not received a shower, still was not receiving assistance with the restroom, and was still having problems receiving his pain medication on time. I texted Jojo and told her that I would be removing him from the facility the following day. At that point I was no longer willing to keep giving the facility chances to provide the level of basic care that should have been happening from the beginning. Only after I complained again by text was he offered a shower, close to 6 p.m. He declined because of how late it was. His pain medication issues were also continuing. Once I made it clear that we were removing him, I suddenly began receiving calls from higher-ups. By then, it was too late. I had already raised these concerns repeatedly and had also contacted his insurance case manager. The final experience on August 15 made an already terrible situation even worse. My husband went to pick up my grandfather as we had already told the facility we were going to do. According to my husband, the head nurse (a tall woman with brown hair and glasses) yelled at him, berated and belittled him, and even accused him of stealing a wheelchair THAT WE BROUGHT WITH US. She then loudly talked about my husband to another nurse in the hallway. The behavior he described was shockingly unprofessional and completely inappropriate for someone working in healthcare. Maybe she was possessed by a ghost of one of the bratty children from the orphanage. I was later told that she behaved this way because we were taking my grandfather out against medical advice. That does not excuse her behavior whatsoever. If the facility was so concerned about his medical well-being, that concern should have been demonstrated through timely medication, assistance with basic needs, hygiene, rehabilitation, and attentive care from DAY ONE. You cannot repeatedly fail to meet someone's needs and then suddenly act outraged when their family decides enough is enough. What makes this even more frustrating is that this nurse reportedly claimed she did not know about my grandfather's condition, yet suddenly had plenty to say and had plenty of questions when we were removing him. My husband asked when my grandfather had last received his pain medication and was not given an answer. He also requested aftercare paperwork and was not provided with it. Thankfully, my grandfather's case manager through his insurance helped us get everything sorted out afterward. We gave this facility multiple opportunities to correct these problems. We spoke with staff, raised concerns repeatedly, went there in person, followed up again, and involved his case manager. Removing him was not some impulsive decision. It happened after several days of watching the same basic care issues continue despite repeated complaints and assurances that they would be addressed.

My 94 year old aunt was having a painful transition and they took her with hospice (Cambridge). I was there for the few days and she was clean, dry and comfortable. Her room was peaceful and the nurses would come by to check on her and me often. One thing that's different here is the staff. They are staff that is invested and have spent awhile with Villa Mesa, this should be a continued goal of the management's part--there are residence that look better than many years ago from the last time I saw them when my employment ended and that is a huge testimony and the way it should be. The residence at Villa Mesa are thriving and well cared for. And even in a transitional period, my aunt was treated so well and so was I. Thank You all. Also, it really is the cleanest place too. Take it from another nurse who has instructed all around the Inland Empire, this place is a good, safe place and it's clean and their nurses are smart. The RN evaluator is also caring, friendly and very capable. If family is involved and communicates they are willing to work with you for the care of the patient. The housekeeping staff is friendly and kind and always want to help. My mom needed doctor appointments and transportation and this was all arranged by the hardworking social worker. She handles the care meetings and coordinates care as if she was a case manager. She handles the hair salon and even additional nursing attention as needed. She walks around and checks in on patients. At discharge she handled all the needed items for home care and worked hard to advocate for my mom. Admission staff was awesome. They were very welcoming and made sure she had coverage with her insurance. They are great to work with. The doctor that oversees the facility was not easy to communicate to in part due to her availability. She is a full time doctor seeing patients in her office during the day. She made an effort to call back after hours but my own schedule also got in the way. It would be best if the doc attended the care meetings upon admission so goals and care can be discussed clearly. It was difficult to pinpoint care plans. Staff would send message to doc that family want to discuss care and it was hard to receive info back quickly or at all. Maybe if the doc was present at limited but fixed hours on site it would help. PT/OT was good and made an honest and persistent effort to assist recovery. It helps if family is involved and asks questions regularly. My mom was independent prior to her medical issue so the goal was set to return her as close to that as possible. You have to make it known to them where she was before very clearly. Otherwise she may be released too early and becomes a fall risk. The food staff is very nice and concerned. They made an effort to give her Greek yogurt often for her digestion. The food is otherwise not very tasty or presentable and comes from the main facility. I'm sure the quality in Claremjnt Manor assisted living is better than in the rehab unit. My mom ate mostly in her room and so could not ask for extra items from the kitchen as those in the dining room. The dining room was filled with behabing persons that need assistance eating and this was hard to see for her to see while eating. She is a social person otherwise. I would definitely recommend this facility. They have helped her recover through 4 events in the past year and a half. No place is perfect but Claremont manor care center does a great job. They want to help and truly care. We are lucky to have them in Claremont.
