I stayed for about 30 days and I was pleasantly surprised. The staff was professional, knowledgeable, and friendly. The staff went out of their way every day to make sure my every need was met. I came in with a serious health issue, and am going home a much better man. I hope that I never need their services again, but if me or my Family ever do, I would choose Oak Glen Acute Care again. **I would not recommend Oak Glen Post Acute to anyone that ACTUALLY likes their loved one** My grandfather arrived on August 11, 2026, in the afternoon following surgery. What happened over the next several days was completely unacceptable. On his first day, he was not given a shower or an examination. His pain medication was not given in a timely manner and was sometimes hours late. Even worse, throughout his entire stay, no one came to assist him with using the restroom. For someone recovering from surgery and sent to a rehabilitation facility specifically because they need additional care and assistance, I find that absolutely inexcusable. I spoke with a supervisor, Jojo, about the problems with his pain medication and the lack of assistance. When I called my grandfather on August 12, the same problems were still happening, and he STILL had not started physical therapy. At this point, my grandfather was already fed up with the facility. I convinced him to give me an opportunity to go there myself and speak with staff in person before we made the decision to remove him. On August 13, I went to the facility and again spoke with Jojo because, quite frankly, she seemed like one of the only people there actually doing work. I specifically explained that my grandfather needed to start physical therapy, needed a shower, and needed his pain medication on time. She appeared surprised when I told her that he had received neither physical therapy nor a shower since arriving. I was assured these things would be addressed. While I was there, I was also extremely unimpressed with the condition of the grounds. The property looked dirty and poorly maintained, with dying plants and an overall neglected appearance. I learned that the property apparently used to be an orphanage, and unfortunately the entire place gave me the feeling of a place where forgotten people are sent and left behind. After watching how my grandfather was treated, that feeling only became stronger. On August 14, he FINALLY received a physical therapy session, three days after arriving. However, he still had not received a shower, still was not receiving assistance with the restroom, and was still having problems receiving his pain medication on time. I texted Jojo and told her that I would be removing him from the facility the following day. At that point I was no longer willing to keep giving the facility chances to provide the level of basic care that should have been happening from the beginning. Only after I complained again by text was he offered a shower, close to 6 p.m. He declined because of how late it was. His pain medication issues were also continuing. Once I made it clear that we were removing him, I suddenly began receiving calls from higher-ups. By then, it was too late. I had already raised these concerns repeatedly and had also contacted his insurance case manager. The final experience on August 15 made an already terrible situation even worse. My husband went to pick up my grandfather as we had already told the facility we were going to do. According to my husband, the head nurse (a tall woman with brown hair and glasses) yelled at him, berated and belittled him, and even accused him of stealing a wheelchair THAT WE BROUGHT WITH US. She then loudly talked about my husband to another nurse in the hallway. The behavior he described was shockingly unprofessional and completely inappropriate for someone working in healthcare. Maybe she was possessed by a ghost of one of the bratty children from the orphanage. I was later told that she behaved this way because we were taking my grandfather out against medical advice. That does not excuse her behavior whatsoever. If the facility was so concerned about his medical well-being, that concern should have been demonstrated through timely medication, assistance with basic needs, hygiene, rehabilitation, and attentive care from DAY ONE. You cannot repeatedly fail to meet someone's needs and then suddenly act outraged when their family decides enough is enough. What makes this even more frustrating is that this nurse reportedly claimed she did not know about my grandfather's condition, yet suddenly had plenty to say and had plenty of questions when we were removing him. My husband asked when my grandfather had last received his pain medication and was not given an answer. He also requested aftercare paperwork and was not provided with it. Thankfully, my grandfather's case manager through his insurance helped us get everything sorted out afterward. We gave this facility multiple opportunities to correct these problems. We spoke with staff, raised concerns repeatedly, went there in person, followed up again, and involved his case manager. Removing him was not some impulsive decision. It happened after several days of watching the same basic care issues continue despite repeated complaints and assurances that they would be addressed. read more

I know that these skilled nursing facilities are understaffed and their employees are over worked. For what they have to work with, I give them alot of credit. The meals were always warm, the milk was always cold and everyone was very friendly. They didn't lose any of my moms clothes or personal belongings and always took such good care of her. She liked them all and that says a lot for my mom!!! She was there 100 days, so I think she would know!! A special shout out to JoJo, Dulce, Maria and to the kind young lady at the front desk. I have been to several nursing homes and Oak Glen is the best! It is old, but clean and the staff is wonderful. The air conditioning needs work, but they are working on it - you can tell they care because you see guys going around checking the temperature in each room daily. It didn't bother my husband because he's always cold anyways. The staff is very attentive, caring and friendly. Jojo (Admissions) and Dulce (Case Manager) - they are knowledgeable, friendly and tended to any needs I had and gave me answers to all of my questions without hesitation. Ray (Physical Therapy) is top notch!!! I will truly miss him. Sabrina (front desk) is welcoming and friendly. The Medicine Administrators and Wound Care staff -Marbrisas, Flavia, Reina, Irene and Eric were very knowledgeable, friendly, patient and caring. They explained anything I asked about my husband's treatments and needs and were available when needed. If they were busy, they got back to me quickly. Sarah (RN) was always very helpful and caring. She would even sing with my husband, which was great in lifting his spirits. CNA's -Omar, Cade, Allya, Aaron and an older CNA man who worked nights (my husband doesn't remember his name) were extremely helpful and friendly. If I ever asked them for something, and it wasn't their job, they would get the person who were able to help. Evelyn (Meals & Food) was friendly and considered all my husband's dietary needs to make sure he was happy and eating. Everyone there were so supportive, helpful and caring that I will miss them all! They treated my husband and I like family. Thank you Oak Glen! Although my husband nor I never want to return, if needed, we would want to stay there. My only suggestion: After CNA shift change, CNA's coming on duty, should check with each of their assigned patients because shift change takes time and patients may be waiting awhile for assistance. read more

My mother was admitted to Oak Glen Post Acute after suffering a broken hip, and I can't say enough about the care she received during her stay. From the moment she arrived, the staff made her feel comfortable, safe, and genuinely cared for. The nursing team was attentive and compassionate, always taking the time to explain her care and check on her regularly. The physical therapy staff were especially wonderful--they encouraged her, supported her progress, and played a huge role in helping her regain strength and confidence after her injury. My 87 year old uncle was transferred to this facility from the hospital. It was an unexpected surprise to see and feel the beautiful welcome from this facility. The day my uncle was admitted he was family. They've given my uncle the care and respect that any patient deserves. The medical staff will attend to the patient when the patient calls for their assistance. The admitting staff at this facility has demonstrated the professionalism with their communications with the patients and their families. They will help the patient and their families with any resources that are available that will help with the care. I would highly recommend this facility to anyone. Today is a whole new generation. The quality of care depends on who is working on what day. But if you bring it to the attention to the head office staff they do look into it. The. I will begin at the beginning. From the moment we were welcomed to Oak Glen we knew it was going to be the answer to our prayers for Richard's rehabilitation. We were greeted as family, and the excellent Care was constant and loving. The environment was so clean, thanks to the maintenance staff and welcoming. The connection from Eisenhower hospital to Oak Glen Post Acute made efficiently by Ginalyn in marketing, she was amazing, making arrangements for a hotel stay until I could make other arrangements for a longer stay, JoJo in admissions, Dolce case manager, Dorene business office manager, Sabrina the receptionist, so friendly and happy to greet everyone, Ray and his staff in therapy, Evelyn dietary Director and kitchen staff have prepared the best meals ever, Hail and her staff in activities and the amazing nurses and their staff and of course the doctors. Richard is leaving Oak Glen in the best condition. He has been in months thanks to their care and rehabilitation. The discharge process has been so easy for us. From the moment I entered Oak Glen Post Acute I was greeted by the administrator and several businesses office people including admissions director. I felt at home right away, and that was just me entering the building. Later I met Maria the social worker she took care of my needs and gave me feedback. Every LVN nurse helped me when I needed help with my medical needs. I am exiting and moving on to assisted living thanks to the physical and occupational therapist for successfully getting me stronger, I am now able to walk. The staff is very responsive, they have a great attitude. My problems were quickly resolved and I found my stay to be very productive.

My husband was there for 3 weeks and from the moment he was there until he was discharged the staff was extremely nice professional whole environment was pleasant I work in the nursing field and it a thankless job but patients are our primary concern their needs being met and my husband was a very difficult patient but from the admission to the janitor everyone so kind and understanding they go over and beyond the call of duty I was very much impressed a place that I would recommend to all my friends and love ones read more
