The staff always kept me well-informed about my mother's health. This is key for families of dementia patients. For this kind of care, it's the staff that matters most. The key players are the unit coordinators, and we had great ones: Kelly and Jenna. Both were very patient and attentive. Jenna eventually felt like a friend of the family. She took a personal interest, and went above and beyond to resolve issues in my mother's care along the way. I was there constantly during one period of time, and really saw how things work. I was amazed at the quality and number of people involved in my mother's care. Besides the coordinators, I was very impressed by Supervisor Marilyn; NP Liz; Night nurse Christen; Med nurses Garcelle, Angie, and Danielle; Aides Carline and Christia; OT/PT's Gary, Pierre, and Jennifer; Speech therapist Ann-Marie (regarding chewing/swallowing); Social Worker Donna; Recreation assistant Jennifer (who helped me Facetime with my mother when I couldn't visit); and Violetta (who *tirelessly* kept the whole unit clean). Pretty much everyone really knew my mother and the details of her care! My mother REALLY did not like taking medicines, etc., but the team displayed some amazing skills in gently getting her to cooperate. The comfort care period here went much better than home hospice care would have. The team took care of all medications, cleaning, laundry, and other tasks so I could just be there for my mother emotionally. In all, Smithtown Center is staffed by a large team of very nice people who were very helpful to my mother and me. Thanks, Smithtown Center! One last thing is, that in comparing notes with others, I learned that Medicaid patients got the same, high quality level of care as the self-pay patients.

She was eating with no teeth in her mouth. wh changing her diaper, stool was left in her diaper and vaginal area where I had to take off diaper and clean after aid. This went on for weeks as well. There is no such thing as low sodium diet and mom has been not 1 working kidney and heart failure which makes it dangerous for her to have too much sodium. The food is also really bad and cheap. Chicken consists of a chicken patty, and lots of pork being served without choice but a sandwich. The physical therapist deprives my mother of the time allowed, so she hasn't made almost no difference since she came in 4 weeks ago. Mom isn't Mobil and can't teach anything by herself and after I leave everything is removed in front of her so she can't get a drink of water, a snack, her tissues, her books to keep busy or anything else. When changing mom's room they left everything of hers, including what will keep her from falling out of bed, in the hallway. I came in at 7pm and had to put her room together to keep her safe. If thinking about coming her I would say don't if you care about your loved ones!

The menu for the day listed something culinarily palatable, yet my diabetic Mom was served a PB&J instead; Umm... No. Once we called them out on that, her food changed, but was either so cold or so distasteful, Mom couldn't consume it. Mom lost about 40 pounds over the five or six months she was there, just from being served nasty food, day in and day out. The Nurses/Medical Assistants are very nice, sweet and extremely respectful while they interacted with Mom. However, when they weren't doing a task directed by the management (such as changing bed clothes, getting Mom dressed, moving Mom from the bed to the wheelchair, etc.), they literally ignored her. My Mom was forced to sit in a pile of her own excrement until a Nurse or Nurses Aid was instructed to do something about it by management. Absolutely disgusting! Next to Mom's bed was a curtain, for privacy while dressing/undressing. At night, there were numerous times when a Nurse would tactfully pull the curtain to separate my Mom's bed from the rest of the room (which startled and woke my Mom up) and then sat in a chair on the other side of the curtain and SLEPT. Mom listened to the Nurse or Aid snoring on the other side of the curtain during the night. So... Too busy to tend to my Mom's bathroom needs yet NOT too busy to NAP?!? COVID outbreak; Mom got sick. I fought with them for three days, making four or five phone calls a day to the Operations Manager (named Laidy), the Resource Manager (named Marie) and various other people in-between JUST to get someone to contact a doctor and have the doctor electronically prescribe Paxlovid® for her, to help her through. They kept trying to push me off until the day a Doctor makes his rounds in the home, once a week. Paxlovid is only available for five days and we were on a time crunch but they didn't care. I had to threaten legal action to get Mom medication. Ridiculous!! The ONLY thing about Ross Rehabilitation that was great; Physical Therapy Program. Mom's Physical Therapist was named Jill and Jill was a doll; So compassionate, patience, enthusiast and uplifting. She helped my Mom a lot. The only thing my Mom looked forward to each day was her Physical Therapy and spending time with Jill. The gnats flying around the room. The phone system that only -sporadically- worked properly. The napping staff. The cold and/or inedible meals. The blasé concerns for my mother's health during COVID. A call-button that did absolutely nothing. Zero concern for personal hygiene. One-Star earned ONLY because of the Physical Therapy, offered.

The food was inedible, the therapy milked every patient for every day they could. Twenty three and a half hours of bed or wheelchairs, and half hour of therapy. Few people rarely graduate to a walker there. While there are a few exceptional staff,(a few nurses, aides, housekeeping or maintenance) most are more concerned with excuses than their patients. There is virtually nothing spent for activities for the patients, despite the sincerity of the people trying to keep the residents involved. There is one dementia resident whose rights supersede the rights of the other residents. He wanders the halls, entering other resident's (usually female) and stares at them in bed. Complaints are answered with "he's harmless" or "we're working on it" or threats and retaliation. Apparently excuses are more important than the privacy, security, or comfort of their residents. He urinates on the floor, and walls exposing himself to others, but according to management he can't be restricted no matter how many times he violates the rights of others. My significant other had a wet towel and used diaper put on her bed, and another day had feces smears on her blanket. I apologize to her every day for sending her there. This is by far the worst facility I have ever seen. The ONLY person exceptional at this facility is the Admissions Administrator who manages to continue to sell this abomination. He does try to intercede on the residents behalf, but his role is limited. No one should send a loved one here.

My father and I were still grieving the loss of my sister, he had been through so much in a short period of time, including the fact that he moved away from his friends and home in Florida by moving to New York. It was crucial that we found the place my dad could happily call home. I researched many communities, however, this time I had the benefit of bringing my dad on the tours so that he could decide for himself where he wished to settle in. The Arbors was his pick! Lisa gave us the tour and my father was warmly greeted by the staff as well as the fellow residents. The Arbors offers a plethora of activities everyday and my father is hardly in his room. It's a warm and friendly environment. The staff is attentive and my mind is at ease since I know that my father's medical needs are addressed. My father has been residing in The Arbors since 2023 and continues to tell family and friends that he is "living his best life" and has absolutely "no complaints". He has a beautiful apartment, which is cleaned everyday by the staff...it's as if he's living on a cruise ship, except it's on land! Big Thanks to The Staff and Residents at The Arbors, you've made my father and I so happy :)

They only call you when there is an issue. If your family member has "too many issues" they don't want them living there any longer. I thought as an assisted living facility the staff would be of assistance but much to my chagrin they were not. They were rude when you called, did not relay messages and when asked for a care plan meeting it took weeks to get one scheduled. Please, please do your research. They obviously only want "well-behaved" residents. So now we have to scram to find a new place.

I live here, and I need to warn others: this place is not safe. Most days there are only one or two…read moresupervisors for the whole building, and staff don't respond when you need them. I've been left waiting for help for long stretches, and when they do come, they are rough and impatient, like I'm a burden. The building is dirty, the bathrooms often smell, and the environment feels depressing. I've seen residents yelled at and treated with no kindness. Once, I even witnessed someone being restrained for hours without anyone checking in, which was frightening. Supervisors don't take responsibility--they push their jobs onto others who shouldn't be doing them. When inspectors are around, they make everything look fine, but as soon as they leave, we're ignored again. I don't feel respected, safe, or cared for here. If you want your loved one treated with dignity, please find another facility.

My best friend always goes to St. James, and while my father is in another facility due to needing more supervision, I don't feel they care for him the way St. James cares for my friend. The attention and compassion at St. James are unmatched, and they even offer iPad calls, something many other facilities don't provide. The staff is amazing, and I truly believe that comes from strong leadership. The head of this facility, Cristen Wei, was our case manager years ago and was promoted to Administrator. Because of my loved one's needs, we have used this facility for years, and I can confidently say I love all of the staff and absolutely adore Cristen. She is the most patient, kind, and compassionate person, and it's clear that she has created a culture where the staff goes above and beyond for their residents. Also, several times she was soaked and when asked the aides to change and clean her, we were given attitudes or was told they weren't the aide for the room and the aide was on break so we had to wait. Almost impossible to get the dr to come talk. Medication was given extremely late almost every dose. Social workers did not seem to care. My grandmother went in doing well, and went downhill fast. Then they screwed up the paperwork to transfer her to hospice at home and I had to get involved. Finally got her home where she passed two days later. Her death was due to neglect, she was in so much pain and I truly hope no one else has to deal with that place. It is a horrendous place and even had another patient tell us to get my grandmother out of there.

I had a family member who stayed here for two weeks following a hip replacement. During that time they developed a stage 4 bedsore that never healed up until they passed a year later. Whether the problem is the facility being understaffed, neglectful, or spread too thin, my main point is that you cannot leave anyone you care about here with peace of mind. We were there every day in shifts because we had multiple family members take turns since we all work and it was always a struggle to get a staff member in the room to help the patient stand up. For a bed sore to form that quickly, they probably didn't get to stand up at all aside from the times we flagged staff down. I wouldn't recommend anyone to have a loved one stay here unless they can have someone there with them 24/7 and make sure they get attended to.
