They would never…read morebrush her hair so it became so tangled, when we asked that they please just brush it once a day a CNA cut off the tangled hair in the back and left the rest of the hair long. Her personal belongings were constantly "misplaced", her room and her were always unhygienic. We could NEVER get ahold of the nurses station, but when I would be there the RN would be sitting on her personal phone on Facebook. She developed multiple bedsores while there. And that's not even everything that was wrong with place. If you have the option to have yourself or a loved one in any other facility I would recommend never sending them to Menifee lakes. There were 2 CNAs we worked with occasionally that seemed to genuinely care, but other than that the experience was absolutely dreadful and made the already devastating process of losing a loved one even more stressful. Also I don't know when these pictures were taken but they are out of date and don't represent the facility well.

My dad was in this facility for 3 weeks. He had tests and procedures done with no results given so unsure if they were necessary or ordered from his doctor. He arrived with a catheter in place and without our knowledge if his doctor had sent orders, it was removed. In his discharge paperwork it said his retention of urine was unspecified so we are unsure if they were monitoring him. 3 days after returning home he complained of sever stomach and lower back pain. At his urologist appointment she discovered 900 ml of urine in his bladder which average is between 20-30 ml. She couldn't understand why the catheter was removed and placed one back in immediately. Within a day he felt better. We have tried to reach out to resolve things and no one has responded. Overall, the lack of communication and attention were minimal at best.

The woman that Gabby sent to care for my 90-year-old father was off her rocker and had to be removed from my parent's home. Gabby was as shocked as I was because she had used her for other assignments and had never gotten any complaints. Also the woman apparently had good reviews online and had a lot of hospital experience. But bad people can slip through the cracks in any business, especially when they're being hired on a contract-type basis. My caution to anyone using any agency or hiring a private caregiver is to do your due diligence. Do not hire an agency and walk away feeling secure that your loved ones will be well taken care of. Had I not been watching on the camera to make sure that my parents were safe, they would have been traumatized by this woman being in their home all night. Even after getting her to leave, my mom who has Alzheimer's, was terrified that she would be coming back despite me ensuring her that she wasn't. Anyone who is doing long distance care of loved ones living at home, I recommend if you don't have them already, get cameras installed and diligently watch them, especially when a new person comes to the home whether they're from a private hire or an agency. This will ensure that your loved ones are safe and well taken care of.

I had no idea where to start. After being given a few options from various people I began making calls. As soon as I had Gabriela on the phone I felt immediately at ease. She went to visit my mom in skilled nursing so she would be able to find a caregiver that matched my mom's personality and boy did she knock it out of the park. The caregiver she assigned to my mom was kind, communicative, patient, reliable, and the list goes on. I was thrilled, but more importantly my mom was over the moon. When our caregiver was unable to make her shift due to illness, Gabriela texted me right away and was prepared to find a back up. Gabriela herself even offered to run an errand for my mom on the day that this happened. I cannot recommend Constant Companions Home Care enough! We had.a very positive experience and if I ever find myself in need of a caregiver again, I know who I am calling.

For 4 months I was unable to get out of bed. Without exception the staff never gave up on me. In the beginning I couldn't even feed myself. Everyone worked very hard to make me believe in myself, they succeeded. I can say the only reason anyone would, want to do the difficult work that everyone does here is because they truly care, it certainly isn't for the money. My recovery was slow and difficult, but I realized from the beginning it would be a marathon not a sprint. The physical therapy department is staffed with well-trained people who care and always create a positive and friendly environment. The activity department work very hard to provide everything from popcorn and movies to bingo games and many other fun projects. After being told I might never walk again, last Friday I walked almost a full mile. I can now do all the things necessary that we take for granted, showering, dressing myself, putting my shoes on and lacing them. Etc.., etc. I am finally ready to "graduate" from here and move on to another much lower level of care. I will miss all of the amazing people here who have made this possible, they truly feel like family. So, yes, I have no problem recommending this facility with only one reservation. The food, for whatever reason is not up to the level that rates 5 stars, but because everyone else is so exceptional I am willing to reluctantly overlook that shortcoming.

My uncle was a very difficult patient but the care person diffused the situation, gained his confidence and made his last 2 months of life, comfortable and dignified. They took complete care of his physical, medical and most importantly his emtoional needs. They were in constant communication with our family in updating his condition. At the time when Hospice became involved in his care, they continued to provide for my uncle as if they were his primary care givers. My uncle felt confidence and care from them and minutes before his passing his caregiver read with my uncle, his favorite passages from his bible....i couldn't ask for more personal, kind, and loving care for my family member.

My 89 yr old dad had emergency surgery, and went to a rehab/nursing facility. He was experiencing 'sundowners' at night. (total confusion, combative, danger to himself and others, and a fear of him pulling out either his pick line, or catheter.) I contracted (through the agency) the same caregiver he had at home, to be a 'sitter', with him, from 9:00p-9:00a. I was to pay $160 for 12 hrs. The same amount I was paying for 24 hr care, which included light housekeeping, driving, meal prep, help with personal hygiene, etc. When I questioned the cost, I was told that because it's in a hospital setting as a sitter, it would mean that the caregiver would have to BE AWAKE WITH HIM AT ALL TIMES, different from being at the home, and sleeping at night. I understood. The caregiver texted me, the second morning, stating that she was 'sorry, but he pulled out his pick line, while 'she was in the bathroom.' What???! This resulted in my father having to be ambulanced back to the hospital, to have his pick line re-inserted! (A pick line is serious, not just an IV) Because I wasn't thinking clearly, I believed her. At the time I was exhausted and a basket case from what I had been dealing with, with my dad that week (life support included, and my 88 yr old mom at home experiencing early signs of dementia.) Think of my horror, when the night nurses pulled me aside later that afternoon, to tell me they had caught the caregiver laying down on the couch, IN THE TV ROOM, not once, but a couple of times, throughout her shift!!!!! The TV room, isn't even close to where his room was located. When I contacted the agency owner in a rage, she tried DEFENDING the caregiver by saying that 'in hospital settings, it's not unusual for things to happen at times.' I'm sorry, 'things happen at times', when 1 nurse is taking care of 5 patients and can't be 5 places at once. I paid $160 for 1 on 1 so 'THINGS WOULDN'T HAPPEN!!' Do you leave a toddler in the bathtub by himself for 2 min? Let alone, go lay down in ANOTHER room?? Do you leave a toddler alone near a pool? 'THINGS HAPPEN' in 5 seconds!!! She was supposed to ring for a nurse, and inform her/him that she needed a bathroom break, etc., so they could keep an eye out. The nurses informed me that sitters are common for some patients, and that they had never seen this happen before. That sitters never leave the room, and ONLY for a bathroom break, with someone covering for them. The nurses told me that when the caregiver informed them what had happened, she actually told them that 'she wasn't where she was supposed to be at the time.' And all of the nurses involved agreed, that they didn't buy that it happened when she was so called 'in the bathroom.' We also had determined that some money had 'gone missing' from his wallet, in the time the caregiver took my father to the ER, and left with his clothes and belongings (already in car by the time I got to my dad) to go back up to the house to be with my mom. Needless to say, we never mentioned this, because obviously, unless you catch someone in the act, you can't prove anything, but it was very unsettling and we really didn't want to believe it. So NO, I CAN'T PROVE THIS, but it's seems like an awfully big coincidence AND more suspicious, since now my trust in this person was completely shattered.

One of us was there every day and we would come at different times my Dad was always clean and on his good days was very happy. Even though his Dementia had gotten worse and sometimes he didn't recognize us they would remind him that his family was coming to see him and he would wait for us to arrive. He became very attached to the staff and they became very attached to him. I wish we could mention you all by name but there are too many angels working there. We decided to take him home for the last few days of his life so we could be with him. We cried and so did the staff as we were going to miss them all so much. My doctor recommended Sundance Creek Post Acute in Banning, so I went online to do a little research. Sundance Creek did not have the greatest reviews but then again the other facilities in this category did not fair that well either. So Sundance it was! It is established that online reviews for rehab centers can be mostly negative and grim. I decided that since my stay went as well as it did, it was incumbent upon me to make sure that my positive experiences deserved to be shared with others so that there is hope for those who are seeking a facility that offers promise. Following is my takeaway: EXCEPTIONAL CARE: All of the CNAs and LVNs were caring, helpful, and even funny, but I have a special shout-out to Louis Obioha. He has a truly caring CNA heart. If you had any anxiety before his shift started, his calming presence, while very caringly addressing your needs, leaves you with an "all is well" feeling throughout the rest of his shift. MEDICATION CART: "Singing Danny", Karla and Vanessa (LVNs) brought their knowledge of all the medications that you should be taking along with much cheer to the daily med passes. EXPERT THERAPY: The physical therapy team is top-tier and abundant. Their knowledge and encouragement were the primary drivers of my recovery. FACILITY & COMFORT: The facility is "clean as a whistle" thanks to Mr. Longie, who is hard at work by 4:00 AM., who takes great pride in his clean and shiny floors! The housekeeping staff were very thorough with their daily sweeping, dusting and mopping of our rooms. My room was very clean and bright, the bed was comfortable, and my linens were changed daily. LEADERSHIP: The Director, Mr. Brett Kendrick, is an empathetic leader who ensures every patient's needs are met. He wears many hats. Organizer. Peacemaker. Overseer. Facilitator and is able to pick up the slack of others when necessary. He truly is the anchor of this facility. SOCIAL LIFE: From bingo and crafts to church services, there was always something to do in the activity center. My only minor gripe is the food. For the Love of Benji. Can nothing be done to make institutional food palatable? That said, I have to tell you that my choice to stay at Sundance Creek was the absolute correct decision to make. I do not regret the option that I chose. If you need skilled nursing and rehab, Sundance Creek is absolutely worth your consideration. I am including a few pictures of what happiness looks like.

What my family and I witnessed was not just disappointing, it was inexcusable! From the moment she was admitted, there were immediate red flags. Despite the presence of numerous staff members, basic standards of care were not met. My aunt was repeatedly left in soiled garments for extended periods, and critical medications were delayed or entirely missed. The neglect was not subtle....it was blatant. Angelica, who was assigned to her floor during several shifts, exhibited a level of indifference that was both shocking and dangerous. Rather than tending to my aunt's needs, she seemed more concerned with avoiding responsibility. On one occasion, I personally witnessed Angelica ignore multiple patient call lights, including my aunt's, for nearly an hour. Melissa, another staff member, displayed an alarming lack of compassion and professionalism. When asked direct questions about my aunt's care plan and physical therapy schedule, she responded dismissively and without any clear knowledge. It took over a week for any form of physical therapy to be initiated. And by then my aunt's mobility had severely deteriorated rapidly. Amanda's conduct was perhaps the most disturbing. She was observed speaking rudely to several patients and was especially careless during basic hygiene care. There were several days where my aunt was not bathed, and her wounds which should have been routinely cleaned and monitored were visibly worsening. Amanda never once acknowledged this or reported concerns, despite it being her direct responsibility. The facility itself was in disarray. It smelled strongly of urine and feces. Patients were heard crying out for help, and some were even seen wandering unattended. This is not a safe or healing environment. When I brought these issues up with the director of the facility, the social services director I met with passive indifference and redirected me to a case manager, Melinda, who seemed more interested in placating us as a family than taking action. Every conversation felt like a bother rather than genuine concern. To add insult to injury, when we decided to transfer my aunt to hospice care due to the rapid decline in her condition they still were completely unaware of what was going on and of her case. She was treated worse than a dog. There may have been a few nurses who tried their best, but isolated acts of kindness cannot undo the widespread and consistent negligence that led to my aunt's decline. This experience has been nothing short of traumatic for me and my family. I urge anyone considering this facility to think twice. The level of carelessness, unprofessionalism, and outright neglect we encountered has had lasting consequences. No one deserves to be treated this way especially not our vulnerable loved ones... there needs to be a better standard of code for this facility and they need to be held accountable for their actions! In other words if you have a family member in need of care I would steer clear from this facility. Hell you'd probably be better off taking care of them at home with no experience.
