Communication with the doctor and home health didn't happen because I didn't know or convey side effects to the following: Do not use newfangled digital machines on everything! They are not tried and true and nothing like old fashioned methods. I talked, conveying concern to family members these were not good..turned, souring making injuries and a health trip to the hospital after home health had to remedy streaks from wounds! Tell your doctor they've done studies. Just say that was a bad move. Let's not try that again causing streaks! Next, I provided, asking to cover one of the wounds with a sticky band aid on hand! Bad mistake for home health because it went beyond blistering and caused panic! After getting The Wound Pros they said do it one way and I forgot and no one fully understood the potential of the wounds again! But it happened! Lastly, and no one could have anticipated and individuals don't need to know the details of repeated hospital admissions and rehab -- even a gentle alternative was too much for the wounds but through home health they got this patient moving again.

You truly have a heart like Jesus, as He said, 'I have come to be a servant of all and show you the Father's love through my actions.' Your daily actions, such as always entering my home with a sweet greeting and a smile. No matter what may have occurred on your drive to work you never let it change your kindness and compassion towards me. Your daily respect for my autonomy and personal choices have been a great relief and blessing to me. Last year, 2025, was enormously difficult for me physically, emotionally and mentally. I was badly injured by a caregiver who really didn't want to be with me. She wanted to run my life instead of letting me make my own decisions. Everything was a struggle. I am still dealing with the pain and suffering from her carelessness almost a year later. You, however, are the complete opposite of that caregiver. You show genuine care, compassion and empathy to me. At the same time you keep a close watch on me for my episodes, for any unusual reactions, for changes in my body or mind that may indicate something physically is about to happen or something that is happening without my knowledge. Your daily care of me is exceptional. Everything from reading my body signs or noticing that I am in the middle of a call, task or just not ready. Yet, you come back or tell me it's getting late for something we have scheduled. This may be as simple as my breakfast or as important as helping me to get ready for my doctor appointments, my bloodwork or therapy. You encourage me to try many things that can help my health, mood or my outlook for the day. Such as, helping me relearn my Spanish while I help you with English, or walking around the house with my gait belt and walker. You keep me on top of my goals. These can be as basic as making my bed every time I get up (in the morning and after my nap). In fact, this helped me create and continue this habit. Another goal of mine is building my strength and stamina enough to walk around outside my home. This has not happened yet, as I have been primarily bed bound since February 19, 2025. You encourage my stretching in bed to keep my body as limber as possible. This helps reduce injury in case of a fall. My personal Internist, Dr. Fondren of Newport Beach certainly appreciates your encouragement and reminders to help me to regain the strength I have lost since February 2025. (when a different caregiver injured me). Once I have regained a little more endurance, then I can start physical therapy. Finally, my family has been at peace with you being my main caregiver and I love how well you and Gabbi work together caring for me. Just knowing how dependable you both are, how you communicate with each other changes, concerns and even my meal planning and cooking. My sister and my son (they both call me almost every day) are amazed that I finally have a team like you and Gabbi. My mother was being released from the hospital into home hospice, and we needed around-the-clock care on very short notice. She was bedridden and required intensive, attentive support. Allen Tahir was our primary contact, and he was extraordinary. With professionalism and kindness, he quickly assembled a team of competent, compassionate, trustworthy, and reliable caregivers so that we could bring my mother home as she wished. Every caregiver who came to our home was wonderful--skilled, gentle, and respectful, not only of my mother but of our entire family. When my mother sadly passed, Allen sent a beautiful bouquet of flowers along with his heartfelt condolences. The flowers were so meaningful and lovely that we featured them in her memorial service. Tami took the time to ask thoughtful questions about my mom and truly understand her needs. After presenting several options and discussing them with me, she refined the list down to two homes and ultimately recommended the one she genuinely felt was the best fit. Having been through this process before -- my grandmother and my father both lived in local care homes -- I've seen many facilities and can easily tell which ones are well run and which ones are not. I worked with Jan, who covers the Ventura area, and she was an absolute pleasure to work for. She was very friendly and personable, asked me questions about my father's needs, budget, and area of interest. She then prepared a list of 5 different facilities, set up tours, and toured the with me to ensure all of my questions were answered. She also had a wealth of knowledge about each facility, and helped me decide the best fit for my father. After moving him in, she also checked in regularly to see how the move was going, which was a very nice personal touch. I would highly recommend this service for anybody who needs to help a family member move to assisted living. It made the whole process easier and more manageable.

Being on non-weight bearing status for an extended period of time required special adaptation training and acquired skills. RN Petra was very thorough & professional in her review of my situation, including supervising medications and limitations. She made recommendations to check with my doctors and made sure I had everything I needed. I felt she listened well to any concerns and questions I had so that I was comfortable being at home with assistance from family members. She shared a story on a situation she'd dealt with that gave me a lot of hope about getting through this time. Physical Therapist Kristi is a delight! She's been in the profession for many years, which means she brings a wealth of experience. Even more, she applies that wisdom with a fresh sense of figuring out what works best for the individual patient. Several times I was discouraged with the challenges I faced to follow the doctor's orders, while still needing to do practical things, like get up the stairs. Previous injuries (and fears) came into play. She made sure that I had time to rest and regroup before moving forward. And she voiced the right measure of encouragement without making me feel in any way patronized. Well done! Her reassurance and problem-solving skills gave me the confidence to try again and triumph. Her personality is so well suited to her job. The joy is the way she shares it in such a genuine way. I felt truly cared for, heard, and encouraged to make strides. I achieved measures of independence that were valuable, like being able to get back to work. I still need help from others until the cast comes off. However, Kristi also reduced the load others have to carry for me by teaching me safe ways to care for myself. We even collaborated on a new way to handle stair climbing that produced an "aha!" moment. Worthy of a high five! Based on the experiences with Petra and Kristi, I feel that there are likely other team members at Access TLC with similar skills and outlook for the way they care for patients.

A brief exchange with front office personnel makes obvious marked deficits in the areas of reliability, communication, professional conduct, emotional regulation and follow-through. If this is any indicator of company culture and overall professionalism company-wide, I would encourage those not limited by service area restrictions to strongly consider an alternate agency, one capable of meeting the sensitive needs of those they service, and whose customer service interactions better reflect the company's mission and values. For those subjected to the monopoly Access TLC has on the Ventura county service area, I cannot stress enough that you will likely be better served by avoiding any case-handling by office assistant, Gabriela P. This individual is tasked with the critical role of proofing and submitting paperwork (on your behalf) to state-level agencies. Several business days after initiating contact and verbally confirming receipt of documents and stating they would be submitted I received confusing telephone call in which she claimed no independent recollection of our conversation and reported an issue with the paperwork (which she had cleared days prior). This resulted in further authorization delays because the documents had not even been submitted. When confronted with an account of prior communication she simply blamed the situation on her having to "manage hundreds of client forms" (as is her job). Rather than apologizing or admitting fault Gabriela became argumentative and tried to take corrective action to swiftly cover her mistake. When pressed multiple times for an alternate resolution she interrupted repeatedly, mid-sentence, and in a bizarre act of gaslighting, accused me of actually yelling at her. After asking for her supervisor I received the same maltreatment from "leadership" who was again dismissive of the confusion and mistake they created. It was an unfortunate and frustrating encounter. The ordfeal was nothing short of a messy disaster. I disagree with AccessTLC's policy of delegating high priority tasks to an office assistant, rather than a case manager. For reasons mentioned, a more senior person should be appointed to serve in this role. Please be sure to request written or email confirmation from this organization on every aciton they claimed to have taken. A paper trail is necessary. When possible, contact any involved third parties to ensure a task has actually been completed irrespective of any claims made by AccessTLC. Do not trust, only verify. You cannot fully trust an organization that employs individuals who operate in this way. Beware of the "Gabriela's" of the world who seem to exist only to makes the lives other others more challenging
