PLEASE DO NOT TAKE YOUR LOVED ONES HERE! My mother was admitted yesterday afternoon and when we checked on her at 10am today she hadn't eaten or had her meds yet. My mom had a stroke and lost the use of her left side. So when we questioned the meds they advised that meds were left on table (which she couldn't get to by herself) nor did they tell her about them. There were several vips of meds sitting there. Mom also called for assistance to go to the bathroom. After almost an hour she had not been helped and she was trying to get to the bathroom herself. We are moving her to a new facility today. This facility should not be allowed to continue business as a 24/7 care/rehab. Someone needs to look into this establishment. read more

WARNING!!!!! We placed my mother-in-law at Allegiant Wellness after she suffered a stroke, trusting that she would receive compassionate and attentive care during a vulnerable time. Unfortunately, our experience was deeply upsetting and unacceptable. One of the most disturbing issues was the way patients were treated regarding basic bathroom needs. Regardless of whether residents had some mobility or ability to transfer themselves, they were placed in diapers and told to simply use them rather than being assisted to the restroom. My mother-in-law is capable of getting herself from the bed to a wheelchair but cannot safely make it to the bathroom alone. She repeatedly called for assistance for over three hours and was ignored. When staff finally responded, they acted inconvenienced and stated they "had other things to do." The lack of compassion and urgency from parts of the nursing staff was shocking. Several interactions felt argumentative and dismissive rather than caring or supportive. No family should feel like their loved one is a burden while recovering from a serious medical event. The food provided was also extremely poor quality and appeared to lack proper nutrition for rehabilitation patients who need strength and recovery support. Within 24 hours, we made the decision to remove her from the facility and return her to the hospital because we no longer felt she was safe receiving care there. I would strongly encourage families to thoroughly research and closely monitor the care their loved ones receive before choosing this facility. read more

Took my wife there after her stroke for rehab rehabilitation The food was not edible. The service was nonexistent. They left her medicine on the tray and neglected to tell her that it was there never ever take anyone you care for to this place. It is terrible. read more

Zero STARS!!! WORST of the WORST!!!!! The administrator and the director of nursing are Incompetent. They intake as many patients as possible while employing less than 1/2 of the staff they need. Consequently the patients are neglected elders who are trapped with inadequate hygenic care which has lead to ECOLI - bacteria spread and Urinary Tract Infection and Isolation. NEVER, NEVER, EVER consider this place if you care for your loved one. It is a place of TOTAL neglect. read more

Our dad was sent here for pt and 4 days later still no pt and didn't know when would start . They his legs started swelling and seeping fluids and not one person up there seemed concerned. So we took it into our hands and got an ambulance to take him to hospital and found out not only did he have fluid in his legs but had moved To his chest . They also were not suppose to be giving him salt in his food and every meal was loaded with salt and salt on His tray . WTF???? He literally would have died there if we hadn't stepped in and got him medical attention. Do not I say do not take your loved ones here unless you want them to die ... read more

If you love your loved one, then do not consider allowing them to be placed here for rehab or care. I know they try hard, but not hard enough. We had our mother placed here and it didn't go well. Twice. Her food was poor, the care was at time negligent, and overall we took her out early and brought her home for rehab in-house. While not ideal it was better than leaving her there. read more

Was rehabbing after cervical /spine surgery, I began to have pain that was more intense only to find out pain medicine that was given to me. Every 12 hours was supposed to be every four hours. I continue to decline. The pain was shooting down my arm. My fingers were numb. I called the surgeon. He said he wanted me to come and see the ER because I was showing signs of a blood clot. They said oh well will call you a MedSTAR then someone else walked in 15 minutes later and so we are not calling you an ambulance and I said well OK well I can call my own ambulance so I called 911. They were very upset. Then I called 911,3 of them, walked into my room and said who told you you could call 911 the amount of intimidation for you to leave there in an ambulance was like a series from American horror story I ended up being readmitted into the hospital needed an MRI needed x-rays. I'm now on my third day in the hospital getting steroids in my IV ?. The one good thing that I can say is there were two ladies who were doing my physical therapy. I'm sorry I can't remember their names, but they are the Albert and Costello of physical therapy. These women were absolutely incredible. I would've taken them home with me. I hope someday that I can thank them for their compassion and their professionalism. It was the funnest physical therapy I've ever had in my life absolutely the bright spot of my stay. read more

I guess the best way I can explain our experience with Allegiant is by telling you the change we saw in our relative during the time he's been here. He arrived in mid-May after a fall and a hip replacement He was completely disoriented, non-mobile, full-lift and malnourished. (At the hospital he was spoon-fed and only ate 20% at mealtime.). The night he was admitted to Allegiant I had quite a shock! The hospital discharge orders did not have his pain medication on the list. In a panic, I asked the Allegiant RN on duty for help. Instead of waiting to contact the hospital in the morning to correct their error, she contacted the rehab's attending physician. Within the hour, Charlie received pain medicine. In the early days of his stay, his appetite was still poor, despite supplemental Ensures and coaxing. I contacted the nursing department. An appetite stimulant was ordered. Within a few days he was eating 75% of his meals. The care team was kind to him, with their help and that of the excellent physical and occupational therapists he improved. Especially kind to him was the Activity Director, Vivian. Charlie can transfer himself with supervision, has gained weight and is neurologically improved. Back to his old cheerful self!. i do not know anyone here or have any tie to Allegiant. I am a family member of a reaident. read more

I am currently a resident here and I am not happy!Caseworker at Harris Southwest said it is rated Five Star. Definitely not! I believe they are short staffed as it takes forever to get my basic needs met, i.e., help with toilet, etc. And don't get me going on the food. Today's dinner was supposed to be sliced ham, baked beans, fried okra, cornbread, and fruit cobbler. Instead we got ham, blackeyed peas (second time this week, overcooked rice, and a cookie! I've been here nine days and have have one decent meal since arriving! read more

Some of the staff here are amazing however, there are always exceptions to the rule. Allen R, the admin, is good at passing the buck instead of being real with information. He, along with, Tiffany, the social worker, authorized the moving of my mom, patient with dementia, from this location to another location without any family permission or involvement. Doris, the director of nursing, does a poor job of covering up the truth and tries to deflect family member's by blaming another, who was not able to make that call. I know this place is new and pretty but they are starting off on the wrong foot. The nursing staff is the reason this review has 3 stars. They were amazing and if management had been better it would have been a higher star rating. read more

just now new this facility is horrible. they are not attentive to the patients needs...in fact, i truly believe that they do not care about the patient. and if you are going there for rehab, forget about it, because they sure will forget about your rehab, there were days that they did not even show up for rehab. this facility looks nice on the outside, but they are not doing right by the patients on the inside where it matters. a major disappointment. i have since had an opportunity to speak to someone else who went to this same facility and had a very similar experience. smh...this place appears to be just a "moneygrab." i am also questioning some of these reviews. read more

My mother has been here twice. The first time was ok but she wasn't there long; just recovering from illness. The second time was for physical therapy after a fall and it was a complete nightmare. The staff is extremely rude, there is no access to patient records, she was not receiving the therapy she was supposed to be getting and if I wasn't there every single day checking they would've let her go without food and showers. Do NOT put your loved one here. They do not care. read more

This is THE WORST rehab facility to ever send your elderly patients !!! my mother wasn't given her blood pressure meds on time or DAILY, neither was she assisted in eating either! Allowing her to sit in her feces No doctor floor nurse called my family once she got there from the hospital. The only call my family received was did my mother have a DNR order!!! Then complained that our mother being so tiny the damage that would cause her body ..... was appalling!!! Once our family realized the inhuman treatment of our mother, We went to administrators to request discharge of our mother and then they called Doctor ACOSTA to release her and he refused, he hadn't even seen our mother but REFUSED SO the next day we took our mother out without his consent and brought her home!!! We had been in contact with our mothers PC Doctor and nurses before taking our mother out of that place, that we accessed with our own eyes that ALLIGIANT is not medically appropriate for humans and medically incompetent !!! Worst place ever my brother was here for rehab. Two nurses shana and Linda dropped my brother while putting him in a wheelchair. Then still waited 1 1/2 hours to call ambulance. He had a brain bleed . He had to have brain surgery . Then they denied responsibility read more

Wonderful, changes in the new Administrator. Staff is caring, friendly and truly care about the patients and the facility.

The staff is very caring here and the therapists were exceptional. Julie, Katye and Jeremy were all wonderful therapists who treated my mother exceptionally well during her weeks at Allegiant. There are many nurses and CNAs on staff who provided excellent care for my mother. Doris the nursing director stays on top of the staff and made sure my mother received good care. New, clean facility. read more

Allegiant is amazing! I highly recommend Allegiant for any skilled nursing care or therapy needs. My mom was here for over a month for a complicated medical problem, and the staff was patient, caring, and very attentive. Thank you for taking such great care of my mom! read more
