I urge all potential patients to be extremely cautious. ​Fabricated Medical Records and Misrepresentation of Care ​The Progress Note documented after my visit is a deliberate fabrication that fails to accurately reflect the brief, negligent encounter I had with the doctor. ​False Physical Examination: The record falsely states that Dr. Waravdekar performed a "comprehensive body examination." This is unequivocally untrue. The doctor performed only a brief cardiac and pulmonary (heart and lungs) check. No other body system was evaluated, making the documented exam a severe and unethical misrepresentation of the clinical service provided. ​Irresponsible Psychological Diagnosis: The documentation under "Psychological Evaluation" is medically negligent and appalling. After a mere five-minute encounter, Dr. Waravdekar concluded that I "do not have depression or anxiety." This conclusion is completely invalid given my history: I have been under the continuous, long-standing care of a psychiatrist and a therapist for the past seven years, with twice-weekly sessions. The attempt to rule out serious, chronic mental health conditions in such a cursory and unfounded manner is deeply troubling and appears to be manufactured solely to justify a more complex and expensive billing code (likely submitted to Medicare). ​Unacceptable Lack of Service and Professionalism ​I scheduled this appointment specifically for severe insomnia. I explicitly informed the front desk staff that I needed treatment for persistent sleep issues and already possessed a CPAP machine, making the reason for my visit clear. Despite this, within five minutes of speaking with Dr. Waravdekar, he dismissed me, stating he could not help and that he "only deals with CPAP issues," instructing me to see a specialist. ​This suggests gross incompetence by the office staff, who failed to properly triage my stated needs to align with the physician's admitted scope of practice, resulting in a wasted appointment and unwarranted billing. ​Lack of Accountability and Failure to Correct Records ​I formally requested the immediate and complete correction of this fraudulent progress note within seven business days of my visit. The practice has failed to comply. ​Furthermore, on October 13, 2025, I called the practice and spoke with the Office Manager, Kim, who provided an alarming response. She stated that "no one in the practice has an oversight of Doctor Neil, as they own the business." This statement demonstrates a shocking lack of accountability and suggests that Dr. Waravdekar operates without supervision, facilitating the creation of fraudulent medical records with impunity. ​Next Steps ​Due to the practice's failure to correct the fraudulent medical record, and the admission that they lack oversight of the physician, I am proceeding with my promised actions. I will not pay the bill for this negligent visit. ​I am reporting the false diagnosis and the entire fabricated report--which I view as a blatant and likely widespread scheme to fraudulently inflate charges to Medicare--to both the Medicare Anti-Fraud Unit and the State Medical Board. ​I encourage anyone who has experienced similar issues with Dr. Waravdekar or this practice to come forward. She originally went into Neuropsychology, but it seems she obtained a Psychology degree and degrees in 4 areas of medicine. Katherine has always been super compassionate with me. I was hyperactive. The teachers said I had a learning disorder. The doctors said I had a high verbal IQ. I went to prison because I had issues, maybe with being adopted, drug addiction, and a lot of depression. I just couldn't love or trust people. No intimacy. Katherine sent me Principles of Neural Science while I was in the joint. No matter how busy she was she took the time out to listen to me. 35 years down the road, just a year ago, I'm on the verge of suicide. Done attempted already. I come across an MRI of a depressed brain relative to a normal brain, and I can interpret it completely due to Katherine. I stumble upon the drug ketamine and hear about how it fuels "synaptic plasticity". I know what that means too due to Katherine. I learned about "tryptamines" and kicked the shit out of my depression. Katherine went to sleep disorders because her father suffered from insomnia I suspect. Her father was a speechwriter. Kathrine is high intelligent. Her mind is incisive like no other I can recall, and I'm throwing in Neil Peart, my idol, too. I can only assume for whatever reason your interaction with her didn't go as planned. Sometimes that happens.

These folks were reasonable to work with, tried to keep things going (positively) even after the appointment experience went sour. Office manager stepped in to seek resolution too. THE REST: I am not taking issue with any one person. I am taking issue with what appears to be the need to review practices, procedures, written instructions for new patience. Those items may seem unimportant, but in reality those components set the tone for a patient/medical staff relationship. I set up an appointment in February for earliest opening, June 1, 2026. Was referred by my doctor to the practice so I waited. Apparently the Pulmonologist are considered top quality and worth the wait. Received a "new patient - prepackage" of paperwork which was completed for submitting at time of appointment. (also completed multiple electronic information request from the practice/Advantia/Privia . ) In the prepackage, ion the cover sheet was stated in caps and in one part underlined and partially in bold letters: ".....you must bring your paper referral at the time of your office visit. Please note: WE CANNOT SEE YOU WITHOUT THIS PAPER REFERRAL." Prior to that it stated that a patient must bring a paper referral at the time of the office visit if their insurance requires it." Guess what stands out if you are a new patient?! Paper referral! Well apparently my insurance had a relatively new policy of wanting electronic referrals sent. [Noted however is that medical practices I went to in the prior month (3) flagged me and asked moving forward that an electronic copy be submitted via the insurance company portal. None of those appointments were cancelled or rescheduled. ] When I arrived, with paper referral in hand (per written instructions) I was promptly told that my insurance required an electronic referral, my appointment was going to be cancelled, and I was going to have to pay the cancellation penalty (cost provided in writing in registration package: $75 (new patient consultation) $125 (PFT). After the confusion had passed, catching my breath, and questions were asked, I have to be honest and say it "over my dead body " moment when it came to paying a cancellation fee. ( I had arrived 30 minutes early for intake.) I have had experience with having team/staff and I understood, that the young woman at the front desk was following her marching orders. Voice was firm, but also kept repeating that the problem was not a person but poor communication, poorly written instructions. It was not acceptable to then pronounce a monetary charge. (My own primary doctor's office was on the phone with me at the same time and was trying to push the paperwork through from their end. ) After several back and forth, the manager was asked for. Again, despite sharing about the Bold Print CANNOT SEE YOU WITHOUT THIS PAPER REFERRAL, the confusion it caused and bewilderment that (while other medical offices kindly do this) no caution was provided that a electronic and not paper referral would be needed. It is common practice when the text/email/phone confirmations come through reminders about the type/form of referral needed is address. Eventually after discussion with the office manager, and no progress I left. The matter of the fee had been resolved, and I appreciated that. But I still needed the appointment, with multiple doctors waiting for the results. As we were leaving, the office manager approached us in the hall way, came up with a resolution and the appointment was worked in. I also have to give a shout out to the admin staff at my primary office for their in the moment contributions to resolution. People don't typically go to the doctor because it is a fun thing to do. And while not my case, people take off work, spend money for public transportation (or their own fuel) , make all kinds of arrangements to make medical appointments. Staff are typically trying to do a good job. Foundational, the intake process and first interactions with patience (new patience, or returning patience) sets such a tone and is largely the public/customer service face of a practice. Got what I needed, but this was not a good experience. Blood pressure was elevated, and the experience was stressful, and upsetting.
