I left to go shopping at Walmart. I come back to my apartment and they used all of my clean towels that were hanging up and drying. They used it to clean up their mess from the water heater. My bathtub was clean and they left dirt in my bathtub where you wash yourself. They tracked dirt all through my apartment and salt due to the weather that we have been having. I do not recommend this corporation to anybody you people have just lost a tenant and probably will lose more. Buckner properties would be ashamed. My dad was here for rehab…read more The place looks secure. It isn't. Anyone can drive and/or walk in at any time of day and no one will question why you are there. Many unlocked doors at all hours. The "security gate" is rarely attended, and even then they do not check incoming or outgoing traffic. The staff, although friendly and tries their best, is not adequate for the sheer number of temporary and permanent residents. They also lack basic resources, like working equipment, much less modern equipment. The amenities are terrible. No wi-fi. I realize many of your patients are older, but a great deal of them are into technology and would love to use their laptops for their own entertainment. I tried to find a cart (or luggage cart) to haul some stuff to my dad while he was here (a stereo and DVD player and storage) but nothing like that is to be found. Really??? In a place where people stay and are moving in and out all of the time, similar to a hotel, you have no way to help your patients feel more at home?!?!? Ridiculous. The food, although meeting nutritional guidelines, is quite bland and mediocre. Finally, the complex itself is an endless maze of nothing making sense. Their is no central check in, so good luck finding who you are looking for. I've been trying to not write this, since their is a chance of my dad going back, but we will pick a different rehab facility next time.

There was no further care St Vincent could do. St Vincent Hospice is a nice place, friendly Nurses and all, however, little patient care for those that are not truly terminal. It's all about the comfort and easing them towards death. We were not about that for my Aunt. The one Nurse emphasized the need to sign the DNR=Do Not Resuscitate form. My cousin refused. We were looking at hope for her to heal well and get back to living (my Aunt's wishes). We had her transitioned to Home Health from Hospice. It's just the beginning so far, however, there always light at the end of the tunnel. We continue to pray and have God lead the way with Jesus interceding on our behalf. With God all things are possible!!! Amen. Not knocking down this place. It may be ok for some people. It may work for them, however, for us, my family. It did not work. We are on the road towards recovery along with hope.

I wanted to…read morewrite a positive review ASAP, because the day my father was being transferred from home hospice care to the St. Vincent facility, I did a quick google search and nearly had a panic attack after seeing the bad reviews on Yelp. COVID-19 note: As of the end of March 2020, they have a limit of 1 visitor per patient--not 1 person at a time, 1 person period. It's really unfortunate, but obviously necessary. You'll be screened at the door (questionnaire about travel and symptoms, then they'll take your temperature using a forehead thermometer) before you can come in. Obviously there are many parts about a terminally ill parent going into hospice that are awful, but it had nothing to do with the hospice or their staff. Everyone we encountered---doctors, nurses and CNAs---were all amazing. They were great about making sure that my father wasn't in pain, and did so quickly. We never had to wait a long time after pressing the call button for someone to come in, and they were always helpful in suggesting medications that might help his symptoms. My dad was there for 6 days before he died, and I can't say enough about how thankful I am that he was able to be free of pain in his last days. Erin, MJ, Denise and Jess were particularly wonderful. The facility is very quiet. No one is hooked up to beeping monitors, and there aren't any overhead speaker announcements like you hear in hospitals. Patients can watch TV or you can play music for them. Each room has French doors that look out onto a peaceful courtyard with a pond, so you'll often see ducks/geese. There are birdfeeders outside each room as well--volunteers normally fill these, but they're not right now. We brought some of our own birdseed and filled them ourselves. Some amenities I can think of off the top of my head: * family bathrooms with showers. They have towels/washcloths and there are baskets of toiletries if you need something and have forgotten yours. * refreshment station - Coke/Sprite etc. fountain drinks and the really good hospital ice, plus coffee and tea are available for patients and visitors. * they don't have a cafeteria for family/visitors, but you're welcome to leave and bring back food (you don't have to get re-screened if you leave and come back right away) or get DoorDash delivery. * the chapel and other social/meeting areas are currently closed. They didn't reach out to the patients daughter when she passed.. she had to hear from a third party. And when the patient had had a bad fall prior to passing the person incharge named Jason insisted on not calling the fire department and handeling it himself which was a mistake and she couldn't get up for over 30 min and ended up getting help anyway from the firefighters. The food sucks, the patient called it a hell hole it was so bad. The laundry always went missing, the nurses take up to an hour to come when you hit the help button even for emergency!!! they are insistence we get their belongings out of her room so they can make more profit for a new patient to come in. She hated every second in this place and I'll do my best to make sure people don't go here. If you care at all for your loved ones don't take them here!!! And rest in peace to my friend always, you didn't deserve this horrid place, the food, the treatment, the rudeness. They were truly horrible To her in her final days. And now that she has passed they are still being horrible and rushing us packing up her stuff. Called that we have 2 days to do so... she just passed. :( My only positive note is that Winnie was truly special to this patient. She was caring, has patience, was kind and always a glimmer of hope for her. The true bright light and glue of this facility.

Creekside Health and Rehabilitation was a huge disappointment to our family. Our sister's initial…read morestay in the rehab wing was okay, not great and not quite subpar. Due to her health restrictions we were forced to make the decision to admit her as a resident. We greatly regret that decision. The majority of the nursing staff was dismissive of her health needs to the degree that vital medication was not dispensed in a timely manner. She was frequently left sitting in excrement for hours, once almost 24 hours, resulting in wounds on her buttocks and back. Her dietary restrictions were regularly ignored, resulting in the family ordering food being delivered to her. Unfortunately we were operating from out of state so we couldn't monitor her care in person. She has since been released to hospice care where her aides are appalled by the condition her many wounds. The greatest travesty is the lack of oversight for a location recommended by Medicare & Medicaid agencies. The management staff need to be held accountable. If at all possible, avoid this place at all costs.

A nightmare that I want to forget, In 2018 I broke my neck, had extensive surgery to put me back…read moretogether again. I stayed in St. Vincent hospital as long as I could under the supervision of Goodman-Campbell. When insurance said that I needed to be released from the hospital the ambulance drove me to this rehabilitation facility. They punished me by insisting that I sit up without assistance for five minutes. I screamed in pain and slunk out of my wheelchair and was laying partially on the floor before a nurse came back into the room. She chastised me for yelling. I said that I needed my wife, and wanted to call my attorney. She denied my request. That night I had to sleep in a hospital bed that I soaked with urine.....I had a spinal cord injury after all. After trying to sleep in my own urine overnight a morning nurse came in. He was so compassionate compared to what I experienced heretofore at Hamilton Trace. He cleaned me up and changed my sheets. Two hours later my wife was allowed to visit. The chief nurse said that I had to return to the hospital after existing one and one half days in hell with this outfit. Yes, I waited in pain until another ambulance came to return me to from where I came. Terrible experience. I can't even think about the pain these people let me suffer saying that I needed to sit up for five minutes and then they chided me for screaming in pain, Bad show all around. I pray to God that this memory will not haunt me, causing nightmares.

Medical staff, nursing, PT and OT were generally top notch. Did have a clunker every now and then…read moreon weekends, especially at night. They appeared to be agency nurses, not staff nurses. Low rating applies to operations outside of medical staff. Room cleaning only happened when it was requested. No fault of cleaning personnel, they were severely short staffed. Food was decent but getting the order correct happened about 50% of the time. Procurement failed to order correct products, such as briefs that fit. Thank god for Amazon. At home medical equipment was not ordered in time for discharge, caused additional expense and a bad experience. Overall the health related goals were met and went home improved, so if you can deal with the other stuff, might be a good fit.
