I have been coming here since the beginning of this journey. I just renewed again and the process was amazing! Send a request for an appointment at noon. Get a text within hours to set up time and fill out paperwork. Too easy so far. Last night at 7pm from my living room I met with the dr via phone. By 2 pm next day, I have my card from the State in my email. It couldn't have been more convenient and quick. Awesome service. They do it all for you. I think today was my third time using AZ Green Life. The appointment is always quick and done efficiently. Friendly folks to chat with(this time the owner) every time. Everything is explained so you understand the process. The Doctors visit was new, they do them virtually now. I was in and out in no time at all and about an hour/hour and a half later my email from AZ came per usual letting me know my application was approved. I'm a first-timer who came in with no medical records after doing pain management treatments for a year. Overall, I am pretty satisfied with my service here. Some less than stellar reviews made me a little nervous about the place, and their website schedule app now goes to a 404 Page, but considering they had more positive than negative comments, was the cheapest I could find, and when I called, they offered a same day booking, I decided to schedule. When I got to the office, I was surprised by how many were waiting. But my consultation, application and payment process took about two hours, which all things considered, is not really that bad. When I met with Dr. Ashley, she was incredibly warm, friendly, and seemed like she understood my situation. Everything about getting my card was easy and fairly painless, the doctors were super friendly as well as the lady in the back, the only reason it is not a 5 star review as they did not honor my appointment time of 1pm and took 2-3 walk ins ahead of me, getting me back at around 1:45. The gentleman upfront was not very friendly about it and said he wouldn't do anything and didn't offer to maybe pass my complaint on or grab someone who could help me. It's good to know as I would've came in a couple days ago instead of waiting for their first available appointment time. Very misleading and thankfully everyone else was on the ball and got me out of there fairly quickly and this didn't cause a domino effect in my day thankfully! I hope the gentleman at the front desk who was helping me out either gets an attitude adjustment or some proper training in customer service. read more

Easy and painless process. I was worried that getting my card in AZ was going to be a huuuge hassle, but it wasn't....at least, not here. The staff is knowledgable and they walk you through the process from start to finish in a fast and efficient way. They were also understanding with my appointment set ups as in, it was easy to set up my appointment-- I had to unfortunately reschedule twice for my initial appointment because I had to constantly fly in and out of town for work that month, but it all worked out with no trouble. I wish I could give this place negative stars. I came in and filled out my paperwork... no problem. There was a wait... no biggie. I'm a very patient person. When they called me back I spoke to a white fit male doctor, he asked me some questions and he told me come back with my doctors paperwork. Now mind you I just paid $125 to see this doctor and I asked him about that as well. He look frustrated when I asked. I did not get loud or give attitude when I asked about my money. He answered my question and gave me my options. I chose to get my money back and go somewhere else. I left his room and spoke with a gentleman at the front desk and he told me to wait for my money in the back. While waiting in the back the previous doctor I spoke with open the door and hits me in my lower back with the door knob. It was not a round knob, it was one with a handle. I was just telling him how I have severe lower back pain and he was just pressing on my back... so as I'm typing this my back is hurting. The worst part is I move to the side once getting hit and look at the doctor... he rolls his eyes. So yea... they aren't the nicest people. This wasn't the only rude thing done today. The front desk associate asked if a Latino women was legal and asked me if I was on food stamps... while these are common questions they weren't asked to everyone. These events happened on 2/01/18 around the time of 13:10 and 14:15. As I write this my back still hurts. read more
