During a time when our family needed support the most, everyone at Avalon treated us with kindness, compassion, and genuine concern. They consistently went above and beyond to make sure not only that my mother was well cared for, but that our family was doing okay too. Care Quality: totally inconsistent Currently a patient who has been in the health care industry for 17 yrs I can actually give you an non bias review of the overall care given here at Avalon. Been in and out of here two times and am currently here 9_/12/26. Both times so far they have screwed up my pain medication where I have suffered at a pain level of 9 almost 10 for over periods of 3 hrs or more. That's totally unacceptable and all nursing could say is "oh we're sorry " which to me is total bullshit. Medications are always a #1 priority, especially pain medication. The total inconsistencies of the quality of care is what makes me not recommend this place for quality skilled nursing facility. They need to improve on all levels of their business, for me to recommend them at all. read more

My 91 yo friend stayed at Avalon for almost 3 weeks. Although the place is a bit old, I was surprisingly impressed by the care and the friendliness of the staff. Although it looked a bit hectic at times due to shortage of staff at times, most of them were very caring and attentive. The facility is not the most impressive t place but the staff was very friendly, professional & caring. It was the staff that made it very comfortable for my friend. We had a great experience with everyone there. As for the parking, l just parked crossed the street by the school so it was not a problem for me. read more

After my uncle was discharged from Pali Momi, finding a rehabilitation center that truly prioritizes the health and well-being of their patients was one less thing to worry about, thanks to Avalon. Their knowledgeable nurses and nurse aides took great care of him, and especially the dietitian for providing the proper nutrition for my Uncle. The list of things to thank them of can go on and on, but let me end it by expressing gratitude towards the admission office for making the whole experience a walk in the park. Nursing facility in Kalihi, Honolulu, Hawaii... When my auntie was discharged from Pali Momi Medical Center, she was brought to Avalon Care Center. I had never heard of it before, so I went to visit... several times. Parking was tight, but I was lucky to find a space every time! Maybe I visited at unpopular times? I was given an option for the orientation. In person or by phone... I chose a phone interview because I wasn't sure about the parking. Staff seemed competent, and they offered options and let me ask questions. Inside, the facility was clean, and the staff were friendly and had good communication skills. Auntie even commented that the food was tasty! When auntie was discharged to home, I asked if she could go back to Avalon because she had Physical Therapy, Occupational Therapy, exercise activities, BINGO, etc. Shucks... there are no Long Term Care beds there. Awww... I was able to attend a Caregiver Training to see how auntie transferred from bed to wheelchair, wheelchair to toilet, wheelchair to car, and auntie even walked with a walker! What progress! Our family is deeply grateful for the exceptional care my grandpa received during his time at this care home. From the very beginning, the staff demonstrated a level of compassion, professionalism, and attentiveness that brought our family tremendous comfort, especially during such a sensitive chapter in our lives. My grandpa's room was always kept clean, organized, and comfortable. It gave our family peace of mind knowing he was in a safe, well-maintained environment where every detail mattered. The care team consistently checked in on him, ensured he was comfortable, and always communicated with us with patience and understanding. I want to give special recognition to Scott, the transporter, whose kindness consistently went above and beyond. He was always so helpful, patient, and respectful. Whether it was assisting with transportation or simply offering a friendly interaction, his support made a meaningful difference. Another person who deserves immense appreciation is Karen from the Business Office. She was a tremendous help to my grandmother throughout the entire experience. Karen was always kind, caring, and treating my grandma with genuine compassion. She made challenging days easier with her warmth and professionalism. Even though my grandpa has since passed away, we take comfort in knowing that during his time here, he received the best care possible. The staff's hard work, patience, and time did not go unnoticed and will always be remembered with appreciation. The RNs, CNAs, and nurses assistants were all passionate of their job and duties in spite of having hectic schedules. The PTs and OTs were awesome too! Although the sessions were short, it was packed with challenges that were suitable for the patients. Every one was encouraging and supportive of both residents and patients who were there for rehab. The facility seems old on the inside and kind of creepy and the food is not the greatest. What truly stands out are the staff they have . Most of them are very polite and eager to help. Ayla (nurse), Liezelle (nurse) and Veggie (CNA) all stand out especially. They make me feel like their very own ohana in the care that they give. They really love their job and you can see it. My first impression walking in was that this was a facility with proper staffing and it would be a safe place. Especially since they have alot of elderly patients. Many people struggle with care and affordable facilities, unfortunately the result of that could come at a bigger cost. My dad was at Avalon for a few weeks for rehab, Well, needless to say my dad got scabies while he was there, a uti, an infection in his spine and an abscess in his hip when he was discharged from there the same day he ended up in the ER and diagnosed. We have papers to prove the timeline These infections are all contributing factors to why he cannot walk. Further more the Avalon social worker on site refused to give a patient assessment for my Dad prior to his departure . The reason we were given is "Because he is no longer going to be in their care as of Sunday" even tho we asked for the assessment way before Sunday while he was under their care while they were getting paid by insurance.. but once they heard the payments would end on Sunday they wouldn't do it. Well, the picture is clear it was because Avalon didn't want anyone to know his condition upon release because that was not his condition upon arrival. also. When I visited my dad I noticed he had lost more weight, 10Lbs quick in a week, we immediately took action and started sending him a meal everyday to ensure he was actually eating because although Avalon was providing food daily my dad wasn't eating all his food and his rehab nurse was so happy when I bought him food one day while He was doing rehab..this man literally had tears in his eyes that's how concerned this nurse was for my dad. we sent the food purely to ensure dad would eat his food (a shake and a fish sand which) his favorite meal... once a Day..... out of our own pockets. the result was the dietician called me up while I was at work upset that I sent my Dad food stating that they are feeding him, it's like these people are so caught up in their own egos and emotions that they don't even care about the people that are there who need help. Nobody said they weren't feeding him.. If you are an Avalon employee and you feel the need to withhold Help from someone in need because your emotions you should quit today. The job you signed up for is not for you. And if you are looking for a place for your loved one and this place was recommended take what I'm typing seriously. Not everyone there is selfish and willing to put people at risk because of their own personal issues.. I'm sure Avalon has some great people there, some of the nurses and aides were very professional and I could clearly see they had compassion and empathy for the people they are there to serve... but the few that hide in offices like for Example sign on door that says "Social worker" her name is Harlene. Maybe find a job where your I don't care about what's right I care about how I feel" doesn't affect someone who has done nothing to you and needs help and can't help themselves because their disabled. Lastly, on his 2nd to the last day there I walked in to urine on the floor, and my dad hungry. If you have time to call People up about stupid things make sure their rooms are cleaned and they are fed first. After all this was said and done I then got word from a friend of mine that her mother was there and it was not good for her either. Pls do better. Again this is not a reflection of the entire staff I'm sure Avalon has some Great people there but some need to be removed. read more

When my grandpa was admitted into Avalon Care Center, our family was incredibly grateful for the quality of care that was given to him. I am beyond thankful for the excellent service the Avalon nurses and aides had given to my grandpa during his entire stay! I cannot emphasize enough on how outstanding the staff was with their efficient communication, impactful involvement with patients, and commitment in fulfilling their responsibilities. As for anyone, there was certainly a lot of worry and uncertainty having a loved one in a care facility. However, after the first day meeting and interacting with the Avalon staff, we knew this was a great place for my grandpa. A kind of place where patients can feel comfort and trust in! Dangerous facility to leave your love ones with. This facility lacked proper equipment to care for my father. We were requested to bring a wheelchair and walker for my father after he was transferred from hospital the same day. The crowded room had four patients with no bedrails and safety equipment. The emergency exits were draped with caution tape. Waited over an hour to speak with any staff member. Faculty lacked empathy and was not willing to address our safety concerns and offered to move my father after we told them we are taking him OUT of their facility-too little too late! read more

I hate to talk bad about people or any business. But this place needs to be updated and prepared for any patent coming In. I arrived on a sat. Understandable but asked for a wheelchair or walker. Said no more it's locked up. I said wtf! Asked for med late afternoon they said coming. 8pm waiting. In massive pain. They said never come yet. I Said wat!!! Coming soon. 11pm finally got my med. I'm so mad and in pain I want to leave! Sunday comes someone finds me a wheelchair. Finally. Now I can get up at least. Nurse Wilma is my nurse. She's an angel. She takes care of my issues. But nobody listens. She leaves and after 3pm no one comes to check on 2 of us. U want stuff or need it press the button until they come. My partners one was unplugged. Unreal. Now it's Monday. All staff is here. But yet once they check the round that's it. Don't get me wrong there are some good nurses but overall I hope if I have to do this in the future I hope to be at the villas. Avalon if you are reading this please give the biggest honor to Wilma. She should be employee of the year here! Hope I see more nurse like Wilma. read more

I'm just giving everyone a heads-up about that place. My friend was admitted there for rehab on a Saturday from a hospital couple of days after surgery once there no nurses or anyone came to check on him for several hours. He buzzed for someone to come help him to use the bathroom and no one came for awhile. He wasn't even seen by the case manager to sign paperwork for couple of days, but they were sure fast to give him a bill they even came by and asked for payment. While there they had a covid breakout, patients and nurses were getting it. There was no temperature taking of visitors, sometimes there wasn't anyone at the front desk. Also, not even once did the doctor there came to check on him. Just so sad to see the elderly patients there being treated like that. read more

If I had a choice I would give 5 negative stars. First day my mom arrived the nurses will playing around outside of the rooms, talking and laughing sooooooo loud. I stepped out of the room and said "keep the noise down. Common sense this is a rehab facility and can you see some of the patients are sleeping." Pay close attention to the staff if you ever see them, because most of the time they are MIA or in the doorway of a patients room on their phone. Also pay attention and see that the staff take pictures when they are evaluating the clients dress/naked with their cell phones. You will also notice the kitchen staff who delivers the food is the same staff who changes the patients undergarments, including their diapers. Visit your family everyday as much as possible and when you see something they are not suppose to be doing, say something and document. Last thing is I found someone else's medication at my moms bedside and it didn't belong to her roommate either. I notified the nurse and she said the nurse manager will get back to me, received no call. The staff leave patients in the hallways for hours in wheelchairs with their plates of food. Everything is stored in the walkways, which is a fire hazard. This place daily smelled like feces and it sad when you hear some patients saying they have to go to the bathroom and staff is no where to be seen. I highly do not recommend this place at all. Cannot give a true rating for the facility, management seem to coerce for payment, suggested places for further placement for Rehab of Injured Client is POORLY lacking! After being referred to couple of places, makes me wonder if those suggested places are investigated for proper REHAB or is placement done without looking into the facility. At one of the two suggested places, experienced seeing a Grey MOUSE run under a bed. Place is hot without AC but with fans that just blow the hot air around. How is it possible to have 8-9 clients will need to share ONE BATHROOM! IF medicines are to be dispensed, an Registered Nurse Practitioner needs to be there 24/7. Management needs to upgrade with proper training, investigate their suggested placed for further care or face possible law suits if a Client gets injured under their short care! After all, suggested places for further Rehab also implies those places are "sanctioned" by Avalon! read more

Believe the one star postings. I tried to stay positive while my cousin was at this place and brushed off her missing laundry, no nurse or staff around, and rude aides. I understand being a care giver is a demanding job but Avalon's lack of professionalism is troublesome. Some workers initially present a side that they care and want to help but there's no follow up. Before being discharged, we sat with a social worker who stated that my cousin qualified (lack of balance, shuffling walk, her townhouse has stairs, etc) in getting a reclining bed, a wheelchair, and a commode through Medicare or her medical insurance. Avalon will work on getting these equipment. After a few days of no bed or wheelchair I called to inquire on delivery. The social worker is on vacation and her supervisor states that my cousin, who is back at home, does not qualify for a bed or wheelchair because during therapy she was able to "walk" 200 feet. She does not "walk", she shuffles and she still has a balance issue! All I got was a "sorry, but the bed and wheelchair are out-of-pocket expenses and the commode has a deductible..."probably around $200". No Thank you! Queens' "first open bed" policy for non acute patients to transfer to a care provider is a "brush off" and a money grab for Avalon. If you're stuck with going to Avalon...on your own, look around for other care providers to transfer to. Believe me, Avalon won't even care that you want to leave. Probably their attitude is "good, go. We don't need a grumbler like you." read more

Thanks to all of the staff at Avalon. They were very nice, professional and accommodating. They addressed my mom's special dietary needs with their dietician who actually visited with us. Nurses and aids were quick responding and the physical and occupational therapists were very knowledgeable. The administration personnel made everything go smoothly. My mom is 99 and she wants to return there after additional surgery. I believe this is the reason that this facility is award winning. You can see their awards in the lobby. read more

It's been 4 months since my Dad passed away. I feel like they didn't do enough for him or if they did try it wasn't enough for him to be discharged from their care. They said he was ready to come home. So on the day he was discharged he was having a hard time getting to the handivan!!!! Was no one aware or cared enough to take him to the handivan or notice he didn't look well??!! And when the handivan finally dropped him off he was just holding himself up with his walker to the wall. Nobody told me he was waiting there for a while and he looked nothing like my Dad!!!!! He was suffering he needed to still be in the hospital!!!!! And Kaiser chose Avalon Care Center for his rehabilitation!!!!! I'm just a grieving daughter who lost her dad to a health care system that don't care!!!! read more

My mother was recently here at Avalon. This place is super awesome because the employees makes you feel at home, from the moment you walk in the door you're always greeted by a nice young lady helping you sign in and making sure you know where to go, to going upstairs and socializing with her CNA, nurse, and social worker. Therapy is super awesome too! They showed me how to get my mom in and out of the bed, transfer her to her wheelchair and into the car. They offer so many activities such as bingo, watching movies, hair cuts, and special entertainment where people come and perform for the patients. The food was always on point! Kitchen never ever disappointed my mom, she loved the food. Facility is super clean too! No cluttered rooms or hallways. Only down fall is the health information manager or whatever she calls herself. That person isn't a good fit for that job! I request records from her 2 weeks before my mom got discharged, I called many times for her and she never once answered! Once we left the facility she called and said her records were ready. Also, their parking lot is super tight and crowded! Everytime I went there was no parking stalls open. I had to park blocks down from the facility! Anywho, I would recommend this place to anyone looking for a care home for their loved ones. Just don't request any hospital records because you'll wait forever!! Hehehe. read more

Was debating on whether or not I should do a few reviews on a few care centers we had family member and close friends stay for a short period of time for rehab. I decided to put 2 stars instead of 1 star to get people's attention and also for the staff that actually do good here. Our family member was sent here as they are the only few that would take patients in after the hospital with the needs and conditions medication etc. So our choices are limited compared to a better condition person. The outcome was to be rehabilitated back into walking condition after a medical condition that made em sleepy. Well come to find out they do things on a set schedule. Food for instance, if you're sleeping you miss out. Important meds, not awake you miss them as well. After a few weeks of ups and downs, minimum PT. BTW the pt was great did do good when was able to. Few of the nurse and Dr was informing. Well time was up and was being released. Go there to pickup and patient was practically out cold. Like not normal but can't do anything as they were released and if they were to take them back you'll be charged out of pocket. Called our PCP Doc about the situation and told us if next day not the same go ER. They released the patient who was practically almost unresponsive like they were sedated. We had such a hard time taking them out the car and into the wheelchair like dead weight Next day at ER they told us he had multiple bruises. One on leg and had a blood clot that needed to be treat. Also had a big bruise on body near kidney and was told kidney was failing, probably from not taking the medications needed as well. I know a lot of these places are all similar. It's your job career choice to want to help people. Not be in it for only the money. Makes your think would they do the same to their own family member? Even the competent roommates were saying how terrible the place is. People always screaming like they want out So my thing for people out there. Research and read into places before sending loved ones there. Unfortunately the good places out there are usually fully booked or cost way too much for normal everyday people to make it in there. If you do have to send someone places like here. Check on them daily, talk to others around there to let you know what's going on when visiting hours are over! read more

My mom is being cared for by a ruthless condescending Nurse or aid. Spoke with the nurse manager about her rude employee and it went in one ear and out the other. Don't send your loved ones here, unless you want them to feel abandoned. This is how I felt and wrote this line immediately after my grandmother passed away in Avalon. After some time I am able to write this with a more objective view and not so emotionally. My grandmother had been here for 2 years and passed away while in the the care of Avalon. My mom and aunty were main contacts, but due to COVID were not able to be in contact with my grandmother as frequently as they'd prefer. Yes, they'd have a Facetime session here and there. Prior to COVID, my mom and aunty were there pretty much everyday. If it weren't them, it would be one of their other siblings. Bottom line, someone went to see her everyday, if not twice a day. We'd have lunch and/ or dinner with her. She was never lonely. I get it, it's COVID and there are restrictions. It was a very lonely time. Within the 1 year of COVID and visitation limitations, it was pretty evident that my grandmother was looking "older." Her hair had gotten longer, her nails weren't as well groomed and cleaned, and her face looked aged. My mom and aunty always made sure that my grandmother was well groomed, because honestly, my grandmother was that way. She always had her hair nicely trimmed and combed. Her nails were always cleaned and painted. Including her toenails. She always put face cream on too! It was important for my grandmother to always feel and look well kept, like she cared about how she looked. Cause, she really did. This made her feel good. We did not see this being done. It may or may not be a part of their duties there, but if they won't do it, then allow convenient access for a family member to come in to do so. Prior to coming to Avalon my grandmother didn't speak too much, but she was getting better. She would talk, even if it was just little sentences. During this time, she would hardly say anything. She would just look at you on FaceTime. Appointments are difficult, because you may or may not get to see your loved ones right away. That is what happened. My aunty had an appointment, she went. During that visit, she could tell my grandmother was declining. She could go any moment. My aunty insisted my mother go right away. My mom tried to get an appointment, but it wouldn't be for a couple of days. Even with the pleading that my grandmother could go anytime, it was not gonna happen. So when she passed the following day. You can imagine the hurt, the anger, the everything a daughter/ son may feel when they can't be there for their parent. The crazy part is they allowed 20+ of us to go in to see her in her bed after she passed to say our goodbyes. Yes, we could only go in pairs, but that's still 20+ of us going in and out of Avalon. Don't get me wrong, according to my mom and aunty there were a few good nurses here, but they aren't always going to be the nurse who's with your loved one. To my husband - Please don't send me here. To my mom - we won't ever leave you here alone. read more

One star for the overworked and fantastically good and caring social worker Chanelle. (I think that was her name, sorry if I got it wrong, but she's truly amazing; if this review was only for her, 10/10.) Otherwise I'd give this facility negative stars. My mom was sent here after a surgery to recover. No PT unless I called multiple times and they finally begrudgingly helped her for about 5 minutes one time. They did not help her shower even after we all asked several times. They put her in a room with a roommate who had some serious mental problems and tried to attack another roommate with a broken pair of glasses, the cops came, my poor mother was terrified, the whole shebang. "Visiting hours"... they give you about 20 minutes crammed together on a patio in the parking lot with all the smokers. Gross and scary in Covid. Staff is irresponsible and lazy and "lost" the cash my mom had them hold for her. Please stay away. This is more like a long term board and care for elderly with mental illness than for sweet little old people recovering from surgery. I could not spring her fast enough. read more

Never in my life would I recommend this facility, or its sister facility Hale Nani Rehab., to anybody with a loved one needing long-term care. Here are a list of reasons why this place is severely lacking professionalism, empathy, and basic healthcare skills: (1) My LO has lost multiple valuables while here (dentures, cell phone, cell phone charger, hearing aids). Staff do not take responsibility either. (2) Getting ahold of staff through the phone is IMPOSSIBLE. Lines are always busy. No call-backs. Because of COVID you have to call to set up an appt. to visit your LO -- takes us two or three days of calling to do that. High staff turnover means no one is available to answer you. (3) Lack of attention or urgency when it comes to care. LO has sat in their filth multiple times, call light unanswered. This resulted in my LO falling multiple times (bruises to prove it) trying to go to the bathroom. Its something new every time we visit! We dont always get a call from the nurses either when a fall occurs! (4) Staff here are not friendly. They dont greet you or give you updates on your loved ones. Just another person to them. We dont get updates on medication changes, incidents (like falls), or overall well-being for our loved one unless we actively ask the questions. Its been a very disappointing relationship with Avalon since admitting our loved one here. Plans to move them are in place as this facility only diminishes the quality of life for them. read more

Where do I start! My 99 year old mom was there after she broke her hip for rehab. The therapist are FANTASTIC , however, with the exception of very few individuals DONT send your loved one or anyone there. The CNA's , with an exception of ONE, are not happy there and it shows . Favoritism and laziness are apparent. The cleaning staff spoke negatively about about cleaning up after someone leaves . Mom got covid(staff wasn't wearing masks), then but I see they are now and they have another breakout there as of yesterday. My was throwing up constantly abc the nurse told us it may be the coffee she drank as the vomit looked like coffee grains. Mom drank cooler every morning for 50 plus years and while there too(3 weeks) before this started. I brought it to their attention again... same BS Long story short, I asked to have an ambulance take her to the hospital. They were also aware of her existing condition!! I have met with Kailani and the nursing supervisor regarding my concerns and hopefully they will be addressed read more

My family member stayed there and had a great experience. Us people always going have something to say whether good or bad but those guys work hard. Just talking to the staff, you know they care. A few months ago Avalon took care of my Friend and let me tell you they did an excellent work! They got him home smoothly. The day he checked in from QMC till he got discharged out of Avalon was very smooth and easy for him! Without avalons help he would of been in the hospital still. I'd love to thank Dio at front desk and along with the receptionist staff for always being there for us. Also I'm not sure but I think his name is Nai? He's the administrator, thank you for always addressing our concerns. Although due to Covid restrictions (pandemic) we were very glad on how they handled things. Being very concern at times they really made things at ease for us knowing my friend is at safe! When you have visitation outside you know very well they take Covid restrictions very serious and that's why I gave this place a 5 star! Avalon Care Center is located on Kamehameha IV Road in Kalihi. There are parking stalls in front of the building as well as on the sides of the building. The two story facility is located near a bus stop as well. Back in May 2017 I had accompany my mom to Avalon Care Center a few times. We were there to see her male companion who recently got his left leg amputated. As we entered Avalon Care Center for the first time I noticed a vending machine on the right hand side. The stairs and elevator is on the right hand side as well. The receptionist was on the left hand side as well as a couch and couple chairs. The physical therapy room was straight ahead. There's a hallway between the reception area and the physical therapy room. The admissions office is located down the hall. Inside the physical therapy room the patients are able to watch the flat screen television while exercising on the stationary bike. Patients are encouraged by the nurses to lift small weights or lift their legs to strengthen their muscles. There are small bouncing balls and a balloon for patients to toss to the nurses as well. Patients rooms are located on the second level. Nurses station and a community room is also located on the second level. There are four patients per room. Sometimes only two beds would be occupied. Clothes for the patients can be stored separately in a closet. Curtains around the bed for privacy. One bathroom can be found in each room. The few times I showed up with my mom at Avalon Care Center was to help encourage her male companion in exercising. Majority of the times he refused to exercise or listen to the nurses. With encouragements from family members he was able to do his exercise. While staying at the Avalon Care Center the patients are fed three times a day. There are activities or small events on certain days in which members from the nearby church or organizations would visit and entertain the patients. Avalon Care Center is just a temporary solution for patients. The elderly and disabled patients are able to return home in a familiar loving environment within two weeks. I've never stayed here, but I've visited friends and family here many times. This is the place you come when you are recovering from surgery, or if you need more care than you can get at home. It's not a "five star" facility in itself, but I am giving it five stars because it's a decent place, it's clean, and the staff are extremely friendly and helpful. They really care about the patients. Parking sucks here--but you're allowed to double park, as long as you leave your info and your cell number so that you can move your car when needed. There are restrooms for visitors, and a vending machine for drinks, so you can grab something even if you're here at an off time. read more

It felt like a place where people left their loved ones cause there was no other financial means. It was dirty, unorganized, and understaffed. I went to get my mother after 2 hours of being transferred from Queens Hospital. Her roommate is asking for sleeping pills at 6:30pm, the guy across is grumbling cause his roommate won't get out of the bathroom, and he needs to go. The nurse's response was how they are understaffed. They say my mom is a fall risk, so the hospital picked that place To live for 2 weeks. Any 80 year old is a fall risk. We as a family should have been able to tour the facilities before she was taken there. They insisted it was her choice. I am my mother's daughter she would never choose a place like that. People, please be the voice for our Kapuna. Don't let someone's medical insurance be the reason why case mangerers make decisions for your family. Just over 12 hours we found so many other choices. This is an institution folks! Some of the nurses care and some don't! My gripe is really about management they treat the staff poorly and expect good moral. The turnover rate is horrendous! The only very good thing about this facility is the food they cook for their patients, that's about it! The nurses are either overworked which leads to patient care being compromised or just plain don't have any common sense. They worry about freeing up a bed as my mom had to go to the emergency room and they wanted us to clear her things out on the same day! Unbelievable! read more

I have had the pleasure to service Avalon for the past 3 years and they have been very professional and caring towards their residents. They always greet me with a smile and have good communication. I've dealt with the admissions team, administrator, social workers and nursing staff who strive to provide excellent care. If you ever need to admit your loved one to a nursing facility, I would definitely choose Avalon! Staff is very caring, friendly, and they treat every resident like family. Unfortunately, the COVID pandemic hit, which lead to many changes in policies and protocols. During this time, Avalon still managed to give the highest quality of care to not only the residents, but also the families of residents impacted by the COVID pandemic. This place is the most unorganized and unprofessional place ever! I would give it ZERO STARS if I could. No one ever answers the phone, calls back when you leave messages, and gives out false information about patient's care. They lost my mom's identification and Medicare cards and never bothered to apologize. She has been at Avalon for a month and the wounds on her hips looked as though they never treated it properly. And this place rips you off by stating patient has to stay beyond the allowed ten day stay and makes you pay $125 per day. I'm so glad to have gotten her out of there and into a foster care home that truly knows how to care for patients. Beware of this place and do not send your loved ones here! read more
