As it relates to the services provided to my Mother: 1. I am well pleased. 2. The Team of providers are very aware of her needs. 3. Staffing levels reduce the timing of addressing her immediate needs of needing to be cleaned and changed. 4. Menu that comes with the food tray does not agree with what is on the tray. 5. After visiting several facilities in the Brandon area, I am grateful to Miss Henderson and Miss Woody who quickly responded to Mother's need for placement and put me at ease regarding the quality of care that she would receive at Aviata Oakfield. They have kept every promised made. 6. A plan of care was never presented to me for consideration and planning. 7. I am also thankful for Paulette and Nurse Tye who were the first to provide services (intake) to Mother. 8. It is admirable that the Administrator is hands on with staff and clients. 9. Grateful I am for Clara (Business Office) who patiently assist in guiding me through the steps necessary for permanent placement. 10. Nurse Jerome excellently guides the team responsible for providing care of my Mother. 11. I am pleased with staff with whom I have had the pleasure of engaging in conversation or requests for service. 12. Discovered that I have a dear friend confidentially admitted. 13. The driveway needs immediate attention to repair pot holes. 14. The exterior of the building needs attention in some areas. 15. The grounds need some tender loving gardening care. 16. Better system of the patients personal clothing needs to be made. Though carefully marked, many pieces of clothing are missing. 17. The custodial team keeps the common areas very polished and clean. 314P could us some floor cleaning. 18. I am appreciative of the 24-hour accessibility. 19. Customer friendly training is obvious with most of the employees. 20. CLOSING: Overall, I am pleased with the Facility and have established a personal connection not only with Staff but also with many of the patients. This is not a facility for anybody who has medical issues. There is no skilled nursing, no follow up on any condition the patients may have that are delicate. Patient illnesses are not addressed, they are left with infections without a doctor to diagnose or prescribe antibiotics / medication for. I have a family member that developed cellulitis and this was left unattended. After over a week of having advised the staff of the condition, my family member ended up in the hospital with an advanced infection in the bloodstream and in delicate condition. Do not send your loved ones here - staff is agreeable but that does not solve much when patients do not receive appropriate care and there's no communication from one attendant to the next. No one seems to have any information when asked about what care is being provided. I don't recommend. read more

My mom was at this facility until I came one afternoon and asked the nurse for protocol to get her sent to hospital. I walked in the room and could barely arouse my mom. The week before I went to the nurse and asked her to check her out because her legs were swollen. Nurse told me it was because she just lays in bed and not moving and likes to eat. Well fast forward to me finding my mom barely able to open her eyes and not talking. Nurse told me I needed to speak with head nurse to get her sent out. Why am I doing your job and why didn't you notice something was off with your patient. I spoke to head person who was in fact super nice. I believe her name was Dr Gwen. I got her sent out to hospital and she had blood clot in her legs as well as Covid and EColi infection. Nurse/ facility had no idea! Her heartrate was high and blood pressure was low! DO BETTER!! They sit around at nurses station looking at their phones. They don't check their patients for any changes. You ask a question and they seem to be bothered! I have a friend's husband in here currently. She tells me no aid or CNA helps with her husband to get better. She has to hang around to make sure he gets some help. I am power of attorney for someone staying there full-time now. I had Expensive medicine delivered and it was put in a drawer in his room and not given to nurse to administer. Then they gave him half the medication. I caught that at a later date and he has cancer. They caused a major injury on his foot, so he was unable to get the rehabilitation needed to walk again. Avoid this place or watch them like a hawk. read more

My father was sent here from Tampa General 2 years ago because they were the only rehabilitation center with an available bed. He was scheduled to stay there for at least 4 weeks. He was there over 24 hours before we were allowed to see him. As soon as we we worked round the clock and we were able to arrange for care at home and he only had to stay there for 4 nights. If had stayed a couple of days more I believe he would have died in there ! Brandon Health and Rehab is not fit for animals. In fact if it were an animal shelter it would have been shut down years ago !!! Sad that people and laws protect animals more than elderly people. After working here for two months as a PCA "patient care assistant" I had to quit. The CNA's are very lazy and the nurses "do what they can" to get to the patients in time, but they do not help at all when you need assistance with changing, feeding, nor bringing medication on time. since most of the staff is agency, they don't really care for the patients at this facility. read more

Listen to the negative reviews. My mother left Brandon Regional able to sit up & walk 15 feet & left this facility bed-ridden after 14 days later. I was told when she entered this facility, they had a 14 day quarantine policy to follow CDC guidelines (even if patients are Covid negative). My mother has Stage 6 Dementia - know your rights because they told me there were no exceptions & no visitors - there are - Compassion Care Visits - Look it up. Day 1 & 2 I got updates, after that, I had to call if I wanted information and was given basic - she's okay. Day 6, I got a nurse on the phone that informed me my mother was refusing food/drink, resisting care, it was taking 2 people to change her, the only thing she had eaten the day before was 1/2 a pudding. Keep in mind she isn't aware she takes meds, they are supposed to be given to her in pudding, crushed. I had to drive there, on a Sunday, argue with the receptionist to be put on speaker phone with the same nurse (because no one else was supposedly available) , who finally sent the Unit Manager to the front to speak to me. The Unit Mgr. had no knowledge of any of the issues, said the notes weren't updated, told me my mother had been downgraded to pureed foods days before, with no one making me aware. I was still told I could not see my mom, NO EXCEPTIONS. Day 7 - After 3 hours of trying to speak with an Administrator, I went in armed with CDC Guidelines, CMS Compassion Care information, Gov. orders on Facility Openings and demanded to see anyone that was in charge. I spoke with the Risk Mng. I explained I was not leaving without seeing my mother - It was her right and mine. Under the Compassionate Care guidelines, a patient does not have to be dying, they can be having a hard time transitioning, refusing foods, etc. My mother met almost every need on that list. They allotted me 30 minutes. It took them 10 minutes to clean her while I stood outside her door. She was severely dehydrated, weak, over-medicated and had terrible leg pain. Did I mention outside her room was a leaky air vent dripping onto the floor into a puddle, with no caution sign & with patient walking around. I filed complaints with every government department I could. I visited her every day after that, with no time limit. I had to tell them to change her meds because she was over-medicated, I had to tell them to check her leg pain and when they did, they discovered a blood clot - the day before they planned to give her the J & J Covid Shot. I had to tell them they tested her eating capacity and down-graded her to pureed foods because they tested her over-medicated and half-asleep. I had to keep telling them to crush her pills. I had to tell them to bring her water and straws, because she wouldn't have any, couldn't reach the cups if she did or the table would be 4 foot away from her. Again, she's now bed-ridden. Would you have had to do this in a good facility? No. If your loved one cannot speak for themselves, DO NOT SEND THEM HERE. I could go on & on, but do I really need to? My mother is safe at home now, under her Primary Care Dr. I didn't even trust them to set up her at home care. No need for a facility response, there's nothing you can do to make up for the lack of care, service and compassion you promise. read more

This place is horrible. I left my love one there while I went out of state to burry my dad. Well when I came back to get him I was told that he had COVID. No one from the facility bothered to call to inform me of this situation I just walked into it. Went to talk to staff about this situation and they assured me they hat he would be good there that they could monitor him and see about him. I would do pop up visits and caught him not being changed his meds not being oven on time etc when you go to talk to people about it all they give you is their attitude. I wasn't expecting the Hilton but he didn't get the bare minimum. Left in his room with the tv turned up loud , he is blind so he can't see the call button . Worst facility in the state! Nothing positive I can say. Dirty, can never find a nurse, if you do their head is buried in a computer screen. Won't even look at you when trying to talk to them. My mom passed away and I have been owed a refund for two months. I can never reach anyone on the phone, once I visit they claim the phone system is down. I will never understand how this place stays in business. It has some of the lowest scores and horrible reviews in Hillsborough County. My mother was sent there after a fall, shame on Brandon Hospital for the solid recommendation they gave, and shame on the state inspectors that are supposed to be holding these people accountable. My moms is out now but not before a terrible experience, a rehab that should have improved her actually has regressed her to a point that I'm not sure she will be able to bounce back. This place is unsanitary, 90% of the staff is contract labor with hallways and rooms filled with the generation that paved the way for many of us treated with no respect. My mother suffered physical abuse at the hands of one of the people that was supposed protect her and her fracture became a full break. Told to go in her pants and left in a broken bed until they were forced to correct it. Please do not send anyone to this place it is not fit to even house pigs. read more

I curse the day my Dad was sent to this hellhole. He was transferred there after hip surgery for short term rehab at the beginning of July 2021. He was neglected, ridiculed, left in a diaper in his own waste for hours with a post surgery catheter that was NEVER removed. They wouldn't answer the room buzzer and constantly told him it was broken. They would close the door so they couldn't hear the cries of pain. We were there every day. My Mom was there 5 to 6 hours a day. My Dad was diabetic and the meals they fed him were not diabetic friendly. His bloodsugar was always skyrocketing. He would tell them that he couldn't eat the food and would ask for something else. They would tell him kitchen is closed, too late or they would bring him something else and just throw it on the tray. They did not make sure he had fluids to drink and most of the time we would have to hunt someone down to get assistance. The place was filthy and smelled so bad. We discovered a prior resident's pee filled clothes still in one of the drawers there. The air conditioner vent had black nastiness in it, as if the filter had never been changed. The bathroom had crusty urine all over the floor around the toilet. Dad kept telling them that his butt hurt and they continued dismissing the complaints until 3 days after he was admitted. He already had a sacral pressure ulcer. By day 5 it was a grade 3 pressure ulcer. Wound care was finally called in. Dad would call us in the middle of the night in so much pain from the lack of movement in his legs. PT was suppose to be started immediately and it was not. Dad had previously had bloodclots and had severe neuropathy and muscle spasms. We tried to call but it was after hours and nobody would answer. We did not know that Dad also had a pressure ulcer on the left heel. That one they kept covered up. I wish to God I would have got him the hell out of this place. being on Medicare, there were not many options nor openings at another facility. Fast forward to 3 weeks later. My Dad was stuttering, was confused, shaking uncontrollably. My mom, myself, my son and my daughter in law refused to leave the facility, until they called an ambulance. There was a nurse there in a screaming match with my Mom, telling her nothing was wrong. That my Dad did this to himself and was in that condition upon arrival. This could be further from the truth. When my Dad was admitted to St Joseph's, he had 2 infections and septic encephalopathy. He was kept there and stabilized. Next he was sent to Hawthorne Villages, which while cleaner, no better. From the beginning of July my Dad was in the hospital a total of 9 times and rehab 4 times. The 9th infection was too much and my Dad passed 2 days ago. He fought so hard to survive this horrific treatment he received but couldn't anymore. He kept asking us "Why did they do this to me?" How do you answer that? I filed 2 acha complaints at the end of July. I will continue to fight in memory of my Dad, who was taken from me too soon at the hands of this horrible facility. I never want anyone to have to go through this again from this day forward. I don't believe all the people who work there are bad but not standing up for the people who have no voice, to me are just as Guilty. Also, The on staff Dr there is Guilty as well. He drives up in his fancy car, sticks his head in the door of the residents and charges a pretty penny for his appearance. He is also the same on staff Dr at Hawthorne. Henry Odukomaiya is his name. Believe all the bad reviews that you can find about this place. My mother arrived on a Friday afternoon, although her hospital papers indicated that she was on a regular diet, mi brother let them know that she needed a liquid diet as she was suffering from Trush in her mouth. They insisted on a mashed diet that she couldn't eat. Although we continue to insist, during the four days that she was hospitalized they never changed to a liquid diet and continued to bring the "fine" mashed and dry food that she could not eat. Saturday morning a nurse passed by who did not speak Spanish, since my mother did not respond because she did not understand, she yelled at her with an attitude that if she was deaf, mom informed her that she did not speak English, she turned around and left. At 9:00 a.m. she asked for help because she needed to go to the bathroom, no one came to assist her, so she ended up relieving herself, after several direct calls from me and my brother to the administrator, it was not until almost 1:00 p.m. they went to clean her up. Her first words when I called her that morning were, "Get me out of here." The bath that she received in those four days there was given by my sister-in-law; the wife of another patient told us that during the time her husband had been there, no one came to bathe him, so she was the one in charge of doing it. The only positive thing that can be said about this place is from the day I was present, a very kind Nurse Practitioner arrived and the nurse on duty spoke Spanish, the latter did everything possible so she could take the medicines. That day, although I reported it, no one from maintenance ever came until past 6:00 p.m. to remove the bags of feces that we had in the room since 10:00 a.m. If you do not have enough time to dedicate to the family member hospitalized in these facilities, do not see it as an option, IT IS NOT. The least these patients going to rehab deserve is a little bit of dignity. This is my experience with a four-day stay, I don't even want to know what happens for the long-term patients. The only reason I gave this DUMP 2 stars is that SOME of the staff are quite kind and compassionate: Katrina, the nurse, is a joy; Steve and Andrew, the father and son physical therapist duo, are excellent and have greae"bedside manners; Donnell and Alex got my wife past the hardest part of physical therapy and prepared her to come home on 7/3/20; and there are more of the nurses and CNAs who did a great job while overworked (2-3 shifts in a row) and SORELY underpaid ($8.00/hour -- ridiculous!). There are, however, MAJOR PROBLEMS with this facility! While the front desk personnel, Donna and Patty, always are friendly and transfer me to the people I need to speak with, those persons are RARELY available. Andrew, the head of Social Services, is good about calling back but apparently has little pull with making meaningful changes. Norest, the Executive Director, although seeming to form a rapport with me, has become do DERELICT in her duties that the facility under her care should be THOROUGHLY investigated by the Department of Health! Some of the staff are talking and laughing loudly outside my wife's room stall hours of the night when she is trying to sleep and prepare for a day of therapy. It took FOREVER to see an official doctor to ask about the medications she was being given. It took a WEEK for Maintenance to get around to fix the air conditioning in her room. The WORST thing of all has been the quality of what passes for "food" there: Some days it is pretty good (especially the chef's salad; some days it is slop; last night they served RAW potatoes. This (and most) mornings, they served egg beaters -- quite horrible! This afternoon they served her RAW FISH! Tonight's food was raw also. When she asked them last night and today to take away the raw food and bring COOKED food, they brought her back NOTHING! If I hadn't brought her snacks the last couple of weeks, she would STARVE! We are praying that no one else already there gets sick or goes without, and that no one else goes there at ALL! I'm a current patient at Brandon Rehabilitation. I am recovering from spine surgery and it is essential that medicine prescribed is given at a timely manner to ensure I maintain theraputic level. They cannot perform this basic nursing function. Very understaffed. I have to constantly ask for medication and , at this moment, I have waited 2 1/2 hours for the RN to respond. The worst place I've ever seen in my life my mother was recovering from being in Brandon hospital for fluid on her lungs she was admitted to Brandon Health and Rehab Center approximately a month ago I come and visit her on a regular basis to check on her she had bruises on her legs and arms that we're not there at the hospital she also had urine stain clothing laying about the bathroom and put in drawers to hide them the smell was Unreal and her room and bathroom was not clean there also seems to be theft going on as she is missing jewelry and she had it when she came in it was in inventory sheet if you value your loved ones do not bring them here very dissatisfied there is no excuse for this kind of elderly abuse and I'm seriously thinking about reporting them to the State of Florida for elderly abuse read more

Absolutely the worst facility and service I have ever experienced. I was initially place in a bed that was more meant for a child and I had to sleep in a fetal position which was not conducive in helping my back injury. I only saw a nurse when she administered medicine. Not once did they come in to check on me. Having one nurse to tend to more than 20 patients is unacceptable. On top of that, I never saw a doctor until I approached him in the hallway. The rehabilitation area was horrible. The equipment was severely outdated and many patients were left unattended due to insufficient equipment. While these patients were left unattended some rehabilitation personnel did nothing but sit in front a computer ignoring them. Most of the time when I received my meal either it was not what was on my ticket or most of it was already gone. Unacceptable. I will make one positive note that the nurse's assistant was very good and attended to me regularly. Very please with her. This facility needs to be investigated and shut down until significant changes can be made. read more
