My dad went in for rehab after breaking his hip. The outside is a little outdated along with the inside too. Let's just say I gave them $100 worth of clothes. We had two signs up saying family will wash his laundry and a laundry basket in his closet. For his clothes to disappear and could never be found. Know the food smelled and looked disgusting. If I didn't bring him food he didn't eat. And they even lost a couple of his meals I bought him. The CNA's even felt comfortable enough to sneak into his snacks. And not to say the would only change his diaper 3 times a day. His clothes would be soaked with urine. Know the rehab part was good. They listen to concerns and tried to figure things out with us. And to say this is one of the top rated nursing centers. read more

I spent close to a month here. Some of the Patient Care Assistants were excellent and some were not. I was disappointed that a couple of my nurses really did not undersstand how a blood pressure is supposed to be done. The office workers and the Physical and Occupational therapists were excellant. I would have rated this place 4 stars except for my disappointments w a couple of my nurses. read more

Worst experience ever. If you're the size of a child the bed is right for you, hope you don't need railings. Be sure you take your medications with you when you leave, they consider ita courtesy to release them to you. Definitely not looking after the welfare of the patient. read more

I have seen several SNF (skilled nursing facilities) in the area, this facility is by far the best overall. The staff does a wonderful job caring for the residents, physical therapy takes pride in rehabilitating patients to their full capacity and the facility is kept clean. read more

My mom is at this facility now. I have found the care and compassion of the staff to be excellent. It is so good that my mom asked to be placed here in a long term care situation. Unfortunately those beds are already taken. This team of doctors and nurses have contacted me with any changes in my mom's condition and medications and any change in care that was needed. If I could give them zero stars I would. My dad was there for "rehab" after a fall. He caught COVID while there and Beachside never called us once to update his condition while he was quarantined. I was taking care of my mom who caught COVID from my dad while visiting him at Beachside. My brother had cancer, so he couldn't visit my dad either. After 10 days of quarantine, my mom and I went to see my dad. He was in bed, completely incoherent, could not move, and looked like he was on deaths door. It was very clear to us that my dad could not swallow, so who knows the last time he ate or could even drink water. I have visions of the staff plopping down a food tray and then just walking away to pick it up later....untouched. Beachside never called us, never called a doctor, never called the paramedics. They just left him in his bed. I told Beachside I was getting him out of there and I needed discharge paperwork to do that. When I got the discharge paperwork I saw that Beachside had put him back on a medication that he was not suppose to be on. My dad also had a stage 2 bedsore that had only been treated with an ointment....no dressing covering the wound and of course no mention of a bedsore on his discharge paperwork. I could go on and on about the lack of care and compassion my dad received while in Beachside's "care". After getting him out of there, my dad died 2 days later......yes, DIED! I will NEVER forgive Beachside for what they did, or didn't do for my dad. Beachside couldn't even pick up the phone to call his family and tell us about his deteriorating condition. Or I don't know.....you have nurses there, how about calling a doctor or the paramedics to take him to the hospital?!! Instead, they drugged him up and forgot about him!!! Our hearts are broken. If you love your family members, don't ever take them to Beachside. We don't want another family to go through what we went through. read more

My father had fallen and had transient ischemic attack (TIA), or mini stroke and also has Vascular Parkinson Disease, I would like to thank Beachside Nursing Rehabilitaion Facility Center for giving my father the best care needed. from the CNA's to the nurse's, and PT/OT therapist, and the front office. The staff gave my father one on one with all administrative, medical and therapy needs. I would especially like to thank: Tracey Davies/D.O.N. for all her assistance with patiently answering all our families' questions and meeting our needs for the best care for my father. John Manalo/ Director/Case Manger for assisting and taking the time to explain with all my fathers administrative needs. JoJo / Director of PT/OT and her staff for helping my father get back on his feet and educating and teaching my father how to utilize his walker. My Mother in Law stayed in this Facility and ended up with Pneumonia. She was really sick and they left her in her wheelchair and she was practically falling out on the floor and no staff was around to come and help her. I had a confrontation with a lady named Jo Jo whos in charge of rehab. I asked her for help and she flat out told me that she was not her patient. I thought every single patient in the facility was the responsibility of all staff. I never knew they could not help with all patients. First Facility I heard anything like that from. We ended up moving my mother in law and I thank god we did. They are understaffed and not one thing was done to address the issue with the Rehab lady's behavior and the management told me she refused to apologize, but knew she should have handled the situation differently. read more

My elderly mother-in-law just spent several weeks at Beachside. She was unable to get in and out of bed, so meals were brought to her, she had help going to the bathroom, etc. The nurses were nice. The food was excellent, more choices than what they show on the menu are available, so if you have someone at this facility, have them ask about the extra menu items. It is clean and well staffed, but don't expect to see doctors very often. As facilities like this go, this one seemed very good. If your person gets stuck in a room with three people, expect to get no sleep. read more

She needs help getting stronger and is getting lots of physical therapy. They will put new patients in with the current ones without isolation, thereby infecting your loved ones. This facility also has a HIGH rate of UTI's within the patient population. My mother had fallen and broken her hip and had a hip replacement and I had to find a skilled nursing rehabilitation center quickly and after visiting 9 different SNF's decided to bring her to Beachside. First thing I noticed is that it smells good. Second I noticed is that the place was very clean. Third thing I noticed was that patients were up and dressed and engaged in therapy or activities and the most important thing I noticed was that the staff was cohesive, collaborative and enjoyed working with each other. The therapy team was stellar and helped my mom achieve her goals and allowed us to bring her home in 3 weeks. As someone who is an Occupational Therapist I recognize a great team when I see one. No one wants to have to send their mom to a nursing center, but when my mom fractured her hip, I am so glad she did rehab at Beachside. The whole team was supportive of her and my family during the recovery process. The place is clean and I especially enjoyed the outdoor area where gathering with her almost felt like home. Clean facility. Attentive CNA staff for the most part of the visit. Very small area since three beds to a room. They did keep my mom clean the whole time she was rehabilitated there. Side note: Nursing staff will NOT tell you when to follow up with your doctor. It was a little frustrating that the nursing staff was not able to help us in that arena. It's essentially up to the family to ask the questions (sometimes you don't know what to ask so it's trial and error). For example, if the patient has a broken hand and was rushed to ER and then admitted to Beachside, make sure the family gets ALL of the family doctors name down, surgeon , ER doctor and make the follow up appts because Beachside won't do that for you. Even THOUGH the hospital discharge nurse gives you the paper work to give to the nursing home, you need to keep up with it. Medications : Beachside WON'T tell you when you need to refill your meds, you will learn the hard way that you need to call the doctor on your own to make appointment before meds run out. Another Trial and Error and lesson learn. Don't rely on the Beachside staff to do it for you. I wished it were easier but it was an exhausting process. The stars are for the CNA staff and the lack of stars are for the process and follow up. read more

I was a patient at Beachside for a few weeks following a laminectomy, back surgery and I'm now recovering well at home. Their therapists are exceptional and everyone who works there seems to try a little harder to make your stay comfortable and productive. I believe my care is responsible and the reason I'm now doing so well. Of course your surgeon and your own personal motivation are also very important, but Beachside taught me what to do to remain safe and move me to a full recovery. read more

THIS WAS SUPPOSED TO BE AN UPDATE FROM MY FIRST REVIEW FOR BEACHSIDE. THIS UPDATE IS STRICTLY FOR THE PHYSICAL THERAPIST AT BEACHSIDE. KEEP IN MIND THEY ARE A SEPARATE ENTITY FROM BEACHSIDE. I've only been there a couple times when Ryan and Michelle have worked with her. They treated her with compassion, were very patient and encouraging.My husband has been there more often during her PT and had nothing but nice things to say about them! My 92 yr old MIL who has dementia was brought to Beachside on May 6th to recover from hip surgery. My experience in the beginning deserved a 1/2 star but they got 1 Star because you can't give a 1/2 star. Here is some of the concerns I had. 1. CNA helped my MIL to a SHARED bathroom while she was in an isolation room because she had shingles. There a WARNING sign outside her room to put gowns and gloves on before you entered her room. A trained CNA should no better than to have a patient in isolation use a restroom that 2 other patients are using. 2.Their policy for when a patient pushes the call button for assistance(CNA).We were told by an administrator that a patient shouldn't have to wait no more than 5 minutes. MAJORITY of the time the wait was 15 or longer. That's a long time if you need help in or out of bed,in pain or have a headache and need your meds. The WORST was during the first few weeks after surgery they had her in diapers. As she got stronger they brought to her room a portable commode. Either way she needed assistance and that's a long time for someone to wait in bed to use the bathroom! Ridiculous! THIS UPDATE IS FOR TODAYS VISIT- JUNE 2nd. there are shift changes at 3:00pm & 11:00pm. I'm not quite sure about mornings. I arrived at Beachside about 8:30 pm. My MIL was in bed asleep almost sideways and her feet were hanging off to the side almost touching the floor.I woke her up to see if she needed to use the bathroom. She was confused. When I got ahold of her feet to get her comfortable she didn't have PJ bottoms and her diaper had come undone on one side. I could tell she was wet when I tried to secure it. Then I saw the protection pad and the bed sheet were wet.The patient next to her has a 'sitter' so I asked if anyone had come by to check her.She said she got there at 7:00 pm and hadn't seen anyone. I thought the CNAs name written on the white board was Jasmine. I asked a CNA that was close by if she was Jasmine. She said NO so I asked if she was on her break. She told me Jasmine doesn't work here.I told the CNA that my MIL was asleep w/ feet hanging off her bed,no PJ bottoms,a wet diaper, the pad & sheet were wet and if she knew who her CNA was, her response was " I will find out for you". THE SAME CNA IN THE HALLWAY THAT SAID SHE WOULD FIND OUT WHO HER CNA WAS WALKED IN AND STARTED TO CHANGE HER. That's when I realized the CNAs name on the board was VANESSA. I asked "are you Vanessa?" and she said YES!! Why didn't she say that when I was questioning her? When I asked when was last time she checked her. She said 6:00 but she was fine. That meant from 6:00- 8:30 my MIL was laying there like that. I told her that's ridiculous! Her diaper was already wet but didn't change her so the protection pad and sheet got wet also. I was disgusted.When she changed out the wet pad for a dry one but didn't look like she was going to change the sheet. So in a disgusted tone I said " aren't you going to change her sheet? It's wet!she snapped and said "She didn't no because I didn't tell her". I DID IN THE HALLWAY! I thought that was her job not mine. I did my part by trying to find out who her CNA was only to find that she pretty much lied. This is a trained CNA! Someone from administration got in touch with my husband to schedule a meeting and talk about "a care plan". He got that call a few hours of my updated review on Yelp. read more

It has only been three days but so far my opinion of this place is "poor" at best. No one has answered the phone today. I have called 3 times. My sister is there for physical therapy 3 times a day for 2 weeks. She has had one physical therapy session yesterday and today was told the person who is responsible for doing her therapy has not come to work yet. I remain hopeful that things will turn around but this is where we are at. read more

My 87-year old Mom arrived at Beachside after hip replacement surgery following 6 weeks of negligent care at Flagship, a skilled nursing facility (SNIF) in Newport Beach. I chose this facility for her after visiting many other SNIFs and reviewing the Medicare web site which rates such facilities. I highly recommend Beachside however, you would be wise to do your own homework. Things change and you want to ensure you act on the most current information you can obtain. Beachside won me over for being the anti-Flagship. They were helpful when I dropped in to look around, the place was clean, relatively small, and they had a very good staff to patient ratio. What I couldn't know until my Mom was actually a patient there was the level of care and attention that staff - all staff - show to both patients and families. At every level the staff at Beachside greet visitors, initiate conversations about their loved one, are available and responsive to requests, and seem primed to engage rather than avoid. My Mom was confused by her predicament but treated respectfully and with care by all staff which was not the case elsewhere. I too was treated with respect and care. From the sign out front touting (I think this was the wording) compassionate care for its patients, Beachside's mission statement seems to have been taken to heart by each employee. When the big things are being done right it can be easy to overlook the small things but the small things were also done right here. Each patient has a direct dial phone beside their bed (just try reaching your loved one at Flagship). Each bed also has a small mounted television with headphones to allow patients to watch without noise-polluting the room they share with others. At Flagship, TVs were wall mounted, blasting away without any headphones provided. Is everything 100% perfect? It is not and because I think they really care I offer up the following suggestions for improvement: * Per protocol, patients are sometimes moved into isolation. Following even a brief isolation period they're not guaranteed to be placed back in the same room they came from. Because change is so disorienting for patients, moving them back into their previous room should be prioritized. * This facility has several 3 person rooms which feel overcrowded and a bit inhumane. Two-person rooms are much more desirable. * Even in her compromised state my Mom was completely bored by the activities and expressed as much. The day I observed her participation she was left to color in a Halloween picture. This activity barely engaged her on day one and did not improve during the duration of her stay. You need to up the volume and variety of activities to truly engage patients minds in a meaningful way. It's my sincere hope that my Mom never wind up back in any SNIF but if that's not the case I'll cross my fingers and hope there's a bed available at Beachside. read more

My father-in-law was a patient here last year and I was really dismayed at staff response here. Hardly anyone acknowledges your presence, and when you ask questions, it feels like you are intruding on staff time. I went to the kitchen to ask about substitute meal options and was totally ignored when I tried to get help for my FIL (kitchen door was open and 2 ladies were standing close by). Then when someone finally came to answer my questions, the lady totally brushed me off and said substitutions need to be made 2 hours in advance, not 1 hour as listed on the substitute menu. Maria is very nice and helpful as is Ashna. Otherwise staff would get 1 star as a group. I would NOT recommend this place for anyone. read more
