I urge you to carefully consider this before contracting with any hospice provider. Unfortunately, our experience with Faith & Hope made an already painful situation even harder. In April 2025, my mom suffered a massive stroke. She was placed under Continuous Care (CC), which meant she required critical, around-the-clock nursing support. We were told her case would be re-evaluated after three days. However, the care was stopped in less than 24 hours and we were given only 4 hours' notice that services would be discontinued. At that point, it was clear our needs no longer aligned with Faith & Hope's philosophy of care. We scrambled to hire private help on extremely short notice. That night, my mom was in significant pain & experienced agitation. She needed her medication adjusted and an increase of the dosage frequency to help manage her symptoms. The next day, we made final attempts to Faith & Hope to reassess mom's eligibility for CC, even though the on-call night nurse, when we asked, did acknowledge that we were free to seek another hospice provider. We waited all day for a response from Faith & Hope, but heard nothing. Faced with another evening without adequate care, we transferred my mom to a different hospice. They gave us the right care without hesitation. Sadly, our mom passed away only 4 days later. The only positive experience we had with Faith & Hope was with Nurse Theresa. She cared for my mom with compassion, dedication and a true commitment to patient advocacy. Theresa's thoughtfulness and support helped guide my brothers and me through an incredibly challenging time, and for that we are deeply grateful.

    I'm writing this review after the passing of my parent, who was a patient of Buena Vista…read more Throughout my family's experience with Buena Vista Home Health, we experienced glaring failures that I feel responsible to warn other families and patients about. The care that my parent received (and didn't receive) fell short of the prompt, coordinated, and compassionate standard that Buena Vista claims. Here are the issues that deeply upset my family: 1. ***Schedule changes and communication failures*** My parent was admitted to hospice with terminal cancer on a Thursday. We needed help with medications, at-home care, and so much more guidance. Our hospice nurse Deneshia rescheduled her initial visit TWICE so that my parent wasn't seen until Monday. For a patient at the end of life, every delayed visit is time that cannot be recovered. Repeated rescheduling of an initial hospice assessment is unacceptable and reflects a breakdown in staffing, prioritization, or accountability. When Deneshia came to deliver medical supplies, she framed the visit as if she were "doing us a favor," noting that she was "only supposed to come once a week." In reality, that visit was to deliver supplies that we should have received at admission. Presenting the correction of Buena Vista's own delay as a favor to our family was inappropriate and added to our sense that my parent's care was not being prioritized. 2. ***Medication delays and errors*** Prescribed medications were delayed or entered incorrectly, and critically, my parent's Ativan was not delivered until the night before they passed. For a hospice patient, timely access to comfort medications is not a convenience; it is the core of the service provided by hospice. Buena Vista Home Health was not proactive in coordinating with my parent's physicians and pharmacy to ensure medications were in the home where they were needed. This delay caused avoidable distress at the most vulnerable point in my parent's life, and it should have never happened under hospice supervision. 3. ***Missing standard supplies at admission*** When the admitting nurse Brian completed the assessment visit, he noted that the initial care bag contained only bare materials. Standard supplies that should have been provided at the outset but were missing until the following week included: -Shampoo and body wash -Cleaning supplies -Foam dressings -Calazime -Urinals My family and I got so frustrated waiting for these supplies that we ordered them ourselves online. The absence of foam dressings and Calazime was especially concerning given the risk of pressure injuries in a bed-bound patient. These are baseline items that a well-run hospice program should have delivered on day one, not days into service. 4. ***Shifting clinical responsibilities to my family*** Our hospice nurse Deneshia shifted responsibility onto me, rather than taking it on herself. The only time she ever physically touched my parent was during her introduction and to take vitals. She never demonstrated how to perform any aspect of my parent's care. Deneshia repeatedly referred to our family as the "primary nurses" and to herself as "secondary." I find this framing troubling. If the family is expected to serve as the primary caregivers, then the hospice nurse's role in supporting, teaching, and demonstrating care becomes even more essential--not less. I happen to be a nurse, but the standard of hospice care cannot depend on whether a family has a trained healthcare worker in it. Had I not been present, I am left to wonder what care my parent would have actually received. I asked Deneshia to provide a list of my parent's medications and how specifically to administer them. I never received a medication list or instructions. I am shocked that Deneshia tried to explain the mechanism of COPD to me when my parent did not have COPD. Any education that a hospice nurse provides should be specific and relevant to the patient in front of them, not generic information that does not apply to their patient's care. Between this, the lack of hands-on care, and the absence of any real teaching, I'm left questioning what purpose the hospice nurse served at all. 5. ***Responsiveness at the time my parent's passing*** On the morning of my parent's passing, I found my parent pulseless and not breathing. My family called Deneshia three times without any answer. When Deneshia finally returned the call, her tone conveyed that our calls were an inconvenience to her. Her tone only shifted when we informed her that my parent had passed. After I informed Buena Vista of these failures, their response was to offer their apologies and to let my family know that they would incorporate our grievances into their clinical education. I share these experiences because I don't want another family to endure what mine did.

Figuring out how to best take care of my elderly father after he fell and broke his hip was overwhelming to say the least. Admittedly, Humble Haven was farther away from me, and my dad's doctor, than I had hoped for or planned. But a placement agent sang their praises --- and choosing Humble Haven was the best decision I ever made. Truly. The place itself is wonderful: Clean, comfy, and staffed with amazing caregivers who are truly patient and compassionate. The meals are all freshly made and tasty and nutritious. Personal taste and preferences are always accommodated. There are also very few residents so they get a lot of attention. And I am not exaggerating when I say that it really feels more like a home than just a house; More like a family than an assisted living facility. The thing that makes Humble Haven truly amazing, however, are the owners, Nicole and Albert. As I said, I was ill-prepared for everything that I've had to go through with my dad. (When something like this happens, who isn't?) Nicole, as a nurse, not only used her medical knowledge to explain things to me when my brain was spinning from all the doctors and medications and treatments --- but she truly acted as an advocate for my father, making sure he got everything he needed and deserved from Kaiser. To say she has gone above and beyond is an understatement. I don't want to take up pages and pages listing all the millions of ways she has helped me --- so I'll just say this: If you are on this Yelp page with a difficult decision to make and you are reading this because you're trying to decide if this is a place that you or a loved one should stay, whether for a short while or indefinitely ... Do it. Don't hesitate. You simply cannot get better care than at Humble Haven. I'd give it 10 stars if I could.

It is peaceful, quiet and the owners and caregivers are sincerely caring. Over the years he'd been at another RCFE facility which was neither peaceful or quiet, (both things his geriatric doctor said were crucial) as well as a small memory care facility (again neither peaceful or quiet, and downright upsetting for him). Humble Haven proved to be such a blessing for both of us! This house is managed as a home and the comfort of the residents is the top priority. I was there a lot and I never felt like there was anything more important than how the residents were feeling. They are on a schedule, well fed, exercised when appropriate and given ample opportunity and conditions for quality rest/sleep. There is plenty of interaction between caregivers and residents and the owners are in-and-out a lot, interacting with residents, too. The fact that owner Nicole is a nurse is such a bonus and she was always available by phone and text. Her husband Albert is a great handyman, sometimes cook and caregiver and never afraid to work hard. I realize this review may sound too good to be true, but anyone dealing with dementia in their family is aware of how hard it is to find living accommodations that you can feel good about. Humble Haven fixed that for me. I couldn't care for my dad in my home and felt bad about that in the past. Once I moved him here, I felt good about his situation and knew I was making the best decision for him. If they have an opening and you need a special place for a loved one to thrive, this is it! I had many questions and often called several times a week or even a day. The staff was always completely helpful and courteous, never making me feel like I was being a pain and always getting me answers right away. They were always on top of my mother in law's needs and increased visits any time something was a concern. Beyond that, when I had concerns about her health they would send out a nurse, often within the hour, to check on her and report back to me. Nino was incredible helpful and would get back to me immediately every time I called. It was such a reassurance during a very difficult time. Jason was the nurse who would visit, often three times a week. Knowing I wanted more detailed information than most people, he would send me pictures of his reports, text me every time he visited, call when things were more complicated, and always stoped to talk when he saw me visiting, even if it wasn't his day to see her. He became a confidant and someone I felt truly comfortable asking the hard questions. He really treated my mother in law like family and always showed her respect, even when he came the day she passed to declare time of death. Without any hesitation, I would recommend Blue Butterfly to a friend. They are professional, caring, and reliable. They saved us thousands of dollars and so much stress.

    Hard to believe a medical service can be as attentive as a fine dining restaurant, but that is what…read morethis was. We used Five Star for about five days. Medicare and Optum covered the hospice side. I paid separately for 24/7 caregivers through Sincere Care Services, which I chose because I wanted Cantonese and Mandarin speakers so my mother would be more comfortable. Five Star was recommended by the Optum physician on staff. Separately, a family member recommended them too, through a friend whose father had passed the week before. I checked their ratings on Yelp and Google before moving forward, and I am extremely happy with the service we got from start to finish. This is the point where you decide between more hospital intervention, which can be painful, and accepting the situation and moving to hospice. Hospice is the wedding coordinator for a peaceful death. Things can still go wrong even when you plan everything perfectly, but Five Star was there at every step to handle whatever came up. The rough sequence for anyone about to do this: 1. Decide on hospice. 2. Line up caregivers. They are separate from Five Star, but Five Star will point you to good ones. I wanted Chinese-speaking caregivers and Mark made it happen. 3. Coordinate with the hospital on transportation. 4. Be home to receive the supplies. 5. Be home when your loved one arrives. Have a bottle of distilled water ready for the oxygen machine. 6. Have the nurse and the caregiver both there so you can learn the comfort meds. 7. I used 24/7 caregivers on 12-hour shifts. I made a point of being there whenever a new caregiver came on, so I could walk them through the medications and get their phone number. Nurses came once a day to check on my mother since she was close. You do not have to be there the whole time, but I tried to be there for shift changes. One thing I wish I had watched more closely was bowel movements. My mother had been constipated since the hospital because she was not eating. Another tip: put together a playlist of beautiful music to play in the room. Otherwise it is silence, or your loved one is listening to caregivers talk and scroll on their phones. Even when your loved one is not responsive, they are still listening and drifting in and out. Tell them everything will be okay. Thank them for everything they did for you. I think it helps them go peacefully. Anytime I needed something, Mark was a text or a call away and he always answered. Again, they are the wedding coordinators for a loved one's passing, and they will do everything within their power to help you through it.
