CNA also left her on toilet and never came back so mom went to her bed by herself which would of been their liability if she fell. She was very weak. No communication between staff infact the care team invited me to a care plan meeting while she was being released. Furthermore the place smells of urine bad thru out the whole building. I feel the staff bullied my mother and it's a shame a certain CNA didn't get fired. Glad my mom is home she's definitely a lot safer

I've got to say I was quite impressed at the level of care they are getting. Their clothes were clean, hair was washed, they looked well-groomed (haircuts and shaved), and they got new glasses. When I arrived, I was surprised to find that they weren't even in their room. They were out on the porch enjoying the scenery and landscaping. My grandpa was telling me about all of the birds he's been watching for the past few weeks. I haven't seen them so happy in such a long time. I'm forever thankful for Oakwood Garden of Deland. PLUS: This is a very secured nursing home. There wasn't a single exit that didn't need a code to get through. This is wonderful since my grandpa has dementia and is always talking about "Escaping Jail." The Bad: The aisle ways are constantly crowded with residents, carts, and personnel. Its a challenge to walk through.

I immediately took him out and did not let him stay another day, I complained and they told me they were looking into it and never called to give me any updates. If you don't believe me then walk in unannounced and then ask your loved ones what they really think of this place. Take care of your family at home, don't leave them with someone who doesn't know or care about them. Horrible, horrible place.

The care and concern that all staff members have for the residents is in comparable. This is a small facility, 40 beds, but the staff was very adequate. The building was spotless and odorless. Each resident had their own TV, the rooms were large, beds modern and comfortable. Large closet, two dressers, plenty of wall space to hang pictures. Excellent social director, and therapy staff, who knew and were concerned about all the residents. I was lucky to get my Mother in this facility.

My mother has dementia and is severely impaired but both my parents strongly resisted needed care. Things peaked to a crisis and we had to have our mother admitted to the hospital until we could find her safe care. They were able to admit our mother in just a few days and from the minute she walked through the door, she hasn't stopped smiling. The worked with us on every step and were patient and caring. I started out as a home health aide, Colleen the owner has ALWAYS been encouraging to her employees to better themselves, so much so that she convinced me I could become a CNA, so I took the necessary classes and tested with the state, and became a CNA, to which Colleen said, " now it's time for you to become an RN! I strongly encourage anyone looking for a GOOD job, to come to Onyx. We have MANY employees who are amazing to work with. We are a true "family" in every sense of the word.

We had an agency for a couple of years that was absolutely horrid! I knew they were awful, but kept hoping they'd improve. Finally we had enough and I happened upon Onyx. I can not express how thankful I am! This agency is all I had hoped for and more. The owner is a wonderful lady with excellent credentials. Unlike the last agency we had, she puts her caregivers through up to date training, and they are all so reliable and professional. When I call I'm treated with respect, so unlike the last agency. They were always unpleasant and hurried. They are nurses at Onyx, and their experience in the field certainly shows Her aide has been outstanding, and works extremely hard to help my mom. The last agency all the aides wanted was to be taken to a restaurant or watch television. I have been desperately hoping for a little help with my mom, and Onyx has provided more than a little, they are life savers!!!

My mom was here for about three weeks, and we are so relieved she is now back in her assisted living facility. First, the great: the PT staff is amazing here -- many of the same staff who were there in 2015 were still there, and mom couldn't say enough good about them -- caring, competent, compassionate, patient-centered. Her weekday CNA, Millie, was an absolute gem, and cared so well for my mom. Thanks to Millie and the PT folks! And now, the awful: The weekend "staff" was appalling. Mostly agency staff who didn't know patients and, frankly, were either overworked/understaffed and/or didn't care. Mom waited extended periods of time for pain medication, her calls to the bathroom went unanswered for long periods of time (leading to accidents), and she was left on the toilet for 40 minutes one evening. Responses to our complaints were, for the most part, curt and uncaring. Discharge was a complete disorganized mess. I've worked as a long-term care nurse, and I know this job is hard. I know the system is set up to not work well, and it's even harder in this environment. But a little bit of caring and compassion goes a long way, and we saw it only in a few pockets at Carlton this time around. (And no, I don't want to talk to someone at Carlton to resolve this; they had their chance when my mom was there. Take a hard look at yourselves, folks.)
