No dirty urines, always 100% compliant and basically flawless history. Anyways, in 2019 i transferred to Crossroads, which allowed me to transfer in with my monthly take homes right away. Then this year i had to move to ARS for insurance reasons. Initially they said it could be a month and i had to do 4 groups to get my take homes. I thought that was a bit much but still, i did that and then after i did that, they change what they said and now say oh it has to be at least 90 days to get takehomes. Despite tranferring my records and showing no reason. Its not just that they wont give me my takehomes, but the way theyve handled it and keep changing the requirements. I wouldnt have switched if i had thought i couldnt bring my takehomes. So anyone thinking about joining from another clinic, beware. I gave it 2 stars cuz i actually really like the nurses. And my counselor is a nice lady.

The current supervisor has a private practice in the side, and rest assured that is her main concern. Do not expect support from HR or any leadership. Yes, I was let go and am resentful, which also means I have no fear of telling the truth. Not only do caseloads exceed legal standards, they pressure counselors to produce quantity and not quality work.

We arrived at 11:00 a.m., and the condition of the facility was immediately concerning. The room was poorly maintained, the TV did not work, the sheets and pillowcases were rough, and the overall environment felt unwelcoming. After just one night, I received a 6:00 a.m. call from my mother, crying and begging to come home. She told me that she rang her call bell multiple times for help using the restroom, yet no one responded. As a result, she was forced to lie in urine throughout the night. When staff finally entered her room, they were irritated and dismissive rather than compassionate. I addressed these concerns with the daytime staff and made it clear that my mother had an advocate looking out for her. Instead of improving her care, the staff seemed to retaliate. That very night, she was again mistreated. The next morning, at 6:30 a.m., my mother called me again--this time trying to plan her "escape" because she was so desperate to leave. After speaking with a friend who is the General Manager of a Home Healthcare company, he made one call and secured her a bed at Lake Manassas Rehabilitation. The difference between the two facilities was night and day. When I arrived to move my mother, Evergreen staff initially claimed she could not leave for three days. I insisted she was leaving that day. At 2:30 p.m., they handed me a release form, which I signed. Immediately afterward, they informed me they were no longer responsible for her--even though she was not scheduled to depart until 5:15 p.m. I spoke with the manager and firmly instructed them to void the form and continue caring for my mother until her transfer. Evergreen felt cold, neglected, and utterly disorganized--more like a graveyard than a rehabilitation center. I have never witnessed such substandard care in my life. They know exactly who I am, because from the moment my mother arrived, I made it clear that she would not be ignored. I strongly advise others: Do not bring your loved ones to this facility. Ever. I am so grateful to GOD that my mom was moved within 48 hours!!!

My mother was there for 20 days, and in that time she had staff members completely ignore her--as in she asked for a diaper change and the staff member shrugged and left and she waited several more hours before someone helped her. She also had staff members who did everything they could do to make her comfortable, such as comb her hair, help her brush her teeth, stop to chat. The night shift was particularly helpful and compassionate. Her room was dirty, as in dirty discarded gloves that sat on the floor next to the bed for days at a time and an incredibly dirty bathroom and toilet. I was also very unhappy with the total lack of communication from the directors/managers. They did not include anyone, including my mom, in her treatment plan, nor did they alert me, her guardian, when she had any medical issues that needed to be treated. Thankfully she was cognitively fine and could tell me what was going on so I could THEN contact management to find out what was going on. So, if you or your loved one ends up there, ADVOCATE, COMMUNICATE, BE VOCAL. They do not initiate contact, but they will respond to some extent.

    These are the things that are important to me and it's why I would highly recommend this therapy…read morelocation: 1- there are 4 therapist plus sometime PT Student in training would help the patients. Without enough staff, it's hard to make appointments that fits anyone's schedule. With this much help, plus their open hours begins at 7/8am-7p, I am able to efficiently make 2-3 appt schedule weekly and get to finish my therapy session in 1.5 months! So happy 2- the amount of space and the tools that they have is essential for efficient recovery. This place have 5 very long tables to lay on, plus clean sheet with pillows, 5 leg workout machine, 2 bikes, and many exercise balls/dumbbells/ stretch bands , 2 private patient rooms, a lot of space in order for all the 5 PT staff to help 5 patients at a time. 3- we can't forget how important (Riley) front desk secretary is when processing appointments and insurance and politely greeting everyone! She was so easy to get along and always answer the phone quickly. 4- if I get hurt again, I will come back here again and again. They helped me recover my foot injury 2 years ago and now my lower back and UI ARE getting amazing help from Dr.Barber and Student-Chloe. THANK YOU
