Be warned They are chronically understaffed. If you don't mind you loved one waiting 30 minutes for a response on their nurse call buttion this might be a place for you. The issue here is the call system was working, it showed the call at the nurses station, but there was no alarm after 10 minutes of being not answered. The facillties service level is it should be answered in 10 minutes. My relative was helped into the bathroom and forgotten for an hour in his wheel chair. He could not signal for help. The food looks like the cheapest food they could find. As a good point they made efforts to meet dietary requests, as they should. The staff was helpful when they were available. My guess is the money is being siphoned off into director salaries and not being used for everyday staffing. The excuse was they were short staffed that day. If it only happend once I wouldn't be warning other folks this this review. Something happend every day. We are looking for other facilities. read more

Disappointing lack of communication. Husband has dementia and was a patient here from 6/23/26 thru 7/8/26. Medicare coverage ended on 7/4 Because we needed to find a Memory Care Facility for him, we paid the estimate given of $1424 for the additional 4 days. We have now received an additional billing of $1245.92 for therapies done on those 4 days We were never given an estimate for this amount. We only expected the room and board for those 4 days till we had a place for him to go. In general husband's care was marred by a general lack of communication. Never met or conversed with a physician. Husband was itchy and had a rash on the weekend, without seeing him in person (that I know of) an antiviral was prescribed on the chance he had shingles. When I asked days later, I was told he did not have shingles. I had difficulty getting information even re: his showers. There was a team meeting re: husband's care ... Notice was given to husband who put it in a drawer and never gave it to me DEMENTIA... In general I had difficulty reaching administrative or medical personnel....frequently not on site. This facility needs an active Patient advocate. read more

The sink is outside the bathroom so that is unsanitary. The staff doesn't care about diabetic diets or providing insulin to help mitigate the high blood sugar. The TVs have the same remote frequency so each patient changes their roommates' tv channel so that's annoying. You can just walk in and out of this place no one checks you in our out. No one cares. The patient could leave and they wouldn't even notice. read more

My wife was there she was not cared for she wasn't given a shower for 2 weeks staff wasn't very attentive to her needs I don't recommend this to anybody Thursday night she asked for shower and was told they give a cold one there water heater was broken. My grandma was their for 3-4 weeks, and they weren't taking care of her wound as they should of. she was supposed to go home a week before but they weren't changing it like they were. so we complained and it was fixed. They would get mad when my grandma need help to the bathroom or the shower. they never checked up on her like they should have or on anyone else there. i personally don't recommend this place at all. read more

DO NOT send your loved ones to this place, this facility is horrible they do know care, they take advantage of patients that can defend themselves, my father was there for a short period of time and he told us they wouldn't give them medication or move them from their beds, they keep patients drugged, no bath the entire time he was there, not allowed to use the restroom if they require assistance. They hide their cellphones and when you try to speak with anyone there they always say they need to call back. DO NOT let Anyone you love there. read more

My mother was 'treated' here in May and June of 2020. The staff took her phone away from her so she could not call her family. There was no phone in her room. She was not bathed for 5 days. She could not walk without her walker, and because of Covid, the family was not allowed to bring a walker to her. The facility did not provide a walker for her. They billed for PT services that they did not provide. This place was truly horrible. read more

SHAME SHAME SHAME. This Albuquerque facility left my sister in bed with a blood infection for 3 days and did not send her to the hospital. Her son went to visit and she was already half dead. He had to call 911 himself. She also had bed sores and pressure sores on her feet. Needless to say, she passed away 5 days later. read more

My mother has was transferred to Canyon Transitional Rehabilitation Center almost one year ago. In this time, she has received excellent care. She is very happy here and has become more engaged and her health has improved greatly. Every single staff member that I have encountered at this facility has been very friendly and helpful. Anytime I have had a question, it has been addressed quickly. She is bathed regularly and is always cleaned, dressed, and groomed every morning. Her room is cleaned daily and housekeeping is always cleaning the common areas. She enjoys the daily activities very much. The activities director provides very creative, fun and beautiful projects. There is a sense of community in the activity program that has given my mom so much joy. She looks forward to the movies, music and the snacks that she enjoys so much. She loves the staff and friends that she has made here. I have asked her several times if she has any concerns and she tells me how happy she is here. My experience with "Genesis" aka Canyon Transitional Rehabilitation Center, in Albuquerque was horrendous... I did not chose them in the first place... If I had any say in the matter, I would NOT have chosen them for "rehab" after two foot amputations... No choice in the matter I was told... Aside from the congenial medical support staff, the people in supervisory positions were the most abusive people I have ever had the displeasure to experience... I will site several examples of WHY you should avoid them like the plague... Aside from what I consider Medicare Fraud, I will describe the maltreatment I received by both administrative and medical staff... Following the amputations, I was treated with a "wound vacuum" which consisted on being hooked up to a machine that required me to remain in bed while the machine helped accelerate the healing process... I was also given an array of antibiotics which were administered via a "PICC line" a device called a Peripherally Inserted Central Catheter, or "PICC line," which is a thin, soft plastic tube -- like an intravenous(IV) line -- that allows you to receive medicines and fluids... A nurse places a PICC line into a large vein in your arm and guides the catheter up into the main vein near your heart where blood flows quickly. the antibiotics... That apparatus was attached to the main vein in my left arm... It was held on by a flimsy strip of cellophane tape... It was pulled off, not once but several times... Their consulting "PICC" consulting technician was referred to come in to replace the line when it somehow (mysteriously) became dislodged... Not intentionally, of course... Keep in mind that this person was a highly-paid "consultant" who was called in to replace the dislodged PICC ~ not once, but THREE times... The last time,. I suggested something heftier than a flimsy piece of cellophane tape... I think you get my gist... The overnight staff included a "head nurse" ~ a man who to all purposes looked like something the cat dragged in from off the streets... Filthy nasty and unshaven with tattered clothing who got off on YELLING into my face from inches away... Not only because of the disconnected PICC, but also when his "lunch break" in the middle of the night came into my room and told me that I had wet the bed! Not only had I NOT wet the bed (how childish of them to even suggest this, I hadn't wet the bed since the 1950's!), they did this on their "break time" at 3:30 a.m. Not only was there no urine, but the wet spot was on the very edge of the bed... It was water! Keep in mind, my feet were unceremoniously slammed to the ground when they quite literally pulled me out of bed... My feet! I was there to heal my feet, not have them slammed... You may be thinking: "This guy sounds a little nutso"... But, I am here to tell you about my experience ~ the truth and the whole truth... I am not exaggerating... I have better things to do than to revisit my time at Canyon aka Genesis... MEDICARE FRAUD: Got your attention? Many instances which I am going to report to CMS... Medicare was billed for non-existent services which never took place... Three consecutive days of a particular therapy that was never administered... Each day, the time increased... The last one was for a 45 minute session that NEVER HAPPENED! (But, wait... There's more... Lots more... If you are interested in more details, I have them... The last contact I had with the administrative staff was when they prohibited me from leaving the facility to take care of urgent personal business... Just days before, they sent me to my bank to get documents)... I was told: "Don't come back"... As if I would ever want to... BED BUGS!!! Horrible place. A few dedicated employees in a sea of laziness and incompetence. My mother went there after a car accident. Despite having a broken neck, she never once saw a doctor, although her insurance was billed for daily doctor visits. When we finally moved her to a better place, the doctors and nurses were appalled at the meds they had mom on, such as an appetite stimulant and an appetite depressant both. By the time we found room elsewhere for her, her health had declined to the point she couldn't recover. I firmly believe she would still be alive if she had never gone to Canyon. read more

Avoid this facility like the plague. They are extremely understaffed. I would like to register a complaint against Canyon Transitional Rehabiliation Center in Albuquerque,NM. After a hospitalization for COVID we were told the COVID Quarantine facility for Albuquerque Seniors awaiting Assisted Living placement was Canyon Transitional. We were forced to provide payment for the month up front, nearly $10,000. We never signed intake papers and our mom suffered. She was not showered or dressed for 5 days. She experienced a skin tear on her lower leg that is still being treated. She was not allowed a personal visit with her hospice nurse. She was not provided her pain medication as prescribed. When we called on this the nurse was rude and stated she needed to ask - I begged her to take the meds in and she huffingly agreed.The COVID unit had one nurse for two hallways of rooms. On transport, although the social worker & nurse confirmed the day & time, our mother was not out of bed when it arrived, she was not dressed, or packed. Our mother was brought out in the pajamas she had worn for 5 days, although we had provided clean, warm clothes. She had no coat, no shoes, no blanket on her at 9:45am at 40 degree temps and was left in her wheel chair in her pj's and socks next to the transport. The nurse was more concerned with signatures for meds than covering our mom. I finally took a sweatshirt out of the bag of belonging and the transport driver helped my mom put it on while I put shoes on & the nurse stood there with her paperwork. Horrible 100 percent, I was treated bad from the first moment wheeled in,Toilet in room clogged I took pictures floor flooded nothing clean before I got there still had someones dirty pajamas on bed I was supposed to stay in I took many pictures and walked to room down the hall,stayed in tv room chair till picked up at 9:15 am next morning main door was unlocked,no way would I of stayed there and absolutely no way would I put a family member there ,This place is not for rehabilitating of any sort it's for old people with out family to stay as nursing home only, Pee odor in entire building mostly old sick very sad to see no communicating from any staff ,nursing,doctors, for 14 hours until one nice lady from rehab approached me at my chair 8:15 am 15 hours after sitting in chair , first conversation with a human being!! I have many things to say about the horrible treatment there from seeing from the inside, None of which is this lady from there rehab department,she was polite and concerned about my night there and helped to get me out of there ASAP, I've had bad dreams for several nights now of the degrading treatment I was submitted too, Do not put anyone you care or love in this place !!!!' I would like to speak on the behalf of the staff as well as the administration of this facility they do maintain and inquire the best of Care as any other facility in the metro area it is a constant revolving door of people coming in with severe to life threatening physical and mental illnesses and when they have been rehabilitated they walk out on their own power or leave the facility on there on transportation with outstanding recovery experience. There are not too many places you can go and be respected and care for on the maximum level of health care needs as you would here at Canyon. read more

This facility is horrible! It lacks leadership and organization. My father was placed here, it took forever for staff to tend to him and they lost his belongings. Including sending my father to unscheduled appointments. My father would use the nurse button but no one would come, so he would then call me to call the front desk. No one wanted to give me someone higher up to talk with each time I would ask to express my concerns. Even the Genesis website did not help! I felt like I was the one giving customer service to the staff, but yet never received it back. They don't answer their calls and if they tell you they will call back they don't! I pray this facility changes it's procedures and practice more professional development or job duties. Such a poor excuse for a rehab center. It had been a long term nursing home and still demonstrates that even if one is there for short term "rehab". The "rehab" is barely existent and most of it proved to be either useless or actually harmful/dangerous (as was discovered once home and receiving proper care from other sources). The administration at Canyon is both unreliable and unprofessional. For weeks and not days "no one" knew where the director of this facility was. It came to be that she was the lady coming in and out of her otherwise locked office all day long. But no one ever knew where she was. That happened for two weeks although we passed her every day, sometimes numerous times a day never knowing who she was. The problems at this place are chronic. They claim to know about it but seem to do nothing about any of them. A patient fell out of her bed and laid on the floor for over an hour. She only received help after another patient had to call an outside 3rd party to inform them and ask them to call the nurse's station. There is no way for a patient to communicate with the nurses other than a simple, old-fashioned call button (which then goes unanswered). There are no phones in the rooms and if one does not have a cell phone with them, they're out of luck. The staff, again, mostly administrative, were often incorrect, lazy and/or late in dealing with problems, issues, orders, and setting up home care resources upon discharge. This place is only in it for profit and why they now claim to also be a skilled nursing/rehab center. They can charge Medicare and insurances companies more if categorized as such - even though they don't truly proved the services. read more

I would like to take this opportunity to express my sincere appreciation for the professional treatment bestowed to me while at Canyon Transitional Center. Everyone at Canyon, which includes Misti Melendez the Admission Director, the PA, nursing staff, administrative staff working behind the ward floor counter, TNAs, Chefs, and those working in the kitchen staff, which I observed, gave one hundred percent effort towards the benefit, and care of Canyon guests. Every day I observed that each and everyone of the employees that I had contact with, were always cheerful, accommodating, and willing to assist without complain or a frown on their face. There were five employees, that in my opinion, and observation gave one hundred and ten percent towards their work during their shifts at Canyon, that in my opinion were exceptional: Not knowing their last name, I can only provide their first name. Bryan the Physical Therapist. Not only did he help me heal by his clear instruction, professional bearing, and knowledge of his duties, but I observed him helping other Canyon senior guests with compassion, and untiring expertise to a degree that helped lifte their moral. He is truly exceptional in applying his occupation. Billy one of the Managers of Physical Therapists. She has the unusual ability to put you at ease while she works with s Canyon guest, to find out if they a fit to live alone. I observed that she is equally diplomatic, and sensitive towards all Canyon guests. While taking my physical therapy I observed her talents in working not only with Canyon guest, but also with other Physical Therapist working under her supervision. She is professional in her leadership, and a great credit to Canyon Transitional Center. TNAs Heather, Shah, and Juanita, also displayed exceptional talent in taking care of Canyon guests. While a guest in room 109 bed 2, there were two different guests that occupied bed 1 in the same room. Their relentless efforts to make me, and the other two guest comfortable were notably unusual, and exceptional. Since I had trouble sleeping, I observed them coming back, day and night to quietly check, and see if we were sufficiently comfortable, and that we had cold water to drink. Such dedication should be acknowledged by their superiors. I should mention that the food, in my opinion was exceptionally tasty, well prepared, and served. Although I needed to get well as soon as possible, and get home to assist my handicap spouse, I had a good experience, and learned a great deal in how to deal with my wife's handicap problems. I agree that they are understaffed but when you can find staff they have been polite, kind, and professional. Does not have as much of a nursing home feel as some rehab facilities. Visiting hours are open, phone to the room, food was good for the breakfast, bacon was good.....so far...only had my friend there for a few days. Do get a hold of the "welcome folder" which tell you your rights, like you can have a shower daily with staff assist. It helps to have a friend or family check on basics when you come into the facility...like check to put the heat on in the room, ask for extra blankets if you need them. read more

If you care for your loved ones, do not take them here. It is over run by inadequate, uneducated individuals. Every time I would call, I felt like I was speaking to a kindergartener. It seems to me that this place needs a lot of staff training in procedure and also hospitality. They did not provide me with adequate updates of my father's wellbeing and their lack of detail and professionalism is truly astounding. My father entered the facility to regain his strength after he had a fall and went into rhabdomyelsis, but also has had some mental confusion. Unbeknownst to me, they sent him via ambulance at midnight back to the hospital. Even though I am listed as his power of attorney, I was not the first contact. I was told that they attempted to call a Susan (who is my mother his ex wife, who also by the way, never received such a call). It just amazes me that no one has returned my call nor have they told me about where his belongings are located. I was relieved to know he was back at the hospital with caring individuals who provide me with daily updates. When I went to pick up my father's belongings, I was looked at like I was speaking another language. One lady was so rude she didn't even take the time to look up at us from her paperwork. A gentleman in a wheelchair asked her where to get out, and she just ignored him. If that was my father, I would at least expect her to treat him with kindness. We took my mother to this facility (as recommended by the hospital) after a 3 week stay in ICU following a stroke. She was there for 10 days at which point they had to transfer her back to the ER at 1:00 in the morning because her breathing was irregular and had been for hours. Her rate dropped to 69 ...(should be 90+), before calling for an ambulance. Once at the hospital, they found she had the following conditions; an extreme case of UTI, pneumonia, dehydrated, septic, bowels were impacted and bleeding, and anemic. Not only did the ambulance drivers tell us not to return her to Canyon, but the nurses in the emergency room told us to talk with a lawyer. Other nurses and Dr's have told us that even though she was not carrying a fever, which was the excuse this facility gave us for missing the infections, they should have seen the signs. Please do NOT take your loved ones to this facility. Their neglect almost killed my mother. read more

I currently have a family member at his sorry excuse for a rehab/skilled nursing facility. They are ignoring her. Things have been requested of the staff for her and they are not being provided. We have not been contacted by anyone there to provide an update. She's been there almost a week. Her case manager hasn't been in to see. One person tells you one thing, another tells you something else, and if you speak to third person you get even more different information. I won't go into detail right now since she is there. They all seem to have their heads up their a***ses, and definitely DO NOT CARE about my loved one. AT ALL. Like in one of the reviews that don't show up for your to review unless you look below and click on another button to get more reviews. A fall went ignored by the staff! Very unacceptable!!! Ginger H. you need to get on the ball and fix these problems. I for one would love to have you SHUTDOWN! 100%. If she gets hurt while under YOUR care, if she dies under YOUR care....It won't be pleasant. I don't know you all can go home and sleep knowing how piss poor a job your are doing.
