My 92-year-old mother was admitted to rehab after a brief hospitalization at SSH for a left-sided stroke. I live in New Jersey, but I'm closest to her and have been involved in her care. My only contact with the staff has been by phone, and every single one of my interactions was unpleasant. The very first call I got was from the discharge planner, who was incredibly abrupt. I tried to talk to the charge nurse for the unit, but I left three messages and didn't receive a return call. After my mother started getting bills at home (which she shouldn't have received), I left 2 messages for the administrator and 2 messages for billing, and didn't receive a call back. The only calls I did receive were from a nurse who wanted to put my mother on opiates for pain, without optimizing her non-opiate pain meds. I requested an update in 2 days, but of course nobody called me back. Since my mother is only partially verbal, she can't complain about the lack of attention. I can. I'm a physician, and I can tell from 300 miles away that CareOne Weymouth is suboptimal. I'm only glad that I got her transferred to another facility. For a facility to have an average of 1 1/2 stars when the lowest rating is 1 star, it must have some serious deficiencies. If they can't bother to return phone calls, it's no wonder they receive poor reviews on other aspects of care. read more

I was there for 3 horrible weeks The cleaning person came by once Thank god I went back to south Shore Hospital. I had surgery for 24 screws and 2 rods. Our experience with CareOne at Weymouth has been awful and deeply alarming. My grandmother was admitted five days ago for short-term rehab and physical therapy, and what she received instead was neglect. When my sister and I arrived one day, we found her lying in bed, clearly struggling to breathe -- wheezing and out of breath -- and no one was doing anything. Staff were walking past her like everything was fine. We had to ask for someone to check her oxygen levels and vitals. That alone was shocking. It turns out her oxygen level was dangerously low -- it dropped to 86 -- and she had to be rushed to the hospital by ambulance. She could have died if we hadn't shown up and spoken up. That is absolutely inexcusable. On top of that, she never received any of the physical therapy she was admitted for. She wasn't even given her daily medications on time -- sometimes not until the afternoon. And she went five full days without being showered. Every time we asked about these things, the staff blamed short staffing. But being short-staffed is not an excuse for ignoring patients' medical needs. This facility is disorganized, understaffed, and unsafe. If we hadn't stepped in, I truly believe something worse could have happened. Families should not have to worry about whether their loved ones are safe in a healthcare facility. Please think twice before placing anyone in CareOne at Weymouth. We trusted them with our grandmother's care -- and it nearly cost her life. ZERO STARS IF I COULD!!!! read more

They need more staff day and night time.. To make any human sit in their own fecese/waste for over half hour is not a caring facility! To not adhere to pain management is in humane. To making patients wait over an hour to relieve themself or make them spend half hour on a bed pan after they finish is not sanitary and unsafe! For family not to have contact people or information is not acceptable to not be able to reach a patient advocate or social worker in a timely manner is just not ok!!! Not changing bandages or cleaning wound vacs on time can be detrimental to anyone let alone people with weakened immune systems.. people go into some times forced into these places in the weakest moments of their lives, they depend on people to care for them with dignity and respect and you do not get that at all at this place!! read more

This is my second rehab stay at CareOne Weymouth in the past 3 years, a total knee replacement in December '22 and a spine surgery a week ago. I highly recommend this facility. None of these places are perfect but if you need skilled nursing and rehab after an injury or surgery it's excellent. Someone is constantly cleaning whether it's mopping, dusting, emptying the trash, or cleaning your bathroom. The staff tries very hard to accommodate your needs but they're not mind readers. You have to advocate for yourself, or have someone who can on your behalf, and they will try their best to meet your needs. The food is not great but you can tell the dietician what your preferences and dislikes are and they will do everything possible to deliver on your requests. You can also call dining services or speak to your nurse or CNA if you'd rather have something else that's not on that day's menu like a burger, grilled cheese, chicken salad sandwich, grilled chicken, etc. It's not a gourmet restaurant but you can get the nutrition you need to recover. The nurses and CNA's are very kind and pleasant as are the therapists. Speaking of therapy, it's top-notch. They know just what to do to tailor your therapy plan to your needs and they make it fun. Kudos also to the wonderful and caring internists, Dr Cummings and Dr Raduazzo. There's a very pretty patio in the back with a water feature where you can sit with family or friends and eat or just hang out. A couple of times my visitors brought food from the outside (pizza, subs, salads...check out Gusto's) and we sat out there under an umbrella enjoying the last rays of summer sunshine. There are two things I find a little annoying but given all the bigger issues in the world they seem rather insignificant: 1) being woken at 5 am for bloodwork or a pill that could have waited until 8 am; and the loud and harsh alarm that goes off every time someone opens the back door without entering the code. Those two things make it difficult to get a good night's sleep but overall, it's much more peaceful than other places I've been either as a patient or a visitor. read more

Care one is a long term as well as short term skilled nursing facility. The care I received overall was commendable; once you get to know the people and the people get know you, manners matter, please and thank you are simple ways to let people know that their help is appreciated. I made friends very quick about 90 percent of the staff will do the best they can for you as humanly possible within the confines of the state laws. The therapists I met were the salt of the earth. The facility is top notch house keeping is always cleaning and the maintenance tend to be fixing things that is even suspect in their eyes. Management are very courteous. And the food is good. I seen some of the other reviews and they must have been over privileged people writing or maybe emotionally lashing out. This place deserves a better reputation. Dan Flynn read more

The staff does not care about a patient who is just there for physical therapy after breaking a hip. They don't help them get out of bed to go to the bathroom. They put them in diapers and they leave them filthy. They don't wash them up and they don't know how to transfer a person and my brother has fallen twice so I'm going to report them. read more

My dad has unfortunately, needed a few rehab facilities and Care One was a great place for my dad. The nursing staff there was fabulous and really did a great job with managing his pain and caring for all of his needs. Arianna was especially wonderful and very attentive. She was on top of all the changes and made sure my Dad had everything he needed. The rooms were clean, the staff was attentive and they were great with communication. My brother was to this rehab for his MS and cognitive behavior. Let me tell you they are the most incompetent people I have ever dealt with. No one calls you back. They all pass you off to someone else. The worst part is they lie to you when asking concerning questions about his after care or long term care. These people all staff including doctors honestly do not want to be bothered. They give you nothing but BS I do not understand how they are still in business. I have heard horror stories since dealing with them. All I wanted was some support and direction in moving forward and I did not get that. My brother is legally blind walks with a walker has sever MS with poor cognitive abilities. They could care less. Most useless place for anyone. Do not send a loved one here they are not helpful at all. Everyone is robotic ....i do realize most of these places are under staffed and over worked employees but that doesn't mean you can be careless to those in need. Useless!!! There is No CARE @ Careone Weymouth. This is more of a warehousing of patients that the hospitals ship out for bed space, but, who are not ready for home yet They DO NOT offer any type of medical care other than providing your daily medicines (not pain meds, those have to be requested which is hard for some the patients to do in their state.) Even the staff are critical of the way things are handled. Physical therapy IS performed here ( only if your insurance will cover it, and then it's just walking around the hallway once.) You are served 3 meals s day , on a very rigid schedule. Never HOT, always strange combinations of food. KEEP YOUR LOVED ONES FAR AWAY FROM HERE. I have to say that the aides (CNA's) that cared for me on a consistent basis, meaning, I saw them on a regular schedule, were the best. Caring, kind and understanding. If it was not for them I would have gone mental! PT/OT people were great, and I have my favorites :). There were a few nurses and I felt really listened to me and were great. But not all of them. But the management, social worker, and other administrative staff were horrible! They did not give a rats a$$ about you or your situation. The food was mostly disgusting! Order from Uber Eats if you can. I gagged every time they brought breakfast. Lunch/Dinner was hit or miss. Since you share a room with another patient, there is no privacy at all, heaven forbid you have to use the bed pan while your room mate has their whole family visiting! The aides & nurses are under staffed and sometimes one aide had to care of as many as 14 patients by themselves. Plus most work double shifts just to make enough money to pay their bills. SO I assume they are paid a very low scale pay! This is not conducive for great care, they are exhausted. They have a very hard job! And one nurse for 25+ patients is ridiculous and they are the only ones that can administer your meds, so you have to wait your turn as they go down the corridors administering meds room by room. There were patients that yelled all night/day long for someone to either close their door, or they just needed attention. I truly feel for the CNA's and the few kind nurses. But I can't in all good conscious recommend this place to any one! Unless things (management) changes this place will never be the place to put someone you love. read more

I made many trips to the nurses station because I knew better than to ring the buzzer and every time I asked them a question I felt like I was bothering them. This place is horrible. The staff hates their job and it shows. How can the kitchen be allowed to serve what they call food!! My biggest complaint is the nurses have no idea how to use the Lidocaine patches!! They continually took one off my mother and slapped another one on!! Those are to be used , 12 hrs. On and 12 hrs. Off!! Using them the way the staff did there could've caused the patient to overdose, get really sick, even fall into a coma!! The head of nursing should be embarrassed!! They shouldn't have them if they don't know the correct way to use them!! If you have a choice, don't chose CareOne!!! read more

Pretty awful. A lot of the people who work here hate their jobs, and it shows. Call bells are hardly ever answered and forget about getting answers to your questions. There are a few people who seem to care, but even they are being beaten down by over work and lack of interest by their coworkers. read more

Doesn't deserve a one star. DO NOT SEND A LOVED ONE THERE. I was sent there from the hospital and spent 6 weeks. I was no better off the day I left than when I went there. The place is horrible. Staff is miserable. Couple times was given wrong meds but I told them. There were maybe 2 aides 1 nurses that were good the rest were Useless. Long wait times for staff to answer call lights. Many times I waited 45 minutes or more. I was told to just pee myself instead of bothering the aides. Food sucked my tray was never right and food was cold. The day I left I was given my release and that was the last time the staff helped me. I had to wheel myself out to car and transfer into car with no assistance. Again. DO NOT GO THERE read more

The most horrible, unorganized rehab I have ever encountered! The physical therapy is maybe they'll do it one day then maybe a couple days later then maybe for five minutes or whatever they feel like they are so inconsistent. Some days you have to beg them to get the patient out of bed but on a day when there's a big meeting or the discharge day was the best service. They do not push their patients at all. The patients are treated like it is a nursing home instead of a rehab, but then again, they're so inconsistent with their care! You press the call button they might come or they might not even come at all! we had to Put my mother to bed on several occasions because they never came and then never checked all afternoon on her or asked how she got back in bed. Then they try to give the patients the wrong medicine a few times. I could go on and on, but they should not be in business. read more

Honestly I would not put a family member here at all. my partner had cervical stenosis surgery with 22 staples. He has to wear an Aspen collar to keep his neck stable. 1. on 2 occasions nurses were to change the bandage. They wanted him to move his head forward so they could do their job. This is a no no due to the ability of dislocating screws. PT stated that was wrong. Why didn't PT and nurses coordinate this? 2. the bell was rang for pain medication at night. the CNA went to get a nurse. NO ONE SHOWED UP. 3. Another time he needed pain medication and it was time for him to get it. we rang the call button. 15 minutes later I asked why they didn't respond and the reply was "it's only been 15 minutes". This was unnecessary. I then found out the pain meds were PRN and we were told it was scheduled. His nurse was at lunch and there wasn't another nurse to help out? 4. The nurses may handle 12-18 people at a time which doesn't allow care for those patients who need it. 5. Many people came into the room without name tags. I don't know if they called a CNA to change his dressing. 6 He was in the bathroom and he pulled the cord for some assistance as he can't turn to clean himself. NO ONE EVER CAME AND HE HAD TO LIFT A COMMODE! they came up with many excuses. 7. A nurse came in and said here are your meds. she never asked how he felt nor did she even say hello With that being said there are a couple of wonderful care takers and the PT and OT workers are phenomenal. They also couldn't locate a Walker nor a commode? They are a care facility! The food itself would kill a healthy person. If you care about your loved ones you would not put them here. There's zero communication between the parties! Do not put a loved one who needs care in here! read more

My husbands grandmother is at CareOne in Weymouth and ALL of her stuff is missing. We called and left MULTIPLE messages and only once we were connected with someone else were we told that her case worker wasn't even THERE anymore and no one had notified us!!! She's been falling out of bed and no one told us and all of her belongings are GONE. They don't "care". They're not doing us a SERVICE. We PAY them. It's their JOB to care it's what they're paid to do. Not keep family members in the dark about what's going on with the health and well being of our loved ones. I am disgusted and will be moving her out of there. Don't waste your hard earned money on this crap hole. read more

If I could give a negative stars rating, I would. Horrible facility that almost killed my dad. Had we not gotten him out of there when we did and back to the hospital, I'm certain he would have died at CareOne. He was only there about 3 days, after being transferred from South Shore Hospital. He had a mild stroke about a week prior, from which he had recovered significantly thanks to the therapists at South Shore Hospital, and he had pneumonia for which he had been treated with antibiotics while at the hospital. On his last day at the hospital, his voice was strong, there was almost no slurring in his speech, and he was very alert and coherent. When I talked to him on the phone at CareOne 3 days later, he was barely able to communicate with me at all and was very confused and exhausted. My brother who was there outside his window almost all day of the days he was there noticed the same exhaustion and confusion. We both expressed our concerns to the nurses, but they were dismissive if not downright rude. Communication was horrible. My brother never got calls with updates on our dad, but was only able to get information when he called and then it wasn't very thorough at all. When we insisted on transferring him back to the hospital, the hospital doctor assessed him as MASSIVELY dehydrated and immediately started him on IVs. The next morning, his swallow capability was assessed as significantly deteriorated during the time he was at CareOne, so he had to be taken off food and drink. His pneumonia had also worsened during his time at CareOne. The negligence and incompetence of the staff at CareOne definitely would have killed my dad if we hadn't gotten him back to the hospital. If you care at all about your family members or friends, don't let them go to CareOne! read more

If I could have left 0 stars I would of. This place almost put my mother in the grave. The nurses are incompetent, and the aids are mean. I observed a nurse trying to give my mother her bedtime meds at 4 p.m. I asked her why and then she lied to me about the prescribed time for her medications. The prescribed medication was 10 pm she told me 6 pm. My mother was on a bed alarm that was not functioning. She had 3 falls and 2 hospitalizations within 3 weeks. They were giving her her metformin, not checking her blood sugars, and rarely checking her vitals. She went out in an ambulance twice for dehydration and confusion. She was dehydrated because no one gave her fluids and confused because her blood sugars were 50, she wasn't eating, and they were giving her metformin!!! The rehab was nonexistent they left her in bed, and she developed a stage 1 pressure ulcer on her sacrum. The aids were rough with her and her roommate when they washed her my mother said it felt like they wanted to scrub her skin off. They would put my mother in a wheelchair in the TV room alone with the TV turned off watching the wall. I witnessed it all because I was there every day. The food was gross and cold and looked like cat food. I took my mother out after her second hospitalization and moved her into another facility. She is now thriving and walking with assistance eating and drinking. I didn't realize the poor care and the reputation speaks for itself. I could write a lot more about the neglectful care that she received. Please do not put your loved one here, this is so heartbreaking. I feel bad for the residents who do not have an advocate. This place needs to be SHUT DOWN!! read more

Terrible facility. Little to no communication from the Staff to the family of the patient, especially at a time when visitors are not allowed into the facility to visit due to Covid 19. Very neglectful & rude to the patients. Whoever is in charge should terminate the employee named Dora. She left the patient in bed all day, numerous times because she said the patient was too fat to get out bed. Instead of getting help, Dora left the patient in bed all day! Not impressed with the physical therapy team or the social workers on Staff. I have had family utilize other facilities in the area & never had these kind of issues with them. read more

This place should be called CARE NONE! Absolutely Horrible! One star for the physical therapist that assisted my mother. They did a nice job with the PT/OT part of her stay. Otherwise, she received little care and assistance. I think the CareOne needs to take a good look at the staff and reassess how they take care of the patients. I did not feel that my mother was given quality care. The staff is not welcoming, never make eye contact, do not look at you while they are talking ( for some of the patients they are hard of hearing and need to see your lips to help understand ) there is insufficient follow through, and they are not effective communicators. Although the facility appears clean, there is a constant smell of dirty diapers. My mother was admitted for rehab she was non-weight bearing, due to a fractured hip. Communication and documentation very poor at all levels in this company. Nurses aids Staff had my mother putting weigh on her leg while pulling her up with her underwear to use the toilet. (They should be using a waist belt for safety, and they should be helping her not put weight on her leg) I filed a grievance and meet with Ms. Kennedy (Director of Nursing) and Mr. Coleman (Assistant Director Nursing). They said they would address the issues. I did notice improvement, however, it wasn't consistent. Old bad habits are hard to break. The staff at times would be short with a patient or snap at them when they asked for them to repeat what they said because they didn't hear or understand them. Very disappointed with personal care. After several requests to have my mother helped with bathing and hygiene, they only showered her once in her entire stay, ( almost a month) I would ask daily different staff members for this. The food was terrible. Usually cold, at the beginning they didn't have milk. The portions were large and overwhelming to the residents, the majority didn't eat the food. (lots of waste) There was never any face cloths or cloth towels in the bathroom for my mother to wash up. Staff relied on the use of pull-ups for they were not around to assist with going to the bathroom. If you ring for the nurse, don't expect an answer for a long time. So if your drinking a drink, call now for assistance for the bathroom by the time you have to go they might be there. Many times I would need to go to the nurses' station and let them know that my mother or another resident needed help. This place is understaffed & staff is burnt out. The patients want to desperately go home. The environment is very depressing. You get an irritable unfriendly vibe when you walk in the halls. I feel I had to be there every day and be my mothers' advocate. Some of the staff appeared numb, didn't know how to be compassionate, many residents sit in their rooms staring at a TV or off into space. The few staff members were warm and welcoming, but it was very obvious that most to them no longer loved their jobs and where overworked. There were many occasions where some of the staff would roll their eyes, mutter under their breath and were visibly inconvenienced if asked for or to do something. Personal and attentive care appears to be non-existent. I did observe tours of the facility and how they moved patients around and had it appear as a loving community. (It was all staged) very sad to see. I gave out my contact information several times and still, it was incorrect in their records, record keeping was non-existent. I asked for board games to play with my mother to pass the time, however, the activity room on the second floor was always closed. Suggestion A gentle smile, a compliment, a tender hello, a warmhearted good morning etc. makes the day a little more manageable. Based on our experience, I would NOT recommend CareOne. read more

My grandmother was recently sent here for rehab services and I am very unhappy with the level of care she received. Bedridden patients should not be left in bed wondering when help will come after they have rung the call bell. Not even an acknowledgement that they see you need help or asking what you need to prioritize calls based on emergency etc. The food was inedible most of the time, but don't worry they will give you a free plate for your visitor if they can stomach it. Basic patient care such as bathing, toiletry/catheter, and moving from bed to chair were at the mercy of who was on staff. If our family didn't have someone with my grandmother daily I have serious concerns if she would be going home or back to the hospital due to negligence. Most of the staff are overworked or just don't care. This facility should only be a last resort as long as the patient can advocate for themselves and is not bedridden. read more

I would highly reccomend finding another care facility for your loved one. The people in this facility are unapologetic about their apathy. My dad had copd and needed a bi pap machine while sleeping. I went to Care One when My dad was being transferred, and alerted staff that he needed bi pap. They were very pleasant and assured me machine would be in his room when he arrived. I was extremely dissapointed when that evening there was no Bi pap in his room, and his nurse that evening was exceptionally rude to me. Told me I had no clue about my fathers treatment. I was with my dad everyday for eight or nine months, and learned about his condition as much as possible by talking to nurses, doctors, and physical therapists. The morning nurses apologized profusely for the error, but this should have been when I moved my dad to another facility. In the hospital Care one was touted in specially treating copd, and I have to say there was nothing special about care offered. My dad most likely spent hours with no oxygen, and nobody on the overnight shift checked on him. Other facilities have monitors that will alert staff about potential issues. I would suggest that you do your research finding care for loved one. I would give negative stars if I could. read more

My brother was at Care One for ten days in December. He was sent for rehab after colon cancer surgery. He was thin but ambulatory when he arrived at Care One. He got very sick there , in a frightening way. He was discharged and returned to the hospital with acute kidney failure, infections, severe dehydration, and was extremely sick and weak, unable to stand. Note, he is developmentally delayed and speech-impaired, but understands what is going on. While at Care One, he asked his blessed caregiver (she was there a lot) several times if he was going to die. We did not realize how close to death he had become. His experience at CareOne included a clogged/unflushed PICC line which meant no fluids getting in which meant extreme dehydration and kidney failure, unsanitary conditions ( feces-covered towel left on the counter, urinal instead of getting patient to use the bathroom resulting in urine left on the floor) which may have led to infection, inflamed area around the colostomy bag because not changed properly, and more. He almost died and we are grateful to South Shore Hospital critical care unit for saving him. He required dialysis and oxygen and massive amounts of medicine when he returned to the hospital, we believe as a result of his stay at Care One. He is off dialysis and oxygen now, but unable to stand. We hope with PT he will get stronger, first he needs to get back to the condition he was in when he arrived at Care One. Perhaps the staff at Care One were overly busy. That is often the case; for this I blame the institution, not all the people who work there. One nurse I saw was lovely and gentle with my handicapped brother. And I felt sorry for the phlebotomist who was totally anxious about the amount of work she was assigned to do. But there should be no excuse for the unsanitary conditions nor for the incompetence which we observed. read more

Not much English spoken. Nurses on my 2nd night literally had a fist fight. Need to use bathroom, ring bell and no one comes. Ring it over and over. You very seldom see any staff at all other than an elderly woman who works there and for some reason would hide in my room and watch tv. The food was so bad and never hot. The coffee cups are a dark color, plastic and filthy inside. I showed them the and no one cared. I could go on and on but having nothing good to say says enough. Agree with all the negative reviews. I regret not reading them before because my Dad may still be alive. He went in to recover from a foot infection. Early on, especially on weekends, he was ignored by nurses and aides. My brother heard him mocked 'complain, complain, complain' because they wouldn't give him even over the counter stomach relief. Fast forward 2 weeks and recovery seemed to be going fine. Then on a Thursday he visibly and vocally had low energy. I know now that's a symptom of infection/sepsis. In 3 days no health care professional picked up on it. That Saturday I called him and he was non coherent and i asked the front desk to check on him. After I called back (another consistent problem was returning phone calls and follow up) the person who checked on him said he was just tired. 24 hrs later they realized he was in bad shape and sent him to the hospital where he died 10 minutes later. This place is an absolute atrocity and I hope someone reading this changes their mind if given a choice of facilities. I share in the blame but I'm also not a trained health care professional. Wife has been there almost a week, broken ankle rehab. The first night they wanted her to wear a diaper, she said no way, She can get up to use bathroom. Repeatedly ask for ice water with meals, get milk and coffee every time. Ring the buzzer, good luck. Nobody at nurses station to ask a question.
