Medical and physical Therapy. My husband was at Carson for 2 days before he was sent back to the hospital for breathing issues. First night there. Can wanted to get his weight but would not let him use urinal first. Second night I was called to help my husband to set his Bi Pap machine because the care staff was not trained. Machine had been broken by staff. Easily fixed by manager RN. After staff disrespected my husband and myself calling me rude for advocating for my husband. On the second day there was a meeting with w RN's and Dr. PATEL the Dr who works for the facility. My husband told the Dr. What had happened thinking he would do something immediately. He informed him he had to investigate. My husband and I were dragged into a meeting with no representation with the Dr. Patel, Shannon, RN and Belinda, RN. We were told by Dr. Patel he did not want to take care of 65 year old patients that he works only with 95 and above basically Geriatric people that live there and are above 90yrs of age. We we r e told by Dr Patel he didn't want to work with a 66 year old. I laughed I thought he was kidding. My opinion is he didn't want t o work with a pa t ient that would advocate for himself or had someone that would advocate for him. Horrible Horrible Bedside manner. Then when my husband asked Shannon the RN what would she do if it was her father she, replied, she has referred all her family members to CareOne. Of course, she does because no one would mess with an RN's family member. My husband is now back at Lahey Hospital, because the staff at Care one had him off his o2 for 20 minutes and never figured it out. Dr.Patel was an ignorant Narcistic man eho had a proble taking care of patients that can advocate for themselves. The staff isnot skilled in what they need to be skilled in to be a medical facility. The nurses or case Managers are not there fir the patientsI met several staff members. I can name 2 that were aware of my husbands needs. Patrice CNA and Cynthia, RN night manager one night nurse that worked with Cynthia. My husband did not get sleep in the 2 days he was there. I would not recommend this place nor wold I call it a slilled Nursing Facility. Neither umless you work there and are a Nurse Manager would I place. If I could give this facility negative stars, I would. In mid‑March, my mother fell and broke both of her tibias. I live in North Carolina and wanted her in the best rehab possible. The hospital told me this was the highest‑rated option, and I pushed hard to get her admitted. It is now one of my biggest regrets. From the moment she arrived, I received constant calls -- sometimes every hour -- of her crying hysterically. On her very first day, she called because she had accidentally used the bathroom on herself. With two broken legs, she couldn't get up and didn't even realize it had happened. Instead of helping her, the nurse aide yelled at her and accused her of doing it on purpose. Her roommate confirmed this. I reported it to management, and nothing was done. Soon after, she was placed on a bedpan and left sitting in her own waste for an hour. When I called the front desk to get someone to help her, staff went into her room only to tell her it "hadn't been an hour" and that she "needed a watch" if she wanted to tell time correctly. Again, her roommate confirmed this happened. Another incident: while changing my mother, a nurse pulled on the blanket so hard that she fell onto my mother's roommate. Later, when my mother politely said, "Please be careful, I don't want you to fall," that same nurse claimed she no longer felt "safe" around my mother and refused to care for her at all. I eventually drove 15 hours across the country to see my mother and scheduled a meeting with the case worker/social worker, PT, OT, and nursing staff. By the time I arrived, she had been there two weeks -- and had not received a single bath or shower. During the meeting, the case worker refused to look up from her computer, claiming she was "typing notes," though she was clearly just scrolling. Every concern we raised was dismissed. When I explained that staff were refusing to help my mother, I was told, "Well, she made the nurse uncomfortable." Apparently saying "Please be careful" is now grounds for withholding care. I asked why my mother had a Foley catheter. They said, "She wanted it." Since when is a patient's casual request enough to insert a catheter with no medical basis? I pointed out multiple errors in her chart -- including listing her as stage 4 kidney disease when she is stage 3, and failing to note her stage 4 liver failure. I was told I "misread it." I raised concerns about her worsening tremors and stutter and was told, "Oh, that's her medications, we'll just lower the dose." I also emphasized that I am her next of kin and healthcare POA, yet I had received zero communication. Even after I left, she stayed another month without a single update from staff. In the month and a half she was there, she received one bath. One. She repeatedly told staff that her incision didn't look right and that something felt like it was poking out of her left leg. She was ignored. We later learned that a screw in her leg is literally coming out -- and her incision sites are infected. On multiple occasions, I was on the phone with her when staff brought medications. She would ask if her pain medication was included, and they would say yes. She would check, and it wasn't. When she pointed it out, they would respond, "Oh, I need to see when you last had it." Shouldn't that be checked before handing out medications? My mother was repeatedly spoken to as if she were stupid. I had to remind them that she has worked in healthcare for over 30 years. Every time I filed a grievance, her treatment got slightly worse. Yet when I was physically present, staff tried to paint her as the problem. I know my mother -- she can be a handful -- but when multiple patients report the same issues, there is only one common denominator, and it is not her. I would not trust this facility to care for my sick dog, let alone my mother. I will never allow her to return here. read more

Very caring and loving staff. They treat you like family. PT and OT are very patient and caring people. They take their time with you if you are in pain. Very friendly and never get upset if you cannot do something the first or second time. I cannot begin to say how nice everyone is here. It is like a home away from home! read more

I am currently a patient at CareOne Wilmington, 62 year old male with Cancer in L3 (lower back) and receiving PT/OT post-op L1-L5 fusion. Basically getting my mobility and locomotion back for the return home and enter into radiation/chemotherapy. I also used this facility for pre-op, my L3 vertebrae was severely compromised by the Cancer and I needed help managing this situation. So I am reviewing this facility as a relatively young patient that needs short term rehabilitation. I can't say enough positive aspects, concerning my care, at CareOne Wilmington. The facility is clean, my nursing staff, including aids, are right on top of my needs and are very personable, I enjoy our interactions. The PT/OT staff are very professional and have helped me move forward quickly, in my journey to be self sufficient post-op. As far as pain management is concerned, I am very comfortable, the nursing staff makes sure my pain is under control and I have, not once, been in a position where I experienced out of control pain. I also hear how the staff treat the more senior patients, including the nursing home aspect at CareOne, and, from my perspective, the staff is attentive and very pleasant towards that demographic of patients. Like any review(s) posting, there will be individuals that just can't be satisfied or have unrealistic expectations that make it impossible to meet those expectations, this, of course, is relative to my humble opinion. I can only review CareOne based on my personal experience. Bottom line, I came here pre-op and, after receiving excellent care, used CareOne Wilmington post-op and I am very satisfied with my level of care at this facility. My advise, when looking at all of the CareOne reviews, is to ignore the extreme biasing and focus on the average reviews. In any case, I felt compelled to write a review, as a relatively young patient of CareOne, and simply state my experience and, so far, I have been taken care of quite well. One more aspect that I should mention, CareOne makes sure that I get to all of my appointments while I'm their care and I appreciate not having to worry and facilitate my transportation to and from Dr appointments while I'm the care of CareOne. I hope this review helps anyone trying to access CareOne and in making the decision concerning the care of oneself or a lived one. read more

My mother had a recent stay with CareOne Wilmington and my family couldn't be happier with her treatment and recovery. The staff was very engaging and helpful, the food was tasty and she was taken care of exceptionally well during her 3 week stay. We were very happy with the cleanliness of the facility and very much enjoyed the outdoor patio space while there. Mom is now home and recovering! Thankfully she learned some good habits while at CareOne that will help her to continue her rehab and us to facilitate her continued well being. read more

There are several issues with this FOR PROFIT facility. 1. As stated by other reviewers, there is no concern whatsoever for patients with diabetes. My dad spent most of the time wearing only socks and not his diabetic shoes. Occupational Therapy (OT) and Physical Therapy (PT) should have known better, but even after I told them, they still didn't bother working with him while he wore his shoes. 2. OT and PT didn't bother doing anything for the entire first week my dad was there. They only started after I informed them that one of the VA services my dad received would require a one week notification prior to his discharge. 3. My dad was at Care One recovering from congestive heart failure. At no point in his 22 day stay did they give him the meds the cardiologist prescribed to prevent another episode. The reason? Care One said it was because they were afraid his blood pressure was too low. My dad's PCP said the drugs in question were very expensive and a profit facility won't pay for it. The VA would have had covered his meds. 4. The meals served were high in sodium (e.g., pork chops) - completely inappropriate and potentially deadly for someone with congestive heart failure. 5. Upon discharge, Care One claimed they called the VA with all those new prescriptions they decided he should have. They didn't. There's no record of any such phone call. 6. Care One did call a random primary care physician (i.e., not his actual VA PCP) to get him an appointment post-discharge. 7. Care One was supposed to evaluated his physical functioning and send that report to the VA for additional services. They sent everything to a FOR PROFIT company instead, preventing him from receiving his OT and PT from the VA. No idea what that bill will come to. 8. Care One claimed his dry weight upon discharge was 191.5 pounds. It was 199 pounds and he was retaining water in his legs again. 9. His legs were very badly bruised with open sores upon discharge. That wasn't the case when he left the VA hospital. 10. The admissions director kept gaslighting me with about the admissions agreement. - She claimed that the health care proxy and power of attorney were no longer valid and had to be re-executed using Care One forms. WRONG. The law is quite specific. Those documents do not expire or otherwise go away. The only way your health care proxy and power of attorney are invalidated is if you sign new forms. - She claimed that if I signed, rather than my father, that I would not be financially responsible. Ya. That's not how that works. - The agreement requires that the person being admitted sign over all sources of income to Care One by the 10th of the month after admission. Heck no. - In the end we didn't come to an agreement about the admissions paperwork until the day before he was discharged and I had crossed out several sections. Curiously, there are several charges to Medicare that were made prior to the agreement being signed. read more

When I needed them to help with my mother when she was on hospice, I had to wait for 6 hrs. It was very disappointing.

I have read many reviews from folks with loved ones at CareOne Wilmington. My mother was there for nearly 7 years prior to her passing two weeks ago. She was a fall risk, agitated and belligerent when she went in to CareOne with dementia. Many other places would not take her. She could barely walk and initially refused to use a wheelchair. They worked with her and us. They got her on meds that calmed her (something she'd needed for years). When she was agitated they calmed her. They got her to a wheelchair, and had her on a "fall risk" alert. Everyone at Careone was professional and respectful at all times. My husband and I often popped in without warning and never found the place dirty, or staff lollygagging. They work hard for little pay. They do what we are unable to do for our loved ones. Is it perfect, no...There were issues at times with laundry or getting staff to put a bra on hero. They always reached out when things went missing. They looked for missing items. They laughed with me about my need for her to have a bra on. It became a running joke between us. When my mom took a turn for the worse, Miriam called to tell me. We were able to spend her last few days with her, and bring her grandkids and brother and my brother in. The staff brought in "care carts" with snacks and coffee and water for us throughout our visits. When she passed, every person on that floor had tears in their eyes. They must lose patients constantly, but they were sad for her and for us. If you had an issue with CareOne, it was not with the current staff. Rehab frequently called to start services for her for eating, sitting, etc. They often checked in to make sure was at her safest. Unless you are willing to do what they do on a daily basis, please give everyone the benefit of the doubt. They work hard in a difficult situation, made more so with Covid-19. We are forever grateful to the staff of CareONe Wilmington. read more

I am writing this review only to say that my Mother was a patient here for rehab post hospitalization. iI was told they had hairdressing services and scheduled an appointment and left a check with the facility for services after speaking with hairdresser Catherine Skerik (Facility being well aware of her status) Catherine called me on the day of my Mothers appointment to inform me that because of COVID restrictions and my mothers vaccine status she could not perform service. That admission was June 21/2021 ( personal check date) and was just cashed November 10/2022. how do you honestly cash a personal check for personal service 17 months later. This just posted to my personal checking account. ??? Really Care One?? Really Catherine?? do you respect all elders mental status this way?? very disappointed with the morals and ethics of all in general. shame on the facility and shame on the ethics of the hairdressor. Horrible experience with my dad! So disappointed in Care One. He fell three times within 48 hours. I was told he was using up all there resources! Hello, WE WERE PRIVATE PAYING FOR CARE ONE TO TAKE CARE OF HIM! We told them he needed his hearing aids on in order to communicate with him, they assured us they'd make sure they were. That was a joke, the very next day there he was sitting in front of the nurses station at 12:30 pm with NO HEARING AIDS on! We leave clothes the night before by the next morning they're GONE and no one can seem to solve the problem or keep it from happening. Had the unfortunate experience of having my grandfather at this facility. The staff from the director down to the janitorial staff should be ashamed of themselves. They lack the compassion and care required to appropriately tend to the patients in their care. I can't articulate the lack of professionalism this facility harbors. This is NOT a facility where I would choose to go for rehabilitation care ever again. While the staff was basically pleasant, the quality of care at this for-profit nursing home and rehab left much to be desired. When I arrived, my bed had a bottom sheet - period. I had to request a top sheet and blanket. There were no amenities, i.e., toothbrush, toothpaste, soap, glass or water container, etc. available. The food was acceptable, but seemed high in calorie and low in protein, vitamins or fiber. There was little or no concern given to patients with diabetes. PT and OT staff provided a minimum of instruction and attention patient improvement. For my last two days there, I received no OT, as I had covered 'everything', and PT told me to practice walking on my own. I am sure additional supervised exercises or encouragement would have been helpful! My strongest issue was the lack of cleanliness given to a bathroom shared by two people with digestive issues. Only once in the entire week that I was there did someone come in a spray and wash the sink and toilet! While the waste baskets were emptied and items dropped on the floor were swept each day, additional cleaning and sanitizing the bathroom should have been included for the healthy and safety of the patients. Sadly, this is not an 'old' facility. But the lack of basic equipment in the PT room, or the need to 'search' for an appropriate wheel chair or a functioning rollator for a patient, seems very short-sighted when the focus here is to care for patients trying to regain their mobility and independence, etc. read more

If you know care one, you would know that employees were told to write positive reviews or risk loosing their jobs if the place shuts down. I caught the staff trashing and laughing about the residences. it is dirty, aids sleep all night and don't answer call bells. they know when the state is coming in or a new family is visiting and clean the place up. rehab looks great but long turn is smelly and dirty. rehab has more staff for show. they only make money with short term so they send away long term residents whenever they can. read more

My family and I found it very hard to find a good nursing and rehabilitation center with care that is home-like and has a low nurse-to-patient staffing ratio. We found this at Wilmington Health Care Center. My father went to Wilmington Health Care Center after leaving the hospital. Due to his illness, he was extremely agitated and displayed unsafe behavior (fall risk). His room was located in the Harnden short term rehabilitation unit. Ashley is the unit manager. She is wonderful! I can't say enough good things about her. Every member of the staff, especially Eashu and Felix, was professional, patient, caring, and kind. They provided my father with great care and compassion. My father's doctors, nurse practitioner Christine, nurses, CNAs, physical and occupational therapists, and unit manager provided me with prompt notifications if there were any changes with his physical or mental health. We knew it would be a matter of time before my father's passing, but the care he was provided was outstanding. They did everything they could for my father with kindness and support for my family. My family and I felt very confident and assured leaving my father in their hands. We knew he was safe, well taken care of and looked after. They treated us like family. I can only speak highly of the care and support he received while he was there. read more

After spending 3 weeks at Care One Wilmington for rehabilitation following a fractured pelvis and cracked ribs I can't say enough good about this facility. I have been treated in other Rehabs following 2 separate knee replacements and my experience at Care One was superior to the others. I am an early riser and Every morning the cleaning staff was working the halls and dining areas . During the evening the nurses were very quick in responding to needs by myself and my room mate and we were never subjected to a long wait time. I had surgery. They didn't have any pain medicine the first night I was there. The second night I had the pain medicine. The third night I didn't have any pain medicine because it wasn't there. There is no excuse for not having pain medicine when you just had surgery. The food was horrible. I couldn't eat it many times. They lied about the pain medicine coming in plus they said they have extra in a vault which they never opened. They told me the PT would be there the next day and no one came. The aids were friendly. My mother has been at Careone since Jan of 2015. She has dementia and was placed there longterm after brain surgery in August. Between Aug and Jan mom was in and out of the hospital and 4 different rehabs. CareOne is clean. The staff are caring. PreCovid they had weekly music hours (people coming in and entertaining the residents). Friday cookouts on their gorgeous patio. While I have had issues regarding mom not always wearing a bra or clothing going missing, staff was always caring and responsive to us and her needs. My mom can be difficult, harsh and verbally abusive when she is distressed. They deal with her with love and humor. I have had some staff be less ideal, but that is everywhere. In general the staff work hard to care for OUR family members and I appreciate them. Would YOU want to spend your days doing this job? They make it so I know my mom is safe and cared for and I don't have to worry! read more

Their care has been well managed. Both individuals have varying degrees of Alzheimer's. The staff is well trained and very caring. I am there on a continuous basis and have found nothing to complain about. In my year of employment there, I have witnessed more lazy,incompetent, poorly dressed, and unprofessional CNAs and nursing staff than ever in my life and I've been employed in healthcare all of my life. in this case I was not part of the nursing staff during my time there. The apathy, the non involvement, the unprofessionalism, and the lack of supervision, has left me determined to never send my family members there for anything.CNAS always using I-phones while on duty, dressed in sloppy street clothes and the lack of caring, astounded me. Care one of Wilmington is one of the best facilities that we have had experience with, and we have been to several other places over the years. My mother first went for rehab, but transitioned into long term care and now lives there. The staff on her unit are absolutely wonderful. they are good to my mom, and are very caring. the place is clean and looks nice. if we have any issues, we bring it up to the staff and the matter is dealt with. I am in total disagreement of previous bad reviews. My sister spent 3 weeks here for rehabilitation. She was afraid it would be another horrific experience like she had in Lexington. I have visited every day and every single staff member is pleasant and helpful. The nurse and CNA are always right outside the room and when you call for help they are there in a flash! Special shout out to Victor and Rony. The OT and PT are wonderful, you can tell that they really love their job and sincerely want to see you improve. All of them went out of their way to accommodate my sister's various handicaps (she has MS). The activities director always has fun things everyone can participate in. The food is very good (pleasant surprise) and every Friday they have a BBQ out on the beautiful large outdoor courtyard (weather permitting). The facility is super clean, the staff couldn't be better, I highly recommend this place for short term Rehab. read more

My father was in a car accident and from the hospital he was sent to Care One in Wilmington. This place is FANTASTIC! He was on the rehab unit and all of the staff were very caring and professional in every way. The Rehab department was excellent! He was making progress every single day. My Dad liked the food and never complained about anything. They do a meeting with new patients and even the administrator is there to welcome you and see if you have any concerns. Dad is home now and doing well with the services that Care One set up for home rehab. I would highly recommend anyone to go to Care One. read more

This nursing home is now called CareOne at Wilmington. I went here looking for a new resthome for my sister closer to me. Josh Cambridge, who has since been fired asked me if I was doing this behind her current nursing homes back. How unprofessional! Since when is it against the law to look for a better place for your family member at your family members request? Amanda Boutwell who is director of admissions refuses to even return a phone call. Amanda Boutwell should give people the respect they deserve even if she doesn't think her place is a good fit. Amanda is heartless. Don't let the overhaul of this place fool you. Terrible place to leave anyone. A friend was sent here for rehabilitation by his PCP (who is affiliated with this facility) he was treated like a person with dementia. Forced to sit in a wheelchair at the nurse station not allowed to go back to his room. Pushed into the recreation room for games. Rehab time was approx 1 hour/ day 23 hours sitting around. At discharge I pushed my friend out in the wheelchair from the nurse station no one assisted. I wanted to leave the wheelchair at the curb. Very upsetting. I would give NO stars if I could. What happened to my previous review??? I would hate to have people not find out the truth about this place! My aunt was there in the dementia unit. She had mild dementia, but they labeled her advanced dementia, then proceeded to convince the family they had no choice but to leave her (and her money) with them. While she was there, I never saw anybody paying attention to her. She never had assistance with dental issues, nobody assisted her to brush her teeth, nor did she have any assistance with any other personal issues. When I tried to take her home, the case worker insinuated she knew about my "criminal history" and that because of it, she could not be discharged to my care. I asked what she was talking about . She said "YOU know". I asked them to produce proof of this "criminal history". When they couldn't they finally had to admit they had no reason not to release her to my care, it had to be by their terms. I had to fire all the help I had arranged for her care, because they made it a criteria THEY had to do my hiring because they had to make sure she got "good care". Also, they required 15 days to arrange this care for me. The help they arranged cost more and were less competent then the people I had hired. When we did get her out, she had sufficient competence to attend to a myriad of legal issues. One day I received registered mail from them. The postman who delivered it was horrified and asked me about it. Then he had his say. His father had spent time there, and his story was very much like mine. They had over-diagnosed the father, told the family he was dying and would never get out of bed again. And made them feel bad about not paying them all his money. The family got him out, and after two months, the man was walking again and on his way to recovery. One day I asked a neighbor to give me a lift to Wilmington Health Center. At first he said no, he didn't want to go near the place. Then he did, but shuddered all the way. He let me know he had had a horrific experience with them also, but refused to say what it was. My parents were failing at home and very unsafe alone. My father ended up in the hospital and had several falls and my mother was developing early dementia. My family knew that my parents were no longer safe at home. As an employee of Care One at Wilmington, I wouldn't want my parents to go anywhere else. I only trusted the staff at Care One to care for my parents because I know what quality care they give and how they truly care about their residents. They treated my parents as if they were their parents. My dad has since passed but he passed with dignity and love from a staff with a heart of gold. My mom has flourished at Care One. Everyone calls her "Mama C". My mom always loved music and singing and loves the entertainers that come weekly. Nothing makes me happier than seeing her sing and dance to the music. The staff have taken her under their wings and when I go home at night, I know she is safe and well taken care of. You can trust in the care provided at Care One Wilmington. I not just saying this because I am a staff member but because I have witnessed such compassion and care given to all that come here, otherwise I would not work there and I would not have brought my parents there. I would highly recommend Care One @ Wilmington to anyone who needs long or short term care. read more

This place has downhill quickly! They renovated to make the first floor look beautiful which it does but the second floor is very old and the lighting is poor. The staffing ratios are terrible. You hear the staff complaining all the time they don't have time to take care of all the patients on their assignment. There is a lot of turnover. My mother entered the Wilmington Health Care in good health for therapy to strengthen her legs. The first few days were good, but then we saw the unprofessional side of them. It took a week to label her clothes which meant they did not do the laundry until she almost ran out of clean clothes. The food was fair to poor and they did not document when they took an uneaten tray away (which was frequently. We complained numerous times that she was not eating, or acting right, and that she had a bad cough. We were told, she had a cold, and that new people have to adjust. When we saw her getting worse, we called an outside doctor who had tests done on a Tuesday. The tests showed she had pneumonia, was dehydrated, urinary tract infection, and was bleeding internally. She was sent to the hospital and died the next day. She was only in Wilmington Health Care for less than a month. When we went to pick up her belongings the staff did not acknowledge her death and seemed not to care. Her belongings were packed in garbage bags stuffed in an old box. This was so hurtful to see her stuff handled like trash. Except for one or two staff people, I have no respect for any of the other staff and would NOT recommend this facility to anyone based on our experience. read more
