When leaving they said it was a $15 co-pay and $75 for an infraction. Was under impression it was totally covered by my healthcare. Could not review paperwork given to me due to my eyes had been dilated. Once home I called my insurance company and was told EVERYTHING was covered as this was a regular eye exam. The Eye Associated had coded it as a Medical eye exam. Days later when I checked their appointment person they default all appointments to Medical! To make matters worse, when I looked at their future appointment and $15 and $75 receipt, they applied everything to another patient with the same last name as mine!
