The therapy room fails to provide proper HIPPA protection. Exam/therapy tables are around the outer perimeter of the room. With different therapy equipment in the center of the room. Wide open visually and also it wasn't difficult to hear conversations regarding other patients. The doors to the room are wide open for fire code and such needs, I could record all I want from the hallway looking into the therapy room - the next thing I was told completely violated my patient rights and civil rights. When Emily Miller, OT, and Marie Schaff PT came out to ask me in. I asked if I can have my girlfriend record me getting the wheelchair and mobility evaluation done. I was told we can't do that due to HIPPA. No reasonable level of HIPPA is provided at RHI. I they have patients walking around for therapy in public spaces, the OT and PT room is one massively open space and not separate therapy rooms with sound and visual HIPPA protection. My simple request to record my activity should had been allowed. But RHI rather violates my rights by pretending HIPPA is the concern. 11 AM and 12 PM 02-16-23 This is what I experienced at: 4141 Shore Dr Indianapolis, Indiana 46254 Seen by: Emily Miller, OT and Marie Schaff, PT will be completing the wheelchair evaluation. & Cory R Moore, ATP - Assistive Technology Professional 7802 Moller Rd Indianapolis, Indiana 46268 Cory R Moore, ATP was present observing and took every measurement of my TiLite ZRA Series 1: (Also it was noted that they had my Serial # and all details of my wheelchair when originally prescribed.) Components: 1 Solid seat pan. 2 Rear drive wheels = Spinergy SPOX 24 inch. w/ solid real feel rubber tires. 1 TiLite ZRA Series 1 = 1 Titanium custom frame. 1 Open loop Titanium footrest. 2 Mono leg forks. 2 Soft roll casters. 2 Ergonomic Handrims I need a new TiLite ZRA Series 2 - the two down stems that are not made or sold any longer are loose and failed beyond repair. The forks connect here. Wheelchair prescribed in 2008 = 15 years old. Measurements: Seat position = from edge of solid seat pan - rear edge of seat pan. No measurements with me sitting on my cushion had been made. Frame leg angle: 80 degrees. Center of Gravity Footrest position Seat back position I was asked: How many hours do I spend in my TiLite ZRA Series 1: Over twelve hours? Does the use of the TiLite ZRA Series 1 allow you to: Get to the toilet? Yes. Allow you to bathe/shower? Yes Allow me to get to the kitchen. Yes Allow you to make meals? Yes Allow you to partake in helping with household chores? Yes as best I can. Allow me to be independent as much as possible. Yes So without the use of the TiLite ZRA Series 1 you would not be able to function? Correct. Any other wheelchair will not work for my medical needs. Only 2 parts of the wheelchair and mobility evaluation: Strength and range of motion were performed. On 12-21-22 Medicare had me get a full wheelchair and mobility evaluation in my home per the Medicare Wheelchair Replacement Policy. I was told that I only qualify for a STANDARD WHEELCHAIR the RHI employees & Cory R. Moore ATP. I made sure to ask both Emily Miller, OT, and Marie Schaff, PT they being RHI, not Numotion employees. I expected them to respond honestly. Can I try a manual wheelchair here to show how it affects me? Nothing was said. I did notice both the OT and PT look at Cory for his reaction. Cory stated no need, medicare won't cover a replacement since they didn't cover this one. How about an in-home evaluation to make sure I can even use a Standard Wheelchair in and around my home to complete daily living tasks? Same thing, both looked in fear at Cory for his reaction. We don't provide loaner wheelchairs. Cory R. Moore ATP stated after I asked: Yes, you will get the same medical benefits you got from your TiLite ZRA Series 1 as you will now from a Standard Wheelchair. When I mentioned asking The Joint Commission to look into my medical safety. Cory explodes into an uncontrolled rage, with pure hate on his face, and a look of nothing to lose: NO YOU WILL NOT THREATEN THE JOBS OF THESE TWO RHI EMPLOYEES! IF YOU GO DOWN THAT PATH I WILL SEE TO IT NUMOTION OR ANYONE ELSE WILL NOT HELP YOU! YOU ARE TRYING TO COERCE US INTO GETTING THE PROPER EQUIPMENT YOU WANT! DO YOU WANT ME TO WRITE IN YOUR CHART HOW YOU'RE COERCING US!?! This is when I left the therapy room to get the information of the Director/CEO of RHI.

    If you need Physical Therapy, look no further because Community PT Carmel is a great place to go…read more I have been to a lot of Physical Therapy practices over the years with my knees, shoulder, hip, and feet, as well as with my teens and mother-in-law. Of course the PTs are all competent and the facilities all have the equipment you need (wooden steps, bikes, tables, straps, etc.) but you have to find a PT with the personality that works with you and an environment that is comfortable. I have seen my share of mediocre experiences -- large chaotic rooms full of dozens of PTs & dozens of patients, or a room of PTs talking about their weekend more that focusing on their patients, or front desk personnel that give you a list of appts. only to find out later that each appt. is with a different PT (does NOT work well)! I or my relatives have been to PT at St. V., Methodist Sports Ctr., ATI, Body One, IU, Carmel Rehab & more. Community PT Carmel is my favorite for many reasons: it has a convenient location, easy parking, walk in building and it is right there (car to front desk = 1 min. max.), little wait, good & helpful front desk personnel, and just 3 PTs and therefore no more than 6 in the room so it is easy to focus and get attentive care. I have had sessions with two of their PTs for knee and feet issues: Amy Guillaume and Brett McDonald, and both are very good at what they do, as well as being just nice and pleasant people who communicate well and care. It is a good, calm, effective experience from the beginning to the end -- Community PT Carmel!

They didn't reach out to the patients daughter when she passed.. she had to hear from a third party. And when the patient had had a bad fall prior to passing the person incharge named Jason insisted on not calling the fire department and handeling it himself which was a mistake and she couldn't get up for over 30 min and ended up getting help anyway from the firefighters. The food sucks, the patient called it a hell hole it was so bad. The laundry always went missing, the nurses take up to an hour to come when you hit the help button even for emergency!!! they are insistence we get their belongings out of her room so they can make more profit for a new patient to come in. She hated every second in this place and I'll do my best to make sure people don't go here. If you care at all for your loved ones don't take them here!!! And rest in peace to my friend always, you didn't deserve this horrid place, the food, the treatment, the rudeness. They were truly horrible To her in her final days. And now that she has passed they are still being horrible and rushing us packing up her stuff. Called that we have 2 days to do so... she just passed. :( My only positive note is that Winnie was truly special to this patient. She was caring, has patience, was kind and always a glimmer of hope for her. The true bright light and glue of this facility.

    A recent cut in staff has left patients in poor care. A care plan should have been held within 72…read morehours, but as of day 11 my loved one has not been recognized as important to management. We are transferring out. The CNA's simply cannot keep up with only one on a wing and must leave some in neglect. It is not the CNA's fault! Management has simply made the decision based on monetary concerns, not in the best interest of their patients. The food is terrible! Who would like a baked potato with nothing? No condiments are offered and the water situation is a critical element. Without water patients die. My loved one was left by the night shift in a soaked diaper, just a gown covering one shoulder with barely a sheet over his private area and no ice water at 8:30 a.m. Some days water didn't arrive until 10:30 to noon! The state should in my opinion shut this facility down.

As my friend who lives there says, "It is all glitz for the children of "prospective inmates" and state inspections, and when no one is looking (which is most of the time) the patients seem to be treated poorly, to say the least. Do not blame the staff, though, this is a management problem and probably indicative of what is happening with lots of extended care facilities these days. Oh, and about that chicken back, the person who actually saw it, an individual who spent most of her career in the food service business, said that she peeled the skin off just to be sure there was no meat off, but she already knew there would be very, very little. if any. "Chicken backs are not served as entrees," she said. "They are for soup or to flavor gravy."
