My mother has been at this location for 6 week and the lack of basic care has been so subpar. My mother's safety has been at risk since day one (See Pictures). The staff sees nothing wrong with how she is on the wheelchairs in the pictures. Staff that doesn't know how to use the equipment properly, she goes the whole day without being checked. When pushing her button for help it goes hours with out anyone coming to see what she needs. She was there for 5 weeks and was never showered once. Simple things like brushing her teeth everyday can not even be done regularly. The final straw was when my mom was having a bad day and was nonresponsive when trying to wake her up. The young lady giving her pills decided since she could not wake her up she would take the pills and shove them in her mouth. Then after shoving them in her mouth tried to give her water and because she could not get any water in her mouth decided to leave the water and walk out of them room. Leaving the pills in her mouth for the potential to choke. This facility only has one care and that is the money that is being paid by the families. Your loved ones best interest is not their priority. The worst part is the facility puts the blame on the family and does not take accountability for their staff and their lack of care and training. I had several meetings with the directors and nothing got better.

BUT, after this year , I must say that your strengths are far beyond comparison. Here are several things that are an example of your stellar service. Mom enjoyed your pool for many months, telling visitors not to come at certain times as she could NOT miss her Aquatics class. (Not a single facility that I toured had a pool except you) Both PT, OT and the other support services have been so helpful in coming forward with suggestion of products and things that will help Mom, before I even knew she needed them. When Mom was beginning to show signs of decreased verbal skills, staff called, recommended and enrolled her in Speech Therapy Caregivers of all shifts are so willing to share things (both the good and the difficult) about Mom's habits, quirks and routines. It says a lot about their care and interaction with Mom. It is a comfort to myself and our family to know that they are aware of all this and share it as well. It speaks volumes of your caring staff. They know what movies she watches, what books she is reading, where and what she does on Wednesday nights (and ask about it), amazing! The Nurse has been spectacular. Prompt at addressing all of my many questions and concerns, she literally jumped out of her chair once and said " Let's check on that". She follows thru with so many little things and has many suggestions to help Mom and myself, as Mom continues to change and decline. The personal touch on a weekend I was out of town: She left me a phone message saying she was working the weekend, and was watching and assessing Mom. Just give a call if I needed anything or had questions. It is difficult for both the individual and the family to travel this path. All these examples of the personalized care that your staff provides Mom and in extension our family is priceless. Thank you to ALL of your staff. You should be proud of the service and care you provide. Our family couldn't have found a better home for Mom. Thank you

    Our family's experience with Brookdale Brookfield Woelfel Road over the course of nearly four…read moremonths (summer-fall 2025) was mixed. While there were some positive aspects, significant concerns about staffing, communication, and quality of care ultimately led us to move our mother-in-law to another facility. On the positive side, the food was very good. Residents were offered choices and could eat in a restaurant-style setting, which she genuinely enjoyed. Additionally, some staff members were exceptionally kind and went above and beyond in their care. However, there were consistent issues that impacted her well-being. Planned activities were frequently canceled due to staffing shortages. Because she is not mobile and requires assistance to attend activities, she often depended on staff to inform and help her participate. Without regular family advocacy, she was not consistently included. About a month into her stay, she was moved from a shared dining table with other residents to sitting alone--this occurred on her birthday. Given that she was adjusting to a major life transition and grieving the recent loss of her husband, this change was especially difficult. During her stay she was not consistently being taken to meals and lost a lot of weight in a short amount of time. We also observed concerns with staff presence and responsiveness. At times, staff appeared disengaged, sitting on phones or eating in common areas, making it unclear whether they were on break. Staffing levels seemed particularly limited during evenings and weekends. More concerning were issues related to personal care. During her stay, she lost 10 pounds and experienced multiple UTIs. There were several occasions when my husband arrived in the morning to find her still in the previous day's clothes, in wet incontinence garments, and with soiled bedding. While staff sometimes stated she had "refused" care, we were not confident that consistent or sufficient efforts were made. The call button system was also problematic. It required residents to hold the button down for an extended period before activation, and response times were often delayed. Some staff members were unfamiliar with how to properly reset the system so my husband had to show them how to use it. Although we requested the simple old style call button, we were told none were available, despite observing newer residents being given the old style buttons. We also encountered issues with missing personal belongings. A gold chain was replaced with a non-gold one, and a new TV remote went missing. Additionally, our mother-in-law reported that a staff member asked her to write them a personal check, which we immediately reported. Another resident nearby also reported missing cash. As her condition declined due to UTIs, she required the use of a Sara Stedy transfer device. We had to purchase this equipment ourselves and train staff on how to use it, as they had not received prior training. Eventually, we were advised to move her to memory care for more support, but instead we chose to relocate her to another facility. Since moving, her condition has improved significantly. She has regained strength, is walking short distances, maintaining her weight, bathing regularly, and is consistently dressed and cared for. While there were some compassionate individuals on staff at Brookdale, our overall experience did not meet expectations for the level of care being provided, especially since we were paying for the highest level of care. In speaking with long-term residents, we also heard concerns that the quality of care has declined over time.

I chose Brookdale Senior Living/Capital Drive - Brookfield for his long term assisted living care. I was impressed with the intimacy of the facility because they only has 24 residents and they specialize in Dementia Care. Their staff is caring, attentive and they get to know each persons individual needs. They know my voice when I call and they are respectful to both his needs and mine as his caregiver - Are they perfect NO - Do they make every effort to take care of any situation and communicate - YES - If I had to do it all over again I would still pick Brookdale - Their Staff is the BEST

    My father was in rehab here and it was a horrible experience. The people that work there don't…read morereally care about the residents. My father was supposed to be transported to dialysis two times a week and they messed up not one but two Saturdays in a row not sending him with the lift he needed to get into the chair for dialysis. The dialysis place was only 10 minutes away From Newcastle, so you think they would go back and get what they needed so he could get the medical care that was required. But instead, they let him sit there for hours until they could swing back and bring him back to their facility without having had dialysis. The fact that this happened a second Saturday in a row was completely unacceptable. Not only were we not able to get a baseline for two days a week dialysis, my father needed to step up and be on three times a week dialysis now. Thank goodness, we found a place in Oak Creek that actually cares about its residents. They are personable and treat residents there like people. The beds are thin and so uncomfortable that my father slept in a chair most nights. I would not recommend this place at all.

And the residents always seem so happy and were so welcoming, it felt like a big family! There were so many things for her to do, from exercise classes to happy hours and socializing! They took such great care of her during her stay that she didn't want to leave!
