These are all updated reviews since I live here and have had so many complaints come to the attention of Obusman and APS after six months of physical,mental,and financial abuse .I rarely get care ,called racist slurs ,dropped ,leg crushed in hoyer ,head hit against wall during positioning and there is one nurse for the whole large facility who can seldom be seen .Have seen the executive manager twice during these six months and once was when passing in hall Management never comes to see when called about a problem..I haven't been allowed in any activitys or even gotten up for bathroom.These are fake reviews and hope I screen shot the real negative ones .The staff is very unprofessional except Anna and Carlos two older caregivers who are very compassionate,caring and punctual .The reviews updated in April due to a meeting regarding their negligence.Hope things change since being able to finally have this meeting .I wish I was never placed here and may post the real reviews if found .Tells alot that they felt it necessary to update reviews after my complaints and grievances .If you believe these reviews you will be very disappointed.Food here often not nutritional.for health so have to buy my own or many days do not eat.I an kept in bed the majority of time and have waited up to four hours for medicines.Ask yourself how likely is it that all reviews are now positive I am optimistic about change after today's meeting about my care .Never given an itemized statement of what I am paying for Need to mention another young medic and care provider named McKenzie and did not mention because she is good but generally too busy to help bu tshe will welcome residents with care ,love and compassion read more

My family and I had a mixed experience during my mother's 2.5-year stay at Farmington Square Beaverton Assisted Living. The first two years was in their transitional memory care unit, then .5 year in the full memory care unit. Here's a breakdown of our experience. I always like to start with the good: Care Staff: The care staff at Farmington Square were generally good. They showed genuine care and concern for my mother's well-being. However, there were some employees who appeared to be primarily motivated by their paycheck, which was a bit concerning. I would like to point out some staff members who I believe went above and beyond: Cory, Megan, Isabel, Michelle, Brianna, and the van driver Rob. Through their actions, they all reinforced my feeling that my mom was being well taken care of. Food: One standout aspect of Farmington Square was the quality of the food. My mother consistently enjoyed the meals, and they were well-prepared with a variety of options and good portions. She was given snacks throughout the day, and someone was always ready to provide her with a cup of coffee. Billing and Communication: Unfortunately, our experience with billing and communication from day one left much to be desired. I stopped my direct withdrawal within a month of starting because they were withdrawing the wrong amount. Billing issues were particularly problematic. I find it interesting that anytime there was an "honest mistake," it always favored the facility and not the resident. We were charged room rent for 15 days after my mother's passing. While this may be legal, it is certainly unconscionable and gives you a sense of their moral compass. There were multiple instances of poor communication between the staff and our family, which led to frustration and confusion. This was both unexpected and disappointing. Management: Our interactions with the management team were less than satisfactory. They seemed disorganized and unresponsive when it came to addressing our concerns or resolving billing issues. This lack of effective management was a significant source of frustration during our time at Farmington Square. In summary, Farmington Square Assisted Living provided decent care and excellent food for my mother. However, the issues with billing, communication, and management were significant drawbacks to our overall experience. If these areas were improved, it could be a much better place for residents and their families. I'm not sure if I could recommend Farmington Square Beaverton to anyone who is private-pay; it seems to me that Farmington Square's number one goal is to separate you from as much of your money as possible. read more

I would not recommended sending your family or ANYONE to live here at Farmington Square Beaverton. I used to work here as a caregiver and the type of care/ quality of care is not what I would wish upon anyone. Residents ending up with "random" bruises and how convenient the camera in their bedroom was unplugged! People that work here are just money motivated they are not looking out for your loved ones please look into where you send your family if you care about their wellbeing!! read more

We moved our mother to Farmington Square after a terrible experience at another memory care facility. The difference was like night and day. Mom was at Farmington Square for about a year before she passed on and we feel the care she was given was excellent from the day she arrived to the day she passed. All of the staff here deserve a big thank you for the way they handle their difficult jobs. The love and care was especially evident in the last few days of her life. GF's Mom (70+, after surgery) stayed there for a lil over a week... HORRIBLE nursing staff... They kept getting her meds wrong, and attitude towards her was along the lines of "and what can you do about it?!"... When our concerns were expressed to management the staff tried to shield itself saying that THEY found out the error on the medicine that she was going to ake and took it out of her cup.p Granted, old people can be difficult at times (have taken care of two of my aging grandparents thru their final days), but to treat older folks like this is just WRONG. So much for paying out of pocket over $1k a week for grandma to b stressed about gettting her daily meds right. I REALLY hope all the other people that r there all "comatose" on theirnchairs is just because the r not feeling well, than for the staff giving them whatever medine whenever they feel like it... Due to the POOR and HORRIBLE attention to grandma, we are taking her out of this dump onto a better facility. Save ur loved one a lot of stress and find a better place than this "skilled facility"... What a joke of place... Sad to find this out first-hand... Shame on you Farmington Square (Beaverton)!!! read more

Farmington's highly skilled and compassionate caregivers and staff provided outstanding care for my mom until the end of her life. She came to live at Farmington after a difficult period. She had been in the hospital and then a rehabilitation center after suffering a hip fracture at a retirement community's memory unit in Portland's West Hills. That community looked like a high end hotel and was considered the best in the Portland area. It was expensive but I assumed it meant my mom would be receiving high quality care and that was extremely important to me. I quickly learned that a beautiful facility and high fees do not translate to good care. The staff were pleasant but did not seem connected to the residents and took care of their needs in a perfunctory manner. They were also unskilled when it came to working with memory impaired individuals. When I called to check in, they seemed to have no idea how my mom was doing and my mom was rarely bathed. This concerned me and so I ended up paying extra for personal caregivers several times a week. It was at this facility that my mom was intentionally pushed over by an 18 year-old unskilled caregiver and suffered a extremely painful and debilitating hip fracture. After rehabilitation, my mom arrived at Farmington Square. She was crying and clinging on to me as this was her third transition within a span of 2 months. I was not able to accompany her inside as Farmington was abiding by strict COVID protocols. The staff there went out of their way to help my mom feel at ease and helped her get to know other residents. They also reassured me. Whenever I called to check in on her, they clearly seemed to know and care about her and provided detailed updates. When I spoke with her on the phone, she seemed happy. Once limited visits were allowed, they were very accommodating. In June of 2021, my mom's health declined and she was placed in hospice. Initially, I was going to bring her to my home, but I realized her needs exceeded my capabilities. I felt like I was letting her down but after spending time at Farmington with her, I quickly realized that she had wonderful relationships with the residents and caregivers and it was home to her. My sisters and I stayed with my mom 24/7 during the last weeks of her life and we were able to observe the consistent high quality care that my mom as well as all the other residents received. These outstanding caregivers were all emotionally mature individuals who were naturally compassionate people. They were attuned to my mom's needs and she was receptive to their caregiving because they were well-trained and had developed a trusting relationship with her. The staff and administrators even took care of me and my sisters. They brought in trays with coffee, ice water, and snacks and sat beside us as we struggled to say goodbye to our mom. Through these experiences I have seen the very worst and the very best of care for a loved one. Farmington exemplifies the very best by focusing their efforts on hiring extraordinary caregivers, which meant that my mom was always clean and comfortable and experienced the joy of meaningful relationships and feeling loved at the end of her life. read more

My father was at the Memory Care unit at Farmington Square for the last 7 months of his life. The staff were all wonderful, caring people who took excellent care of Dad. They not only took great care of Dad, but they were also wonderfully caring and supportive of my mother. We'd had Dad in another facility that looked fancier, but the staffing ratio was terrible and we got him out. We were so happy to find the loving care at Farmington Square! read more

My 82 year father was at Farmington Square Beaverton for almost 3 years and we never had a problem with the staff there. They were nothing but awesome. After my father had a stroke, he was diagnosed with Lewy Body dementia. He was having very aggressive behavior and was denied by every memory care facility that we had tried to get him into. On the day before he was to be released from the rehab facility he was in, after his stroke, we contacted Farmington Square. They came out to evaluate him and said they would take him in. From that point on, my father continued to improve. They were instrumental in helping even out his meds and make sure he was as comfortable as possible. He eventually began to call Farmington Square home. We all were very happy with the care he received there. They were kind and caring to all of the residents, not just my father. When my father was in the last stages of his life, to no fault of Farmington Square, all of the staff, even the administration, were very kind and understanding. Making sure that my father, as well as the rest of my family, were comfortable. They even invited my family to join in their staff Thanksgiving dinner. I would recommend Farmington Square Beaverton to anyone. Thanks to Teddi, Rachael, Jen, Carlos, Nancy, Steven, Alex and to the rest of the staff at Farmington Square Beaverton. When my mom needed to move from independent living to assisted living (with a path to memory care), my wife and I spent a LOT of time looking at local options. We visited many facilities and got a backstage view into the entire spectrum -- from pretty horrible and depressing to pretty wonderful and uplifting, and everything in between, each with a different set of care options and costs and insurance eligibility. After much deliberation, we picked Farmington Square because the staff seemed so incredibly compassionate, caring and skilled. It was the right decision and, even though mom's health took a turn for the worst almost immediately and her stay there was, sadly, relatively short, we couldn't be happier with the support and kindness we received. The facility is nice enough, the care options ranging from assisted to full memory care and the costs are right in line with other local providers, but the people who work there -- at every level, from the nursing staff to the kitchen staff -- are truly wonderful. They went out of their way to welcome mom (and us) into the community and provided incredible and comprehensive care as she worsened. We were also struck by the long-tenured careers of some of the key staff, some of whom have been there for close to 20 years. They made a sad end more bearable and we give them the highest recommendation. read more

Farmington square is a wonderful community whether you are looking for a place for your loved one, or looking for an employer. While it is part of a ever growing and expanding group of communities, it is still a family owned and oriented business. The owners of the community come through frequently and are always working on improving and updating not only the grounds but providing new technology and tools for the care team to provide the BEST care for their residents. You can tell they truly care about what they are doing, and their mission. I know of families that have returned multiple times and have had two, three or even four of their loved ones reside within the community over the years, simply because of the great relationships they had formed with the care team, and the quality of care they received. There are many people that continue to come for events at the community years after their loved one have passed on because they are a part of the family now. I recommend to come in for a tour if you are looking for a place for your loved one and you will see why they are the best choice in senior living. read more
