So when I went in, the office was closed for lunch but the woman teller was closing the door and was kind enough to go get a form for me and explain how to submit it. I didn't realize it was closed for lunch (1pm-1:30pm) but she still helped me, was very pleasant, and she was 15 minutes into her lunch! Thanks to all of the tellers at this location!
