Turns out why they would want to discharge an elderly because Medicare had stipulations on weight statistics on patients. They LIED and also stated the patient gained 2 pounds which is absolutely should be criminal. This place definitely needs to be investigated!!!! The facility is nice looking. BUT.... If your elderly family member goes in as a patient here please keep a close eye on them. The meals they serve to them are COLD (Approximately 80% of them). The food is horrible. If the patient calls for care, they respond LATE or forget. My family member sat in their own urine for over an hour (this happened twice with me there). My family member lost over 50 lbs in just one month and LIED about it when they were discharged. They stopped patients pain meds for a short time even though patients doctor prescribed it. They denied patient VITAMINS even though patient stated they were weak feeling and requested them. They did NOT keep patient hygiene up to par. They lied and stated patient denied care. They deliberately made patient miss doctor appointments. Patient asked multiple times to speak to a doctor onsite but was DENIED. Seems like this place is run by nurses only. They manipulated Medicare and discharge too soon even though patient also had insurance and need further care. I would NOT recommend this place to anyone.

Greatest lesson learned for our family is to never ever prioritize the convenience of being physically near your loved one over quality of care. We faced multiple challenges, but here are the top cons: 1. Bedsore/Wound Care Issues & Lack of Care: Despite entering the facility with clear skin and no open wounds other than a surgery scar, my father developed bad bedsores (puss-filled, hole in flesh) which is simply caused by neglecting and delaying the repositioning of patients and not attending to their basic hygiene needs in a timely manner. The family reported the initial bedsore, not the staff, and the wound care physician didn't see him for nearly two weeks after. Dad needed to have a surgical procedure by then. 2. Lack of Medication Management: Meds have been administered multiple times without nurses performing the essential blood pressure check first, which could cause a major medical crisis. He was also given an increased number of pills at one time, which was greater than the max he should have received. After multiple inquiries, we still don't know any details. My father decided he did not want to ask anymore because "at least he did not die." 3. Severe Gap in Overall Medical Wellness Coordination: We didn't see the different disciplines working together to provide holistic health care. For example, he was being moved in a hurtful manner, etc. by nurses and aides who told us they were not aware that he even had a hip replacement! He was a heart patient and had bedsores, but there was no attention to diet, which is key to healing. There were multiple instances of one team member reporting an issue that was then not addressed by the nursing team. We waited about a week to have an issue with stool be addressed, which also caused major concern for infection with the bedsores. This is detrimental, especially for the elderly. Despite reporting his worsening condition, including pain and diminished mobility, there was a notable absence of timely medical intervention -- the attending or servicing physician did not see him for two weeks even though we were being told that he had been seen. Finally, we did not have an official, informative "plan of care" meeting until 15 days after admission. When my dad became very ill a few days ago, after waiting for some time, we decided to request that he be taken by ambulance to ER vs. chancing waiting even longer on a response from staff given their history. He was admitted to the hospital that day. 4. Consistency/Quality of Care Gaps: Having temp agency workers in and out resulted in a compromise in the consistency and quality of care provided. Patients and families should not have to tell staff what's going on and what's needed medically. It's important to note that these issues occurred even with the family paying out-of-pocket for a day-sitter and having our immediate family checking in on him + stay with him throughout the afternoons and evenings. That indicates to us that as soon as the family or friends are not around, things are dropped. Despite these issues, the therapy provided, particularly by Mr. James, was commendable, with thorough explanations and personalized attention. There are several others who I don't have names for, but PT and speech are included, and my father bragged about them. Additionally, I can say the Director of Nursing, Karla, addressed our inquiries and concerns when I brought them up to her. The administrative staff also demonstrated professionalism and helpfulness in navigating insurance and care-related matters. We met some very caring and professional care team members during 2 two-week stays in the last 45+ days (Stephanie, Johnnie Mae and 2-3 others who I can't name at the moment, but unfortunately, the majority of the others are no longer employees. I could not, in good conscience, send my father back here.

It's like they forgot how they got you in there in the first place..... Promises and reeling family members into that SHIT HOLE of a care facility. The don't care once you have signed all the paperwork, the already see dollar signs. My family member was in this place, and she was sick most of the time. No one would come after the call button not even her yelling for help. She laid in her own urine for 6 HOURS, and nobody checked on her until we the family members checked our cameras that we installed and found this out. That same night they dropped her in the shower, and she broke her shoulder. It was all downhill from here. She never really recovered from the fall. even after the fall NO physical therapy. She was in better shape before we had her admitted her into Sylvan Shores. Today we no longer have my family member with us after 2 months and if we would have known that this place would not be taken care of her like they said they would, we would have never put her in this place. And she still would have been with us. If you are thinking about putting your family member in this facility DO NOT!!!!!!! Horrible, Horrible, Horrible. My problem is getting in the place when you drive over an hour to get there . And no one is around to let you in before 8am . You either freeze outside in the winter or sweat to tears in the summer until someone comes to let you in . Another problem is getting my mom's food heated up . There is only one microwave in the Emplaye Lounge and they aren't happy when you come in to use it .It would be nice to have another microwave in another area of the place . Thank you ! I placed a food delivery order for my loved one that was very specific -- it clearly stated on the label of the bag to hand the order directly to "said patient. I confirmed with several staff members that the order was received, yet the patient never got it. This was a $46.00 meal for my fat, and no one at the facility can explain what happened to it. When patients are already saying the food provided is not edible, and then outside food meant to help is never delivered, it becomes not just disappointing but unacceptable. The lack of accountability, communication, and basic follow-through is negligent. When you entrust a facility with the care of a loved one, the very least you expect is honesty and reliability. Unfortunately, this facility has failed in both.

Other nursing homes are better if you have the money, but if you don't have it, this is as good as it gets. The food is just ok. I have a friend in a private room and she has her own toilet and sink, but the room is small. The nursing staff comes and goes, she can get really good nursing care some of the time, and she can have really bad nursing care, just depends on who is her nurse at the time. But the Administrator here is very warm and friendly and caring and easy to work with, she wants the best for the residents, just let her know if there is a problem. The grounds are very nice and well kept. This place is old, one of the original nursing homes in Baytown, but it has been maintained and has recently went through some renovations, fresh coats of paint, replaced sinks - toilets. My friend was walking when she came in, but not soon after. I believe they want the residents in a wheelchair because of fall risks. Your family member has to be diligent and persistent in their own care, if they are not willing to push themselves, they will soon be in a wheelchair and eventually will need help just getting into the bed. They want them to sleep at night, so sleeping pills are given. I would say the worst part of this is that when the resident buzzes for the nurse, the nurses don't respond sometimes for 45 mins or at all. I'm guessing too many residents per nurse. That is a long time for a resident to wait if they need help going to the restroom. There are activities, and the residents can make friends, if your loved ones needs economical round the clock care it is not a bad to place to be. In a tense time when I was under immense pressure to find care for my Aunt who was in a bad situation with current caregiver, I made dozens of calls to many different people. When Melinda Bass, owner of Ascend Hospice answered the phone, I could feel her genuine concern and compassion. Not only did we discuss my Aunt's potential for onboarding hospice care, but she went above and beyond by helping me find a place for my Aunt. I felt the genuine concern and the willingness to help. From her suggestion, we found the perfect place for my Aunt. It all felt like divine order, like angels were all around us. In addition to the immediate care they took of the situation, they are working for the greater good as it relates to my Aunt's care by coordinating with her PCP. I strongly recommend considering this group when researching hospice care for your loved one. Even just one phone call can change your (their) life - especially in the difficult times when hospice care is being considered. It's very emotional, but this Ascend is a safe harbor, qualified and ready to help. I've cried many tears of relief. Melinda, Laura and team may be the best in the business.
