I cannot say enough AMAZING things about my experience at Form MD. From the moment I walked into the office, everything felt calm, welcoming, and beautifully run. The staff is incredibly kind, professional, and made me feel comfortable every step of the way. Okkkk....Dr. Pollei -- not only incredibly talented, but also so nice, genuinely funny and just an overall joy to be around. He immediately put me at ease, explained everything clearly, and made the entire process feel effortless. It should also be noted that he was my second consult for an upper eye bleph-- the first doctor I went to, like many others, was trying to oversell me and told me I also needed a surgical brow lift. Dr. Pollei assured me that I did NOT. Finding an honest doctor that does amazing work AND can be trusted is gold & should be everyone's main objective. My bleph turned out better than I could have imagined. It looks so natural and refreshed, and my whole face just feels brighter and more awake without looking "done" at all. This was one of those experiences where you feel completely cared for -- from the office environment to the results themselves. I had high hopes going into facial plastic surgery at this office, but unfortunately, my experience was disappointing. I had researched and interviewed 5 other doctors before making my choice for Dr. Pollei. I had an upper blepharoplasty where not enough skin was removed, leaving little visible improvement. I also had fat grafting to my cheeks, which turned out noticeably uneven--one side looks fuller than the other, and it's been that way ever since with no change after nearly four months. Every time I voiced concerns, I was told to "wait longer" for the results to settle, but even after months, the issues remained. To make matters worse, he offered Botox (at an additional cost) when I came in seeking a solution--despite already spending thousands on procedures with him. He kept telling me if the problem persists he can simply add filler. Yet again, expected me to pay for his poor work. The final straw was when neither the doctor nor his staff scheduled another follow-up. I was essentially ghosted, with no further communication from the office. For such a big investment, the lack of follow-through and accountability was really disheartening. I would not recommend this surgeon. You truly get what you pay for, and in this case, that wasn't much. read more

Dr. Ziai did an amazing job closing a rather large MOHS surgery - on a moments notice, I might add, since I was sent over from my dermatologist after she operated on me. He took his time and did such an excellent job, you can hardly see the incision site. AND i was worried that I would lose a lot of hair - and he worked without having to shave my head! His staff was professional, friendly and knowledgeable in answering all my questions and giving me thorough instructions on how to take care of the surgery site. I moved 2 years ago and went to several different places looking for someone who is good at injectables. I finally found her! Alicia is amazing! She took the time and listened to me about what I like and dislike. After two weeks, I came back in for a follow up and she saw there was a slight difference and she fixed it. I didn't even see it! I was a loyal customer to another aesthetics clinic, but recently converted to Form MD when I found out my favorite aesthetician, Cheryl had moved to Form MD. After having to go with a different aesthetician at the previous business, being informed at the last minute Cheryl no longer worked there, and having a HORRIBLE experience, I hunted down Cheryl online and found she's now working at Form MD. I had her do my most recent Botox and am so happy with the results again. So tasteful, very modest with her Botox usage, and close to painless. She informs me of my options without EVER pressuring me. I initially thought Botox was simply a pain free procedure and people were overreacting, but after my last experience at the other business, I now realize Cheryl is just talented.
