    My aunt came here for rehab after heart surgery. They were rated 5 stars on the hospitals list of…read morerehab centers, so we thought we were sending her to a good place. We were so wrong! After being there for a little over a week, she was sent by ambulance to a hospital for shortness of breath & chest pain. Once there it was discovered that she was bruised black over her entire sternum all the way around her side & was bloated. She was FULL of blood!!! 1 liter of BLACK blood was taken off of her in surgery that was over 24 hours old. This means something happened to her at Kingston & no one there is talking. Her medical team suspects she fell due to all of the bruising on her body as well as the fact that her sternum is separated. She is still in the hospital fighting for her life! If you love your loved ones, DO NOT send them here!!!!

I have broken legs (yes, both of them) and am bed bound and completely dependent on these people for everything. I have to wear diapers. A few of the patient assistants are good, but I think the majority are just here to collect a paycheck. I had a BM and was left lying in my own excrement for an hour. Response time is often abysmal, and there's always an excuse no matter the time of day. Here's a typical excuse: "I had to go to a meeting." So you just leave your patients without coverage? I don't think so. They lie. The food here isn't fit for pigs to eat, and as a Type 1 diabetic I have been fed a steady diet of carbs and sugar that I refuse to eat. I frequently can't stomach the food and they give me too much insulin which sends my glucose level plunging. Think long and hard before coming here or leaving a parent or other loved one.

What most people need to realize, is the importance of building a relationship with the staff caring for your loved one. It is a team effort. This is a care facility. They cannot and should not be expected to give private duty nursing services. It is unrealistic for any facility to be expected to do this. You have to be willing to reach out to the staff and work with them. Come to them with concerns, but also realize a thank you goes a long way when you do this very hard work. I can't begin to thank the staff for the care dad received especially during his final days. I stayed with my dad to care for him and make sure he knew he was not alone. I believe that was my honor and duty, but this staff cared for me as well as my dad. The concern for my well being touched me to my core. I will not forget the compassion my father and I received by all the staff. The flower arrangement that was sent to the funeral home was so beautiful and very appreciated. Staff who became friends were there at the funeral home to support me and my family. Their concern did not stop as dad was taken from the facility. If you are looking for a facility to "team care" for your loved one, stop looking, the Lakes of Sylvania should be strongly considered. I will only refer folks to this facility.

    Our experience with Lakes of Sylvania was beyond tremendous. My loved one experienced care in both…read morethe assisted living wing and also in the skilled nursing area. The director, the nurses, the physical therapists, the chef, the cleaning department and various departments ALL provided the best of care possible. The communication which existed between Lakes of Sylvania and her family was also tremendous. Each day an updated Covid report was to be found in my email and a weekly video was posted to inform all families members of the events of the week. I was able the connect in person with the staff whenever I so desired. My dear one got the BEST of care and compassion from everyone within the walls of Lakes of Sylvania. I would give this campus my highest recomendation. I owe them my deepest gratitude.

Some medical staff are also very attentive to resident needs and observe standard of care requirements as implemented by adequately educated professionals. Deficiencies on the part of other staff are not due to staffing volume but could be remedied by doing what you were trained to do with quality and care. Money may need to be managed in a way better to meet resident basic needs; for example use some of the money spent on landscaping extras to fix non-functioning bathroom fans, attach bedside light cords/bed controls to the bed so within reach instead of hanging loose, falling onto floor,etc. Allow for resident to watch/listen to their own TV by providing audio via remote or another device that does not disturb roommate and others in adjacent rooms. It would also help to have staff wear ID badges so the resident know who is delivering a service.

Our family personally had a horrific experience that could've resulted in the DEATH of my father. A nurse came to give my father an insulin shot.... Except my father is NOT diebtic or has ever been diebetic. My father had a stroke and needed PT to gain strength and get back home! My poor father was confused when the nurse came in (he has a bit of dementia.) Thankfully a family member was there when the nurse came in with a needle of insulin and spoke up for him!! From that moment on, we decided to have our family members rotating for 4 nights - staying with my father for fear of him being yelled at, neglected or given life threatening medication. Thankfully his insurance was finally approved and we left to a much better, safer and cleaner facility! PLEASE DO NOT NOT TAKE YOUR LOVED ONES TO ARBORS AT SYLVANIA!!! My heart breaks thinking of the precious elderly people still stuck there being neglected and mistreated. They deserve so much better and so did our family. Our experiences at Arbors at sylvania are law suit worthy!!!!

It's nothing active physical that I'm worried about, It's my dependence on staff to enjoy their work and job since with any movements my O2 saturation can and has gone below 42% at which time I totally lost visual acuity but not auditory. The last thing I did hear was after a code was called on me. One person said "he's blue, another couple were laughing " After calling the code everyone was in the room seeming to be apprehensive/unwilling to do anything. Then I lost all consciousness. I guess 16lpm of o2 and the rebreather brought me back. When I started my journey to this facility via St. V's I was able to breathe with room air up to 3-6lpm of O2. Now if I'm absolutely still or sleeping I require from 8lpm to 10 lpm just laying on my side. If I maneuver to pick something up or moving the absorbent pad underneath me my O2 levels will drop into the low 70% range and require the O2 rebreather be used and the high-flow hospital Air on 16lpm to slowly recover. Nurse Shawna figured out a new trick, she said I refused all my meds except for Magnesium, metoprolol, and metformin. Since my Oxygen drops so fast and by 40-50% sometimes. It causes me anxiety, that feeds into my breathing problems and makes me more anxious. Getting only 2 of my many medications and Nurse Shawna saying there's nothing she can do once she puts the order in is way too much power over the life and death for one person over their patient. As soon as the new shift came in they immediately called the respiratory therapist and she started a breathing treatment and elevated my Oxygen up to 15lpm. Nurse Shawna would make hospice nurses blush. Dr Kevorkian could make Shawna an equal partner. I'm sure she has a lot to bring to the the gurney.

    My mother spent 7 days here after receiving a poor diagnosis from the Flower Hospital palliative…read morecare doctor. The nurse liaison was also way off base. They insisted our mother would be best served in this facility. LESS than 12 hours later we were told by the nurses and social worker that our mother didn't belong here and we needed to find someone else immediately. This was less than 24 hours after receiving a terminal diagnosis. The care was horrible. They made her defecate in her Depends because "it was too difficult" to move her. They confined her to her bed and refused to let her even sit in a chair for the same reason. My mother weighs 150 pounds. We were told my mother bit someone and was constantly agitated. This is after 24 hours of asking them to provide the proper medications for her because "they couldn't get hold of them". The clinical director later said she absolutely did not bite anyone. They ruined her chances of getting into a hospice nursing home because of the biting report and keeping her on Haldol to keep her sleepy and compliant. They they told us the doctor was taking her off of inpatient hospice and we would need her to begin paying $270 a day to keep her there. The nurses were rude and brusque. They literally laughed in my mother's face when she asked for something that didn't make sense because of her dementia. This was one of the worst experiences of our lives. My three sisters and I couldn't wait to get my mother out of this facility. It made us sick to watch how she was cared for. The building may look nice but it's rotten to its core.

The individual caregivers are remarkably attentive and the individual nurses are caring. However, what makes Charter Senior Living of Oak Openings nearly impossible to work with is the management team, especially the current Executive Director and the Director of Wellness/Nursing. It took the ED nearly a week to tell me how often rooms were cleaned when we expressed concerns about other residents using our mom's toilet or other residents urinating or dedicating in her wastebasket. The Director of Wellness/Nursing Director refused to accept orders from physicians about basic healthcare including orders to determine UTIs. I did more of his job than he did. They are not problem-solvers. They tell you all the reasons that problems are happening, but seem incapable of solving them. We had a resident who was fixated on our loved one. She unplugged her lights, tv, and modem. She stole pictures off of our mom's walls, stole her belongings, and took her hearing aids. She eventually threw water at a family member who was visiting our loved one and presented our loved one with her poop. Despite this being in direct violation of the code of conduct explained in the contract, they did NOTHING. The Management team explained all of the reasons that this was happening, but was not willing or able to solve the problem. A male resident showed up in our mom's bed and wouldn't leave. Again, a lot of explanations about why it happened but no solutions for how to solve it. How did Charter Senior Living of Oak Openings respond to these problems? They discharged our loved one! Yes, they were cited by the Ohio Department of Health, but the stress that they placed on our family during an already difficult time was unimaginable. Having a loved one go to a memory care facility is stressful. They made it worse.
