We have an unusual plan because it is for retired military, but it should have been accepted. They don't bother to tell you until you have to pay the bill at the end of the session. My family and I are through with them. I hope others find this information useful and don't have to pay $250 for their kid's cleaning. Update: After calling Humana, we found out that Dr. Fazio is covered under our plan and we are filing a claim directly with Humana. This proves that the insurance people at Aspen aren't doing their job properly.
