This review is specifically for the skilled nursing and rehabilitation services. After a 1 week…read morestay at the local hospital my grandmother was recommended to Meadowood at O'Connor. She was septic with pneumonia. She was alert, moving, talkative and cheerful when she arrived here and within her 1 week stay she took a drastic turn for the worse. I feel this place is severely understaffed. Each day we noticed the lack of care she was receiving from the staff. We could also hear other patients in rooms nearby shouting for help and nothing was done for them either. The staff also mistook her for another patient so the fact that they could not even remember or keep track of her needs was alarming. They were giving her all the wrong foods, liquids, and hardly any medication to improve her health and get her strong enough to come home. Even after filling out their little cards for specific food and drink instructions the changes were not made. Within 1 week she went from cheerful and talkative to being so weak that she was unable to feed herself or formulate full sentences. The PT staff said she was progressing so well she was walking herself around to the dining area and would be released soon while the actual nurses were saying she was so weak they couldn't even assist her to the bathroom anymore. Talk about inconsistent and polar opposite feedback from the staff about the same person... since they couldn't help her to the restroom anymore she was going in the bed and constantly needed to be changed which they would take 1-2 hours to complete which is a bit dehumanizing and humiliating to have to sit in and after they changed her and the sheets they did not put her blankets back on her - mind you this was in the middle of winter. She was shivering and curled up in a ball with blankets on the floor when we saw her. She was severely dehydrated yet the entire time they did not have her on any IV fluids to get her body the nutrients it so desperately needed. After claiming she would be released for HOME (again I have no idea how they were able to say this to us when they we were clearly not talking about the same person) she was actually transferred back to the local hospital. From there the local hospital deemed her health was so irreversible that all there was left to do was to take her to hospice care. She passed away only 4 days later. I blame this facility for her rapid decline and I also blame myself for allowing her to stay here. We should've taken her out after all the mistakes and neglect here. When she arrived here she was her full self and when she left here she looked skeletal. She lost her figure and you could tell she was dying. Please think twice before taking your loved ones here and keep a close eye on the care they are receiving. If you feel like this is not the right fit, listen to your gut and find a different facility. It may look nice from the outside but the care is some of the worst I have seen.

My mom was in another memory care facility which was fine, but I immediately noticed a positive difference at Summerfield in the level of care and communication. When our grandchildren visited mom they made wonderful memories joining her in activities, games and getting to know the other residents. Mom received loving care in her last days. The med techs and caretakers went above and beyond, showing us respect and mutual concern for mom's comfort. My mom was well cared for, and staff were very attentive. However, six months later, Del Monte Assisted Living bought the company. Employees of many years suddenly were let go. I live out of state and had put my trust in this facility. Communication was horrible with me leaving messages of concern on the phone about the care of my mother, and no call backs. Heat was off for 3 consecutive days and when I contacted the owner, he said he wasn't aware of the situation. Another time, there was no cook available to prepare dinner for the residents, so the company provided delivered pizza for all of the residents. My mom later told me it was cold, hard, and she threw it away. Also, several times, there was no hot water for my mother to shower, so she missed having her twice a week shower. and when the water was "warm" she had to wait until her next shower day to take a shower. So there were times she didn't' get a shower for that week, prolonging her shower to 10 days. Or the staff "ran out of time on their schedule" and again, didn't get her shower. She voiced her concerns and later legitimate complaints to staff, and they just told her "I don't know!" The facility lost my mom's new wheelchair. No one helped her look for it. I called several times, with no call back. Due to my concerns of my mother's safety, I pulled her out of the facility. I gave plenty of notice for her meds to be ready when she left Del Monte. We were given an empty bottle of one of her prescriptions, and we requested two times on same day for them to have it filled, but no call back. Also, they gave us an empty prescription bottle of another resident! We called them back about the serious mistake, and they immediately wanted that bottle back. However, I took a picture of the bottle. Total renovation construction inside and outside was started when Del Monte first bought the company. Then all construction came to a halt because the construction company didn't have a state contractor's license. Safety signs were put up all over the facility, and nothing was finished. I called Del Monte and was told that "there was no money to complete the unfinished construction projects." All throughout the facility, it was an eye sore to see nothing finished to completion. I have since then, filed a formal complaint with the State of California against this facility. This is not a 5 star review facility as shown with the online reviews. Check the recent dates of all reviews. Please don't go by the reviews dated before 2022. My mother still has a clear mind for her age to let me know about her care. However, for residents who either can't express themselves or are no longer mentally capable, family members never know of the lack of care that their loved one(s) deserve and are paying for each month. In closing, they increased my mother's monthly rent an ADDITIONAL $650.00 MORE per month. That is when we pulled her out of this facility. Continue in the app

When I went to visit my family member (not my choice to put them here) it looked as if she was drugged... she was not taken care of at all... do not put your loved ones here if you truly care for them... a friend of mine told me she had her dad there and he almost died in there care due to neglect.. and I've seen their shower room and there are tons of towels piled up and tons of shoes piled up. If you want your loved one in good care and alive I beg please don't put them there. And in their prior review they can't even pay their employees.

I met with them several times and was impressed with their care for their residents in memory care and their staff is also as caring and kind as they are. Typically you find highly processed and bland food at homes like these but when mom sat down for her first lunch I was so surprised at the deliciousness of their food which is cooked onsite (I have a good source there who advised me of this). It was so good I wanted to eat lunch too. Additionally, they serve fresh, delicious snacks, not anything pre-packaged. Mom tends to want to sleep much and so we put her on a schedule to keep her busy as she is very high functioning, and when I showed up the following day there she was interacting and happy. The staff are constantly checking in on her and are always available when needed without delay. I'm so happy to have made the move to The Commons for mom as we FINALLY found the right place for her.
