The worst of the worst. The staff is very rude,non concerning, smart ass mouth to residents and family. They play on their phone at the desk while residents need help, laughing joking. They leave information open to anyone to read about patients. Steal items and say oh I'll check the laundry, not really do it."" No call lights, phone, curtains, water, they simply don't care! We had to rescue my husband. He was dying....he was so dehydrated and infected, they didn't bother to send him to the er, we had to! The worst place ever, DO NOT SEND YOUR LOVED ONE HERE!!! Since, I started ive had the run in with my therapy or the therapist people.....ive corporate with them and i knew, my time, was coming to and end buy the thing that gets me is for someone, to discuss My insurance. Absolutely horrific! Absolute WORST EVER!!! They are neglecting my mom, let her sit in urine and whatever else is in her depend. They don't give her her meds in a timely manner. They haven't switched her diet from liquids and I've requested every day for a week. It's a challenge to get these people to get her in a wheelchair to move around a bit, it's absurd! Her call button takes at least 30-45 mins to get a nurse and that includes me calling them to see why it's taking so long and my mom yelling for help. It's insane and I'm working on getting her out of this place ASAP! This place should be shut down. I am also reporting to all the places I can. There's no reason for them to be this dedicated to not giving any concern for the patients In their definitely NOT "care" home. read more

I wish I could give zero/negative stars. One of my family members currently resides at Windsor Elmhaven Care Center in Stockton, CA. He suffered a stroke years ago and was placed at Windsor Elmhaven when his prior skilled nursing facility closed. Luckily for him, the stroke did not did not cause brain damage to his left or right brain; only to his brainstem. He is like Christopher Reeve or Stephen Hawking (brain works, but body does not). He has a tracheotomy and a feeding tube. He also has extremely limited movement in his right hand. With this hand, he is able to write messages on paper and on an iPad. He sends continuous pleas for help to his family; notifying us that the staff do not regularly change his soiled disposable underwear (he is incontinent). He also regularly insists that staff are retaliatory towards him for "complaining." None of his family has the capacity to care for him at home due to his extremely intensive medical needs and are not able to privately fund a transfer to another facility (the costs are astronomical). His family has spoken to the facility, filed police reports, spoken to the ombudsperson, and filed complaints with the state. But to no avail; besides my uncle's complaints, there is no "proof" that this is happening and we are not with him 24/7 to observe any of these things happening. His family will continue to check-in on him and pester the facility to care for him until a change is made. I just wanted to let people know that this has been our personal experience with the facility. read more

I made appointments for the week and wrote them down exactly as we were scheduling however come time for the appointment and they couldn't find that it was scheduled then they said it was a different time that never even came up because I took the last time of 215 for that day but they stated it was at 12 totally Wrong because that time slot wasn't even available ..... then as they begin confirming the rest of the week the times don't even match what I was told on the phone ..... someone switches the times I originally scheduled and didn't bother letting me know on top of that I was told I would receive a call the morning off the appointment to Confirm ... that wasn't even done so that tells me someone isn't doing their job properly ..... read more

What a busy little world in itself.  My very first experience with convalescent environment.  So much involved.  So much to consider.  So many staff hustling and bustling doing their parts.  Most of which appeared happy with their parts .  It was a little rough at first.  Looking out for my brother (Mr Apilado).  On the other side of a stroke and brain surgery, he didn't know what he needed. After asking many questions and establishing a good communication, all of his needs were pretty well accommodated.  I did learn about the daily menu ordering. Writing "diabetic" and "low sodium" can help the preparation proper process. The Physical Therapy people were so awesome.  This business can be depressing to be away from home, strange place, strange bed, strange people everywhere, many just as unhappy about individual discomforts, all kindness extended from staff can go a long way. Thank you Windsor staff. Oh, one more thing. Whom ever is in charge of answering email on the website, must not have a daily reminder to check it.  Today is Feb 26, 2019, Its been couple few weeks since I emailed, have yet to receive any response 3/11/19.  One information request.   One nursing concern. In October of 2021 my father was sent here from a rehab facility in downtown modesto for rehab post CVA. I thought our horrors were over after leaving , one employee from over there said that Windsor was, "The best of the Best" turns out it was more like the worst of the worst, PAAAHHH. It's nearly impossible to get in due to lies from staff and administration talking about a covid outbreak when everybody knows that is all fake news. The few times I was able to visit were not only haunting but left me with a headache due to a combination of my father's roommates family yelling at staff non stop , "when they getting that tube out my daddy neck" and what I can only describe as a surround sound system playing the intro to 21 jumpstreet on a loop. read more

Need I Say more? Hip surgery. And they refused to allow her to use a bed pan, made her wear a diaper and she begged to be cleaned up but no one would help. Now her bandage is soaked in urine and family had to run to the rescue. This place is a joke. Do not leave your family member unattended. Thankfully our grandmother is healthy young and able to call us to let us know she was sitting there soaked in this condition. It's sad that she had to go through this. Now someone's as to be with her 24/7 to make sure she is cared for properly and to prevent infection to her incision area. You guys should be ashamed. read more

My Dad passed away and left a suitcase and a bag at this place. I stopped today and picked it up. I then drove back home 3.5 hours. When I unloaded the bags out of my car into the garage several large very healthy roaches sped off in all directions out of his bags. I was shocked and managed to kill only one. So I assume my car and garage are infested with these roaches just like this dump is in Stockton. read more

This place is highest level of incompetency the nurses do What they can and they are all very nice under the circumstances Short staffed always, things never work l.e. bed control call lights don't work, like it makes any difference, it takes sometimes an hr or hr an half to get needs taken care of. I do not recommend this place to anyone, actually it should be shut down, how this place is still in business is a mystery to me, I wouldn't send a dead maggot here, but I'm sure there are plenty of them here, live ones as well along with all the gnats flying around. The food is not nutritious, no fresh fruit to be offered. Mostly over cooked soggy frozen vegetables lots of carbohydrates, no fresh vegetables. Rooms get cleaned once a day, garbage containers over flow, all is ignored just like the patients. Horrible horrible place. I'm sorry I got stuck in this place! Absolutely terrible. The management is constantly changing. I think they have had 7 different directors of nursing within the year and several different administrators in the few years my family member has been here. The current administrator, Anthony, seems too busy to even care about his patients and never gives us direct answers. Never follows up with our complaints. They are always short on staff. CNAs are hard to find. One CNA told me they only had 1 CNA for 30 something patients on the respiratory side. The nurses are always complaining they don't have staff, wound nurses are usually not in the building even though we were promised that there would always be a wound nurse on day shift. I've overheard the staff frequently talking about bed bugs and scabies and how the management tries to hide it. A few employees even said they got scabies from the patients there. This facility is always out of supplies. One time they told us that they only had small gloves and they're always out of medications. Honestly I'm not sure how this place isn't shut down yet. Showers for my family member are always skipped and I don't get notified of medication changes or changes of status. I'm always scared about them retaliating against my family member when I complain because they will neglect her for hours if they find out we complained. Looking for a new place even if I have to drive an hour out of town. I was just updated that the respiratory unit is being treated for asbestos with the patients still residing in the unit itself which is super contradicting read more

My sister fell and fractured her hip, after being in the hospital for 3 days she was released to Windsor Elmhaven Care Center. I cant say enough good things about the staff! From the admssion woman who visited her in the hospital to the nurses who took care of her. As in all facilities like this a patient needs an advocate to help to make sure that the patient is taken care of. They had classes and fun things to do during the day. My sister was very happy with her care. I understand that they have gone thru an ownership change so problems from the past are being resolved. I know that if she were to need care like that again we would have no problem with her being there. read more

I was a patient at Windsor Elmhaven Care Center from 2/12/18 to 2/24/18. This is my first time to write a review regarding a skilled nursing facility where I was a patient. I try to be as objective as I can for the benefit of others. In general, I would rate as 2.5 but because of customer services from rank and file employee I gave the center 3 stars. Plus: *** key strength: employee's attitude *** - Employee dedication, hardworking, friendly, and accommodating. - Willing to augment meal with additional drinks when possible and permissible. - Superb gym staff - CNA's (Delia, Felicimo) my OT (Justin), my PT (John, Roberto), and my ST (Whitney) Minus: - Facility odor. - Shift transition - for the employee who works 11.5 hours from 7 pm to 6:30 am. There is no coverage from 6:30 am to 7:00 pm. More than one occasion such as 1/19 I had to wait for more than 30 minutes to have some CNA's to guide me to the bath room. RECOMMENDATIONS: 1) Hire more staff 2) Have dedicated bathroom dispatcher (1 on West Wing, 1 on East Wing) OBSERVATIONS: Current employee is responsible for 15 patients to 20 patients. (overworked, underpaid) (Kaiser: 5 patients) - On my second day there, the billing supervisor along with other financial staffs told me to pay up additional $160.00 per day or leave. It is not their responsibility to resolve the financial difference with Kaiser. It turned out that Kaiser had a break down on their billing charge allocation with Windsor Elmhaven. I request Windsor to work with me and Kaiser to resolve the dispute. They refused to help and said that it is my issue. No help from Windsor management. - On my last day at Windsor Sat 2/24, the nurse in charge was so upset that I got permission to discharge one day from my business case manager (Ruben), my nurse (Carolyn), my CNAs (to stop the nurse from charging Kaiser one extra day, threatened me and wrote me up for leaving early by one day, and demanded me to return. Do not put your loved ones in this nursing home. We almost lost my father because of poor nursing care. I told the nurse assigned to my father that he looks weak and more confused, she told me she'll check on him. The next day I asked the nurse what was the doctors recommendation. He told me he did not know anything about it. I insisted for him to call the doctor. My father was sent to the ER because of difficulty breathing. He was diagnosed of sepsis and dehydration. Sepsis because the infection went to his blood, because Elmhaven did not catch the infection early, dehydration because he was not monitored if he was getting enough fluids. ER nurses told me the patients they receive from Elmhaven are very sick because of poor nursing care. The facility smells bad and dirty. Very poor nursing care. read more

If I could give zero stars I would. This place is the worst! My mother passed away a few years ago. She was on the sub-acute side of the facility paralyzed and on a ventilator. She suffered from sepsis, bedsores, and many other infections. The night she passed, I was notified she was at the hospital by the attending ER physician. No one from the facility notified me. Horrible place. My dad was unable to walk or talk and their nursing staff allowed him to sit in his own mess for hours on end till it created sores and ate away his backside. The care that he received was abhorrent and inhumane. Please take your loved ones elsewhere. With the exception of a handful of wonderful people, the majority of the staff there can't be bothered with helping those those you care for. read more

My mother spent 28 days at Windsor Elmhaven. The level of care that she received was 5 star. The physical therapy, wound management, and level of communication from the management to patient was excellent. My mother, coming from a different SNF had much to compare it to. The new management here at Windsor is very attentive to patient care and the end result shows. The head manager is always in and you can communicate directly with him should you desire. read more

My father in law loves living here. They have lots of social events. We have had trouble with his clothing disappearing. I rate it a 4 even with the clothing issue because the happiness of my loved one outweighs this. It would be 5 stars if they took better care of his personal belongings. read more

This place is HORRIBLE!!!! If you care about someone in your life avoid this place completely. They are completely incompetent. Promise everything and do nothing. We were told they provide physical therapy multiple times a day and there were times we saw nobody. You call someone to change your loved one and wait an hour. They said a doctor would be here to talk with daily and in 7 days we have never seen one! I am taking my mother out of here before they kill her! This place should be shut down!!!!! read more

This place is dirty and frequently smells bad. The staff doesn't give proper care and they don't seem to be much help at all.

My mother is there and she has to wait forever for a nurse to come in the room or even a CNA when she pushes her call Button she has to call me to call them for them to go see her so wait 30 minutes sometimes an hour it's ridiculous I can't believe how long it takes for a nurse to come in other than that it seems like it's an all right place but I would really like to get my mom into a different Rehabilitation Center read more

I still have nightmares knowing I left my husband in Windsor Elmhaven. He was referred to them for rehabilitation after back surgery from Kaiser Hospital. The care a patient receives is non existent. Inmates in prisons get better care. He was bedridden and totally dependent on the care of the nursing staff and their time or availability . We asked for a urinal for him and was told that supplies had only delivered one that day and it was given to another patient. He waited 13 hours before he received one. Leaving him to urinate in the diaper they put him in. One person put him in 3 diapers at once so she didn't have to change him as often. This caused him to have back and leg pains because of having to spread his legs so far apart. He didn't have bed linens or blankets. I found the linen closet which only held 2 wash cloths and a sheet. I covered him with a sheet. He received 1 bath in 5 weeks. He didn't receive meals for 2 days because he was told his diet restrictions were not covered in his transfer file. I brought his meals to him. Twice staff ran out of his pain medication before reordering it which left him in excruciating pain for 10 hours until the new medications came in. On one occasion he was given someone else medication rather than his own. I continuously reported all of this to administration and was told that they would take care of it. Which of course they did not. But the worst of all is the late dark night it was raining hard and I walked to my car in the parking lot. I heard a weak voice calling out "help me". I looked around in the dimly lit area she was in and found an elderly patient from the facility that had gotten out the side door in her wheel chair sitting between two parked cars soaking wet with nothing covering her. I wheeled her back inside and screamed for help. It took several minutes before someone came. There answer was "oh, she does that all of the time". My husbands parting gift from the facility was hemorrhoids, diaper rash, dry scabby feet and constipation. Thank You I just wish I could have evaluated Windsor Elmhaven Care Center with a 0* rather than a 1* read more
